[Aortic root replacement of 231 cases].

OBJECTIVE: To discuss the operative indications, methods, and surgical techniques of aortic root replacement for aortic not aneurysm. METHODS: Between January 1994 and August 1999, 231 consecutive patients underwent aortic root replacement in our institution, and 13/231 were treated on an emergency basis. They were 189 men and 42 women, ranging in age from 14 to 69 years. The diameter of the aneurysm varied from 4.5 to 11.0 cm. Among this group, 145 had isolated aortic root aneurysms, 65 suffered from DeBakey type I aortic dissection, and the remaining 21 were diagnosed as DeBakey type II aortic dissection. Aortic valve regurgitation was involved in all patients. Aortic root replacement was performed with composite valved graft in 229 patients, and 2 patients reserved the aortic valve. RESULTS: Hospital mortality was 3.03% (7 patients). The early complications included re-exploration for bleeding in 6 patients, pericardial effusion in 9, and cerebral infarction, pleural effusion and pneumothorax each in 2 patients. One hundred and seventy-five patients (78.12%) were followed up, with a mean follow-up period of 15.7 +/- 13.1 months (range, 2 weeks to 65 months). One patient died from lower-limb embolism and renal dysfunction at 3 months postoperatively. Three patients died from postoperative anticoagulant accidents. The preoperative and postoperative mean left ventricular end-diastolic diameters were significantly different (68.1 mm +/- 9.4 mm, range: 54 to 112; vs 54.8 mm +/- 8.2 mm, range: 38 to 88; P < 0.001). CONCLUSIONS: Aortic aneurysm without dissection or with chronic dissection should be operated if the diameter of the aneurysm is greater than 5 cm. Once a diagnosis of acute aortic root dissecting aneurysm is made, the patient should undergo surgery as soon as possible if the general condition permit.

Overexpression of Bcl-2 partly inhibits apoptosis of human cervical cancer SiHa cells induced by arsenic trioxide.

OBJECTIVE: To study the biological effect of arsenic trioxide (As2O3) on human cervical cancer SiHa cells and SiHa cells overexpressing bcl-2 gene. METHODS: SiHa cells with overexpression of Bcl-2 (SiHa-Bcl2 cells) were established by transfecting SiHa cells with Bcl-2 expression vector. The sensitivities of SiHa and SiHa-Bcl2 cells to As2O3 were determined using MTT (Thiazolyl blue) reduction and colony forming ability assay, morphological analysis, flow cytometric analysis, DNA agarose gel electrophoresis, in situ cell death detection (TUNEL), Northern blot, RT-PCR and Western blot. RESULTS: As2O3 inhibited the growth of SiHa cells and induced G2/M arrest and apoptosis of the cells. RT-PCR and Western blot analysis revealed that As2O3 induced SiHa cell apoptosis possibly via inhibiting the expression of HPV16 E7 and decreasing the expression of c-myc. However, we found that SiHa-Bcl2 cells partly resisted As2O3 induced apoptosis, which might be related to the prevention of the down-regulation of HPV16 E7 and c-myc gene expression. Nevertheless, As2O3 at a high concentration could still induce apoptosis of SiHa-Bcl2 cells mainly via decreasing Bcl-2 expression and slightly inhibiting viral gene expression. CONCLUSION: As2O3 is an inducer of the apoptosis of human cervical carcinoma cells and the cells overexpressing Bcl-2 can partly resist As2O3 induced apoptosis, but the exact mechanism is unclear.

Molecular cloning and protein expression of EC1-2 and EC3-4 epitopes of pemphigus vulgaris antigen.

OBJECTIVE: To clone and express EC1-2 and EC3-4 epitopes of PVA (pemphigus vulgaris antigen, desmoglein-3) in order to diagnose pemphigus and study the relationship between epitopes of PVA and anti-PVA antibody. METHODS: RNA was extracted from keratinocytes and the cDNA of epitopes EC1-2 and EC3-4 was synthesized by reverse transcription. Amplified genes of EC1-2 and EC3-4 were inserted into the expression plasmid, PGEX-4T-1, and transformed into E. coli BL21 by electric transduction. Recombinant fusion proteins of EC1-2 and EC3-4 epitopes were expressed by IPTG induction. These proteins were separated on SDS-PAGE gels and electroblotted to nitrocellulose to detect the anti-PVA antibody. RESULTS: The sequences of cloned EC1-2 and EC3-4 genes were identical to the sequence registered in PC/GENE. Expressed recombinant proteins reacted only to sera from patients with pemphigus vulgaris, not to sera from patients with bullous pemphigoid, systemic lupus erythematosus or normal persons. CONCLUSIONS: These recombinant proteins are very specific in antigenicity. This may provide a new method for the diagnosis of pemphigus vulgaris (PV) or the differential diagnosis of other bullous cutaneous diseases via patient sera. It is also helpful in understanding the relationship between adhesion molecules and the pathogenic mechanism of pemphigus vulgaris.

[Effect of tamoxifen on apoptosis and drug resistance of breast cancer cells in vitro].

OBJECTIVE: To study the effect of estradiol (E2) and tamoxifen (TAM) on the apoptosis regulatory genes (bax, bcl-2) and mdr-1 in MCF-7 cells. METHODS: Immunohistochemistry, in situ hybridization were used to observe the changes in bax, bcl-2 and mdr-1 genes after treatment with E2 and TAM. RESULTS: E2 had ability to increase the expression of bcl-2 and mdr-1 but TAM decrease the expression of both. Synthetic bax oligodeoxynucleotide (ODN) was used as probe to perform in situ hybridization. It was shown that bax mRNA in MCF-7 cells was down-regulated by E2 and up-regulated by TAM. Significant correlation between bcl-2 and mdr-1 was observed after TAM treatment of MCF-7 cells (P < 0.05). CONCLUSION: TAM regulates cell apoptosis and inhibits development of drug-resistance of MCF-7 breast cancer.

[Primary repair of cartilage defect accompanying patellar fracture with free auto-periosteal graft].

OBJECTIVE: To evaluate the results of free auto-periosteal graft in primary repair of cartilage defect accompanying severe comminuted fractured of patella. METHODS: From January 1992 to August 1998, seventeen cases with extensive cartilage defect due to severe comminuted fracture of patella were primarily repaired with free auto-periosteal graft. In these cases, there were whole patellar fracture in 9 patients, upper two third patellar fracture in 3 patients and lower two third patellar fracture in 5 patients. During operation, "S"-shaped incision along medial side of knee through intra-cavity pathway were used. After fixation of the patellar fracture and clearance of the residual cartilage in the fracture area, the cancellous bone was exposed and trimmed. The free periosteum was incised from the anterior medial side of upper tibia and then transplanted to the region of cartilage defect. The size of grafted periosteum ranged from 3 cm x 4 cm 5 cm 6 x cm. The knee joint was received passive motion at 7 days after operation. RESULTS: All cases were followed up 8 to 74 months. There were excellent recovery in 12 patients and the function of knee joint was normal, better recovery in 4 patients and the function of knee joint was nearly normal, and moderate recovery in 1 patient and the function of knee joint was limited mildly. CONCLUSION: Free auto-periosteal graft is a simple and effective treatment in primary repair of cartilage defect accompanying patellar fracture. It is valuable to apply in clinical practice.

[Predictive factors for successful sperm recovery in azoospermia patients].

OBJECTIVE: To find any predictive factors for successful testicular sperm recovery in azoospermic patients. METHODS: 50 cases of azoospermia were subjected to testicular biopsy. Serum follicle stimulating hormone (FSH), concentration, testicular volume, testicular histopathology, and testicular sperm extraction (TESE) were determined in each cases. RESULTS: Neither serum FSH values nor testicular volume were predictive of spermatozoa in TESE. Testicular histopathology was specific for predicting successful sperm recovery (sensitivity 96%; specificity 100%; accuracy 100%). CONCLUSIONS: Azoospermia patients with high FSH and small testis should be recommended for testicular biopsy and TESE to confirm if there is any testicular spermatozoa.

[MR urography: technique and clinical application in patients with kidney transplantation].

OBJECTIVE: To assess the value of magnetic resonance urography (MRU) for the noninvasive postoperative evaluation of renal transplants. METHODS: 28 cases of renal transplantation were examined with a modified, heavily T2-weighted fast apin-echo pulse sequence and fat-suppression pulse aid. Post-procedure processing was performed with a maximal-projection (MIP) algorithm. RESULTS: 28 patients were studied. MRU revealed abnormal finding in 9 patients: mild dilatation of the renal pelvis without any urodynamic relevant obstruction (2), moderate dilatation of the renal pelvis (3), acute rejection (3), and ATN (1). Ureteral stenosis was diagnosed in 1 patients and treated surgically. CONCLUSIONS: MRU is an alternative to conventional urinary tract images techniques. It requires no radiation or iodinated contrast material. It is reliable and noninvasive in depicting the urinary tract. MRU can be used as a noninvasive technique for assessment of renal transplantation in cases suspicious of complication in the excretory system.

[Effects of p16 and GM-CSF combined gene therapy on immune response of tumor-bearing mice].

OBJECTIVE: To investigate the effects of p16 and GM-CSF combined gene therapy on immune response of tumor-bearing mice (Renca). METHODS: The model of renal tumor-bearing mice was constructed by subcutaneous injection of Renca cells. The tumor-bearing mice were divided into 5 groups at random three days after injection of Renca cells. Different groups of mice were given different treatments. The antitumor immune responses and the survival time of mice were tested. Histopathologic analysis of tumor tissue was done. RESULTS: Enhancement of antitumor immune responses and prolongation of survival period were observed more obviously in tumor-bearing mice after combined therapy than in those of the other four groups. CONCLUSIONS: p16 and GM-CSF combined gene therapy can enhance the antitumor immune responses of mice and inhibit the growth of pre-established renal cell carcinoma.

[Inhibition of growth and metastasis of human giant cell carcinoma of the lung by transfection of antisense VEGF121 cDNA].

OBJECTIVE: To explore the effects of blocking the VEGF/VEGF receptor paracrine pathway on growth and metastasis of human lung carcinoma cell line PG and to evaluate its potential application in gene therapy of cancer. METHODS: The eukaryotic expression vectors bearing either sense-VEGF121 cDNA or antisense-VEGF121 cDNA was constructed and transfected into PG cells. In vitro and in vivo tests such as Northern blotting hybridization, Western blotting immunochemistry analysis, as well as xenografting in nude mice were used to analyze the effect of antisense-VEGF. RESULTS: The transfectants stably expressing antisense VEGF121 were observed to produce markedly reduced 3.3 kb VEGF mRNA and 45 KD, 41 KD, 32KD VEGF proteins. When xenografted s.c. into nude mice, growth and metastasis of the antisense-VEGF transfected cell lines were greatly inhibited when compared with control cells. CONCLUSION: Antisense VEGF gene significantly inhibited tumor growth and metastasis and may provide an experimental example for the development of antiangiogenic gene therapy.

[The prognostic significance of expression of c-met oncogene and its relation to gastric mucosal lesions].

OBJECTIVE: To determine whether there is a relationship between the expression of c-met oncogene and the stage of gastric mucosal lesions or the prognosis of gastric carcinomas. METHODS: Expression of c-met was investigated in 169 gastric mucosal lesions by using immunohistochemistry. Survival analysis was studied with the Kaplan-Meier test. RESULTS: For chronic superficial gastritis, chronic atrophic gastritis, intestinal metaplasia, dysplasia, early gastric carcinomas and advanced gastric carcinomas, the expression rates were 23.5%, 36.8%, 51.5%, 61.3%, 66.7% and 73.7% respectively. The positive rate was higher in intestinal metaplasia, dysplasia and gastric carcinomas than in chronic superficial gastritis (P < 0.05). Expression of c-met increased with the increasing proliferation of gastric mucosa in rank correlation test, which showed a close relationship (P < 0.01). Expression of c-met correlated significantly with histologic type, serosal invasion and lymph node metastases. In particular, the expression of c-met was significantly higher in Borrmann type IV gastric carcinomas (87.5%) than in early gastric carcinomas (66.7%) or Borrmann type I, II (68.7%, P < 0.01). The survival rate of patients with expression of c-met was significantly lower than that of patients without c-met expression. CONCLUSION: The expression of c-met is associated with proliferation and malignant transformation of gastric mucosa. These results suggest that expression of c-met might be a new prognostic factor in gastric carcinoma.

[Effects of cytokine gene therapy on prolonging survival time of allografted skin after scalding in a murine model].

OBJECTIVE: To explore the role of cytokine gene therapy on prolonging survival time of allografted skin after scalding in a murine model. METHODS: Interleukin 10 (IL-10) gene was employed as therapeutic objective gene and fibroblast was as a carrier cell. Gene transcription technique was adopted to establish an experimental murine model in which fibroblast-mediated gene therapy was used to prolong allografted skin survival time after scalding. IL-10 was transferred into fibroblastocyte (NIH3T3) by reverse transcriptive virus vector. The mice were grafted with alloskin after scalding. In addition, collagen capsulized NIH3T-3-IL-10 cells were implanted intraperitoneally in the mice so as to observe its influence on allografted skin survival time and on the changes of their main internal organs. RESULTS: Cytokine gene therapy could obviously prolong the survival time of allografted skin (P < 0.01) without any evident detrimental effect on the internal organs. CONCLUSION: These results demonstrated that skin allograft rejection could be inhibited and the survival time be prolonged with the implantation of the fibroblastocyte transferred IL-10 gene.

[The postburn change in splenic T lymphocyte transmembrane signal transduction and its relationship with the secretion of IL-2 and IL-10 in severely scalded mice].

OBJECTIVE: To explore the postburn functional change in splenic T lymphocytes and the secretory rules of IL-2 and IL-10 in severely scalded mice, and to look for the explanation of the change by means of T lymphocyte transmembrane signal transdunction study. METHODS: The changes in the antigen receptors on T cells (TCRalpha/beta), assisting stimulating molecule (CD28) and the activities of GTPase, protein tyrosine kinase (PTK) and protein kinase C (PKC) that participated in the transmembrane signal transduction were detected, so as to observe the proliferation and transformation function of T cells at different post-burn periods and the secretion of IL-2 and IL-10. Furthermore, the changes in every signal transduction molecules were analyzed with the consideration of their relationship with the change in T cell function activities. RESULTS: The postburn positive rates of the expressions of TCRalpha/beta and CD28 on T cell membrane decreased. The postburn GTPase activity and membrane PTK activity were all suppressed but recovered at 168 postburn hours. Membrane PKC activity exhibited a dual-phase change (increase following decrease), which was closely related to IL-10 level. CONCLUSION: The postburn changes in TCRalpha/beta, CD28 on T cell membrane molecules and transmembrane signal transduction enzymes were important factors contributing to the decrease in IL-2 secretion, suppressed T cell function and the dual-directional changes in IL-10 secretion.

Relationship of the livestock trade to schistosomiasis transmission in mountainous area.

OBJECTIVE: To study the relationship between the livestock trade and schistosomiasis transmission and to provide an evidence for making a strategy of schistosomiasis control in mountainous areas. METHODS: A retrospective survey and analysis was conducted to investigate the prevalence of schistosomiasis in both humans and livestock (cattle, horses, mules, donkeys and pigs), and the number and migration of livestock in Weishan County, Yunnan Province in 1980-1991. RESULTS: A positive correlation was found between the infection rate of residents and the numbers of livestock migration (R = 0.9151, P < 0.01). During 1980 to 1991 the infection rate was increased gradually along with the development of livestock husbandry, especially, from the economic reforms since 1984. In 1984 there was positive correlation in the infection rate both human and livestock (R = 0.8458, P < 0.05). The results show that the infection rates of livestock on sale including cattle, horses, mules, donkeys and pigs are 9.54%, 29.39%, 16.38%, 14.47%, 25.73% and 11.11%, respectively. CONCLUSION: The infection rate of human and livestock arises by parallel. The high frequency of livestock trade resulted in serious spreading of the infection source of schistosomiasis. The migration of the infected livestock might be an important factor in transmitting schistosomiasis.

Lens epithelial cell proliferation and cell density in human age-related cataract.

PURPOSE: To discuss the potential effect of the lens epithelial cell proliferation in age-related cataract. METHODS: In vitro cell proliferation was assayed by MTT method to evaluate the lens epithelial cell density, index, and proliferation capacity in normal lens and all kinds of age-related cataract. Capsulotomy specimens from all kinds of patients who underwent cataract phacoemulsification extraction surgery were compared with the lens epithelial specimens from non-cataract lenses of Eye Bank eyes. RESULTS: Lens epithelial cell density of central anterior capsule (LECD) in female normal lens was higher than that in male, LECD in nuclear cataract (> NIII) was higher than that in normal lens, but in the mature cortical cataract, LECD was lower. Mitotic index of three kinds of age-related cataracts in vivo had no statistical difference, neither did cell proliferation capacity of cultivated cells in vitro. CONCLUSION: The individual difference of lens epithelial cell density and proliferation capacity in vivo may be an important underlying cause for senile cataract in the cellular level, especially for nuclear cataract.

Vitro culture and immunohistochemical identification of astrocytes of infantile optic nerve.

OBJECTIVE: To culture astrocytes of optic nerve and to establish the cell lines for further study of healing process after optic nerve trauma. METHODS: Optic nerve astrocytes of infantile and adults with sudden death were cultured by tissue inoculation or tissue digestion with 0.25% Trypsin and 0.06% EDTA. The second and fourth passage cells were stained with HE and anti-GFAP, S-100 protein, Vimentin, and CD34 antibodies. RESULTS: The trypsinized astrocytes of infantile optic nerve reached confluence in 7 days, but the astrocytes of adults weren't successfully cultured. The cultured cells were in polygonal shape with processes; the cytoplasm was abundant and pink; the cells had light-blue nuclei. These cells were positive in GFAP, S-100 protein and vimentin staining, and negative in CD34 staining. CONCLUSIONS: The results showed that astrocytes of infantile optic nerve can be successfully cultured and trypsinization is a better method than tissue inoculation. The culture of infantile astrocytes is easier than that of adult astrocytes. Immunohistochemistry were used to determine the source and type of those cultured cells.
