Hemodynamic parameters during dynamic exercise in patients with silent myocardial ischemia in the presence or absence of hypertrophy.

Hemodynamic parameters during supine leg exercise-induced myocardial ischemia were investigated in ten patients without left ventricular hypertrophy (group I), and in ten patients with left ventricular hypertrophy (group II). Both groups of patients had previously been diagnosed as having totally asymptomatic silent myocardial ischemia. Both groups were comparable in age, sex, left ventricular ejection fraction at rest, and extent of coronary artery disease. All of the patients exercised to the same extent without angina or its usual equivalent. Left ventricular filling pressure increased from 4 +/- 2 mmHg at rest to 11 +/- 3 mmHg at peak exercise in group I, and from 7 +/- 4 mmHg at rest to 19 +/- 7 mmHg at peak exercise in group II. Left ventricular filling pressure at peak exercise was significantly greater in group II than in group I (p < 0.01). Totally asymptomatic silent myocardial ischemia can be seen with either normal or mildly abnormal left ventricular filling dynamics during exercise. In addition patients with coexistent left ventricular hypertrophy can have severely abnormal left ventricular filling dynamics during exercise without ischemic cardiac pains. Therefore, severe ischemia and/or stretching of the ventricular wall may be necessary, but not sufficient, for the occurrence of chest pain.

Retroviral transfer of herpes simplex thymidine kinase gene into glioma cells causes targeting of gancyclovir cytotoxic effect.

The thymidine kinase (tk) gene of herpes simplex virus type 1 (HSV-1) was transduced into three glioma cell lines (T98, U251MG, T9) using a retrovirus vector. The supernatants of viral producer cell line PA317/LTRNL was used for infection and three transduced cell lines (T98/LTRNL, U251MG/LTRNL, T9/LTRNL) were established. The toxicities of the anti-herpetic drugs, acyclovir and gancyclovir, were evaluated in vitro. The cytotoxicities of acyclovir and gancyclovir to the HSV-1 tk gene-transduced cells increased 100-1000 fold compared to the non-transduced parental cell lines. The cytotoxic effect of gancyclovir was higher than that of acyclovir, requiring a concentration of less than 0.31 microM to obtain 50% inhibition. Deoxyribonucleic acid analysis of the gancyclovir-treated cells demonstrated fragmentation, suggesting that apoptosis is involved in the mechanism of cell death. The HSV-1 tk gene-transduced cells were co-cultured with parental cells and treated with gancyclovir. More than 95% of cells were killed in a mixture ratio of 1:1, suggesting that the "bystander effect" operated in this system. Selective transduction of HSV-1 tk gene into glioma cells using a retroviral vector and treatment with gancyclovir is a promising therapy for patients with malignant glioma.

Primary Ewing's sarcoma of the skull base with intracerebral extension--case report.

A 32-year-old female presented with an uncommon primary Ewing's sarcoma of the skull involving the middle cranial fossa with an extremely unusual extension into the cerebral parenchyma. She was treated with surgical excision, irradiation, and chemotherapy. Two years after surgery she demonstrated no evidence of local recurrence or distant metastasis. Ewing's sarcoma of the skull may achieve a better survival rate under adequate management than this lesion in other sites.

Liposomal transfection of human gamma-interferon gene into human glioma cells and adoptive immunotherapy using lymphokine-activated killer cells.

The authors evaluated the effect of liposomal transfection of human gamma-interferon (HuIFN-gamma) gene into human glioma cells and lymphokine-activated killer (LAK) cells, alone and in combination. An HuIFN-gamma gene inserted in a eukaryotic expression vector was entrapped in liposomes bearing positive surface charges. Liposomal gene transfection induced production of HuIFN-gamma and its secretion in culture medium of human glioma cell lines (SK-MG-1 and U-251 MG). At 4 days after transfection, the cells produced 10 to 50 U/ml of HuIFN-gamma in the medium, whereby the major histocompatibility complex (MHC) class I and II antigens, as well as intercellular adhesion molecule-1 (ICAM-1), were induced on the glioma cell surface. The growth-inhibiting effect of transfection-induced HuIFN-gamma was much stronger in comparison with control cultures exposed to 500 U/ml of exogenously added HuIFN-gamma. In addition, 20% to 40% growth inhibition was obtained in the glioma cells when they were treated with LAK cells alone at a 5:1 ratio of effector to target cells. Liposomal transfection of HuIFN-gamma gene into human glioma cells combined with immunotherapy using LAK cells was more effective than either technique alone. The reinforcement of growth inhibition in the case of combined therapy was quenched by anti-ICAM-1 monoclonal antibody, but not by anti-MHC class I or II monoclonal antibodies. These results suggest that the combined effect of liposomal transfection of HuIFN-gamma gene plus LAK cells into human glioma cells is a potentially useful therapy for malignant glioma, and that the mechanisms of the reinforcement of growth inhibition are closely related to the expression of ICAM-1 on the glioma cell surface.

Factor VIII can be positive in a special type of malignant lymphoma, intravascular malignant lymphomatosis: an immunohistochemical investigation.

This report presents seven cases of intravascular malignant lymphomatosis (IML). The patients exhibited clinical signs and symptoms of multiple cerebral infarctions and polyradiculopathy, but in each case the diagnosis of IML was only established by postmortem examination. The autopsies revealed that almost every organ was involved and that there were atypical cells which had proliferated intravascularly. This was especially remarkable in the central nervous system. Extravascular extension of the tumor cells was rarely observed. It was documented by immunohistochemical studies that the abnormal cells were B cell lymphoma cells with a high proliferative capacity. Factor VIII, an endothelial cell-related antigen, was detected in the cytoplasm of the tumor cells of two cases (28%), but this finding was considered to represent a non-specific absorption of factor VIII from the thrombi.

Radioimmunolocalization of human brain tumor: fundamental studies with indium-111 labeled monoclonal antibody G-22.

Monoclonal antibody (MCA) G-22 reacts selectively with human glioblastoma. Whole immunoglobulin G (IgG) and the F(ab')2 fragment of G-22 were labeled with indium-111, and injected into athymic nude mice bearing xenografts of a human glioblastoma cell line (U-251-MG). Radiolabeled G-22 retained its antigen-binding activity allowing clear visualization of transplanted tumors. Although the 111In-labeled F(ab')2 fragment had a higher tumor-to-tissue ratio than whole IgG, tumor concentration of 111In-labeled G-22 MCA was higher in the latter and it provided much better images for a longer time when visualized by gamma-scintigraphy. These results suggest that 111In-labeled G-22 may be a useful agent for brain tumor imaging.

[A case of advanced gastric cancer with liver metastasis disappeared by oral administration of 5'-deoxy-5-fluorouridine after absolute non-curative operation].

A 64-year-old male was found to have a gastric cancer (Borrmann type 3) on the posterior wall of the stomach. Histological diagnosis was poorly differentiated adenocarcinoma. Ultrasonography revealed a hypoechoic lesion (18 x 14 mm) in the anteroinferior segment (S5) of the liver. The biopsy specimens from those lesions proved to be poorly differentiated adenocarcinoma. Total gastrectomy and distal pancreatosplenectomy were performed. During the operation, other liver metastases were found in the anterosuperior segment (S8) and inferolateral segment (S3). 5'-deoxy-5-fluorouridine (800 mg/day) was administered per os from the 33 rd postoperative day. The hypoechoic lesion of S5 in liver disappeared in the 17th postoperative month. The man is healthy with no evidence of recurrence 47 months after the operation. This case is very important because of the long-term CR.

A rare case of typical carcinoid of the lung coincident with thymoma.

The present report describes a 42-year-old woman with a coin lesion of the upper lobe of the right lung, preoperatively diagnosed as a small cell lung carcinoma of unusually slow growth. The preoperative workup revealed a mass in the mediastinum, diagnosed as a thymoma. Accordingly, a right upper lobectomy and mediastinal lymph node dissection, followed by a radical thymectomy, were performed. The resected pulmonary specimen was histologically diagnosed as a typical carcinoid and the mediastinal mass as a mixed type thymoma with microscopic invasion. The postoperative course was uneventful. The patient is doing well with no sign of recurrence one year after her operation. To our knowledge, the coincidence of a bronchopulmonary carcinoid and thymoma is believed to be the first such case reported in the literature.

Interferon-beta endogenously produced by intratumoral injection of cationic liposome-encapsulated gene: cytocidal effect on glioma transplanted into nude mouse brain.

The cytocidal effect of endogenously produced human interferon-beta was tested on glioma transplanted into the brain of nude mice by intracranial injection of a cell suspension of human glioma cell line U251-SP. When plasmid pSV2IFN-beta, bearing the human interferon-beta gene, was encapsulated into cationic multilamellar liposomes composed of N-(alpha-trimethylammonioacetyl)-didodecyl-D-glutamate chloride, dilauroyl phosphatidylcholine, and dioleoyl phosphatidylethanolamine in a molar ratio of 1:2:2 and injected intratumorally, human interferon-beta was produced in the tumor. The tumor completely disappeared if the injection was done shortly after the transplantation. Even when the tumor did not disappear, survival of the tumor-bearing nude mice was markedly prolonged. In control experiments made with normal brain having no glioma, interferon-beta was not detected.

[Outcome of treatment protocol 8704T for childhood T cell leukemia and lymphoma].

We reported the treatment outcome of Protocol 8704T, which included repeated L-asparaginase, for childhood T cell malignancies. Fifteen cases of acute lymphoblastic leukemia (T-ALL) and 11 cases of non-Hodgkin's lymphoma (T-NHL), aged 3 to 14 yrs (median 6 yrs), were enrolled. Twelve T-ALL had mediastinal mass. Murphy's stages of T-NHL were 6 with III and 5 with IV. Types of histology consisted of 8 lymphoblastic and 3 large cell. Treatment was performed for 2 years. Observation periods were from 14 months to 78 months (median 42 months). Twenty-three achieved remission and 6 of them were transplanted with bone marrow or peripheral stem cells in the first remission. The protocol was continued in 17 cases. Fourteen of them remain in first remission, but one died of measles and 2 died of relapse. The 5-year event-free survival was 76.1% for ALL and 65.5% for NHL. In terms of histology, it was 87.5% for lymphoblastic NHL and 33.3% for large cell NHL (p = 0.19). In terms of phenotypes in ALL, it was 88.7% for ALL positive to CD2, 5 and 7, while 2 ALL positive to CD7 alone both failed. Therefore, it was shown that this treatment protocol is very effective for T-lymphoblastic leukemia and lymphoma.

[Peritoneal dissemination of cecal cancer disappearing in a patient taking 5'-DFUR for 3 years].

A 51-year-old woman presenting with a left ovarian tumor underwent left oophorectomy and hysterectomy with intraperitoneal administration of cisplatin, cyclophosphamide, and 5-fluorouracil on December 13, 1989. Microscopically the tumor was well-differentiated adenocarcinoma. Postoperatively cecal cancer was detected. Laparotomy revealed multiple peritoneal dissemination on March 23, 1990, when palliative ileocecal resection was performed. Microscopy revealed several of the nodules to be adenocarcinoma and the cecal lesion well differentiated adenocarcinoma. The patient was then placed on 5'-deoxy-5-fluorouridine (800 subsequently 600 mg/day). In November, 1991, bright red blood per rectum led to the discovery of rectal cancer. On January 26, 1993, laparotomy revealed rectal tumor invading the bladder but the metastatic nodules on the peritoneum had disappeared. Thus pelvic exenteration was performed. The tumor was well-differentiated adenocarcinoma. The ovarian lesion may have been a metastasis from the cecal cancer, a so-called Krukenberg tumor. The rectal cancer was most likely to be asynchronous cancer. 5'-DFUR seemed effective in remitting the peritoneal dissemination.

Four autopsy cases of CNS lymphoma: consideration of unknown causes of death.

The authors present four autopsy cases of primary CNS lymphoma. In each case death was due to different causes. Case 1 was a typical cerebral herniation. Case 2 was brain stem lymphoma diffusely spread after leukemoid reaction. Patient 3 died because of cerebral and brain stem degeneration without evidence of lymphoma cells. Case 4 was unique and the cause of death was not even established by autopsy (definite unknown death case). Taking together these facts, the authors consider the patho-physiology of CNS lymphoma and indicate that herniation is not frequent after various therapeutic treatments.

[Clinico-pathological investigation for infantile brain stem glioma: report of two cases].

Thirty-three children harboring brain stem glioma were treated at Nagoya University, Department of Neurosurgery during in the 16-year period from 1976 to 1991. Infantile brain stem glioma is so rare and we have only two cases (6.1%) of all 33 cases. This paper reported clinico-pathological investigation of infantile brain stem glioma. Case 1: Patient 1 was born after 38 weeks gestation, and he showed no mental nor physical retardation. His parents noticed his torticollis at 4 months of age. MRI showed exophytic abnormal enhanced mass behind the medulla oblongata. The mass was partially removed by craniectomy, and its pathological study revealed astrocytoma grade II. Since 3 months after the operation, torticollis had been gradually improved and disappeared completely 6 months later. Case 2: Patient 2 born after 41 weeks gestation, two cafe-au-lait spots were seen on the abdominal skin and hemangiomas were seen on the left shoulder and femoral area. For the initial symptoms, left oculomotor palsy was recognized at 2 months old. CT showed intrinsic enhanced abnormal mass in the mid brain up to the pons and then it invaded into right frontal lobe soon. Open biopsy was performed and the pathological examination revealed astrocytoma grade III. After IAR therapy (Interferon-beta 140 x 10(4), ACNU 20 mg x 2. Radiation; whole brain 40.8 Gy, focal 9 Gy), although left oculomotor palsy was improved temporarily, the tumor enlarged invasively and the patient died at 11 months old.

Prognosis of intracranial germ cell tumours: effectiveness of chemotherapy with cisplatin and etoposide (CDDP and VP-16).

A co-operative study for patients with intracranial germ cell tumours was performed to analyze their prognosis and the effectiveness of Cisplatin/Etoposide (CDDP/VP-16) chemotherapy. A total of 46 patients; 30 primary and 16 recurrent cases were registered from 15 participating neurosurgical institutions in Japan. Based on histological criteria and tumour markers, they were classified into three groups; germinoma, germinoma with syncytiotrophoblastic giant cell (STGC), and non-germinomatous malignant tumour. Sixteen patients were treated with CDDP/VP-16 chemotherapy alone and the other 30 patients were treated by a combination of surgery and/or radiation in addition to chemotherapy. Eleven out of 13 patients (85%) with germinoma showed a complete (n = 10) or partial (n = 1) response to CDDP/VP-16 chemotherapy even if their tumours were recurrent and there was evidence of CSF dissemination. For the germinoma with STGC and non-germinomatous malignant tumour, a high response rate; 100% for the former and 78% for the latter, could also be achieved in both the primary and the recurrent cases except in those cases of immature teratoma. Their survival times were still different between them. Two-year survival was 50% in germinoma with STGC and 48% in non-germinoma, while it was 88% in germinoma cases.

Recurrent malignant glioma: detection with 131I labeled monoclonal antibody G-22, positron emission tomography and magnetic resonance imaging.

A 45-year-old man with suspected recurrent malignant glioma was evaluated by magnetic resonance imaging (MRI), positron emission tomography (PET) and 131I labeled monoclonal antibody G-22 (G-22) scan. Following Gadolinium-DTPA, a TI-weighted spin echo image (TR 500 msec, TE 20 msec) demonstrated a large mass with an irregular margin in the left temporo-parietal area. An 18F labeled fluorodeoxyglucose PET study demonstrated marked accumulations in the left temporo-parietal area. Serial 131I-G-22 scintigraphy was obtained for a week after the injection. The uptake was most increased on the 2nd day after the injection. 131I G-22 was specific for tumor-associated antigens.

Patterns of hepatitis B virus DNA integration in liver tissue of children with chronic infections.

Although an integration of hepatitis B virus (HBV) DNA in children with chronic HBV infection has been documented at early phases of the disease, the incidence of this process is not known. Therefore we examined nine liver DNA specimens from chronic HBV carriers ages 5-14 years and one sample from a neonate delivered of a carrier mother, in order to determine the HBV DNA patterns of these patients at different ages and phases of chronic infection. The integrated HBV DNA was detected by Southern blot hybridization and analyzed by molecular cloning. Southern blot showed a smear pattern of HBV DNA integration in four of six chronic hepatitis patients (ages 5-14), as well as in one asymptomatic carrier (age 12). Multiple and random integrations occurred during chronic infections in childhood. The neonate did not, however, show signs of any integrations, suggesting that integration starts after HBV multiplication. A band pattern that suggested clonal growth of integrated liver cells was found in a chronic active hepatitis patient (age 9) and in one of two hepatocellular carcinoma patients (age 11). Molecular cloning in two cases with chronic active hepatitis showed that the HBV genome structure was preserved in five of six HBV DNA inserts. Our findings confirm that HBV DNA integration can occur at early stages of chronic HBV infection. In Japanese children, the process of integration seems to be common regardless of HBeAg/anti-HBe status.(ABSTRACT TRUNCATED AT 250 WORDS)
