[Survey of smoking and human health in Guangdong province].

As part of the national plan in China, 28,979 residents or farmers were surveyed in 1991 in Guangdong province to investigate the relation between smoking and human health. The results showed that the total smoking rate was 40.2%, 58.8% was for male and 1.2% was for female. 56.4% was in city and 38.7% was in rural area. The beginning smoking age was 15-16 years old (5.5%). 8.5% smokers started smoking after 25 years old. Smoking can increase risk of diseases. The incidence of chronic bronchitis, hypertension, gastric ulcer and pulmonary TB in smoking group were significantly higher than that in non-smoking group. No difference was found in the symptoms of panting, cough and phlegm between the two groups. To control smoking is one of the important measures for human health.

[Detection of circulating autoantibodies to beta 2-adrenergic receptors in patients with asthma].

To understand the roles of autoantibodies to beta 2 adrenergic receptors in the pathogenesis of asthma, we investigated the positive chronotropic action of beta 2-selective adrenergic agonist, clenbuterol, on cultured neonatal rat cardiomyocytes. Thereafter, we detected the autoantibodies to beta 2-adrenergic receptors in the sera from patients with asthma as it could inhibit the positive chronotropic action of clenbuterol. In the sera of all patients with asthma (16 cases) there were autoantibodies to beta 2-adrenergic receptors, but not in the normal controls (20 cases). Further study showed that the inhibitory autoantibodies were IgG type. This experiment suggests that the autoantibodies to beta 2-adrenergic receptors may play an important role in the pathogenesis of asthma.

[Clinical significance of microtransferrinuria in diabetic patients].

We studied the relationship between the urinary microtransferrin (TRF) and early glomerular damage in diabetes mellitus. 61 patients with non-insulin-dependent diabetes mellitus and 40 healthy subjects were measured for urinary TRF with rate immunonephelometry assay. The Urinary TRF concentrations were found to be greatly elevated in patients with diabetes compared with those in the health subjects (P < 0.001). The increase of TRF was closely related to age and duration of diabetes mellitus, blood glucose, hypertension, retinopathy, and it was initial parameter to predict diabetic nephropathy. There was a significant correlation among urinary TRF concentration, microalbumin, and N-acetyl-beta-D-glucosaminidase. The urinary excretion of TRF was more elevated than that of microalbumin. It is suggested that excretion of TRF in urine is a more sensitive index of the glomerular dysfunction in diabetes mellitus.

[Clinical analysis of 7 cases of mesenteric venous thrombosis].

7 patients with mesenteric venous thrombosis (MVT) were treated in Anzhen Hospital from 1984 to 1996. The patients were all operated on and the diagnosis was confirmed pathologically. Fine patients were cured and 2 died. We discussed the pathophsiology, pathogeny, clinical manifestation, diagnosis and treatment of MVT. Most of the patients presented with a clinical picture of acute abdomen and the diagnosis was made only at laparotomy. Close observation and proper laparotomy should be given to the doubtful cases. To reduce the mortality rate of MVT, we resected the bowel which looks normal in appearance but comprises the mesentery of thrombosis. Adequate anticoagulant therapy should be given as early as possible.

[Histological and ultrastructural studies of experimental herpetic keratitis in the rabbit].

OBJECTIVE: In order to investigate the histological and ultrastructural changes at different post infection days in herpes simplex keratitis (HSK). METHODS: The corneas of New Zealand white rabbit eyes experimentally infected with primary HSK were studied by light microscopy and electron microscopy. The histological and ultrastructural changes in the rabbit corneas at 3, 7, 14, 21, 60 post infection days were investigated by light microscopy with HE, Mallory dyes and electron microscopy. RESULTS: Infiltration of corneas with inflammatory cells was found in rabbit eyes infected with HSK, polymorphonuclear leukocytes being predominant. Contact of inflammatory cells with fibroblast cells and corneal edema were seen in the stroma on acute post infection days 3 and 7. In severe cases inflammatory cells incarcerated in the endothelium. Only did two cases of the specimens present with herpes simplex virus type I granules in the acute stage under electron microscopy. CONCLUSION: The direct HSK-1 destruction together with immunopathogenetic injury caused by the body immune system may serve as the pathologic base of HSK. Decompensation of corneal endothelial function due to infiltration with inflammatory cells may be responsible for corneal edema.

[Investigation of C1R gene frequencies in three Han populations in China].

To reveal the C1R polymorphism in Chinese, three Han populations in Guangzhou (101 samples), Jilin (105 samples) and Chengdu (111 samples) were investigated with a technique using PAGIF followed by immunoblotting. The results showed in Chengdu the C1R * 1 = 0.5676, C1R * 2 = 0.3424 and C1R * 5 = 0.0856, in Guangzhou C1R * 1 = 0.5248, C1R * 2 = 0.2663 and C1R * 5 = 0.1089, and in Jilin C1R * 1 = 0.5381, C1R * 2 = 0.2619 and C1R * 5 = 0.1714. Three rare genes C1R * 6, C1R * 7 and C1R * 8 were found in the investigation. These indicate that the frequency of C1R * 2 is elevated from north to south which may imply a geographic cline in this locus. The cumulated heterogeneity of C1R in Han population is 61.5% which means that this polymorphic system is useful in anthropolgy as well as in forensic science.

[Analysis and prevention of road traffic fatalities in Sichuan Province].

This is an epidemiological survey of the fatalities from road traffic accidents in Sichuan. All fatal accidents that occurred in Sichuan from January 1994 to December 1994 were analysed. The mortalities per million kilometers of vehicle travel, per 100,000 registered motor vehicles and per 100,000 resident population were 4.35, 42.41 and 3.92 respectively. These figures were compared with international death rates and those reported by other countries. The results showed that Sichuan had higher mortalities per million kilometers of vehicle travel and per 100,000 registered vehicles but lower mortality per 100,000 resident population, compared with some industrialized and rich develeping countries. The majority of the victims were males aged 21 to 40. Road traffic fatalities constituted the leading cause of all accident deaths. The cost of road accidents in Sichuan was estimated to be sixty million Yuan. Further strategies and methods to minimize the rate of such accidents have been suggested.

[In situ DNA labeling apoptosis in breast cancer as related to prognosis].

OBJECTIVE: This study was undertaken to determine the expression of apoptosis in breast cancer and to evaluate it's significance as a prognostic marker. METHODS: A series of 91 invasive breast cancer was analysed for the expression of apoptosis by using the 3-end-labeling method of DNA in tissue sections. The apoptotic indexes were the percentages of apoptotic cells among tumor cells. RESULTS: The end-labeling method allowed a precise evaluation of the expression of apoptosis. Apoptosis occurred in 91.2% of breast cancer patients, and apoptotic indexes were divided into two groups, 0-0.21 and 0.28-0.62. Low apoptotic index was related to axillary lymph node metastasis (P < 0.01). In survival analysis, higher apoptotic index was related to disease free survival (P = 0.0095) and overall survival (P = 0.0348) in the entire cohort. Cox's analysis showed apoptotic index had no independent prognostic value. CONCLUSION: The apoptosis was a spontaneous phenomenon in breast cancer tissue, and the expression was different from each other. Further analysis was needed to clarify the relationship between apoptosis and prognosis, especially the response to adjuvent therapy.

[Endoscopic and CT evaluation of three types of intranasal ethmoidectomies].

To evaluate the curative effects and complications of conventional, microscopic and endoscopic intranasal ethmoidectomuies, 130 cases of sinusitis and polys (stage II) were operated on with intranasal ethmoidectomies. There were 40 conventional, 40 microscopic, and 50 endoscopic ethmoidectomies, followed-up with endoscopy and CT scanning. The results showed that 1. the relieve of symptoms in the endoscopic group(94.0%) was obviously better than the conventional (60.0%) and the microscopic groups (70.0%, P < 0.01): 2. the clinical cure rate in the endoscopic group (86.0%) was obviously higher than the conventional (40.0%) and the microscopic groups (52.5%, P < 0.01); the CT scans showed a normal ratio in the endoscopic group(82.0%) obviously higher than those in the conventional (22.5%) and microscopic groups (37.5%, P < 0.01); 3. the complication ratio in the endoscopic group(12.0%) was slightly higher than those in the conventional(5.0%) and the microscopic groups (2.5%, P > 0.05). The intranasal endoscopic ethmoidectomy was more successful than the conventional and microscopic ethmoidectomies, but attention should be paid to the possibility of complications.

[Surgical treatment of the condylar osteoma].

The purpose of the paper is to present different procedures for condylar osteoma surgery and their indications. Five surgical procedures were used for the treatment of 23 cases of condylar osteoma: 1. Simple resection of the condyle and osteoma for 5 cases. 2. EVRO of the effected side and resection of the condyle and osteoma for 12 cases. 3. Procedure 2 plus IVRO of the contra-lateral side for 2 cases. 4. LeFort I osteotomy for correction of the upper occlusal plane, vertical ramus osteotomy bilaterally and genioplasty for 5 cases. 5. One stage resection of the huge condylar osteoma, and repair of the defective ramus in second stage. It is emphasized that good function is based on the normal anatomical relationship of jaws and TMJs, so simple resection of the condyle and osteoma without repair is not an ideal surgery. Various orthognathic surgical techniques could be used in the therapy of condylar osteoma. While the deformity involves maxilla, two jaw surgery is indicated for better results on function and appearance.

[The role of calcium on parotid cell differentiation and secretion in vitro].

To study the role of calcium on parotid cell differentiation and secretion, high extracellular calcium was used to treat the parotid gland cells in culture. Immunohistochemical staining was applied to examine the expression of proteins involved in cell-cell interaction, cell differentiation and parotid gland secretary proteins. The results demonstrated that the expression of CAM and Cx32 increased and more cells had positive reaction to c-jun oncoprotein. No effect of calcium on the expression of EGF-gamma, c-fos, alpha-amylase and PRP was observed. The data indicate that extracellular calcium play an important role on cells differentiation. However, it does not affect the cell secretion.

Stable expression of human cytochrome CYP2B6 and CYP1A1 in Chinese hamster CHL cells: their use in micronucleus assays.

With specific designed primers, CYP2B6 and CYP1A1 cDNA were generated by reverse transcription-polymerase chain reaction(RT-PCR) technique performed on total RNAs isolated from human liver and 3-methylcholanthrene(3-MC) induced human amnion FL cells. Cell lines (CHL-2B6 and CHL-1A1) capable of expressing human cytochrome P450(CYP)2B6 and 1A1 were established after transfection of corresponding eukaryotic recombinant expression plasmid with human CYP2B6 and 1A1 cDNA inserts respectively. These cell lines stably expressed the mRNAs and the enzymatic activities corresponding to CYP2B6 and CYP1A1, respectively. Compared with Chinese hamster 1ung(CHL) cells, the micronucleus frequency in CHL-2B6 cells is markedly increased when exposed to nitrosamines, aflatoxin B1(AFB1) and cyclophosphamide (CPA). This is also in CHL-1A1 cells, when exposed to carcinogenic polycyclic aromatic hydrocarbons.

[Physiological effects of cabin temperature decrease on human].

To study effects of cabin temperature on human body, changes in rectal temperature, local skin temperature, local skin heat fluxes and skin infrared thermogram were observed in 5 male subjects dressed in different thermal insulation clothings under 20 degrees C, 15 degrees C and 10 degrees C cabin temperatures. The results showed that, average skin temperature and tip temperature obviously dropped, and skin heat fluxes increased with falling of cabin temperature, and changes of local skin infrared thermogram were significant. Subjective feeling were comfortable and cool at cabin temperature of 20 degrees C and 15 degrees C respectively. While subjective feeling was cold and there was a thermal debt of body 144 kJ/m2 at 10 degrees C which indicated that it was slightly cold but tolerable. The results may be valuable in determining the lowest tolerable limit of cabin temperature.

[Comparison of evaluation methods on changes of facial microcirculation during induction of motion sickness].

In order to increase the accuracy of motion sickness grading criteria, infrared thermography and Laser Doppler microcirculation blood-flow detector were used in 60 healthy male individuals: before, immediately, and 20 min after paralleled-swing stimulation. Facial skin temperature and microvessel blood-flow were recorded. The results showed that facial skin temperature and blood flow in facial microvessels fell together with the appearance of pallor. However, the degree of pallor observed with naked eye was inconsistent with that found by thermography or facial blood flow. It suggests that the accuracy of grading might be improved with the use of thermography or measurement of facial microvessel blood flow.
