Stalking.

Stalking is a frequently reported crime. It has a strong impact on the lives and mental health of the victims. It is perpetrated by persons who often suffer from mental disorders. This paper is an overview on the subject. It encompasses the various definitions of stalking, a number of classifications or sub-divisions of the phenomenon according to different approaches, a description of the psychopathology of the stalker, the psychological effect of the stalking on the victim, ways of evaluating and managing the phenomenon both in the victims and in the perpetrators, prevention of stalking, and also relevant bibliography. In addition, the paper includes an Appendix containing case series (clinical vignettes which include diagnosis, medical and legal interventions and outcome).

Vladimir Bekhterev: his life, his work and the mystery of his death.

Bekhterev was one of the prominent scientists of his era. He played a crucial role in the study of the development of the brain's organization, neurophysiology and neuropathology and in the objective study of psychological phenomena. Due to a trick of fate, Bekhterev's name and scientific contributions were neglected and consigned to oblivion. In this article we would like to restore the historical injustice done to Bekhterev. Based on newly discovered documents and testimonies, we present little known details about his life history, his clinical work and the tragic circumstances of his death.

Patient rights and law: tobacco smoking in psychiatric wards and the Israeli Prevention of Smoking Act.

In August 2001, the Israeli Ministry of Health issued its Limitation of Smoking in Public Places Order, categorically forbidding smoking in hospitals. This forced the mental health system to cope with the issue of smoking inside psychiatric hospitals. The main problem was smoking by compulsorily hospitalized psychiatric patients in closed wards. An attempt by a psychiatric hospital to implement the tobacco smoking restraint instruction by banning the sale of cigarettes inside the hospital led to the development of a black market and cases of patient exploitation in return for cigarettes. This article surveys the literature dealing with smoking among psychiatric patients, the role of smoking in patients and the moral dilemmas of taking steps to prevent smoking in psychiatric hospitals. It addresses the need for public discussion on professional caregivers' dilemmas between their commitment to uphold the law and their duty to act as advocates for their patients' rights and welfare.

[Commercial sex engagement: medical and social aspects].

Commercial sex engagement represents a common phenomenon that has not been studied as of yet. Relatively few surveys have been published worldwide. The image of the woman that takes part in commercial sex, as shown by these surveys, is similar to a female victim who was sexually abused during her childhood and who was exposed to maltreatment. It is also shown that these women frequently suffer from violence and that a high percentage of these females can be diagnosed as post-traumatics. In the literature, there is mention of their involvement in the spreading of veneral diseases. The state of affairs in Israel can be seen in the Parliamentary Investigation Committee for the Trading of Women and also in the article by BenZion et al. published in this issue of Harefuah. This paper shows the preliminary results of a study conducted among 55 women who are engaged in organized prostitution and reside illegally in Israel. Our editorial attempts to analyze the issue of commercial sex and the results of this research, and stresses certain important social and medical aspects of this subject.

[The establishment of "Ezrath Nashim" Psychiatric Hospital in Jerusalem: selected issues].

This article describes the process of the establishment and early years of "Ezrath Nashim" Psychiatric Hospital in Jerusalem. Economic aspects in the continuous and daily life of the institution are emphasized, and existential difficulties that were involved in its regular management are mentioned. Selected issues such as economic difficulties and maintenance of the hospital are discussed based on historical documents. The resemblance between the early history and the current state of affairs in the mental health field in Israel is presented and stressed.

[Pathological gambling in Israel as a medical and social problem].

This article discusses the phenomenon of pathological gambling in Israel today, emphasizing its medical as well as social dimensions. After a brief historical review of pathological gambling in world and Jewish history, certain psychological aspects of the disorder are reviewed. The typology of gamblers is mentioned, and the relationship between gambling and affective disorders is briefly discussed. Some of the forensic implications of pathological gambling are also mentioned.

The concept of severe mental disorder and the amended law of reduced punishment for murder.

According to the 1995 amendment 300a of the Israeli Penal Law 1977, punishment for murder may be reduced in cases where "a severe mental disorder" is present. As a consequence, psychiatrists are called upon to define the mental disorder and to assess its impact on the actions of the murderer. This constitutes a major challenge both for the courts and for forensic psychiatrists who are still in the process of learning the ramifications of this medico-legal issue. We suggest that, from the psychiatrist's point of view, the law be used only for people suffering from mental disorders which are very closely associated with psychotic mental illness.
