Ontogeny of pepsin secretory response to secretagogues in isolated rat gastric glands.

By use of isolated gastric glands from rats at various ages, we demonstrated that full-term neonate and 1-day-old rats showed no response to cholecystokinin octapeptide (CCK-OP), carbachol, or Ca2+ ionophore. The same glands, however, were responsive to dibutyryl cAMP. A mature response was not found until the pups were 2 days old. Injection of hydrocortisone into newborn rats led to an increase in pepsinogen concentrations in gastric glands and also an increased responsiveness to CCK-OP, carbachol, and Ca2+ ionophore A23187 24 h after administration. Hydrocortisone thus caused precocious maturation of both pepsinogen accumulation and pepsinogen secretory responsiveness of gastric glands in rat pups.

Free pleuroperitoneal liquid movement indicating diaphragmatic tear.

An unusual radiological feature, not mentioned in the literature, is reported. It has been observed in a patient with traumatic rupture of the diaphragm associated with intra-abdominal bleeding. This injury produced a free pleuroperitoneal movement of liquid demonstrated in the supine and erect chest radiographs. Since there is an associated abdominal injury in most cases of rupture of the diaphragm, this radiological sign might be more common. The importance of supine and erect chest radiography is reinforced in cases of multiple injuries.

Assessment of intestinal and cardiorespiratory function in children with congenital heart disease on high-caloric formulas.

Fourteen infants with congenital heart disease were investigated for failure to thrive. Assessment of intestinal function revealed minor absorptive abnormalities (mild steatorrhea in three patients, bile salt loss in four patients), delayed gastric emptying, and abnormal triglyceride loading tests. Low caloric intake (88.3 +/- 19.3 kcal/kg/day) seemed the main reason for failure to gain weight. Weight accession and cardiorespiratory rates were monitored daily during voluntary intake, a high-caloric diet by mouth, and nasogastric tube feeding. Providing 169 +/- 29 kcal/kg/day by tube resulted in weight gain with mild and transient elevation of respiratory rate at the end of the meal and increased heart rate 90 min after the meal. This regimen is a metabolically inexpensive and efficient method of supporting weight gain in children with congenital heart disease.

Causes of diarrheal disease in soldiers stationed in Lebanon and in northern Israel.

A study of acute gastroenteritis in soldiers was carried out during a 1-year period. Causes of diarrhea were compared between 33 soldiers stationed in southern Lebanon and 174 soldiers stationed in northern Israel. Shigella sp. caused 42.4% of diarrheal disease in Lebanon, compared with only 19.0% of cases in Israel. Sh. flexneri was a more common cause of bacillary dysentery in Lebanon than in Israel (71.4 vs. 18.2%, respectively), and caused a more severe illness.

Dose-response characteristics of nebulized fenoterol in asthmatic children.

Fenoterol hydrobromide, a beta 2-selective bronchodilator, was administered by aqueous nebulization to 31 children with stable asthma. An initial comparison of 0, 100, 300, and 1000 micrograms drug in 20 of these patients showed a significant change in forced expiratory volume in 1 second for all three doses compared with change after placebo (P less than 0.0001). However, the differences in peak pulmonary response from 100 to 1000 micrograms were not large (P greater than 0.2). Assessment of spirometric responses of 11 children to 3, 10, 30, or 100 micrograms nebulized fenoterol clearly revealed the dose-response effect (P less than 0.01). When all the FEV1 data were plotted over the entire range of 3 to 1000 micrograms, the resultant log dose vs response curve could be characterized by the ED50, the amount of drug producing half-maximal response. At 15, 30, or 60 minutes, the ED50 was in the range 8 to 10 micrograms. With increasing time there was a parallel shift of the entire dose response curve to the right, manifested by ED50 of 47 and 150 micrograms at two and three hours, respectively, after administration. This decreasing potency of a sympathomimetic drug with time shows that duration of effect and dosage are interdependent variables and must be evaluated simultaneously. Such considerations cannot be derived from cumulative dose-response studies. In our patients, 100 to 300 micrograms fenoterol delivered by aqueous nebulization achieved optimal bronchodilation with no detectable cardiovascular side effects.

Gluten-sensitive enteropathy: value of oral triglyceride loading test in the follow-up of patients on gluten challenge.

Oral triglyceride (TG) loading tests were performed in 31 patients with gluten-sensitive enteropathy (GSE) upon gluten challenge, and the results were compared with the mucosal histology. Of 19 prechallenge tests, 16 were in the normal range previously established in this laboratory (2-h postcibal plasma TG rise greater than or equal to 55 mg/dl) although values were lower (76 +/- 38 mg/dl, mean +/- SD) than those of healthy age-matched controls (116 +/- 34 mg/dl). Postchallenge tests performed in 29 patients (both symptomatic and asymptomatic) were all pathologic (postcibal TG rise 9 +/- 18 mg/dl). Three tests performed in two patients in whom GSE was ultimately not confirmed remained normal (postcibal TG rise 82 +/- 21 mg/dl) during a 24-mo follow-up. A significant correlation was demonstrated between the TG loading test and the mucosal pathology. The test is useful in the follow-up of patients of gluten challenge and in directing the postchallenge biopsy.

Clinical features of viral and bacterial gastroenteritis in hospitalized children.

The clinical course of 119 children, hospitalized with acute gastroenteritis of known etiology, was studied. The severity of disease was measured by a scoring system that showed that the disease caused by Escherichia coli was the most severe, followed in decreasing order by Salmonella, human reovirus-like agent (HRLA) and Shigella. The clinical features of salmonella and shigella infections differed from those reported in other countries. Fifty percent of all patients recovered after 5 days of treatment, but 20% of the patients with bacterial infections continued to have symptoms for 10 days or longer. The relationships between etiological agents and various symptoms were measured by odds ratios. These associations may provide a basis for clinical prediction of the etiological agent.

Ketamine anesthesia for pediatric gastrointestinal procedures.

Ketamine HCl anesthesia was used for gastrointestinal therapeutic and diagnostic procedures in 26 children aged 3 mo to 16 yr. In addition, atropine was used for the gastroscopies and diazepam for the colonoscopies. Ketamine, 5 to 8 mg/kg body wt administered i.m., produced rapid anesthesia of short duration which allowed easy performance of the procedure. No serious side effects were noted, but care must be taken as ketamine is a general anesthetic with potential side effects during and after the period of anesthesia. This satisfactory initial experience should be confirmed by a controlled study of anesthetic agents for pediatric gastrointestinal procedures.

The validity of the transcutaneous oxygen tension method in children with cardiorespiratory problems.

Transcutaneous oxygen tension (tcPO2) was continuously monitored and compared with simultaneous arterial oxygen tension (PaO2) in 68 patients 1.5 to 23 yr of age (mean age, 7.7 yr) with cardiopulmonary disease. Two groups of patients were studied: (1) Patients during cardiac catheterization (56) in whom the PaO2 ranged from 34 to 98 mmHg (mean, 72.4 +/- 17 mmHg) and (2) patients in the Intensive Care Unit (12) in whom the PaO2 ranged from 54 to 158 mmHg (mean, 106 +/- 37 mmHg). Studies lasted for 30 to 210 min (mean, 80 +/- 33.4 min). The overall relation between tcPO2 and PaO2 in the 68 patients studied was r = 0.96 (p less than 0.001). These results indicated that tcPO2 measurement is a reliable method for assessing low as well as high PaO2 values in children and young adults without gross circulatory impairment.

Prevalence of serum antibody to reoviruslike agent in hospital patients in Israel.

The prevalence of antibody to reoviruslike agent was studied in serum from 350 hospitalized patients in the Tel Aviv area by the complement-fixation technique, with calf diarrhea virus as the antigen. There were 217 (62%) serum samples that showed evidence of antibody to reoviruslike agent. The highest prevalence was noted in postpartum women (95%) and in infants under one month of age (93%), the latter probably as the result of placental transfer. The presence of antibody declined rapidly in the first six months of life and rose sharply after this age. These findings accord with the high incidence of reoviruslike agent infection in these age groups, as previously reported. Above the age of three years, the prevalence of detectable antibody remained stable, ranging from 54.5 to 81.0% in various age groups.
