[Pregnancy and labor in the patient with arteriovenous congenital fistula].

The paper presents a case of uncomplicated pregnancy in 26-years old primipara suffering from congenital arteriovenous fistula. The clinical picture of disease was manifested by presence of vaso-dilatations in lower and upper limbs and in the vulva, because of which the pregnancy was terminated by cesarean section. Puerperal and postoperative outcome was uncomplicated. In the puerperal period heparin and large-spectrum antibiotic therapy was administered. No cardiovascular or thromboembolic complications were observed. Authors are of opinion, that in face of menace, the pregnancy and labour in the women presenting arterio-vascular fistula have to be considered as situation of high obstetrical risk.

[Carotid sinus syndrome diagnosed by transcutaneous cardiac pacing].

The diagnostic usefulness and safety of non-invasive heart stimulation has been tested on 20 persons with full symptomatic cardiodepressive type of carotid sinus syndrome. The estimation of the cardiodepressive syndrome has been conducted on the grounds of electrophysiological tests of the heart with the method of endocavitary stimulation introduced by Stryjer and adopted by us for non-invasive heart stimulation. Massage of the carotid sinus was performed during sinus rhythm, during transesophageal atrial stimulation and during transcutaneous ventricle stimulation. In 17 persons massage of the carotid sinus caused sinus node inhibition. In this group carotid sinus massage during atrial stimulation revealed a group of 10 persons with only sinus node inhibition (type A) and a group of 7 persons with the coexistence of sinus node inhibition and an AV III block (type C). Only in 3 persons massage of the carotid sinus merely caused an AV III block (type B). Massage of the carotid sinus during transcutaneous ventricle stimulation showed retrograde AV conduction suppression in type B and C of the carotid cardiodepressive syndrome. To recapitulate the obtained results, it can be assumed that electrophysiological estimation of the carotid sinus syndrome is also possible and safe with the non-invasive method. Such procedure can also facilitate the selection of patients for permanent stimulation of AAI type for the type A syndrome or DVV for types B and C.

[Latent retrograde atrioventricular conduction in healthy persons].

45 patients (13 females and 32 males; mean age-37 years) without an organic heart disease underwent the study, on condition that effective transesophageal, ventricular stimulation was achieved. The transesophageal pacemaker SP-5 made by OBR TEMED ZABRZE was used. Ventricular stimulation threshold and a lock of evident retrograde conduction was estimated during ventricular stimulation with a constant cycle length of 500 ms. To assess concealed retrograde atrioventricular conduction, programmed ventricular stimulation of a 50 ms cycle length was applied between late diastole and left ventricular refraction. Concealed retrograde atrioventricular conduction was diagnosed of a sinus impulse reached later ventricles than that during sinus rhythm preceding an examination. There was no evident atrioventricular retrograde conduction in 15 patients (33%). In 9 of them (60%) programmed stimulation revealed concealed retrograde atrioventricular conduction reaching atrioventricular node. Obtained results indicate, that the transesophageal programmed ventricular stimulation enables to examine concealed a-v conduction phenomenon and can be helpful in approximate localization of retrograde atrioventricular block.

Bactericidal activity of human sera with a physiologic or genetic defect of the complement system.

The serum of patient suffering genetically conditioned C2 defect showed a weak bactericidal activity against Pseudomonas aeruginosa strains. Out of 23 tested strains one was susceptible comparing with 16 ones sensitive to normal human serum. The normal cord serum exerted a bactericidal effect on 8 strains. All sera were found to be active against Salmonella strains tested.

Isolation and some properties of zinc-containing proteins from human placenta.

Applying molecular filtration, the presence of two zinc-binding protein fractions in the human placenta was found. Their molecular weights were 47000 and 7900, respectively. A method for isolation of the high-molecular weight fraction from the placenta was elaborated. It consisted in the fractionation with acetone, Sephadex G-75 filtration, DEAE-Sephadex A-25 chromatography and preparative electrophoresis in polyacrylamide gel. It was calculated that 3.93 g-atoms of Zn are bound to one molecule of the protein. Rabbit antibodies against the zinc-containing protein formed an immunoprecipitate with the whole human amniotic fluid and with two protein fractions obtained following filtration of the amniotic fluids on Bio-Gel P-2.

[Treatment of varicose ulcers with Codogard of Polish production].

Synthetic skin CODOGARD produced by the Research and Development Centre of the Knitting Industry in Lódź was applied in 125 cases of the shank varicose ulcer treated in the Clinic of General Surgery of the Warsaw Medical Academy. Two different types of CODOGARD were used for evaluation: the first one without antibacterial dripping, and the second one with antibacterial dropping Sterinol. On the basis of achieved results it was established that CODOGARD was a successful medicine for the crus varicose ulcer. It causes faster cleaning away of the pus. It was observed that CODOGARD with Sterinol dripping was more effective. Taking into account the fact that the application of CODOGARD is easy, it should be widely used in surgical out patient's departments.
