Bilateral spontaneous pneumothorax associated with metastasis of a malignant fibrohistiocytoma.

The case of a 34-year-old female patient is presented. The patient was admitted because of bilateral pneumothorax caused by the metastasis of a malignant histiocytoma originating in the left gluteus. Bilateral chest-suction was made and the patient received complex chemotherapy. The chest X-ray taken 4 months later showed considerable regression of the lymph node metastases. Neither the time of development of pneumothorax nor its mechanism is known. There are only assumptions about it. Authors have considered their case worthy of publication because the lymph node metastasis of bilateral simultaneous pneumothorax due to histiocytoma has not so far been known in the literature.

Subglottic inflammatory pseudotumor in a 6-year-old child.

Inflammatory pseudotumors have been given diverse--no fewer than 19--names in the literature. The most frequent localization is in the lung, the gastrointestinal tract, and salivary glands. The case presented here at the subglottic level is exceptional. Although the intraoperative diagnosis is not easy, the prognosis is usually good. Malignant degenerations have not been described. In those cases that cannot be operated on or that extend into the mediastinum, radiotherapy is indicated.

Surgical treatment of thoracic hydatidosis. A review of 100 cases.

Hydatid cystic disease continues to hold an important place in chest disease in our country. The authors review their experience from 1977 to 1987 of 100 patients who underwent surgery for pulmonary hydatidosis. Mean age was 36.21 years. Fifty-nine cases were ruptured cysts (RC) and 41 were unruptured cysts (UC). The diagnosis was based on epidemiological, radiological and mainly serological and endoscopic criteria. The indirect haemagglutination test was positive in 100% of RC and 80% of UC, while 70.2% of the patients who underwent fiberoptic bronchoscopy showed pathological changes. The most commonly used surgical procedure in UC was open subtotal cystopericystectomy (89.09%), while wedge resection (41.81%) was the most commonly used technique for the RC. There were no operative deaths and no recurrences were observed for a mean follow-up of 5.4 years. The indications for adjuvant chemotherapy with mebendazole are presented.

Ectopic goiters of the mediastinum: presentation of two cases and review of the literature.

Two cases of ectopic goiters located in the anterior mediastinum are presented. These comprise 5.1 per cent of our cases of intrathoracic goiters and 1.3 per cent of mediastinal tumors. These benign tumors have been rarely reported in the literatures, however, excision by thoracotomy is the surgical procedure advocated for these cases and the diagnosis is generally established operatively. Angiography and tomography have proved useful in diagnosing and localising these lesions prior to the surgical procedure.

Costal chondritis in heroin addicts: a comparative study with postsurgical chondritis.

Infection of the chondrocostal junction occurs infrequently nowadays. However, with the increasing incidence in the last years of intravenous drug addiction, more cases have been reported recently. The authors studied two groups of patients with costal chondritis, one of heroin addicts and the other of patients who had undergone thoracic surgery previously. While in the postsurgical group the patients need some kind of resection for their treatment, in the heroin addicts an early drainage is usually enough.

Unexpectedly long 14S ribosomal RNA gene in Tetrahymena mitochondria.

Extraction of RNA from Tetrahymena mitochondrial ribosomes yields several RNA species, including a "large" 21S molecule, a "small" 14S molecule, a 7S molecule, and other smaller RNAs. The molecular weight of the 14S rRNA indicates that it is about 1,300 bases in length. We have sequenced the 14S rRNA gene and, by aligning our sequence with that of the corresponding small rRNA from E. coli, find that the 14S rDNA is at least 1,635 bases in length. We propose, based on the results of hybridization studies, that this unexpected length is due to the presence of 7S RNA sequence within the 14S gene sequence. The 7S region is apparently lost from the 14S rRNA, yet is still a component of the ribosome.

Hemothorax as rare presentation of intralobar pulmonary sequestration.

In a patient with intralobar pulmonary sequestration the presenting feature was severe hemothorax caused by rupture of the anomalous artery. Only one previous case with this presentation was found in the literature. During operation for intralobar sequestration, the blood vessels of the anomaly must be clearly visualized, in order to avoid further complications. When the condition is suspected, angiography is mandatory for establishment of the diagnosis.

Anti-ulcer activity and other pharmacological properties of lozilurea.

As a result of trials on a large series of compounds, one of these, N' -3-chlorobenzyl-N'-ethylurea (lozilurea, ITA 312) has shown marked anti-ulcer activity. It has shown itself to be active against chemically and neurogenically induced gastric and duodenal lesions in various experimental animal models. It has no major anti-secretory action. The experimental data obtained suggest that the mechanism of action of lozilurea consists in increasing the protective function of the mucus barrier. In the screening trials carried out in order to detect the side effects of lozilurea, it has shown sedative, antipyretic and vasodilatory actions.

Primary choriocarcinoma of the lung. Presentation of a case and review of the literature.

A 67-year-old man with persistent haemoptysis was found to have primary choriocarcinoma of the lung. Only previous cases were found in the international literature. Hyperprolanuria B would seem to be the most important factor for early diagnosis. The recommended treatment is surgery in accordance with the oncologic criteria for bronchopulmonary cancer.

Pharmacological activities and side effects of plafibride.

This paper reports on pharmacological properties of N-2-(p-chlorophenoxy)-isobutyryl-N'-morpholinomethyl-urea (plafibride, ITA 104) and its possible side effects. This work was carried out on CNS, ANS and PNS, cardiovascular system, respiratory and gastroenteric apparatus as well as anti-inflammatory activity and gastric tolerance. The most evident secondary effects were: a certain sedation, as a light tranquillizing agent, a hypothermic effect when it was administered at high doses, a certain beta-blocking and antiarrhythmic activity probably due to its local anaesthetic action. All the side effects appeared at high doses, much higher than the therapeutic ones.

Toxicological studies of plafibride. Part 1: Acute toxicity and its determination after several administrations of plafibride.

Acute toxicity studies of N-2-(p-chlorophenoxy)-isobutyryl-N'-morpholinomethylurea (plafibride, ITA 104) were carried out in mouse, rat, guinea pig and hamster by different administration routes. The effect of pretreatment with plafibride on acute toxicity was also studied for several days. Plafibride had low toxicity. Its LD50 p.o. was about 4000 mg/kg in rat, mouse and hamster. The LD50 i.p. was between 760 (673-859) mg/kg in NMRI mouse and 1050 (830-1328) mg/Kg in hamster. The LD50 i.v. was similar for rat and mouse (about 100 mg/kg). Pretreatment with plafibride did not alter acute toxicity, which implies the absence of accumulation of the compound which would have led to an increase in toxicity. Nor was there any appearance of enzymatic induction, in which case a decrease would be observed in the toxicity of the product.

Toxicological studies of plafibride. Part 2: Subacute and subchronic toxicity in rats and chronic toxicity in dogs.

The toxicity of N-2-(p-chlorophenoxy)-isobutyryl-N'-morpholinomethylurea (plafibride, ITA 104) was studied by repeated administration in Wistar rat for one and three months and in beagle dog for six months. Clofibrate was used as the standard product. In subacute toxicity in rat hepatomegaly and increase in kidney weight were observed in the animals treated with plafibride. A smaller increase in body weight was also observed in the males. The rats treated with 500 mg/kg of plafibride showed slight anaemia. Clofibrate at the same dose (500 mg/kg) caused the same effects as plafibride. In subchronic toxicity in rat for a period of 3 months both plafibride and clofibrate produced hepatomegaly and an increase in kidney weight but did not affect hepatic and renal functions. In chronic toxicity in beagle dog (6-month period) plafibride was tolerated at the dose of 100 mg/kg, causing slight anaemia, hepatomegaly and an increase in kidney weight, although no functional alterations of these organs were noticed. Clofibrate was administered at the dose of 50 mg/kg but was not tolerated at higher doses.

Toxicological studies of plafibride. Part 3: Study of teratogenic activity in rats and rabbits.

Teratological studies of N-2-(p-chlorophenoxy)-isobutyryl-N'-morpholinomethylurea (plafibride, ITA 104) were carried out in rats and rabbits. The rats were given doses of 125, 250 and 500 mg/kg/d p.o. and the rabbits 25, 50 and 100 mg/kg/d p.o. during the whole period of organogenesis. As a positive control, vitamin A was administered to the rats at the dose of 60,000 IU/rat p.o. for 3 days and thalidomide was given to the rabbits at the dose of 150 mg/kg/d p.o. A group was added treated with clofibrate at the dose of 500 mg/kg/d p.o. in rat and 100 mg/kg/d in rabbit. Plafibride caused malformations in both species. In rat a considerable number of fetuses appeared with hydrocephalus, correlated to the dose. In rabbit, malformations were observed at the lower doses whereas at 100 mg/kg the toxic action on the embryos avoided the appearance of malformed fetuses. Both vitamin A and thalidomide were teratogenic in the experiments in rat and rabbit, respectively. The number of malformations and postimplantations losses were significant in relation to the control group in both cases. The standard product, clofibrate, reduced the viability of the fetuses which showed weight loss in rat and lower survival rate in rabbit. Plafibride is advised against in pregnant women.

Toxicological studies of plafibride. Part 4: Interaction of plafibride with other drugs.

The possible pharmacological interaction was studied between N-2-(p-chlorophenoxy)-isobutyryl-N'-morpholinomethylurea (plafibride, ITA 104) and a series of products with analgesic, anticoagulant, antidepressant, antidiabetic, Beta-blocking, diuretic, hypotensive, nootropic, tranquillizing and vasodilator activities. In the experiment the LD50 of each product was compared with that found for the joint administration of the product and plafibride. Plafibride increased the acute toxicity of reserpine and potentiated its hypothermic effect in the mouse; it did not alter its effect on arterial pressure in dog and rat nor its effect on palpebral ptosis in the mouse. Although plafibride did not potentiate the acute toxicity of warfarin, its anticoagulant activity was potentiated, possibly due to the movement of warfarin from its binding sites to the plasma proteins.

Plafibride tolerance trial at increasing doses in healthy volunteers.

Tolerance of N-2-(p-chlorophenoxy)-isobutyryl-N'-morpholinomethylurea (plafibride ITA 104), was studied in a group of healthy volunteers. Bis[2-(p-chlorophenoxy)-2-methylpropionato]-hydroxyaluminum (alufibrate) was used as a reference drug. Treatment with both drugs was carried out at progressively increasing doses from 50 to 1600 mg/d. Both products were administered p.o. Plafibride showed a platelet antiaggregant effect at the dose of 800 mg/d, which was more evident at the dose of 1600 mg/d. Alufibrate showed no platelet antiaggregant effect. Both drugs showed a similar type of hypolipemic effect which was a little higher in the case of alufibrate. No side effects of any kind were noticed with either of the two drugs.
