Prevalence and estimated incidence of Toxoplasma infection among pregnant women in Poland: a decreasing trend in the younger population.

This study investigated the prevalence of specific Toxoplasma gondii IgG in pregnancy, the incidence of congenital toxoplasmosis and the prevalence trend of T. gondii infection among pregnant Polish women between 1998 and 2003. The study population comprised 4916 women who were admitted to the Polish Mother's Memorial Hospital Research Institute in Łódź. Their sera were tested for specific IgG and IgM antibodies to T. gondii, and the incidence of T. gondii infection was calculated from the increase in prevalence rates of IgG antibodies in various age groups. Specific IgG antibody was found in 41.3% (95% CI 39.9-42.7) of pregnant women, and the prevalence of IgG increased with age. The linear trend was significant (p <0.001), with an annual seroconversion rate of 0.7% (95% CI 0.004-0.010). The risk of primary infection was estimated to be 0.5% for 9 months, i.e., an incidence of 5/1000 pregnancies. Assuming a 30% maternofetal transmission rate, 1.5/1000 neonates were infected in utero. Seroprevalence during the 6-year study period decreased from 45.4% in 1998 to 39.4% in 2003, with a yearly decline in prevalence of 1.0% (p 0.02). The most important contributory factor to this decline was the group of women aged 19-29 years, among whom seroprevalence decreased significantly (p 0.007). Specific IgM was found in 244 (4.9%) women.

Diabetes screening after gestational diabetes mellitus: poor performance of fasting plasma glucose.

Gestational diabetes mellitus (GDM) is an established risk factor for the development of overt diabetes. Since the change in diagnostic criteria for diabetes in 1997, it is unclear whether there should be any preference for fasting or post-glucose challenge blood glucose in diagnosing diabetes after GDM. The study aimed at assessing the usefulness of both diagnostic methods in women after GDM. The study enrolled 193 women with previous GDM. Women who did not have a current diagnosis of diabetes were screened for impaired fasting glucose (IFG) and for glucose intolerance with an oral 75-g glucose tolerance test. A total of 45 (23.3%) subjects declared to be already diabetic. Of the 148 non-diabetic subjects, 141 (95.3%) had normal fasting plasma glucose, whereas four (2.8%) had IFG (i.e. FPG> or =6.1 and <7.0 mmol/l) and 3 (2.5%) had FPG> or =7.0 mmol/l. Upon OGTT, among the 141 subjects with normal FPG, 6 (4.3%) were diagnosed with diabetes and 23 (16.3%) with impaired glucose tolerance (IGT); the remaining 112 (79.5%) had normal glucose tolerance. Three out of four subjects with IFG had IGT. The sensitivities of fasting criteria for diagnosis of diabetes and IFG/IGT were 14.3% (95% CI, 8.0%-37.2%) and 17.1% (95% CI, 8.6%-19.8%), respectively. The specificities were 98.6% (95% CI, 97.9%-99.7%) and 99.1% (95% CI, 96.5%-100%), respectively. The kappa for diabetes diagnosis was 0.177 (95% CI, 0.018-0.507). For women with previous GDM, the sensitivity of the new criteria based upon fasting plasma glucose is unacceptably low. In addition, the two sets of criteria are not interchangeable. Therefore, we suggest full glucose tolerance diagnostic procedures in women after GDM, including assessment of post-glucose challenge values.

'In vitro' cytokine secretion by peripheral blood and decidual lymphocytes during the third trimester of normal pregnancy.

If the 'Th2 phenomenon' is dependent on trophoblastic antigens and cytokines, the profile of cytokines secreted by decidual lymphocytes (DL) should indicate stronger Th2 shift than that of peripheral blood lymphocytes (PBL). We studied spontaneous and mitogen-stimulated 'in vitro' cytokine secretion of cultured lymphocytes isolated from peripheral blood (n = 21) and decidua (n = 11) of third trimester healthy pregnant women not being in labor. The ELISA method was used for estimation of IL-2, IL-4, IL-6, IL-10, IL-12, IFN-gamma and TGF-beta. The results (given in pg/ml) were found to be statistically significant in the Mann-Whitney U-test (p < 0.05). Compared to PBL, DL produced spontaneously decreased concentrations of IL-12, IL-6, IFN-gamma and TGF-beta and upon mitogen stimulation increased quantities of IL-2, IL-4 and IL-10. It demonstrates that DL play an active role in local Th2-like immunoregulation and that their cytokine secretion pattern estimated in 'in vitro' conditions is biased towards Th2 activity more than that of PBL.

Psychomotor development in the children of mothers with type 1 diabetes mellitus or gestational diabetes mellitus.

The purpose of this study was to examine psychomotor development in children born to mothers with type 1 diabetes mellitus (DM1) or gestational diabetes mellitus (GDM). The influence of metabolic control in pregnant diabetic mothers and complications during labor on their children's psychological and physical development was evaluated. The analysis included 59 children, 20 of mothers with GDM, 19 of mothers with DM1, and 20 children of healthy mothers. Clinical observations and medical history were recorded and children were assessed using the Brunet-Lezine Psychomotor Development Scale. Abnormalities were found more often in the children of mothers with DM1 whose illness was insufficiently controlled during pregnancy and of mothers with serious hypoglycemia while pregnant. Speech, eye-movement coordination and social aspects were affected.

[Congenital pneumonia in fetuses and newborns ad inflammatory lesions in the placenta].

The authors compared the degree of congenital pneumonia in stillborn and neonates died in first two days of life with inflammation lesions of placenta. The coexistence of those two processes and its significant correlation was found, especially among preterm LBW neonates. This finding supports the hypothesis about infection as a probable cause not only of preterm deliveries but infection of fetuses and neonates as well. Analysis of some cases revealed also that general infection of mother could cause transplacental infection of foetuses.

[Transvaginal sonography in prediction of preterm delivery in patients presenting with signs and symptoms of preterm labor].

OBJECTIVE: The purpose of the study was to assess the clinical value of transvaginal sonography in the group of women presenting with preterm contractions and cervical changes. MATERIAL AND METHODS: We prospectively evaluated 82 patients between 23-34 weeks of gestation presenting in our department with signs and symptoms of preterm labor, intact membranes and cervical dilatation < 3 cm. In all cases transvaginal sonography was performed. The following parameters were assessed: functional canal length, funneling, cervical diameter and the length of the vaginal portion of the cervix. The primary outcome was delivery < or = 28 days from examination. RESULTS: The rate of preterm delivery (< 37 weeks) was 25.6% (21/82) and 17.1% (14/82) of the patients delivered < or = 28 days from the examination. Among the analyzed parameters, the significant difference between patients delivered < or = 28 and > 28 days from examination, was noticed only for the functional canal length (21.6 mm vs 30.1 mm; p < 0.001). The analysis of ROC curves showed that functional canal length had the highest diagnostic capability. Two important thresholds were found--20 mm and 31 mm. For predicting delivery < or = 28 days the functional canal length < or = 20 mm had sensitivity of 57.1%, specificity of 92.6%, PPV of 61.5% and NPV of 91.3%. The cutoff value of 31 mm had sensitivity of 100%, specificity of 47.1%, PPV of 28% and NPV of 100%. In multiple logistic regression analysis only FCL < or = 20 mm (OR 8.18; p = 0.027) was independently associated with PTD. CONCLUSIONS: 1. The shortening of the functional canal length (< or = 20 mm) is predictive of impending preterm delivery and the functional canal length > 31 mm is the indicator of the absence of labor. 2. Cervical sonography can be a valuable adjunct to the clinical assessment of patients with signs and symptoms of preterm labor.

[Peripheral blood neutrophils reactive oxygen intermediates (ROI) production in early pregnancy complicated by diabetes mellitus type I].

UNLABELLED: The components of maternal innate immune system are activated in pregnancy. Increased number of circulating neutrophils and monocytes, reactive oxygen intermediate production (ROI), increased expression of the adhesion molecules can be observed. The activation status and neutrophils response to stimulators in pregnant diabetic women's comprise, to some extend, both situations pregnancy and diabetes, combined effect of both seems to be very intriguing. AIM OF STUDY: Evaluation production of ROI on peripheral blood neutrophils in early pregnancy of diabetic mothers. MATERIAL: 12 pregnant diabetic women demonstrating good glycemic control and without major complications before pregnancy were studied at 8-12 weeks of first gestation, 11 healthy pregnant women match for weeks of gestation, age and parity; 11 healthy non-diabetic women; 13 diabetic, non-pregnant women, were also studied. METHODS: ROI production was evaluated with chemiluminescence method using standard stimuli fMLP, PMA, OZ and also after 15 min. priming with TNF-alpha. We used MLX Microtiter Plate Luminometr DYNEX, USA. with computer program Revelation 97. For statistic analysis Fisher, Kolmogorow-Smirnow and Wilcoxon tests were used (p < 0.05). RESULTS: 1. Peripheral blood neutrophils of healthy pregnant, diabetic and pregnant diabetic women independently from the stimuli used showed an increase in ROI production 2. Peripheral blood neutrophils change their activity, evaluate by ROI production during pregnancy, diabetes and pregnancy complicated by diabetes. 3. Peripheral blood neutrophils of pregnant diabetes women do not show expected impairment of their function. CONCLUSIONS: Diabetes type 1 with good glycemic control and pregnancy, change the production of ROI by peripheral blood neutrophils but these events do not sum up in diabetic pregnancies.

[The prognostic meaning of the selection parameters amniotic fluids samples and frequency occurrence respiratory distress newborn].

Prenatal diagnosis of amniotic fluid enables the evaluation of fetal lungs maturity. The aim of the study is to evaluate prognostic significance of measurement of concentration of lamellar body count (LC) in comparison with other laboratory investigations for prenatal fetal lung maturity evaluation. The study was carried out on 60 amniotic fluid collections obtained by amniocentesis of pregnant women, hospitalized in Polish Mother's Health Centre between August 1998-January 2000. Following tests were performed: phosphatydyloglicerol concentration (PG), concentration of lamellar body (CLC), optical density-reading absorbance in wave length 400, 570, 650 nm. Test PG showed 70% specificity and 100% sensitivity, LC 70% specificity; 100% sensitivity. PG and LC showed good correlation for the assessment of fetal lung maturity. The study involved small number of patients pointed out the necessity of continuation quantitative investigation of LC to confirm diagnostics utility for the assessment of fetal lung maturity.

[The influence of paternal lymphocytes immunization on percentage of peripheral blood CD16+/CD56+ cells in women with primary recurrent spontaneous abortion].

OBJECTIVES: Paternal lymphocytes immunization has been proposed for several years as an efficient treatment for unexplained Recurrent Spontaneous Abortion (RSA), however precise mechanism that underline the benefits of this immunotherapy is still unclear. DESIGN: The aim was to study the influence of paternal lymphocytes immunization on percentage of peripheral blood NK cells (CD16+/CD56+) in women with primary RSA of unknown etiology. MATERIALS AND METHODS: 48 patients with history of 3-5 (mean 3.3 +/- 0.7) consecutive primary RSA of unknown etiology were selected for the study. Immunotherapy with paternal lymphocytes, isolated from 100 ml of peripheral blood, was performed twice prior conception with a 4-week interval. The percentage of NK cells (CD16+/CD56+) was estimated using standard flow-cytometric immunofluorescent techniques for whole blood with one-step monoclonal anti-CD16/CD56 antibodies. Statistical analysis was performed with Wilcoxon test and the p value less than 0.05 was accepted as statistically significant. RESULTS: It was found that paternal lymphocytes immunization significantly decreases the percentage of NK cells in peripheral blood in women with RSA (23.9 +/- 8.5 vs. 16.2 +/- 7.0; p = 0.009). CONCLUSIONS: The data of the present studies suggest that paternal lymphocytes immunization modulate immunity in women with primary unexplained RSA significantly decreasing the percentage of NK cells (CD16+/CD56+) in peripheral blood.

[Oxidative stress in pregnancies complicated by gestational hypertension (GH), effect of oxidized serum lipoproteins (O-LDL)--the role of antio-LDL antibodies].

OBJECTIVE: To explain correlations between metabolism of serous oxidized lipoproteins, their influence on placental function in pregnancies complicated by GH and intrauterine asphyxia. MATERIALS AND METHODS: The group of 233 pregnant women characterised by gestational age between 32-40 weeks were divided into two sub-groups: the study group of 105 women with GH and the control group of 128 healthy pregnant women. In all the subjects doppler flows in umbilical and middle brain artery as well as the serum levels of anti o-LDL antibodies (oLAB) were measured. RESULTS: In the study group mean PI values in middle brain artery equal 1.62 +/- 0.41 while in the control group 0.91 +/- 0.16 (p < 0.01), with oLAB values 1133 +/- 24 for the study group and 619 +/- 15 for the controls. Having analysed vascular flows in umbilical artery, mean PI values of 1.41 +/- 0.36 for the study group and 0.63 +/- 0.12 for the control group were obtained, (p < 0.01). The levels of oLAB in these groups were as follows: 1019 +/- 18 for the study group and 600 +/- 12 for the controls. CONCLUSIONS: 1. In pregnancies complicated by GH an intensified oxidative stress was observed as determined by increased levels of anti o-LDL antibodies (oLAB). 2. Pulsatility indices in umbilical and middle brain artery exhibited enhanced values in pregnancies complicated by GH.

[Parameters of peripheral whole blood neutrophil activity in women with pregnancy induced hypertension].

OBJECTIVES: Pregnancy induced hypertension is believed to be a state of neutrophil overactivity, however all previous studies were done on isolated cells. DESIGN: To study neutrophil activity in whole blood of PIH women. MATERIALS AND METHODS: Neutrophil activity was estimated without isolation, in peripheral whole blood of 23 PIH women and 26 normal pregnant controls. The chemiluminescence test was performed without any stimulation and upon stimulation of neutrophils with fMLP, OZ and PMA, before and after pre-activation with TNF-alpha. The results were corrected according to the haemoglobin concentration and (%) of neutrophils. The percentage of whole blood neutrophils indicating expression of selectins CD18, CD11b, integrin CD62L and mean fluorescence intensity (MFI) of these molecules were studied on flow-cytometry. RESULTS: The study revealed that neutrophil chemiluminescence was not significantly higher in PIH women and after correction coefficient used it was even lower in PIH patients. TNF-alpha preactivation had no influence on chemiluminescence results. Expression of CD11b expressed as MFI value was significantly increased while that of CD62L, expressed as (%) of positive cells and MFI value--decreased in PIH patients. CONCLUSIONS: Changes of neutrophil CD11b and CD62 expression indicate increased activity of these cells in PIH women, however low production of reactive oxygen species estimated by corrected chemiluminescence test especially after TNF-alpha pre-activation, indicates that this form of their reactivity is rather "exhausted" during PIH.

[Decidual lymphocyte subsets in pregnant women].

OBJECTIVES: Materno-foetal immunological reactions in decidua are probably one of the most important elements in pathogenesis of preeclampsia. DESIGN: To compare lymphocyte subsets isolated from decidua of preeclamptic pregnant women with lymphocyte subsets isolated from healthy pregnant women. MATERIALS AND METHODS: Preeclampsia (PE) was defined according to USA National Health Institute criteria. The study group consisted of 21 women with PE and 11 women with physiological pregnancy. All pregnancies were finished with elective cesarean sections. Exclusion criteria were: uterine contractions, infection, chorinamnionitis, diabetes mellitus and therapy with steroids less than 7 days before blood sampling. Decidual tissue was obtained by curettage of the uterine cavity. The fragments of decidua were separated from clotted blood and placed in sterile tubes with 5 ml of isotonic solution (PBS). Then the decidual tissue was mechanically fragmented, homogenized and rinsed in PBS. Routine immunofluorescent marking techniques with monoclonal antibodies were performed. Analysis was done with FACSCalibur flow-cytometer with 488 nm argon laser using CellQuest programme. The following lymphocyte subsets were estimated: CD3, CD19, CD4, CD8, CD4/CD29, CD8/CD28, CD4/CD45RA, CD4/CD45RO, CD56/CD16, CD69. The results were described as percentage of lymphocytes positive for above surface molecules. Statistical analysis was performed using t-Student and U-Mann-Whitney tests. RESULTS: Decidua of pregnant PE women contains significantly increased percentage of CD3-/ CD56 + 16+, CD8+/CD28+ cells and decreased percentage of CD3+, CD19+, CD4+/CD29+ and CD4+/CD45RA+ compared to decidua of healthy pregnant controls. CONCLUSIONS: These changes suggest that deficiency of suppressor activity as well as aberrant immunoregulation exists in decidual tissue of PE women.

[Arterial hypertension during pregnancy complicated by type-1 diabetes--clinical aspects].

In the group of 289 pregnant diabetic women hospitalised and followed-up between 1991-2000 in the Maternal-Fetal Medicine Dept., Research Institute Polish Mothers Memorial Hospital, 44 patients were diagnosed with hypertension arterialis (15.2%), significantly more frequently in women with long lasting diabetes complicated by angiopathy and whose who trend to be obese. Metabolic control did not differ in the group with hypertension and without. In the group of pregnant women with hypertension following symptoms occurred significantly more frequently: proteinuria (29.5%), pyelonephritis (11.4%), anaemia (25%) and the risk of premature delivery (25%). Hypertension arterialis shortened significantly the duration of pregnancy (34.7 weeks of gestation vs. 37.3) and affected the obstetrical outcome such as:-low birth weight and longer time of newborn hospitalisation.

[Administration of natural anthocyanins derived from chokeberry (aronia melanocarpa) extract in the treatment of oligospermia in males with enhanced autoantibodies to oxidized low density lipoproteins (oLAB). The impact on fructose levels].

PURPOSE: To determinate the influence of anthocyanins from chokeberry on the generation of serous autoantibodies to oxidized low density lipoproteins (oLAB) and fructose levels in semen of males with oligospermia. MATERIAL AND METHODS: Study group consisted of 38 males with oligospermia of who were randomly divided into 2 subgroups: in 22 subjects anthocyanins were administered, while 16 men were on placebo; control group consisted of 25 healthy male volunteers. We examined the level of oxidative stress measured by serum concentration of oLAB, together with fructose levels in semen. RESULTS: In the anthocyanin group oLAB titres decreased from 1103 +/- 34 mU/ml before treatment to 742 +/- 24 mU/ml (1st month) and 724 +/- 23 mU/ml (2nd month), p < 0.01. In the placebo group oLAB titres were as follows--1094 +/- 21 mU/ml before treatment, 1114 +/- 36 mU/ml (1st month) and 1117 +/- 33 mU/ml (2nd month), p > 0.05; oLAB tires in the control group: 601 +/- 40 mU/ml before treatment, 609 +/- 38 mU/ml (1st month) and 609 +/- 38 mU/ml (2nd month), p > 0.05. In the anthocyanin group fructose levels in semen increased from 850 +/- 42 mg/ml before treatment to 1121 +/- 26* mg/ml (1st month) and 1230 +/- 27* mg/ml (2nd month), p < 0.05. In the placebo group fructose levels were as follows--832 +/- 36 mg/ml before treatment, 845 +/- 33 mg/ml (1st month) and 841 +/- 35 mg/ml (2nd month), p > 0.05; fructose levels in the control group: 1376 +/- 40 mg/ml before treatment, 1376 +/- 40 mg/ml (1st month) and 1388 +/- 37 mg/ml (2nd month), p > 0.05. CONCLUSIONS: 1. Our results indicate that natural antioxidants (anthocyanins) can be useful in controlling of oxidative stress in males with oligospermia. 2. Anthocyanins from aronia melanocarpa induce enhanced levels of fructose in semen of males with oligospermia.

Levels of mannose binding lectin in early pregnancy complicated by diabetes mellitus type 1--preliminary report.

The complement system plays an important role as a product of innate and acquired immune reaction. It can be activated via three different routes: the classical pathway, the alternative pathway and the lectin pathway. MBL (mannose-binding lectin) is considered to be a pathogen recognising receptor (PRR), an important factor in recognising pathogen associated molecular pattern (PAMP). The aim of study was to evaluate MBL in early pregnancy of diabetic mothers. Higher values of MBL were observed in diabetic non-pregnant women compared to healthy non-pregnant. Subjects early pregnancy seems to decrease MBL values in both diabetic and healthy pregnant women.

[Anti - T. gondii antibody concentration in sera of pregnant women in the sample of Lódź population].

The aims of the study were: to estimate the prevalence of Toxoplasma gondii in pregnant women in the Lódź region; to assess the rate of incidence of suspected recent infection based on the results of the standard test for specific anty -T. gondii IgG and IgM antibodies. The study covered 1920 pregnant women served in 1998 by Toxoplasma Reference Laboratory in Lódź. T. gondii specific IgG and IgM were quantitated by an enzym-linked immunoabsorbent assay (ELISA, Organon). We have demonstrated high prevalence of T. gondii among pregnant women in the Lódź region. In the analysed sample of Polish population the T. gondii-specific IgG with negative IgM occurred in 43.4% (n = 837). In 1.42% of pregnant women IgG and IgM specific antibodies were detected.

[Multifocal fungal infections in pregnant women].

The aim of the study was to estimate the prevalence of fungi in monofocal and multifocal infections in pregnant women. 251 pregnant women were examined for presence of fungi in vagina, oral cavity and rectum. Fungal strains were recovered in 259 out of 753 samples (49.2% of all pregnant women). Presence of fungi in vagina was associated with the invasion of oral cavity and/or rectum in 98% of all women. Monofocal vulvovaginal mycosis was found only in 1.9% of the cases. Bifocal infections depended on the ontocenoses: vagina - oral cavity (32.1%); vagina--rectum (7.5%); oral cavity--rectum (13.2%). Trifocal infections (vagina - oral cavity--rectum) occurred in 58.5% of all examined women.
