Diabetic nephropathy as the mode of presentation of diabetes mellitus.

Diabetic nephropathy have only rarely been described in patients who have minimal or no glucose intolerance. We herein report the case of a 59-yr-old man who presented with nephrotic syndrome and minimal glucose intolerance whose renal biopsy showed the nodular (Kimmelsteil-Wilson) and diffuse glomerulosclerosis lesions characteristic of diabetes. We critically review the literature on this subject, pointing out the pitfalls in diagnosis and establishing strict criteria for the diagnosis of diabetic nephropathy in patients wihout overt clinical diabetes.

Natriuretic effect of propranolol on dogs with chronic bile-duct ligation.

1. Chronic ligation of the bile duct in dogs is associated with salt retention and a blunted natriuretic response to extracellular volume expansion. The mechanism of this phenomenon has not been clarified. 2. We have examined the influence of chronic beta-adrenergic blockade on sodium excretion in dogs with bile-duct ligation during extracellular hypotonic volume expansion. 3. Urinary excretion of sodium and fractional excretion of sodium rose significantly after 5 days of oral DL-propranolol administration to dogs with bile-duct ligation. 4. The antinatriuresis after bile-duct ligation was not followed by a significant alteration in the mean peripheral plasma renin activity as compared with control values. 5. It is suggested that propranolol can partially reverse the antinatriuresis of chronic bile-duct ligation, and that this is mediated by an extrarenal effect of the beta-adrenergic blockade.

Improvement in the anemia of hemodialyzed patients following subtotal parathyroidectomy. Evidence for the role of secondary hyperparathyroidism in the etiology of the anemia of chronic renal failure.

An increase in the blood hemoglobin concentration was found in four of five patients on chronic hemodialysis following subtotal parathyroidectomy performed for the treatment of incapacitating osteodystrophy. In a sixth hemodialyzed patient, there was a striking increase in the hemoglobin level following treatment with 1alpha-hydroxycholecalciferol with suppressed parathyroid overactivity. It is concluded that surgical or "chemical" parathyroidectomy seems to be associated with improvement of the anemia of patients with chronic renal failure.

Renal tubular acidosis due to the milk-alkali syndrome.

A 60-year-old man with a history of excessive ingestion of calcium carbonate presented with azotemia, hypercalcemia and hyperphosphatemia. His acid-base status was initially normal. Following the cessation of calcium carbonate treatment, the hypercalcemia and azotemia disappeared, and the patient was found to be in metabolic acidosis with blunted acid excretion and a urine pH of 6.1. Kidney biopsy showed focal tubular calcification; the tubular damage was apparently caused by hypercalcemia and had resulted in renal tubular acidosis. During the three months of observation since that time there has been a tendecy for spontaneous remission of the renal tubular acidosis. Impaired renal hydrogen ion excretion prevented the development of metabolic alkalosis despite ingestion of alkali initially, and was later responsible for the metabolic acidosis. Renal tubular acidosis occurring as a sequel to the milk-alkali syndrome may aggravate the danger of nephrocalcinosis in this syndrome.

Atrial natriuretic peptide in relative polycythemia and polycythemia vera.

Atrial natriuretic peptide (ANP) levels have been tested in patients with chronic relative polycythemia (RP), polycythemia vera, and in healthy subjects in order to find a possible underlying pathophysiological mechanism for relative polycythemia. No difference in statistical significance has been found between the mean atrial natriuretic peptide levels of the polycythemic patients and the control group. It is suggested that ANP probably plays no significant role in the contraction of plasma volume observed in normotensive patients with RP.
