Churg-Strauss syndrome and congenital factor Leiden thrombophilia as nontypical causes of intracardiac thrombosis.

We present and discuss the case of a 26-year-old man suffering from Churg-Strauss syndrome (CSS). In our patient the disease severely affected both the heart and the lungs. Left ventricular dysfunction and intracardiac thrombosis, probably secondary to the cardiotoxic effect of eosinophils, were revealed by echocardiography. The risk of thrombosis was additionally increased by the coexistence of the factor V Leiden gene mutation. An opportunely established diagnosis allowed institution of an appropriate pharmacotherapy and subsequently to obtain a significant abatement of the clinical symptoms as well as the laboratory manifestations of the disease.

[Obesity influence on renal function].

Obesity is an increasing problem in all over the developed world. The paper describes the substances secreted by adipose tissue and the altered renal hemodynamics in obese subjects. The review pays attention to diagnostic difficulties and discusses the medical trials of this pathology.

[Hyperammonemia type II as an example of urea cycle disorder].

Ornithine transcarbamylase deficiency is the most common inherited urea cycle disorder. Its clinical manifestations as lethargy, vomites, coma and cerebral edema are the effect of the higher concentration of the ammonia in plasma. Hyperammonemia, caused by mutation in ornithine transcarbamylase gene, is often considered as a reason of coma by pediatricians but skipped by internist, although it is the third reason of hepatic coma in adults. This article is the recapitulation of published studies and their implication on everyday clinical practice.

[The influence of anesthesia methods on renal function in patients without symptoms of nephropathy].

The aim of this study was to assess the influence of some anaesthesia methods on renal function in patients with no clinical and laboratory symptoms of nephropathy. The activity of various parts of the nephron was evaluated on the basis of the urinary excretion of albumins (marker of renal glomeruli function), beta-2-microglobulins (marker of proximal tubule function) and Tamm-Horsfall protein (marker of distal tubule function). 45 patients were divided into three groups, basing on the kind of anaesthesia: "O"--general, "P"--spinal and "N"--local. The patients who were routinely found to have no signs of nephropathy showed no dysfunction of renal glomeruli and distal or proximal tubule as determined by the urinary excretion of albumins, Tamm-Horsfall protein and beta-2-microglobulins prior to the operation. Different anaesthesia techniques had no effect on the urinary excretion of beta-2-microglobulins. In contrast to local and spinal anaesthesia, general anaesthesia significantly increased the urinary excretion of albumins. General, spinal as well as local anaesthesia significantly decreased the urinary excretion of Tamm-Horsfall protein. The decrease in the excretion of this glycoprotein suggests that all types of anaesthesia techniques may influence the function of the distal part of nephron.
