Aspects of "reality" and ways of knowing in nursing: in search of an integrating paradigm.

Types of knowledge, theory and research needed in a practice-oriented discipline are considered in relation to different "aspects of reality." Major changes in nursing epistemology are reviewed to show an expanding model of inquiry. It is argued that a paradigm which recognizes a spectrum of reality from "body" through "mind" to "spirit" provides a philosophical basis for understanding the necessity of multiple modes of inquiry and multiple types of knowledge and theory. From this perspective, fundamentally different modes of inquiry are seen as complementary rather than competitive or inherently superior or inferior.

An experimental evaluation of a model child life program.

A model child life program for hospitalized children between the ages of 3 and 13 was systematically developed and tested on a large number of stress, coping, adjustment, and surgical recovery variables. The results indicated that children who participated in the experimental program (N = 68) scored significantly better on 18 of the 21 outcome variables than did control children (N = 160) who did not participate in a full child life program. As in previous research and within the limitations of a quasiexperimental design, the results suggest that this type of systematic child life care has a significant, positive impact on hospitalized children.

Relationship of declines in grain consumption and milk yield to estrus in dairy cattle.

A retrospective study was done to determine if a reduction in daily grain consumption or decline in daily milk yield were accurate indicators of estrus in 25 Holstein cows. A computer automatically identified all cows that left more than 10% of their daily grain allocation in the feed stall or that gave less than 85% of their most current 7-d daily average milk yeild. By retrospectively analyzing breeding records, grain refusal and milk declines were identified as either occurring in conjunction with estrus or nonestrus. Declines in grain consumption and milk yield were frequently associated with estrus, but many cows in estrus did not exhibit such declines. While such declines hold promise as estrous detection aids, it was concluded that refusals of daily grain allocation or declines in daily milk yield were not sufficiently accurate to supplant other methods of estrous detection in dairy cattle.

The effects of stress on children's human figure drawings.

Investigated the effect of stress and preparation for stress on emotional indicators (EIs) of 68 children, aged 4 to 12 years, who were hospitalized for elective surgery. All children were asked twice to draw a human figure, once shortly after admission and again 90 minutes later. In the interim, they received stress (venipuncture or "blood test") or no stress (venipuncture delayed until after second drawing), and were either prepared (information, rehearsal and supportive care) or not prepared (free play) for the stressful situation. EIs increased only in the group tor mental age score of drawing. Pre- and post-measures of coping behavior and pulse also were assessed. EIs correlated with pulse taken at the time of the venipuncture. Examiners who interpret drawings by this method must consider the effect of current stress and the child's understanding of stress.
