[Therapeutic effect of low-dose omeprazole vs. standard-dose ranitidine in mild to moderate reflux esophagitis].

BACKGROUND/AIMS: Proton pump inhibitors (PPI) and H2-receptor antagonists (H2RA) are commonly prescribed for the treatment of mild to moderate reflux esophagitis (MMRE). There remains great controversy in their usefulness as the first choice and the appropriateness. We prospectively compared the efficacy and safety of the 8-week low-dose PPI vs. standard-dose H2RA in MMRE. METHODS: One hundred patients with MMRE were randomized to receive either low-dose of omeprazole (L-OMP: 10 mg, q.d.) or standard-dose of ranitidine (S-H2RA: 150 mg, b.i.d.) for 8 weeks. The H. pylori status using rapid urease test, histological examination and culture, reflux esophagitis (RE) grading, gastrointestinal symptoms using 4-point scale, adverse event and the standard laboratory examination were assessed at baseline and 8-week end point of therapy. RESULTS: Improvement rate of RE [intention to treat (n=82)/per protocol (n=72)] were shown in 69.1%/63.9% for L-OMP and 65.0%/63.9% for S-H2RA group (p=0.697, p=1.000). Complete healing rates of RE were 54.7%/50.0% for L-OMP and 42.5%/41.7% for S-H2RA. No significant difference in healing rate, the rapidity of symptom resolution, adverse events, and laboratory monitoring was found between the two groups. CONCLUSIONS: The low-dose omeprazole therapy produced similar healing rates and safety in the treatment of MMRE. In addition, L-OMP is advantageous in its once-a-day dosing and might be an alternative to S-H2RA, especially in Korean patients with MMRE.

Transient ischemic attacks presenting as hemiballism.

Hemiballism is continuous, nonpatterned involuntary movement characterized by irregular, coarse, flinging movement involving the limbs on one side. Hemiballism is most commonly caused by stroke. However, very rarely a transient ischemic attack (TIA) presents as hemiballism. We describe 2 such patients with hemiballism presenting as TIA.

Serial measurement of surface expressions of CD63, P-selectin and CD40 ligand on platelets in atherosclerotic ischemic stroke. A possible role of CD40 ligand on platelets in atherosclerotic ischemic stroke.

BACKGROUND: Platelet activation is a key step in the progression of atherosclerosis. The CD40 ligand (CD40L) on platelets may be a critical factor to develop the acute vascular events from atheroma. METHODS: To determine the role of CD40L on platelets in atherosclerotic ischemic stroke, we serially measured the expressions of CD63, P-selectin and CD40L on platelets in patients with atherosclerotic ischemic stroke (n = 25) and compared them with those in patients with asymptomatic carotid stenosis (n = 20) and in normal subjects (n = 24). RESULTS: The expressions of CD63 and P-selectin on platelets were significantly higher in patients with atherosclerotic ischemic stroke (n = 25) than in normal subjects (n = 24). The extents of surface expressions of CD63 and P-selectin on platelets showed no significant differences between atherosclerotic ischemic stroke and asymptomatic carotid stenosis. However, the CD40L expression on platelets was significantly higher in atherosclerotic ischemic stroke when compared to that in asymptomatic carotid stenosis. CONCLUSIONS: In our data, among the population with large artery atherosclerosis, the patients with symptomatic ischemic events showed a significantly elevated expression of CD40L on platelets compared to those without ischemic events. Therefore, the upregulation of CD40L on platelets may be a specific marker of platelet activation to provoke ischemic stroke from large artery atherosclerosis.

Effect of nitric oxide on the proliferation of cultured porcine trabecular meshwork cells.

To investigate the effect of nitric oxide (NO) on the proliferation of trabecular meshwork (TM) cells, primarily cultured porcine TM cells were exposed to NO donor (SNAP, S-nitroso-N-acetyl-D,L-penicillamine) with and without its inhibitor (L-NAME, Nomega-Nitro-L-arginine methyl ester). The proliferation of TM cells was quantified by a rapid colorimetric assay. Acridine orange/Hoechest 33342 staining and flow cytometry with annexin-PI were done. As a result, NO inhibited the proliferation of TM cells significantly in a dose-dependent manner and this inhibitory effect was abolished by L-NAME. Fluorescent microscopy and flow cytometric analysis revealed that NO induced apoptotic cell death. The current results suggest that NO inhibit the proliferation of TM cells and apoptosis may be involved in some degree.

Nitric oxide mediates epinephrine-induced apoptotic cell death of trabecular meshwork cells in vitro.

To evaluate the effect of epinephrine on the cellular proliferation and production of nitric oxide (NO) in trabecular meshwork (TM) cells, cultured porcine TM cells were exposed to epinephrine at various concentrations, with and without its NO inhibitor, N(omega)-Nitro-L-arginine methyl ester (L-NAME). The proliferation of TM cells and the production of NO were quantified by rapid colorimetric assays. Acridine orange/Hoechest 33342 staining and flow cytometry were done to evaluate apoptosis. Epinephrine inhibited the proliferation of cultured TM cells and produced NO at significant levels in a dose-dependent manner. However, the antiproliferative effect of epinephrine was abolished by L-NAME. Fluorescent microscopy and flow cytometric analysis revealed that epinephrine induced apoptotic cell death, which was suppressed by L-NAME. The current results suggest that the inhibitory effect of epinephrine on TM cell survival is associated with NO production. NO-mediated apoptosis may be involved in this epinephrine-induced antiproliferative effect on TM cells.

Assessment of genetic diversity of Korean native pig (Sus scrofa) using AFLP markers.

In order to assess the genetic diversity and genetic relationships among the six commercial pig breeds including the Korean native pig, we performed an amplified fragment length polymorphism (AFLP) analysis. Applying the three EcoRI/TagI primer combinations to 54 individual pig samples out of six breeds, a total of 186 AFLP bands were generated, 67 (36%) of which were identified as polymorphic bands. From these polymorphic bands, the three estimates (percentage of polymorphic loci, Neis heterozygosity and Shannon index) of genetic diversity, G(ST) estimates, Neis unbiased genetic distance and two indices of genetic similarity were calculated. From all the calculations of genetic diversity, the lowest genetic diversity was exhibited in the Korean native pig, and the highest in the Chinese Yanbian pig. Given the mean G(ST) value (G(ST) = 0.390) across all pigs examined, levels of apparent breed subdivision were considerable. A UPGMA tree of individuals based on Jaccards similarity index showed that the Korean native pig formed a distinct cluster from the other five pigs. In addition, the tree displayed that all the individuals except for six individuals were grouped into their breeds. Principal component analysis based on the binary data matrix of either presence or absence confirmed the distinctness of the Korean native pig from the other pigs. Our results indicate that the Korean native pig has a low level of genetic diversity and is distinct from the five pig breeds, confirming the results from previous microsatellite data. The findings also suggest that AFLP analysis may be a valuable tool for revealing genetic relationships and genetic diversity among different pig breeds.

Surface expression of P-selectin on platelets is related with clinical worsening in acute ischemic stroke.

Platelet activation has a critical role in arterial disorders. In this study, we showed that the upregulation of P-selectin expression on platelets was related with clinical worsening in acute ischemic stroke. We serially (within 24 hr, at 72 hr, and 7 days) measured the expression of P-selectin on platelets in patients with acute ischemic stroke (n=45) and investigated the correlation between their extents and clinical severity of ischemic stroke. A significant relationship between the P-selectin expressions and National Institute of Health Stroke Scale (NIHSS) was observed at 72 hr and 7 days after ischemic stroke onset. Patients with clinical deterioration showed significantly increased expression of P-selectin on platelets as compared to those without deterioration. These results suggest that the P-selectin expression on platelets may contribute to the aggravation of clinical course in acute ischemic stroke. Thus, adequate manipulation of activated platelets is an important therapeutic strategy in acute ischemic stroke.
