Endoscopic endonasal sinus surgery: a review of 18 years of practice and long-term follow-up.

A study of 1,915 patients operated on with a total of 4,285 endoscopic endonasal ethmoidal procedures during a period of 18 years is presented. The study comprises five groups of patients with (1) acute recurrent and chronic sinusitis--long-standing symptoms >3 months--(n = 733), (2) nasal/paranasal polyposis (n = 710), (3) sinogenic headache (n = 325), (4) mucoceles (n = 59) and (5) other nasal/paranasal disorders (n = 88). This group includes acute sinus infection with complications (n = 38), acute trauma/injury (n = 22), Grave's disease with exophthalmus (n = 12) and olfactory dysfunction (n = 16). The need for a thorough examination leading to a final diagnosis is emphasized. Endoscopy and CT/MRI are standard tools in the diagnostic and postoperative process. Close cooperation and educational teaching of the patient are necessary to obtain highest compliance. Long-standing medical treatment of inflammatory diseases is important to the long-term outcome and is of utmost importance to surgical success. Surgery is recommended to be performed under general anesthesia (total intravenous anesthesia or laryngeal mask anesthesia) with additional local, topical anesthetics, and can be easily performed on a day-case out-patient basis. The surgery is based upon the Messerklinger technique (MT). Peroperative complications are reduced by a skilled and meticulous surgical conduct and an adequate medication preoperatively. During this period of time, necessary controls within the first postoperative month were reduced from 3-4 to only one. Long-term follow-up secures a long-standing optimal result. Endoscopic surgery for more rare conditions, such as orbital decompression, CSF leaks and meningoceles, is recommended to be performed in specialized centers with adequate trained staff and necessary equipment. The possibilities of the picture archiving communication system, giving a number of different projections and angles and video-guided imaging peroperatively, facilitates safe surgical performances in areas usually associated with great risks and hazards. The long-term results are very good during the above circumstances. Reoperations are few and the patient satisfaction is very high.

Interleukin 5, IL6, IL12, IFN-gamma, RANTES and Fractalkine in human nasal polyps, turbinate mucosa and serum.

Polyps are considered to develop as an end result of an inflammatory process. Cytokines and chemokines in the respiratory mucosa may be a key to polyp pathophysiology. The main objective was to identify IL-5, IL-6, IL-12, RANTES, IFN-gamma and Fractalkine in humans on the protein level in nasal polyps and mucosa from the inferior turbinate (IT). Furthermore, the cytokines and chemokines RANTES and Fractalkine were analyzed in plasma. Tissue homogenates and plasma from 13 patients were analyzed by the ELISA technique. All the patients had longstanding nasal/paranasal polyposis. Fractalkine was detected in polyps and IT in two different patients. IL-5 was expressed in polyps and IT. IL-6 was expressed in all patients with a higher level in polyps than IT. IL-12 was present in plasma, polyps and IT, though at an increased level in polyps. RANTES was present at a higher level in plasma than in polyps and IT. IFN-gamma was detectable in polyps and IT. Fractalkine is detected in nasal polyps, which is a new observation. The overall results indicate a mixed T(H)1/T(H)2 cytokine profile in nasal polyps. RANTES and IL-12 are strongly present in plasma, suggesting an ongoing inflammatory "drive". IL-6 and IL-12 are up-regulated in polyps versus the IT. Up-regulation of IL-6 may be explained by increased fibroblast activity dependant on an ongoing local inflammation possibly initiated by an infection. IL-5, RANTES and IFN-gamma are equally represented in polyps and IT, indicating equilibrium between the nasal polyps and surrounding tissue, and that an up-regulation of cytokines in the polyp indicates a potential for polyp growth.

The role of computed tomography in endoscopic sinus surgery: a review of 10 years' practice.

CT scans from 100 patients undergoing endoscopic sinus surgery (ESS) were primarily re-examined in order to assess the quality of the radiological examinations and the information it could provide years later. The material comprised the cases of patients who had undergone ESS by one surgeon (AD) in the 1990s. The indications for surgery were acute recurrent or chronic sinus infections, polyposis of the nasal and paranasal cavities, mucoceles and sinogenic headache. All examinations were described using a scheme modified according to Stoney et al., evaluating the opacity/mucosal thickening, drainage/pathways, the lateral nasal walls, septum and other bony walls. This study has been re-evaluated and an additional group of 20 patients consecutively operated on in 2002/2003 has been added. The initial radiological examinations often proved incomplete, with insufficient descriptions of either the drainage/pathways, the lateral nasal wall or the septum and bony walls, which all are structures of great value to evaluate preoperatively for the endoscopic surgeon. The operative findings correlated well with the CT if taken between 4-6 weeks before the operation. The structural pathology correlated with preoperative endoscopic findings independently of the time lapse between the scans taken and the date of surgery. The development and the gradual implementation of the picture archiving and communication system (PACS) certainly represent huge advantages in radiological assessment. The need for CT scans to be available in the operating room calls for specific computer installations to make PACS readily available peroperatively and in an easily workable way for the surgeon.

Continuous laryngoscopy exercise test: a method for visualizing laryngeal dysfunction during exercise.

OBJECTIVES/HYPOTHESIS: To assess the diagnostic feasibility and patient acceptance of a new developed diagnostic test for exercise induced upper airway flow limitation. STUDY DESIGN: Clinical case control study including evaluation of contemporary ergo-spirometry and laryngoscopy continuously performed during exercise. METHODS: Twelve nonsymptomatic controls and four young females with documented dyspnea and noisy breathing during exercise were studied. All subjects exercised to exhaustion on a treadmill while attached to a fully equipped ergo-spirometry unit and a fiberoptic laryngoscope linked to a video camera and a sound recorder. RESULTS: The test situation was well tolerated. Two control subjects had a minor inspiratory synchronous medial motion of the aryepiglottic folds without limitation of laryngeal airflow. In the four symptomatic subjects, exercise induced inspiratory synchronous medial motion of the dorsal part of the aryepiglottic folds as well as vocal cord adduction and inspiratory stridor was demonstrated. CONCLUSION: The continuous laryngoscopy exercise test was easy to perform, well tolerated, and can be implemented in future diagnostic work-up programs of laryngeal dysfunction.

Cystatin A and HIV-1 p24 antigen expression in tonsillar lymphoid follicles during HIV-1 infection and during highly active antiretroviral therapy.

Cystatin A is a natural cysteine proteinase inhibitor and is found in a wide variety of normal cells. The physiologic role of Cystatin A is not fully known, however. Cystatin A is present in large amounts in follicular dendritic cells, which are important in HIV-1 pathogenesis. We analyzed Cystatin A expression in tonsillar sections from 20 patients at various stages of HIV-1 infection. There was a significant (P < 0.001) difference in Cystatin A fractions between patients and controls, with medians (ranges) of 0.61 (0.46-0.83) and 0.86 (0.78-0.90), respectively. Inverse correlations (Spearman rho) existed between Cystatin A and the rate of follicular fragmentation (rho = -0.658) and HIV-1 p24 antigen expression (rho = -0.622) in germinal centers and the amount of HIV-1 RNA in tonsillar tissue (rho = -0.765). The Cystatin A fraction declined from early chronic HIV-1 infection and was significantly lower in patients with a CD4 count below as compared with above 300 cells/muL of blood (P < 0.001), suggesting a favorable initiation of highly active antiretroviral therapy (HAART) at this level. Regeneration of Cystatin A to normal levels was shown in 11 patients 12 and 48 weeks after initiation of HAART, whereas the rate of follicular fragmentation was still elevated. Thus, we found Cystatin A to be a sensitive marker during HIV-1 infection and for regeneration of follicular lymphoid tissue during HAART.

Viable head and neck tumor spheroids stimulate in vitro autologous monocyte MCP-1 secretion through soluble substances and CD14/lectin-like receptors.

Biopsies from carcinoma tissue and benign control mucosa from head and neck squamous cell carcinoma (HNSCC) patients were used to establish fragment (F)-spheroids in vitro. We have previously shown that autologous monocytes co-cultured with F-spheroids in vitro augment their secretion of monocyte chemotactic protein-1 (MCP-1). Presently, the aims of the present work were to study whether the metabolic activity, secreted products and/or specific receptor/ligand on the surface of the F-spheroids and monocytes are necessary for stimulation of the monocyte MCP-1 secretion upon F-spheroid co-culture. Actinomycin D (1 mug/ml for 24 h) pre-treatment of the F-spheroids abolished the monocyte MCP-1 co-culture response. Co-culture of monocytes and F-spheroids separated by a semi-permeable membrane showed a decreased, but still present, monocyte MCP-1 co-culture response. Conditioned medium from F-spheroids stimulated allogenous monocytes to secrete MCP-1. The addition of glucose or galactose, but not mannose, to co-cultures partially inhibited the monocyte MCP-1 co-culture response. The addition of anti-CD14 antibody diminished the MCP-1 co-culture response. In conclusion, the monocyte MCP-1 co-culture response is dependent on metabolically active spheroids, secreted stimuli, and is augmented by direct contact with F-spheroids, possibly via lectin-like receptors and the CD14 receptor.

Ultrasound of the abdomen and total bone scintigraphy in patients with cancer of the head and neck.

In this prospective study, the patients with head and neck cancer admitted to the Department of Otolaryngology and Head and Neck Surgery, Haukeland University Hospital, underwent examination with ultrasound of the abdomen (112 patients) and total bone scintigraphy (118 patients) in the primary staging of the disease and before the start of treatment. In only one patient did we find metastases of head and neck cancer in the liver by these additional investigations, and one patient was found to have bone metastases. No second primary cancers were found by these extra examinations. One patient was found to have an asymptomatic aorta aneurysm and was in need of urgent surgical treatment. Based on these findings, we nowadays only use ultrasound of the abdomen and total bone scintigraphy as part of the general examination of new head and neck patients in selected cases.

[A four-year-old boy with swollen cheek after a fall].

A four-year-old boy had fallen from a tree in nursery school and had minor symptoms immediately after the trauma. He was admitted to the local casualty clinic the same night, but no signs of local injury were detected. The next morning he was admitted to the department of oto-rhino-laryngology at the local hospital due to increasing local swelling and pain in the cheek. A computed tomography examination demonstrated an extensive preauricular soft tissue swelling and a long central hypodensity from the left ear to the oropharynx. The presence of a penetrating foreign body was suspected and the boy was admitted to our hospital. An examination in general anaesthesia revealed a small skin lesion medial to the tragus in the left external auditory meatus and a wooden stick was seen crossing the oropharynx. The eight and a half centimetre long wooden stick had penetrated from the cartilaginous part of the left external auditory meatus to the right palatal tonsil. It was removed through a pre-auricular incision. No complications were observed and the boy could leave the hospital after four days of observation with antibiotic therapy and tetanus prophylaxis.

Mood, anxiety and sense of humor in head and neck cancer patients in relation to disease stage, prognosis and quality of life.

CONCLUSION: At diagnosis, control patients had the lowest depression levels and anxiety scores, followed by those with limited HNSCC disease and these with extended HNSCC disease. Anxiety and depression levels at diagnosis predicted prognosis via an association with extent of disease. Sense of humor, but not anxiety or depression level, predicted lower QoL and depression levels at follow-up. OBJECTIVE: To study the association between anxiety score, depression level at sense of humor at diagnosis in head and neck squamous cell carcinoma (HNSCC) patients versus TNM stage, prognosis and depression level/quality of life (QOL) 6 years following diagnosis. A control group of patients with benign HN disease was also included. MATERIAL AND METHODS: Male patients with newly diagnosed HNSCC (n =78) or benign HN (n =61) disease completed the following questionnaires: the Beck Depression Inventory, the Spielberger Trait Anxiety Inventory (state) and the Svebak humor questionnaire. Patients with cachexia or those aged >80 years were excluded. In the HNSCC patients, TNM stage, prognosis and QOL/depression level (n =27) were determined following successful therapy. RESULTS: HNSCC patients reported high anxiety scores and lower depression levels than control patients, although there was overlap between the groups. N stage was associated with high anxiety scores and depression levels, whereas T stage was only associated with depression levels. Both anxiety scores and depression levels at diagnosis predicted prognosis through an association with TNM stage. Sense of humor, but not depression levels or anxiety scores, at diagnosis predicted QoL and depression levels at follow-up.

Head and neck squamous cell carcinoma spheroid- and monocyte spheroid-stimulated IL-6 and monocyte chemotactic protein-1 secretion are related to TNM stage, inflammatory state and tumor macrophage density.

Monocyte fragment (F)-spheroid-stimulated and F-spheroid IL-6 and monocyte chemotactic protein (MCP)-1 secretion are related to inflammatory state, macrophage density and the TNM stage of patients with head and neck squamous cell carcinoma (HNSCC). Fragment (F)-spheroids from HNSCC patients in vitro secrete and stimulate autologous monocytes to secrete IL-6 and MCP-1. The aim of this investigation was to study this cytokine secretion in relation to other cytokines, spheroid composition and host factors.In series I (n=14) the densities of epithelial cells, fibroblasts and macrophages were determined in sections from F-spheroids and donor tissue. In series II (n=17) the TNM stage, donor inflammatory state, macrophage density and the secretion of F-spheroid- and monocyte F-spheroid-stimulated IL-6, MCP-1 and tumor necrosis factor (TNF)-alpha were determined. Epithelial cells were partly replaced by interstitial tissue during spheroid formation. Malignant (M) F-spheroids secreted more MCP-1 than benign (B) F-spheroids. No F-spheroid secreted measurable amounts of TNF-alpha. Monocytes secreted more IL-6 when co-cultured with MF- compared to BF-spheroids. Monocyte IL-6 MF- and MCP-1 MB-spheroid-stimulated secretion correlated with macrophage density. In addition, there was an association between MF- and BF-spheroid-stimulated monocyte cytokine secretion, as well as between BF- and MF-spheroid-stimulated MCP-1 secretion. An inverse relation was also noted between the erythrocyte sedimentation rate at monocyte harvest and the monocyte MCP-1 F-spheroid responses.

Anaesthesia in endoscopic sinus surgery.

The aim of this study was to evaluate the methods of anaesthesia used in 1,460 patients (677 females and 783 males), aged 4 to 79 years, who were consecutively operated on with a functional endonasal endoscopic approach (FESS) or by an endoscopic endonasal approach (ESS) by one surgeon (AD) in the period from 1987-2001. The included patients presented a variety of disorders within the nose and sinuses. Many of the patients had sinonasal polypoid disease, which had gradually expanded over years and was causing pressure to neighbouring structures. Several had undergone previous multiple surgical procedures altering the original anatomy. However, the majority of the patients had been treated surgically for acute recurrent and/or chronic infections, others for sinogenic headache, benign tumours/mucoceles and smell disorders. The intention of looking into the anaesthetic procedures being used was to clarify the needs for a differentiated anaesthetic approach to different surgical tasks. It is of crucial importance for an optimal surgical result that both the patient and surgeon feel absolutely comfortable during the operation. Local anaesthesia can be used in minor surgery on selected patients and is definitely suitable even for residents in training. General anaesthesia is preferred in most cases by both patients and surgeons. The conclusion of this paper is that total intravenous anaesthesia (TIVA) in addition to oxygen-enriched air through a laryngeal mask airway (LMA) should be the anaesthetic procedure of choice, at least in our experience.

Risk factors and prognosis of anterior commissure versus posterior commissure T1-T2 glottic cancer.

For better understanding of the causation and behavior of T1-T2 commissural glottic laryngeal tumors, we retrospectively analyzed risk factors and outcome related to T class and type of therapy for tumors involving the anterior (184 cases) or the posterior (60 cases) commissure. The patients' smoking habits and alcohol consumption were similar, regardless of involved subsite. The disease-free interval was longer after surgery than after radiotherapy. The survival rates after recovery upon relapse were similar among subsites, T classes, and types of therapy. In anterior commissure tumors, the larynx remained preserved more frequently after partial laryngeal resection than after radiotherapy, and was more frequently preserved the lower the T class. In posterior commissure tumors, larynx preservation was less frequent and apparently independent of type of therapy or T class. In conclusion, smoking and alcohol consumption play similar pathogenetic roles in either subsite; partial laryngeal resection gave a higher rate of laryngeal preservation than did radiotherapy; and anatomic peculiarities of the subsites influence tumor behavior.

Voice results in patients with T1a glottic cancer treated by radiotherapy or endoscopic measures.

Early squamous cell carcinomas of the glottis can be treated effectively by means of surgery or external beam radiotherapy. The curability rate is about the same for both treatment modalities, but differing results have been reported regarding functional results. We selected 24 patients from a larger group of patients who had been treated for T1a glottic laryngeal cancer without the involvement of the anterior commissure. Fifteen patients were treated endoscopically and nine by radiotherapy. During a routine control videolaryngostroboscopy at an outpatient clinic, an objective and a subjective voice analysis were performed. The objective and subjective voice analyses showed no differences between the two treatment modalities. Videolaryngostroboscopy showed a significantly more pronounced glottic wave at the side that was originally affected by the tumour in the radiotherapy group. This difference disappeared when we looked at both vocal cords. Significant differences between the two treatment modalities were not found in any of the other parameters. Thus, this study shows no difference in the voice quality of patients treated by irradiation or by endoscopy. Therefore, the post-treatment voice quality is not a reason to favour radiotherapy for small T1a glottic squamous cell cancers without involvement of the anterior commissure.

Parenteral vaccination against influenza does not induce a local antigen-specific immune response in the nasal mucosa.

The immune response in the nasal mucosa to influenza vaccination in 23 patients scheduled for tonsillectomy was studied. A statistically significant increase in influenza virus-specific serum and oral fluid antibodies was observed 7 days after vaccination. The numbers of influenza virus-specific antibody-secreting cells (ASCs) in peripheral blood also increased significantly 1 week after vaccination. The numbers of ASCs in tonsils and nasal mucosa were compared with data from a recent study of nonvaccinated volunteers. The numbers of influenza virus-specific ASCs in tonsils were significantly higher in the vaccinated group, but, surprisingly, there was no significant difference between the groups in the numbers of ASCs in nasal mucosa. This suggests that the influenza virus-specific antibodies detected in oral fluid are not produced locally in the nasal mucosa and may originate from a systemic source, indicating that the vaccination may favor a systemic immune response.

An optimized technique for separation of human decidual leukocytes for cellular and molecular analyses.

PROBLEM: Human decidua contains a significant number of leukocytes, phenotypically and functionally different from peripheral blood. In vitro studies of decidual leukocytes require constant improvement of cell isolation technique in order to obtain highly purified viable and functionally active cells. METHOD OF STUDY: Optimized isolation procedure of decidual leukocytes, based on non-enzymatic cell dispersion, Percoll gradient centrifugation and purification step involving immunomagnetic beads is compared with a similar procedure with Lymphoprep gradient. RESULTS: The method based on Percoll gradient gave a viable leukocyte fraction with higher amount of alphabetaT- and gammadeltaT cells compared with Lymphoprep gradient. The leukocytes exhibited a well-preserved ultrastructure and surface marker expression and were suitable for molecular analysis. The magnitude and the kinetics of their proliferative response were comparable with peripheral blood lymphocytes. CONCLUSIONS: The results provide an optimized method for isolation and enrichment of decidual leukocytes, suitable for phenotypic, ultrastructural, molecular and functional analyses.
