[Transvaginal surgical removal of a sacrococcygeal teratoma (author's transl)].

For the operative treatment of presacral sacrococcygeal teratomas perineal, abdominal, and combined abdominal-perineal routes are described. In this paper the transvaginal surgical removal of a benign teratoma after partial abdominal resection of the tumor in a 19-year-old woman is reported. No recurrence was observed within 5 years.

Evaluation of hypercoagulability in users of oral contraceptives.

Soluble fibrin monomer complexes (SFMC) were determined in users of different oral contraceptives. Quantitative gel filtration of beta-alanin precipitated plasma samples yielded the relative (per cent of total fibrinogen content) and absolute (mg per 100 ml plasma) amount of SFMC. Increased levels of SFMC were observed in users of oral contraceptives, when compared to age, parity and socioeconomic background matched control groups. This increase was more pronounced in users of Neogynon (0.05 mg ethinyl estradiol, 0.25 mg norgestrel) than in the one using Microgynon (0.03 mg ethinyl estradiol, 0.15 mg norgestrel). Users of Ovulen (0.1 mg mestranol, 1 mg ethynodiol diacetate) in addition to the rise of SFMC levels exhibited elevated fibrinogen levels in plasma. The data presented indicate a limited state of hypercoagulability in users of oral contraceptives.

[Late cervical pregnancy (author's transl)].

A cervical pregnancy which develops into the last trimester of gestation with a viable infant is a very dangerous and rare condition. A personal observation of such a case at 31 weeks gestation is added to the 8 cases known in the literature. Extreme dilatation of the cervical wall with extensive vascularization is typical for this condition associated with placenta increta or accreta. The uterine body tops the cervical pregnancy like a cap. Cesarean section followed by hysterectomy is the management of choice.

[Histological diagnosis and individual treatment of cancer in gynecology (author's transl)].

Individualization of cancer therapy in gynecology is based on a perfectly precise histological diagnosis. This implies close cooperation between the clinician and the histopathologist. Making a reliable and therapeutically decisive histopathological diagnosis is possible by means of certain diagnostic interventions which provide a maximum of clinical and histological information on the uterus and the breast, and by means of a systematic fine structure processing of the surgical specimens in carcinomas of the uterus, vulva and breast. Only in this way can the clinician take the extraordinary responsibility of an individual treatment of cancer.

[Hemoblastosis and genital manifestations (author's transl)].

The course of the disease in a woman with a malignant lymphoma is described. The primary disease was recognized through an early involvement of the uterus which led to hemorrhages. While observations of this kind are relatively seldom communicated, examination of the literature showed that involvement of the genitals is to be reckoned with relatively frequently with hemoblastoses in the advanced stages. Regular gynecological investigations are therefore essential for women with hemoblastoses.

Secretion of progesterone by human overies perfused in vitro.

A system for the perfusion of isolated human ovaries is described. Krebs-Ringer bicarbonate buffer (pH 7.4) containing glucose, sodium lactate and pyruvate was perfused at a flow rate of 5-8 ml/min at 37 degrees C through the ovarian artery. Oxygen uptake, LDH-activity, pH and progesterone secretion were determined as parameters of viability of the perfused organ. Our results demonstrate that a relatively simple system used for perfusion was able to preserve ovarian function for at least two hours. This experimental design should not be regarded as physiological but in addition to other experimental approaches it might provide information on the physiology of the human ovary. Though lower values of progesterone secretion than in vivo were found, a correlation of secretion and functional stage of the ovaries could be demonstrated.

Metabolism of (4-14C)progesterone and (7alpha-3H)pregnenolone by human ovaries perfused in vitro.

Nine human ovaries were perfused in vitro with [4-14C]progesterone and in addition one ovary with [7-3H]pregnenolone. From the perfusate unchanged progesterone and five different metabolites were isolated: 20alpha-dihydroprogesterone, 17alpha-hydroxyprogesterone, 16alpha-hydroxyprogesterone, 5alpha-pregnane-3,20-dione and 4-androstene-3,17-dione. In the ovarian tissue saturated pregnane derivatives were the main metabolites. When [3H]pregnenolone and [14C]progesterone were perfused simultaneously a stimulation of delta4-5-isomerase and 3beta-dehydrogenase activity by HCG was demonstrated.

Progesterone 20 alpha-dihydroprogesterone and 20 beta-dihydroprogesterone levels in different compartments from the human foeto-placental unit.

The endogenous levels of progesterone, 20 alpha-dihydroprogesterone and 20 beta-dihydroprogesterone in different foetal tissues, in placental tissue and in foetal blood during pregnancy (week 9-25) were determined by gas-liquid chromatography or by radioimmunoassay. The identification of these steroids in foetal tissues was based on the behaviour in paper and thin-layer chromatography, the formation of different derivatives, and the retention times in gas-liquid chromatography. For additional identification of progesterone and 20 alpha-dihydroprogesterone a mass spectrum was obtained. The average concentrations of progesterone (P), 20 alpha-dihydroprogesterone (20 alpha-DHP) and 20 beta-dihydroprogesterone (20 beta-DHP) in foetal tissues were as follows (ng/g wet weight tissue): brain, P 119.0 +/- 45.5, 20 alpha-DHP 86.9 +/- 44.7 and 20 beta-DHP 9.7; lungs, P 257.7 +/- 195.7, 20 alpha-DHP 60.3 +/- 26.4 and 20 beta-DHP 7.8; liver, P 103.7 +/- 84.6, 20 alpha-DHP 95.2 +/- 89.8 and 20 beta-DHP 17.8; adrenals, P 295.7 +/- 90.0 20 alpha-DHP 254.5 +/- 194.6; kidneys, P 214.2 +/- 155.3 and 20 alpha-DHP 76.6 +/- 43.0; intestine, P 273.2 +/- 166.7, 20 alpha-DHP 77.9 +/- 51.2 and 20 beta-DHP 8.7; residual tissues, P 246.3 +/- 178.3 and 20 alpha-DHP 54.8 +/- 44.5. The average concentrations in placental tissue and in foetal plasma (week 12-18) were as follows (ng/g wet weight tissue or ng/ml plasma): placenta, P 2619. +/- 2153.6 and 20 alpha-DHP 461.0 +/- 155.9; foetal plasma, P 378.7 +/- 231.8 and 20 alpha-DHP 283.0 +/- 108.9. These results indicate that some foetal organs contain a high 20 alpha-reductase activity at least during the first half of pregnancy.

[Balanced synthetic diet in gynaecological operations].

A balanced synthetic diet, the so called astronaut-diet was given to nine gynaecological patients with recto-vaginal fistulas or operation for aplasia of the vagina. By the reduction of stool quanitity and the change to an almost sterile stool, wound healing was very much enhanced. This diet is recommended for some operations involving the vagina and rectum.

[Perinatal mortality in foreign workers (author's transl)].

From 1970 to 1972, there were 216 perinatal deaths among 5595 newborns at the I. Frauenklinik der Universität München. 54 of these deaths were children of foreign workers (so-called "Gastarbeiter"). The data have been processed on punch cards and analysed by a computer. The differences noted underwent significance testing by the CHI-Quadrat test. Only statistical significant results are published. The perinatal mortality in the above period shows no difference between foreign and German ward patients. There is, however, a significant lower perinatal mortality in private patients. We feel that this difference is due to a significant lower rate of prematures in the private patient group. The cocial status as well as higher interest and motivation in health resulting in better prenatal care are discussed as causal reasons for this fact.
