Decreased lactoferrin levels in peritoneal fluid of women with minimal endometriosis.

OBJECTIVE: The aim of the study was to evaluate for the presence of lactoferrin (LTF) in peritoneal fluid (PF) of women with and without endometriosis. PATIENTS: Seventy-eight women were studied, including 49 women with endometriosis and, as a reference group, 29 patients with functional follicle ovarian cysts. RESULTS: Lactoferrin levels were detectable in all peritoneal fluid samples. Women with minimal endometriosis had lower PF lactoferrin concentrations compared to both patients with high revised American Fertility Society classification scores and women with follicle ovarian cysts. No significant difference in the peritoneal LTF levels was found between patients with stage II endometriosis, stage III or IV endometriotic disease and women with functional cysts of ovaries. CONCLUSIONS: Owing to its antibacterial properties lactoferrin is probably an important defense factor in the peritoneal cavity, however its role in the pathogenesis of endometriosis remains enigmatic.

The role of CpG islands hypomethylation and abnormal expression of neuronal protein synuclein-gamma (SNCG) in ovarian cancer.

BACKGROUND: The synucleins are a family of small cytoplasmic proteins that are expressed predominantly in neurons. Recently, their expression has been found also in many human cancers. AIM: To understand the molecular mechanisms underlying the abnormal expression of SNCG in malignant ovarian tumors, in this study, we examined the methylation status of a CpG island located in exon 1 of SNCG gene in a panel of ovarian malignant tumors to determine if DNA methylation is related to clinical and histological tumor characteristics. METHODS: SNCG mRNA expression in tumor samples was assessed by RT-PCR. Methylation status of SNCG gene was studied using methylation specific PCR (MSP). RESULTS: Study group included 43 ovarian carcinoma samples (40 primary and 3 metastatic). The expression of SNCG mRNA was detectable in 33 of 43 ovarian cancer cases (76.7%). There were no significant differences in the mRNA expression between serous or nonserous tumors. The presence of SNCG mRNA in tumor samples was not correlated with age and menopausal status of patients, also no correlation was found with clinical stage or histological grading of malignant tumors. In 29 of 43 (67.4%) cases of tumors unmethylated product of MSP amplification was present. In a group of SNCG mRNA-positive tumors there were 75.7% (25 of 33) cases with demethylated or hypomethylated exon 1 of SNCG. The differences between the groups were statistically significant (chi2 =4.46; p=0.034). Demethylation of SNCG was not related to clinical tumor stage (p>0.05), but it was strongly associated with tumor grading (chi2=6.66; p=0.035). Aberrant methylation of SNCG was more often seen in tumors of women after menopause (78.2% vs 55%). In postmenopausal women 18 of 33 (62.1%) tumor samples synuclein-gamma mRNA expression was found, however the differences were not statistically significant. No correlations between SNCG hypomethylation and patient age, clinical stage and tumor grading were found. In 9 of 43 samples (21%) both products of amplification with methylated or unmethylated primer sets were found. In all of these cases SNCG mRNA was present. CONCLUSIONS: SNCG mRNA is expressed in a substantial proportion of malignant ovarian tumors and demethylation is an important event in abnormal synuclein-gamma expression in most of these cases.

[The use of 2-dimensional and 3-dimensional color and power-Doppler in the diagnosis of blood flow indices of adnexal tumors].

OBJECTIVES: The purpose of this study was to compare 2-dimensional and 3-dimensional color and power-Doppler as measured with blood flow indices for the prediction of malignancy in complex adnexal masses. METHODS: Two hundred eighty women with complex adnexal masses (mean age, 46,3 years (range, 13-83 years) were evaluated by 2-dimensional and 3-dimensional power-Doppler imaging. Complex adnexal mass was defined as the presence of at least one of the following features: solid areas, thick papillary projections, thick septa, or purely solid echogenicity. Tumor volume, sonomorphology and Doppler indices (PI, RI), as well 3-dimensional power-Doppler assessment (VI, FI, VFI) were calculated. All tumors were surgically removed and definitive histological diagnosis was obtained in each case. Diagnostic accuracy and areas under ROC curve (AUROC's) were calculated for each test. RESULTS: Seventy one tumors (25.3%) were proved to be malignant, and 209 tumors (74.7%) were proved to be benign. Statistically significant differences between benign and malignant tumors were found for PI (1.06 vs 0.76; p = 0.001), RI (0.62 vs 0.51; p = 0.002), VI (0.55 vs 1.52; p = 0.002) oraz VFI (0.18 vs 0.49; p = 0.001). However, the highest sensitivity of 3D indices for malignant tumor detection was found for FI (42.9%). Specificity of 2D and 3D indices was between 93.6% for VI to 98.2% for RI. Accuracy and positive and negative predictive values were fund to be between 66.8% to 79.8%, respectively. AUROC's analysis indicated that the most useful test for the discrimination between benign and malignant tumors was VI followed by VFI and PI with RI. CONCLUSION: Three-dimensional power-Doppler imaging has diagnostic potential for the discrimination of benign and malignant complex adnexal masses, but specific, possibly most vascularised areas of each tumor have to be examined in detail to improve not yet satisfactory predictive values of this method.

[Ferritin levels in the peritoneal fluid--a new endometriosis marker?].

UNLABELLED: Accumulating data suggests that iron homeostasis in the peritoneal cavity may be disrupted by endometriosis. Increased iron metabolism induce proinflammatory and prooxidative environment in the peritoneal fluid (PF), thus may be involved in the pathogenesis of the endometriotic disease. Ferritin, a protein consisting of 24 subunits, may represent 25% of the total iron found in the organism. The aim of the study was to estimate the levels of ferritin in peritoneal fluid of women with endometriosis. MATERIALS AND METHODS: Forty women were studied, including 15 patients with minimal/ mild endometriosis, 15 patients with moderate/ severe stage of the disease and 10 women without the disease. Ferritin concentrations were measured in the PF using a commercially available ELISA kit. RESULTS: Ferritin levels were significantly higher in PF from both women with stages I/II (p=0.003) and III/IV (p=0.0007) endometriosis as compared to the reference group. No significant difference in the PF ferritin levels was found between women with stages I/II and stages III/IV endometriotic disease (p=0.98). CONCLUSIONS: Increased ferritin levels, observed in peritoneal fluid, may reflect disrupted iron metabolism in the peritoneal cavity of endometriosis women.

[YB-1 protein expression in ovarian cancer].

OBJECTIVES: Unfavourable prognosis of ovarian cancer is due to prompt progression, advanced stage at time of diagnosis and chemoresistance. No protein tissue prognosticators of ovarian cancer are in clinical use yet. YB-1 belongs to a family of "cold shock proteins" and participates at gene expression control at several levels. High expression of YB-1 in tumour tissue correlates with unfavourable prognosis and chemoresistance in some malignant neoplasms. THE AIM: of this study was to determine the expression of YB-1 in benign and malignant ovarian neoplasms and to correlate the expression of YB-1 with clinical indicators of cancer progression. METHODS: Specimens of 11 benign ovarian cysts and 14 cystadenocarcinomas of the ovary were obtained.YB-1 expression was determined by immunohistochemistry. Staging of ovarian cancer was performed according to FIGO. RESULTS: Mean YB-1 expression levels in benign and malignant tumours were 5.36 +/- 4.1 and 2.86 +/- 4.18 points respectively and were not significantly different (p=0.18). No correlation between FIGO stage and expression of YB-1 was found in the group of ovarian cancers, either (p=0.32). CONCLUSIONS: This study demonstrates that YB-1 is expressed both in benign and malignant ovarian tumors. Although there we didn't found any correlation between YB-1 expression and FIGO stage, YB-1 could be useful in the prognosis recurrence after chemotherapy.

Opioid-binding protein/cell adhesion molecule-like (OPCML) gene and promoter methylation status in women with ovarian cancer.

OBJECTIVE: The expression profile of the OPMCL gene was studied to find out if there was any evidence of a CpG island methylator phenotype and if there was an association of CpG island methylation with the gene downregulation in women with ovarian cancer. MATERIAL AND METHODS: Expression of OPCML in 43 ovarian cancer tumor samples and in 4 normal ovaries was determined by RT-PCR. Methylation status of OPCML promoter region was studied with methylation-specific PCR (MSP) method. Possible associations with selected clinicopathologic variables: FIGO stage, histological grade, patient's age and menopausal status were tested. RESULTS: In all normal ovarian samples OPCML mRNA was present, but it was not detectable in 24 of 43 ovarian cancer cases. We have not found the relationship between age and menopausal status with the presence of RTPCR product of OPCML. Hypermethylation of OPCML was not correlated to FIGO stage, however, in 80% of cases with methylated OPCML early clinical stage was also present. Tumor grading and histological type had no significant influence on the presence of hypermethylation of OPCML gene. In 20 of 43 cases of ovarian cancer methylated product of MSP amplification was present. In a group of OPCMLmRNA-negative tumors there were 75% of cases with hypermethylated exon of OPCML and the correlation between these variables was statistically significant (chi2 =17.7; p=0,00003). No promoter hypermethylation of the studied gene was found in normal ovaries. CONCLUSIONS: Reduced OPCMPL gene expression in ovarian cancer in comparison to normal ovaries could be related to the hypermethylation of promoter region. This epigenetic alteration may be the reason of gene silencing and the loss of suppressor function.

Leptin is not involved in the pathophysiology of endometriosis-related infertility.

OBJECTIVE: Changes in peritoneal fluid (PF) composition may affect fertilization as well as early embryonic development. Leptin, an adipocyte hormone, has been shown to act as a link between adipose tissue and the reproductive system. Therefore, we decided to assess peritoneal and serum leptin levels in infertile endometriotic patients. PATIENTS: Seventy-two women were studied, including 30 fertile and 18 infertile women with ovarian endometriotic cysts and, as a reference group, 24 patients with unexplained infertility. RESULTS: No significant difference in the peritoneal and leptin levels was found between the studied groups. Significantly higher PF leptin concentration was observed in patients with stages III and IV of endometriosis as compared to those with minimal stage of the disease. In fertile patients with endometriosis a positive correlation has been found between PF and serum leptin concentrations. CONCLUSIONS: No differences in peritoneal or serum leptin levels between infertile and fertile women with endometriosis suggest that this cytokine is not involved in pathophysiology of endometriosis-related infertility.

Serum acute phase protein concentrations after hysterectomy with and without low-molecular-weight heparin thrombosis prophylaxis.

Background. Low-molecular-weight heparin (LMWH) is recommended for venous thrombosis prophylaxis. There are several lines of evidence that heparins exhibit anti-inflammatory properties in addition to their anticoagulant activity. Therefore, we decided to estimate the concentrations of C-reactive protein (CRP) and haptoglobin, as acute-phase proteins, in the blood serum of patients who underwent total abdominal hysterectomy with and without the LMWH administration. Method. Forty-two women were studied, including 26 patients treated with enoxaparin and a group of 16 women without thrombosis prophylaxis. Haptoglobin and CRP concentrations were measured in serum samples obtained before the operation and also 8, 24, 48, 72, 96, and 192 hr after surgery. Results. Serum CRP level measured 8 hr after the operation was significantly higher in the group without thrombosis prophylaxis, however, no statistically significant differences in the concentration of CRP between studied groups in the other time periods were found. There were no statistically significant differences in serum haptoglobin levels between the groups of patients in all time periods. Conclusion. Administration of low-molecular-weight heparin may affect CRP production, however, further studies are still needed in order to clarify the exact immunosuppressive action of heparins.

[Three-dimensional sonoangiography in diagnostic of endometrial hyperplasia and carcinoma. An assessment of vascularization indices and endometrial volume].

Three-dimensional sono-angiography was executed in 123 women with uterine bleedings in perimenopausal period. With the use of software for automatic measurement of volume of blocks (VOCAL, Kretztechnik, Austria) vascular indices (VI, Fl, VFI) and endometrial volume (EV) were estimated. Based on histopathological estimation of endometrium the patients were divided into the following groups: atrophic, proliferative, secretive endometrium, hyperplasia and endometrial cancer (EC). Mean EV in women with EC amounted to 16.82 ml and was significantly higher (p < 0.005) than in women with hyperplasia or without pathological changes. Mean values of vascular indices VI, Fl and VFI in the group of women with EC amounted to respectively 5.464; 25.995 and 1.898 and were also significantly higher in comparison to remaining groups of patients. The results of the investigation suggest, that three-dimensional measurement of endometrial volume with assessment of vascular indices (VI, Fl, VFI) can be valuable prognostic element improving a diagnostic precision of sonographic estimation of pathological changes of endometrium.

Serum cortisol and acute phase protein concentrations after hysterectomy with and without peritoneal sutures.

OBJECTIVE: Although non-closure of the peritoneum still remains a controversial problem, to our knowledge there have been no studies comparing stress response in women who either have or have not had peritoneal sutures. Therefore, we decided to assess cortisol, C-reactive protein and haptoglobin levels in serum obtained following hysterectomy in both sets of patients. MATERIAL AND METHODS: Forty-nine women after total abdominal hysterectomy with bilateral salpingo-oophorectomy were studied, including 28 patients with non-closed peritoneum and 21 women after peritoneal sutures. Cortisol and acute phase protein levels were measured in serum samples obtained before the operation and also 8, 24, 48, 72, 96 and 192 h after surgery. RESULTS: There were no significant differences in the concentration of cortisol and C-reactive protein in blood serum between groups of patients in all time periods. Serum haptoglobin level was not statistically significant between the examined groups in 8, 24, 48, 72, 96 h, but after 192 h it increased significantly in a group of patients who had received peritoneal closure operations. CONCLUSIONS: Our results suggest that closure of parietal and visceral peritoneum does not provide any benefits during the postoperative period.

The effect of oscillating low intensity magnetic field on the Na+, K+, Ca++, and Mg++ concentrations in the maternal and fetal circulation of the dually perfused human placental cotyledon.

Dual-sided perfusions of the human placental cotyledon in vitro were used to study effects of low intensity magnetic fields (MFs) of 2 mT, 50 Hz (E1, 10 perfusions) and 5 mT, 50 Hz (E2, 10 perfusions). In the control group C (10 experiments) no field was used. Perfusions lasted 180 min each. Increased release of calcium ions from the placental cotyledon was found in the fetal circulation during perfusion when the 2 mT, 50 Hz MF was used. No changes in the release of sodium and magnesium ions were observed compared to the control group. The 5 mT, 50 Hz oscillating MF intensified the release of sodium ions from the perfused cotyledon both to the fetal and maternal circulation up to the 150th min of the experiment. Increased release of magnesium ions was observed only to the fetal circulation between 120 and 180 min and of calcium ions to the fetal circulation between 60 and 180 min. No significant differences in K concentrations were found between the control and MF exposed cotyledons under conditions of these experiments.

[Oxygen transfer and consumption in human placenta exposed to variable magnetic fields in vitro].

The initial investigations concerning the influence of variable magnetic fields (MF) on transfer and oxygen consumption in isolated human cotyledon in vitro were performed. Ten dual closed perfusion of the human cotyledon were conducted in each group. The control group was not exposed to magnetic fields. In studied groups cotyledons were exposed to magnetic field: in the group B1 (B = 2 mT, f = 50 Hz), and in the group B2 (B = 5 mT, f = 50 Hz) for 180 min. Obtained results may suggest that variable magnetic field (B = 5 mT, f = 50 Hz) cause decrease of oxygen consumption in human placenta in 120 and 150 min of experiment.

[Decrease of paraoxonase activity in the first day after abdominal operations in women].

INTRODUCTION: Paraoxonase (PON1) is one of enzymes participating in antioxidative defence mechanism, which protects the organism from the results of action of reactive oxygen species. PON1 is an esterase contained in plasma high density lipoproteins (HDL) which protects HDL from peroxidation and plasma membranes from free radical injury. PON1 serum activity is diminished in some diseases (atherosclerosis, diabetes, coronary artery disease) and after cardiosurgery operations. But there is no information about changes in PON1 activity after abdominal operations. THE AIM: Of this study was the assessment of PON1 serum activity after the abdominal operations in women. MATERIALS AND METHODS: 40 women operated in the 1st Department of Gynaecology of Lublin Medical University were studied, including 34 patients after total abdominal hysterectomy with salpingo-oophorectomy and 6 patients after myomectomy or cyst enucleation. The blood to experiments was collected before operation, and 8 and 24 hours after. PON1 serum activity was assessed spectrophotometrically toward two synthetic substrates: paraoxon and phenyl acetate. RESULTS: PON1 activity toward paraoxon before operation was 146.8 +/- 17.3 U/ml, and after 8 and 24 hours after 107.6 +/- 11.9 U/ml and 119.9 +/- 16.9 U/ml, respectively (results statistically significant). PON1 activity toward phenyl acetate (arylesterase activity) before operation was 182.7 +/- 16.4 U/ml and did not significantly changed after the surgery. CONCLUSION: It is observed decreased PON1 serum activity in the early postoperative period after abdominal operations in women, what can be connected with the utilisation of PON1 in antioxidative defence mechanism.

[Pre-pubic TVT in the treatment of urinary incontinence in women. Description of surgical technique and initial results].

Tension-free vaginal tape (TVT) has within the last decade become one of the most popular procedure for treatment of female urinary stress incontinence. It is still persistent the risk of complications peri- and postoperative in spite of high efficacy of this method. Investigators still have been looking for new variations of this method to diminish the risk of complications and to maintain the efficacy of the treatment. The aim of this study is the description of the pre pubic TVT method and the preliminary short time (3 month) results of treatment. The results of pre-pubic TVT treatment were compared with "classic" TVT method. The pre-pubic TVT technique was performed in 25 women, operated for stress incontinence. The average operation time was 19 min (10-30 min). During the operation in one of patients there was excessive bleeding from venous plexes of vagina during preparation of tissues and in 4 of them it was perforation of vaginal mucosa during introducing of needle with tape. All these complications were recognised and corrected during the surgery. The mean hospitalisation time was two days. Our 3-months observations allow to confirm that efficacy of pre-pubic TVT is similar to the "classic" method, but the frequency of complications is lower in the pre-pubic TVT. There were no postoperative complications and voiding dysfunctions.

Investigation of glutathione concentrations in peritoneal fluid from women with and without endometriosis.

UNLABELLED: Changes in the peritoneal fluid (PF) environment have been implicated in the pathogenesis of endometriosis as well as in the decrease of fertility. OBJECTIVE: To evaluate the concentration of glutathione in PF of women with endometriosis. PATIENTS: Twenty-one patients with endometriosis (I or II rAFS stage, n=11; III or IV rAFS stage, n=10), and 29 patients with follicular or dermoid ovarian cysts (n=17 and n=12, respectively). RESULTS: Mean (+/-S.D.) PF glutathione concentration was 0.22+/-0.01 micromol/ml in patients with minimal or mild endometriosis, 0.21+/-0.05 micromol/ml in women with III or IV stage of the disease, 0.24 +/- 0.03 micromol/ml in women with follicle ovarian cysts, and 0.23+/-0.05 micromol/ml in patients with dermoid tumors of ovaries. No significant difference in the peritoneal glutathione level was found between the groups. CONCLUSION: These results suggest that PF glutathione is not involved in the progression of endometriosis.

Evaluation of myeloid and lymphoid dendritic cells in peritoneal fluid in women with non-malignant ovarian tumors.

PROBLEM: Identification of myeloid and lymphoid dendritic cells (DCs) in peritoneal fluid (PF) and peripheral blood (PB) of patients with ovarian pathology. METHOD OF STUDY: PF and PB were collected from 60 patients who underwent laparoscopy because of non-malignant ovarian tumors. Mononuclear cells were separated by gradient centrifugation. The cell surface antigens were determined by flow cytometry using monoclonal antibodies. RESULTS: Both myeloid and lymphoid DCs were detected in PF and PB of women with ovarian tumors. The percentage of myeloid DCs was significantly higher in PF than in PB. The concentration of PF myeloid DCs was the highest (P < 0.05) in patients with dermoid cysts (0.67 x 10(6)/mL PF) in comparison with the other studied groups, excluding patients with normal pelvis. CONCLUSIONS: Domination of myeloid and not lymphoid cells in PF may support the hypothesis that local PF immune disturbances may play a role in some non-malignant ovarian pathology.
