IgA deficiency, epilepsy, and phenytoin treatment.

In a prospective study of thirty-two children with seizures treated with phenytoin (diphenylhydantoin), five had low levels of serum-IgA before treatment. All of these were among the fifteen who had had febrile convulsions in infancy. IgA levels fell significantly during 6 months treatment in the fourteen patients studied sequentially. Treated children with low serum-IgA had normal numbers of lymphocytes with surface IgA. This suggests that phenytoin causes failure of terminal differentiation of B lymphocytes, and is the first known cause of this, the commonest mechanism of immunoglobulin deficiency.

Plasma corticotrophin levels in addison-schilder's disease.

Raised plasma immunoreactive corticotrophin (ACTH) levels were found in five boys with the sex-linked disorder progressive leucodystrophy associated with adrenal insufficiency (Addison-Schilder's disease) and in a symptom-free brother of one of them. Similar ACTH concentrations were found using two antisera, one against the N-terminal part of the ACTH molecule and the other against the C-terminal part. In one patient the circulating ACTH had normal biological activity as measured using the cytochemical ACTH bioassay. Immunoreactive beta/-melanocyte-stimulating hormone was also determined in one patient and found to be raised.

Narcotic analgesics and delayed gastric emptying during labour.

The rate of gastric emptying in women during labour was estimated indirectly from the kinetics of absorption of orally administered paracetamol (acetaminophen). Gastric emptying was normal in patients who had not received a narcotic analgesic but was markedly delayed in women given pethidine, diamorphine, or pentazocine. The inhibitory effect of diamorphine and pethidine on gastric emptying was not reversed by metoclopramide.

[Study on the inhibition of testicular polyadenylate polymerase stimulated by manganese].

Many different types of naturally occurring substances and of drugs were examined as potential inhibitors of a Mn++-stimulated and poly(A)-primed polyadenylate polymerase that was partially purified from rat testis. By far the most active inhibitors were anionic dyes that were either of the diazo naphthyl sulfonate category, such as Evans Blue, or were halogenated derivatives of fluorescein, like Rose Bengal. The characteristics of these in vitro inhibitions, which did not involve any photo-sensitized reactions, were examined in considerable detail.

Operant conditioning and behavioral analyses using a minicomputer.

An operant conditioning and data analysis software system was developed for use on a PDP-12A minicomputer. The operant software functions in quasi time-sharing fashion to control and acquire data from any peripheral device that operates in the binary mode. In addition to independently running different experiments in near simultaneous fashion, the program also provides information on the current status of each experiment using a cathode ray-tube display. Response data from each experimental subject is stored on magnetic tape and analyzed, off-line, by the data analysis portion of the software system. A discussion of the operation of this system is given for one possible application: visual discrimination training.

A minicomputer interface for realtime operations: an application to operant conditioning.

A PDP-12 interface was designed, constructed, and tested for realtime imput and output of binary information. Within limits this interface can be used with any peripheral device which operates in the binary mode. In addition to its generality of application the interface features include ease of expansion and low cost. A description of its design and operation is give here is terms of a typical application: the control of behavioral equipment (i.e. "Skinner Boxes") for operant conditioning.

Zebra body myopathy. Clinical, histochemical and ultrastructural studies.

Clinical, electrophysiological and histological evidence is presented of a myopathy not previously described. The patient, aged 15 years, was generally weak and wasted. His condition probably began in utero, but abnormalities of the serum CPK and EMG did not become evident until adolescence. Wide variation in fibre diameter, vacuolation, calcification, increased endomysial connective tissue and intra-fibre splitting were prominent in muscle biopsy sections. Electron microscopy showed numerous rod bodies, fibre disorganisation with loss of Z-bands and occasional honeycomb structures. Frequent zebra bodies were present. Because of the large number of zebra bodies seen, the name "zebra body myopathy" is provisionally proposed.

Inhibition of gastric emptying and drug absorption by narcotic analgesics.

The effect of intramuscular pethidine or diamorphine on gastric emptying and the absorption of orally administered paracetamol was assessed in eight normal subjects. 2 Both drugs produced a significant and striking delay in gastric emptying and absorption of paracetamol. 3 It seems inevitable that pethidine and diamorphine will retard the absorption of other orally administered drugs.

Creatine phosphokinase (CPK) levels in pregnancy: a case report and a discussion of the value of CPK levels in the prediction of possible malignant hyperpyrexia.

Creatine phosphokinase (CPK) levels during two consecutive pregnacies of a patient whose sister had died of malignant hyperpyrexia have been investigated. The results are compared with CPK levels of serum obtained from apparently normal people at similar stages of pregnancy. The rise of serum CPK occurring as a result of labour was much more pronounced in the malignant hyperpyrexia suspect.

A double-blind comparison of single doses of etidocaine 1 % and bupivacaine 0.5 % during continuous lumbar epidural block in obstetrics.

A double-blind investigation of some of the effects of etidocaine 1 % and bupivacaine 0.5 % was undertaken in patients in established labour. The results showed that the duration of bupivacaine was significantly longer than that of etidocaine, while the number of patients with good analgesia was equal in both groups. Motor blockade was less obvious in those patients receiving bupivacaine, but in neither group was this a finding of importance. At 5 minutes the Apgar scores of neonates from both groups were similar and any differences seen at 1 minute may be attributable to factors other than the drugs on trial.

Teratoma of the spinal cord. Report of a case and review of the literature.

A 6-week-old infant had a teratoma that involved the entire spinal cord. No reports of another such extensive teratoma of the spinal cord can be found in the literature. We postulate that an embryonic rest was incorporated into the spinal canal and, when removed from its normal inducer tissue, grew haphazardly throughout the spinal cord.
