The communicating branch of the 4th cervical nerve to the brachial plexus: the double constitution, anterior and posterior, of its fibers.

Twenty-four adult cadavers (48 sides) were used to investigate the incidence of a branch arising from the ventral ramus of the fourth cervical nerve (C4) with the phrenic nerve and subsequently joining the brachial plexus. Six brachial plexuses with spinal cords and phrenic nerves were dissected under a surgical microscope to investigate localization of fibers contained in the C4 branch to the brachial plexus. The incidence of the C4 branch was 23% (11/48 sides). Branches from C4 to the brachial plexus divided into anterior and posterior divisions on four sides (4/6 sides). On two sides, the branch did not divide but consisted entirely of an anterior division (2/6 sides). In the brachial plexus, anterior division fibers of the C4 branch were intertwined with fibers from the anterior divisions of the ventral rami of the fifth and sixth cervical nerves. They then passed to the suprascapular nerve and the anterior division of the superior trunk (6/6 sides). On the other hand, posterior division fibers of the C4 branch were intertwined with fibers from the posterior divisions of the ventral rami of the fifth and sixth cervical nerves. They then passed to the suprascapular nerve (2/6 sides) and the posterior division of the superior trunk (4/6 sides). The anterior division of the C4 branch received fibers from the ventral rootlets of the entire fourth cervical segment, whereas the posterior division received fibers from the ventral rootlets of the caudal half of the fourth cervical segment only. The fact that the suprascapular nerve received fibers from both the anterior and posterior divisions of the C4 branch was considered to support our claim that the human suprascapular nerve belongs to both the anterior and posterior divisions of the brachial plexus.

Repair of cardiac defects through a shorter right lateral thoracotomy in children.

BACKGROUND: Median sternotomy is a conventional approach for correction of cardiac defects for many years; however, the cosmetic result is poor. Therefore, right lateral thoracotomy was tested as an alternative procedure with a better cosmetic outcome. METHODS: Between October 1994 and February 1999, 683 patients underwent correction of congenital cardiac malformations during a cardiopulmonary bypass through right lateral thoracotomy involving a shorter incision through the third or the fourth intercostal space. All of the patients were children. The average age was 3.26+/-1.67 years (range, 4 months to 7 years). The average weight was 13.59+/-4.37 kg (5 to 40). The patients had various cardiac defects and associated anomalies. RESULTS: Only 2 patients died after operation, 1 from low cardiac output and the other from severe pulmonary infection. The hospital morbidity was lower. The mean cardiopulmonary bypass time was 58.67+/-35.11 minutes (range, 16 to 430 minutes), the mean aortic cross-clamping time was 35.03+/-24.84 minutes (range, 3 to 205 minutes). The postoperative average mechanical ventilation time was 19.23+/-39.11 hours (range, 2 to 391 hours), and the mean postoperative stay was 8.55+/-12.54 days (range, 5 to 293 days). CONCLUSIONS: The right lateral thoracotomy incision is a safe and effective alternative to a median sternotomy for correction of cardiac defects. Advantages of this approach compared with median sternotomy are less injury, maintenance of the continuity and the integrity of the bony thorax, and prevention of the development of "pigeon-chesting." The scar is less visible, hence, the cosmetic result can meet patient expectations. This procedure is consistent with the idea of minimal invasive surgery.

Critical role of Kupffer cell CR3 (CD11b/CD18) in the clearance of IgM-opsonized erythrocytes or soluble beta-glucan.

Liver macrophages (Kupffer cells) play a major role in blood clearance of both C3-opsonized immune complexes and therapeutic beta-glucan polysaccharides. Human Kupffer cells express three types of C3-receptors: CR1 (C3b-receptor; CD35), CR3 (iC3b- and beta-glucan-receptor), and CR4 (iC3b-receptor; CD11c/CD18). Studies of isolated macrophages have suggested that CR3 is the major receptor mediating capture of either C3-opsonized erythrocytes (E) or beta-glucans. In this investigation, the organ distribution and function of CR3 in the clearance of IgM-opsonized E and soluble CR3-binding polysaccharides were explored in normal vs. CR3-knockout (CR3-KO) mice. Analysis of intravenously (i.v.) injected 125I-anti-CR3 showed that the major vascular reservoir of CR3 was the liver, followed by spleen and lungs. By contrast, clearance of 125I-anti-CR1 appeared to be mediated predominantly by splenic B lymphocytes, as only subsets of splenic macrophages or Kupffer cells were found to express CR1. Clearance of IgM-opsonized 51Cr-E occurred rapidly to the livers of normal mice but was nearly absent in CR3-KO mice. Soluble 125I-beta-glucan exhibited rapid clearance to the liver in normal mice, whereas clearance in CR3-KO mice was significantly reduced. In conclusion, Kupffer cell CR3 plays a crucial role in the clearance of both IgM-opsonized E and beta-glucans.

Optimization of the fluorescence in situ hybridization (FISH) technique for high detection efficiency of very small proportions of target interphase nuclei.

Using commercially available fluorochrome-labeled probes specific for chromosomes X, Y, 13, 18, and 21, we optimized the technical protocols for fluorescence in situ hybridization (FISH) so that the highest sensitivity and specificity were achieved. Also, we compared the optical properties of different types of fluorescent labels in an effort to develop the most efficient FISH protocol, including the determination of which types of labels are the easiest to count accurately. The lymphocytes were purified from blood of normal male and female newborns, normal male and female adults, and a trisomy 21 male adult. Male and female lymphocytes were mixed in five different combinations. For each combination, the male lymphocytes either from newborns or from adults were diluted with female lymphocytes in seven different proportions. For each of these 35 different cell mixtures, 100,000 nuclei were analyzed and scored in a blind fashion. Among the different fluorochrome-labeled probes, the highest sensitivity and specificity were achieved when SpectrumAqua CEP-Y/SpectrumOrange CEP X probe mixture, SpectrumAqua CEP-18, SpectrumOrange LSI-13, and SpectrumOrange LSI-21 were hybridized. The hybridization sensitivity and specificity were higher than 99% for the identification of chromosomes X, Y, 13, and 18, and higher than 98% for the detection of trisomy 21. The proportion of false-positive signals was under 0.005% for XY detection and lower than 0.14% for autosome detection. With these high hybridization sensitivities and specificities, the optimized FISH protocol developed in our laboratory has the potential to detect very rare events, e.g., when the proportion of cells being sought is lower than 0.01%. In other words, our protocol allows the specific detection of one male cell sunken among 10,000 female cells.

Carbon disulfide at a Chinese viscose factory external and internal exposure assessment.

This article presents the results of carbon disulfide exposure measurements in a Chinese viscose rayon factory. The objectives of the study were to identify the external exposure levels at a large factory and to investigate the 2-thiothiazolidine-4-carboxylic acid (TTCA) concentrations in the urine of the subjects who were exposed to carbon disulfide in the working place atmosphere. The metabolism of carbon disulfide in the exposed subjects was also studied in order to demonstrate the best points in time for the internal exposure sampling. The measurement of the amount of personal exposure to carbon disulfide in the air of the workplace was performed by GC-FPD; the presence of TTCA in the workers urine was analyzed by use of a modified HPLC method. The kinetics of TTCA excretion was studied by analyses at different time-points both during and after exposure to carbon disulfide in the subjects. A total of 155 personal samples were obtained. The carbon disulfide concentration in the staple viscose hall was 13.72 +/- 1.12 mg m-3 in terms of the geometric mean +/- geometric standard deviation, and was 20.05 +/- 1.33 mg m-3 in the filament spinning hall. The TTCA values in the subjects who worked in the staple spinning hall were 1.18 +/- 0.43 mg g-1 creatinine and 1.07 +/- 0.38 mg g-1 creatinine for subjects working in the filament spinning hall. The best time for TTCA sampling is at the end of the working shift, the TTCA excretion was stable for a period of 4-12 h after exposure of the subjects to the carbon disulfide. It might be that the Chinese have different anthropometric characteristics; a sampling bias may therefore appear among different races.

Defective radiation signal transduction in ataxia-telangiectasia cells.

PURPOSE: The product of the gene ATM mutated in the human genetic disorder ataxia-telangiectasia (A-T) is predominantly present in the nucleus Compatible with a role in DNA-damage recognition and cell-cycle control. However, ATM is also present outside the nucleus in cytoplasmic and membrane associated vesicles, which may explain the more general signalling defect in A-T. This led us to investigate signalling events initiated by ionizing radiation, remote from the nucleus. MATERIALS AND METHODS: A-T and control lymphoblastoid cells were employed to study radiation-induced signalling at the level of protein activation using immunoprecipitation and immunoblotting. Flow cytometry was used to determine mobilization of intracellular Ca2+. RESULTS: Lymphoblastoid cells from A-T patients were found to be defective in the radiation-induced activation of protein tyrosine kinase p53/p56lyn. In control cells Ca2+ was mobilized in response to gamma-radiation largely from internal stores, and increased significantly over a 20 min period. This mobilization of Ca2+ was either absent or increased very slowly in A-T cells post-irradiation. The same pattern of release was observed after treatment with the radiometric agent, streptonigrin. In addition the phospatidylinositol 3-kinase (PI3-kinase) inhibitor wortmannin suppressed the release of Ca2+. CONCLUSION: These data demonstrate that ionizing radiation activates lyn kinase and leads to the release of Ca2+, and-for the first time-that these steps are ATM-dependent.

[Amplification and sequence of human CD14 gene from human mononuclear cell in peripheral blood].

OBJECTIVE: To introduce a new method for getting human CD14 gene. METHODS: Based on the character of CD14 gene, the authors amplified the CD14 gene directly from genome DNA of human mononuclear cell in peripheral blood by PCR for the first time. RESULTS: A human CD14 gene about 1.1 kb was successfully amplified from genome DNA of human mononuclear cell in peripheral blood, and the sequence analysis has indicated that the CD14 gene cloned in this way is the same as the sequence reported by other authors. CONCLUSION: It is a simple and effective method to obtain human CD14 gene from genome DNA of human mononuclear cell in peripheral blood.

Expression of vascular endothelial growth factor in liver tissues of hepatitis B.

OBJECTIVE: To study the relationship between vascular endothelial growth factor (VEGF) and grading (G) and staging (S) of hepatitis B (HB). METHODS: Immunohistochemical staining was performed with VEGF monoclonal antibody in 250 biopsies of HB. RESULTS: The types of VEGF expression in liver tissues were of plasma type, sinusoidal membrane type and sinusoidal endothelial cell type. VEGF was mainly secreted by hepatocytes, and was also positively expressed in liver sinusoidal endothelial cells, fat-storing cells, myofibroblast and monomorphological nuclear cells. And it was negative in vascular endothelial cells and hepatocytes which had not significant damage, suggesting that the expression of VEGF was enhanced with the increase of G (chi(2)=90.43, P<0.01). VEGF was positively expressed on those whose scores of S were lower, while strongly positively expressed on those whose scores of S were higher. There was also a significant difference between them (chi(2)=115.74, P<0.01). CONCLUSION: The expressions of VEGF are related to G and S of HB

[Phylogenetic relationships and evolution of yeast-like symbionts of the small brown planthoppers based on partial 18S rDNA sequences].

The yeast-like symbionts are isolated and purified from the small brown planthoppers collected from five areas in China. The 18S rDNA sequences of them are determined. Combined with the sequences of other fungi, we have constructed a comprehensive phylogenetic tree. The results suggested that the taxonomic positions of the YLS are in the class Pyrenomycetes in the subphylum Ascomycotina and the YLS have the closest relationship with H. chrysospermus. The YLS of different areas in China and Japan may belong to different geographical populations of one species. We could also draw a conclusion that there are independent acquisitions of various symbionts in differentiated hosts.

[Disorders of hepatic sinusoids and perisinusoidal space in chronic viral hepatitis B and its relationship to hepatic fibrosis].

OBJECTIVE: To explore the disorders of the hepatic sinusoids and perisinusoidal space in chronic viral hepatitis B and its relationship to hepatic fibrosis. METHODS: The histopathology, the expression of smooth muscle actin (SMA), fibronectin (FN), laminin (LN) and collagen type IV (C-IV), and deposition of reticular, collagenous and elastic fibers in the sinusoids and perisinusoidal space in 160 liver biopsy specimens of viral hepatitis B were examined by haematoxyln and eosin (HE), immunohistochemistry and special stains techniques, respectively. RESULTS: The pathological changes of the hepatic sinusoids were collapsing, expansive, occlusive and narrowing. The changes in SMA distribution were characteristic. The spotty activated stellate cells were proliferating, transiting, expressing SMA, and stretching down the wall of sinus or forming fine bundles in G(1) or G(2); they were more active in proliferation, transition, forming bridging and even wide fibrous septa constituted by myofibroblast cells in G(3) or G(4). The other 3 elements, FN, IV-C, LN and reticular, collagenous and elastic fibers deposited in the sinusoids and perisinusoidal space following SMA. The higher the gradings were, the more the depositing elements were. They mainly deposited in the areas of degeneration, inflammation, necroses and fibrous bundles (septa) where the pathologic changes of sinusoids happened. CONCLUSION: The sinusoids and perisinusoidal space is the original sites of fibrosis. Special attention should be paid to the pathological changes in the sinusoids and perisinusoidal space when to evaluate the gradings of histopathology and stagings of hepatic fibrosis.

Rejection of capillary wall background light in an on-column confocal fluorescence detector.

Minimization of the background arising from the capillary wall is of extreme importance in laser induced fluorescence on-column detection for capillary electrophoresis. A sensitive confocal fluorescence detection scheme is described. The intense capillary wall background light was effectively rejected by an adjustable precision slit in a confocal configuration. Rhodamine 640 was used as a standard fluorophore. Detection linearity was demonstrated over four orders of magnitude of concentration and the concentration detection limit was 3.38 x 10(-11) M.

[Primary identification of organic compounds in soybean rhizospheric soil on continuous and alternate cropping and their allelopathy on soybean seed germination].

Organic compounds extracted with ethanol (OCEWE) from soybean rhizospheic soil on continuous and alternate cropping at pod-string were identified by gas chromatography-mass spectrometry (GC-MS). The results showed the compounds mainly include organic acids, alcohol, acetone, aldehyde, naphthalene phenyl and furan hydrocarbon, many of which were reported as allelochemcals. Under this experiment condition, OCEWE did not show any allelopathy on soybean seed germination and radicle growth, which might be related with their critical toxicity concentrations. In addition, the relationship between allelopathy and barrier on soybean continuous and alternate cropping was also discussed.

Safety and immunogenicity of lyophilized, live attenuated hepatitis A vaccine in non-human primate model.

OBJECTIVE: To evaluate the safety and immunogenicity of lyophilized, live attenuated hepatitis A vaccine (H2 strain) in rhesus monkeys. METHODS: Nine adult rhesus monkeys were used as experimental animals. The rhesus monkeys without anti-HAV were divided randomly into the aqueous vaccination group (4 rhesus monkeys), the lyophilized vaccination group (3 rhesus monkeys), and the control group (2 rhesus monkeys). Monkeys were inoculated by intramuscular injection, with control monkeys being inoculated with Minimum Essential Medium Eagle (MEM). Following vaccination, the monkeys were observed for the development of diarrhoea and other adverse side-effects, such as changes in appetite, frequency of defaecation and stool consistency for seven days. At the weeks 2, 3, 4, 6, 8, 10 and 12 positnoculation, the peripheral blood was collected from all animals and assayed for anti-HAV and alanine aminotransferase (ALT) and aspartate aminotransferase (AST), at weeks 0, 4 and 8 postinocuation, needle-biopsy specimens were taken at weeks 0, 4, 8 and 12, all monkeys were sacrificed and tissue samples were taken from liver, lung, heart, kidney and brain for pathological examination at week 12. RESULTS: Animals were immunized with a dose of 7.0 logTCID50/ml which is stable after freeze-drying. During the 12-week observation, no animals showed abnormal elevations of liver enzymes (ALT and AST) and no change in appetite or activity. Two monkeys (one in the aqueous group and the other in lyophilized group) showed possible lesions at week 8. The lyophilized vaccine, in addition to eliciting an anti-HAV IgG response similar to aqueous vaccine (P > 0.05), also showed IgM anti-HAV response at week 2 which was not observed with aqueous vaccine. CONCLUSIONS: These results demonstrate that lyophilized, live hepatitis A vaccine is safe and highly immunogenic in primates, supporting its further evaluation in human clinical studies.

Advanced glycosylation end products increase diacylglycerol levels in cultured human umbilical vein endothelial cells.

OBJECTIVE: To study whether the diacylglycerol (Dia) signaling pathway is stimulated by advanced glycosylation end products (AGEP) and to test the effect of vitamin E and aminoguanidine (AG) on the elevation of Dia induced by AGEP in cultured human umbilical vein endothelial cells (HUVECs). METHODS: The effects of AGEP on Dia levels in cultured HUVEC were studied with radio-enzymatic assay. Quantitative measurements of 32P phosphatidic acid were achieved by thin-layer chromatography and autoradiography. RESULTS: The Dia levels in HUVECs were increased by AGEP modified bovine serum albumin (AGEP-BSA) in a dose-dependent, biphasic manner. The early phase was rapid and transient, peaking at 15 s; the late phase reached the maximal level at 10 min and then decayed slowly. Dia levels in HUVEC exposed to different concentrations (50, 100 and 200 mg/L) of AGEP-BSA (341 +/- 14, 678 +/- 16, and 873 +/- 18 pmol/L, respectively vs control 225 +/- 10 pmol/L) and AGEP-BSA samples with various glycosylation times (4, 8 and 12 weeks) were significantly increased (270 +/- 12, 394 +/- 16, and 556 +/- 19 pmol/L) as compared with the controls. 50 and 100 mmol/L of vitamin E can reduce AGEP-BSA-induced Dia levels from 873 +/- 18 pmol/L to 764 +/- 29 and 441 +/- 21 pmol/L in HUVEC, respectively. In AG-treated (100 mmol/L) groups, the same concentration (100 and 200 mg/L) of AGEP-BSA-induced elevation of Dia was decreased to 312 +/- 8 and 351 +/- 13 pmol/L, respectively. Glycosylated low-density lipoprotein (LDL) did not affect Dia levels. CONCLUSION: AGEP causes a robust stimulation of the Dia/protein kinase C pathway in HUVEC. Vitamin E can attenuate the AGEP-BSA-induced elevation of Dia levels. AG can suppress the ability of AGEP-BSA to increase Dia levels in HUVEC.

[Establishment of in vitro serum-free embryo implantation model].

OBJECTIVE: To observe the growth and implantation of mouse embryos in vitro. METHODS: In order to establish a serum-free coculture system, the whole components of decidual tissue was collected and cultured in vitro. Then 4-day-mouse embryos were added to the well-developed decidual cells, and the following changes of the above system were closely observed under microscope. RESULTS: Mouse embryos were successfully hatched, adhered, implanted and outspreaded under the coculture system. The extraplacental cone and amniotic sac were formed as well. Decidual cells kept stable during the following 4-day coculture. CONCLUSIONS: Decidual cell is an ideal feeder cell for coculture. Our results showed that the modified method of decidual cell culture and the establishment of implantation model were simple, practical and reliable.

[The survey of prevalence of HIV infection among paid blood donors in one county in China].

OBJECTIVE: To estimate the prevalence rate of HIV infection among paid blood donors and to analyse high risk factors. METHOD: All residents between 18 to 60 years old were selected from on county in March 1999. RESULTS: There were 210 paid blood donors among 730 interviewed subjects. The HIV prevalence rate among paid blood donors was higher (9.1% or 19/210) than that of others (0.6% or 3/520). The HIV prevalence rate was higher in plasma donors (25.9%) than that (2.6%) among 210 blood donors. Donors who donated blood/plasma more than 10 times each year had a higher prevalence rate (13.5%) than those who donated less than 10 times each year (2.8%). Donors who had donated blood but stopped donation before 1995 had a higher prevalence rate (18.3%) than those who started donation after 1996 (1.4%). The results of KAP survey showed that blood donors had a low knowledge about AIDS. CONCLUSION: There was a higher prevalence rate of HIV infection among paid blood donors in some regions. It should take measures to prevent further transmission HIV through paid blood donors.

[Studies on schedules for human rabies immunization].

OBJECTIVES: To study the feasibility of interferon (INF) plus an immunization schedule of two dose-two dose-one dose (2 - 2 - 1) of rabies vaccine and a simple 2 - 2 - 1 schedule, and to explore immune regulation mechanism of INF. METHODS: For an IFN plus 2 - 2 - 1 schedule, 5.0, 5.0 and 2.5 I.U. of primary hamster kidney cell rabies vaccine (PHKCV) were injected on the 1st, 7th and 14th day, respectively, and 0.2 million I.U. of IFN-alpha on the 1st day simultaneously. For a simple 2 - 2 - 1 schedule, only 5.0, 5.0 and 2.5 I.U. PHKCV were injected on the 1st, 7th and 14th day, respectively. And, for controls, a routine schedule recommended by WHO was used. RESULTS: On the 7th day after immunization, their geometric mean titer (GMT) of antibody was 1.71, 1.57 and 1.21 for the above three schedule groups, respectively; on the 14th day, 62.77, 58.79 and 28.96; on the 45th day, 76.64, 72.90 and 62.22. Conversion rate of antibody was 85.00%, 61.11% and 65.00%, respectively on the 7th day, and was 100.00% for all on the 14th day. Proportion of those with protective antibody level was 10.00%, 0.00% and 0.00%, respectively for the three schedule groups, on the 7th day; 100.00%, 100.00% and 95.00%, on the 14th day; and was 100.00% for all on the 45th day. Proportion of those with antibody titer more than 1:1 was 55.00%, 33.33% and 20.00%, respectively, on the 7th day. Adverse reaction rate was 0.00% - 55.00%, without significant difference between three schedule groups. CONCLUSIONS: Lower dose of IFN could enhance the effect of immune response to rabies vaccine. IFN plus 2 - 2 - 1 rabies vaccine schedule was better than that of simple 2 - 2 - 1 schedule, while the later was better than the routine schedule.
