Evidence of a role for NK cells in oxazaphosphorine-mediated tumor regression.

The present studies showed that nude mice xenotransplanted with L5222 leukemia responded as did syngeneic BD IX rats to low doses of mafosfamide or cyclophosphamide. Unlike rats, nude mice rarely showed resistance to a second tumor challenge. The observation that concurrent treatment of rats with cyclosporin A did not alter the rate of survival clearly indicated a T-cell-independent mechanism of tumor defense. The incidence of lung colonies from i.v. injected Lewis lung-tumor cells could be enhanced by a high dose pretreatment with mafosfamide or cyclophosphamide, whereas pretreatment at low doses was inhibitory. Since identical experiments carried out in NK-cell-deficient C57Bl/6 "beige" mice did not show such an effect, NK cells appeared to represent a possible effector cell in oxazaphosphorine-mediated antitumor effects. This assumption was further supported by the fact that enhanced NK cell activity could be observed in the 51Cr release assay using spleen cells from mafosfamide-treated L5222-bearing rats. The transplantation of the unrelated syngeneic ovarian carcinoma OV-342 to animals that had previously been cured of L5222 leukemia did not lead to the rejection of this tumor. This indicates that a specific resistance against L5222 leukemia had developed. In contrast, a T-cell-dependent antitumor effect was demonstrated for mafosfamide in the MOPC-315 mouse plasmocytoma. Therefore, we conclude that the effector cell for tumor rejection depends on the type of tumor. This, of course, does not exclude a common target cell for the immunopharmacological activity of oxazaphosphorines.

Functional activities in spinal muscular atrophy patients after spinal fusion.

Spinal muscular atrophy patients present with weakness, greater in the proximal muscles, leading to scoliosis and limited upper extremity function. The purpose of this study is to identify unique aspects of these patients and to understand how spinal fusion affects their function. Forty patients underwent Harrington or Luque rod instrumentation with functional evaluations preoperatively and 2 and 5 years postoperatively. Biomechanical assessment of function is important. Flexibility of the spine is functionally advantageous because distal strength is used to align weaker proximal segments. Postoperatively, lack of spinal flexibility resulted in a decline in gross motor function and increased use of UE aids due to a change in the trunk position in the weaker patients. The stronger patients' activities were maintained. Earlier mobilization in patients with Luque procedures did not improve postoperative function.

Accentuated complement activation in patient plasma during the adult respiratory distress syndrome: a potential mechanism for pulmonary inflammation.

The adult respiratory distress syndrome (ARDS) represents an acute inflammatory lung disorder, characterized by both refractory hypoxemia and a mortality rate approaching 95%. Researchers have proposed that activation of the complement (C) system may play a role in the development of the pulmonary inflammation associated with ARDS. The purpose of this investigation was to determine whether complement activation occurs to a greater extent in patients with ARDS than in patients without ARDS, thereby providing a potential mechanism for the acute inflammation seen in ARDS. In this study, we assessed plasma complement activation by measuring complement activation by-products: C1rC1s-C1 inhibitor complex and C3b-P complex, generated subsequent to activation of the classical and alternative pathways, respectively, and also the terminal complement complex, formed after activation of either the classical or alternative complement pathway. Multivariate discriminant analysis revealed that these three complement activation complexes could distinguish patients with ARDS from those without ARDS (p less than 0.04). Furthermore, these three complexes provided a more sensitive discriminator of ARDS than did plasma levels of C3a desarginine (p greater than 0.30), C5a desarginine (p greater than 0.41) and total hemolytic complement activity (CH50) (p greater than 0.72). We conclude that a temporal association exists between the complement activation and the development of ARDS. Therefore, we suggest that complement activation by-products be included in the armamentarium for ARDS.

Congenital rubella infection after previous immunity of the mother.

A newborn boy was admitted with a congenital rubella infection. Seven years previously his mother had been vaccinated against rubella; 3 years previously rubella immunity had been confirmed. Therefore, intrauterine transmission must have occurred after maternal reinfection during pregnancy. Prenatal diagnosis of rubella embryopathy with serological methods after subclinical maternal reinfection is nearly impossible.

[Basal cell epithelioma of the eyelids].

UNLABELLED: The purpose of this study of 36 patients was to assess the oncological, functional and cosmetic results of surgery in the treatment of basal cell carcinoma of the eyelid and to compare them with those obtained with other possible therapeutic methods. MATERIAL AND METHODS: We report a series of 36 cases of histologically proven basal cell carcinoma principally located on the lower eyelid and the inner canthus. 27 p. 100 of the patients had previously been treated and presented with a recurrent tumour. Mean age was 61 years. Minimum follow-up was 5 years. Reinforced and potentiated local anaesthesia was given in most cases. Tumoral excision was complete in 31 patients, insufficient or borderline in 5 patients. Intraoperative extemporaneous biopsy was performed in 8 p. 100 of the cases. Repair was carried out by direct suture or by grafting, the flap being simple or combined with a mucosal or chondromucosal graft. The lacrymal passages were destroyed in 5 cases and were not reconstructed in 4 of them. There was no postsurgical therapy. RESULTS: As regards the malignancy itself, the recurrence rate was 6 p. 100 and affected patients who had previously been treated. The cure rate was 100 p. 100 in patients for whom surgery was the first treatment, and 94 p. 100 in the totality of patients. Functionally, there was one case of ectropion, and epiphora developed in 7.4 p. 100 of the cases. In this series epiphora was associated with destruction of the lacrymal passages in only one patient. Satisfactory cosmetic results were obtained in 85 p. 100 of the cases, bearing in mind that the assessment was necessarily subjective. DISCUSSION: Treatments of basal cell carcinoma are numerous. We shall analyze their results, advantages and drawbacks compared with those of surgery. Cryotherapy raises technical problems of execution which are responsible for an excessively high recurrence rate. Chemotherapy has recently been enriched with the introduction of Solcoderm, but the follow-up is still too short. Electrocoagulation gives good oncological results, but it can only be used for small lesions. Radiotherapy (contact, penetrating or semi-penetrating X-ray therapy, electron therapy) has a cure rate of 95 p. 100 and gives acceptable cosmetic results. However, it requires numerous sessions, cannot be repeated if it fails and complicates surgical treatment; in the long term, it carries a risk of radiodystrophy. Its functional and ocular complications have been estimated at 9 p. 100. Epiphora is present in 2 to 19 p. 100 of the cases.(ABSTRACT TRUNCATED AT 400 WORDS)

Radiation therapy for carcinoma of the pinna using iridium 192 wires: a series of 70 patients.

From January 1970 to November 1982, 70 patients with carcinoma of the pinna were treated by interstitial irradiation. An afterloading technique with Iridium 192 wires was used. One patient recurred and had a total pinnectomy followed by 60 Gy external radiation. This patient was alive without evidence of disease at 134 months. Three patients who had tumors greater than 4 cm in size at presentation developed late necrosis which required subsequent total pinnectomy. Cosmetic results were assessed in 55 patients and were good with few late sequelae (in 78% of cases (36/46) when the tumor measured less than 4 cm, but only in 1/9 when the tumor measured more than 4 cm). We advocate interstitial Iridium 192 irradiation for treatment of pinna tumors smaller than 4 cm. None of 39 patients with squamous cell carcinoma had biopsy proven cervical lymph node metastasis at the time of diagnosis. Four patients with squamous cell carcinoma (4/39: 10%) later developed a regional nodal metastasis after treatment of the pinna. All four relapsed in the parotid region and were managed by partial parotidectomy and neck dissection followed by external irradiation. One of these four patients died from uncontrolled cervical node disease. In our opinion, when regular follow-up is dependable, it is reasonable to save treatment of the cervical nodes for those patients who relapse with involved metastatic cervical nodes.

[Cutaneous prickle-cell epitheliomas and visceral cancer].

We have studied the incidence of internal malignant neoplasms on patients with squamous cell carcinoma (SCC). This incidence has been compared to expected number of malignant neoplasm in that population by a precise statistical analysis. Two-hundred and eighty-three SCC confirmed by histology are diagnosed on 235 patients (159 men, 76 women). These patients have been followed between December 1970 and November 1982 at the specialized consultation on cutaneous tumors. We have examined all these patients, questioned their doctors, consulted their medical case history, to search an internal malignant neoplasm. Twenty-eight patients had a malignant neoplasm: this malignant tumor has been found 11 times before and 17 times at the same time or after SCC. These 17 malignant neoplasms are the only statistically interesting cases. The annual incidence of malignant neoplasms related to age and sex has been evaluated by consulting French registers. The French incidence has been compared to foreign incidences. This incidence enables us to calculate the expected observed numbers (12 and 5). The expected numbers are 9.15 for men and 2.1 for women. These numbers are not statistically different from observed numbers (12 and 5). The incidence of internal malignant neoplasms on patients with SCC is not different from the incidence of general population.

Disseminated parainfluenza infection in a child with severe combined immunodeficiency.

An 8-month-old male infant with severe combined immunodeficiency syndrome was admitted to the hospital with pneumonitis and, subsequently, died at 21 months of age. During his lengthy hospitalization, parainfluenza virus type 3 (PIV-3) was isolated from respiratory secretions and unusual sites, such as pericardial fluid, CSF, and WBCs. A postmortem examination showed apparent viral pancreatitis, and PIV-3 was isolated from the lung, brain, and pericardial fluid. To our knowledge, this is the first reported case of disseminated parainfluenza infection identified during life. We speculated that a mutant fusion protein may have been elaborated by the virus, allowing dissemination beyond the respiratory tract.

Serotonin content of platelets in inflammatory rheumatic diseases. Correlation with clinical activity.

Significantly decreased platelet serotonin contents were measured in rheumatoid arthritis, systemic lupus erythematosus (SLE), progressive systemic sclerosis, and mixed connective tissue disease. An inverse relationship between platelet serotonin levels and clinical disease activity was observed in both rheumatoid arthritis and systemic lupus erythematosus. SLE patients with multiple organ involvement showed the lowest platelet serotonin values. No correlation was observed between platelet serotonin contents and nonsteroidal antiinflammatory drug treatment, presence of circulating platelet reactive IgG, or the amount of circulating immune complexes. The results are interpreted as indicating platelet release occurring in vivo during inflammatory episodes of the rheumatic disorders investigated.

Infection of the colon of the rhesus monkey by spiral-shaped organisms.

Intestinal spirochetosis, an infection of the mucosa by spiral-shaped organisms, was studied in clinically normal rhesus monkeys (Macaca mulatta) by histology, transmission and scanning electron microscopy. The incidence of intestinal spirochetosis was 42% in 221 monkeys. Spiral organisms stained with hematoxylin and eosin (HE) appeared as a broad basophilic haze on the colonic surface and were strongly positive by the Warthin-Starry stain. Spiral-shaped bacteria include two structurally different organisms: spirochetes and flagellated microbes. They intimately populated the brush border of the surface of the epithelium of the large intestine. They were absent in the crypts and in the small intestine. Infection by spirochetes produced no alteration of cytocomponents of the underlying host structures. Spirochetes and flagellates infrequently penetrated beyond the brush border into the epithelial cytoplasm and also into the lamina propria. Even in cases where invasion was documented, no inflammatory response was found.

Longitudinal assessment of children with enteroviral meningitis during the first three months of life.

Nine children who had enteroviral meningitis during the first three months of life and nine matched control children were evaluated for possible sequelae of their infection. The mean age of the children with meningitis was 46.67 months at the time of testing and that of the control subjects was 47.33 months. The mean IQ level of 89.67 obtained in the control children. Receptive vocabulary testing suggested that the receptive language functioning of the group with meningitis was significantly less than that of the control group. There was no significant difference in head circumference, no detectable sensorineural hearing loss, and no detected difference in intellectual functioning between the meningitis group and matched control subjects.

[Iridium 192 interstitial radiotherapy of carcinoma of the penis. Carcinologic results, dermatologic evaluation (about 87 cases) (author's transl)].

87 cases of epidermoid carcinoma of the penis treated by Ir 192 interstitial radiotherapy are studied. The 5 years survival rate, estimated on a 51 patients group, is 61 p. 100. Among these 51 patients, 12 had a local recurrence. The post-radiotherapy early reaction is estimated on a 79 patients group, with a mean duration of 3, 4 months. The early reaction is correlated to tumor size and not correlated to number of radio-active lines. The later dermatologic evolution is studied in 59 patients. In 22 cases, the scar remained supple, without any further local change. Local changes occurring in the 37 other cases are studied in their clinical aspect, signification and frequency. Of 34 patients followed longer than 40 months, 14 had local complications with 4 cases of local recurrence. The carcinologic reliability of this method is similar to that by surgery. It seems however it must be restricted to tumors limited to glans penis.
