[Fertility after extrauterine pregnancy. A 16-year retrospective study].

A retrospective investigation of fertility following extrauterine pregnancy during a period of 16 years in a central hospital was carried out. Out of 283 women with extrauterine pregnancy, if proved possible to investigate the postoperative fertility in 181 women. It was found that 71.8% of the patients conceived and 49.2% were delivered of living infants while 16% developed renewed extrauterine pregnancies. No significant differences were found in the frequencies of renewed extrauterine pregnancy and the number of liveborn infants between women who had been submitted to radical operation and women in whom conservative operative methods were employed. On the basis of this investigation and review of the literature, the present authors consider, that conservative operation should be employed where this is technically possible.

["Alcohol associated" death in a forensic medicine autopsy sample at the Berlin Free University].

From 1978 to 1988 6431 forensic postmortem examinations have been made at the Freie Universität Berlin. These are about 50% of all p. m. examinations in Berlin (West) of the corresponding period. 22% of these cases were associated with alcohol. On one hand there was an acute influence of alcohol, on the other hand there were indications of chronic alcoholism. A rise of alcohol associated cases or of alcohol concentrations could not be found out.

[Fathers of first infants--preparatory courses about delivery, experience of delivery and paternity leave].

In the Central Hospital in Randers, 233 fathers of first infants replied to a questionnaire which illustrated their attitudes to the preparatory courses about delivery, experience of delivery and attitudes to paternity leave. 65% of the fathers participated in the course and 74% stated that they considered that this had been profitable. Where 77% of the men were concerned, these considered that participation in delivery had been a positive experience. 73% of the men had planned paternity leave around the time of delivery, which emphasizes the need for this arrangement.

[Poisoning with a wood preservative].

Autopsy findings and the results of chemical-toxicological analysis are presented on a case of suicide with a wood preservative containing potassium fluoride. The highest amounts of fluoride were found in kidney and urine; there was no increase of fluorides amount in the brain. The analysis was done by colorimetry after micro-diffusion.

[Fatal diltiazem poisoning].

The diagnosis at postmortem examination and the results of the chemical analysis in a case of a diltiazem intoxication are presented. Two metabolites were identified as desacetyldiltiazem and N-desmethyldiltiazem.

Efficient synthesis of N alpha-acetylarginine methylamide.

A simple preparation of Ac-Arg-(p-TosH or HCl)-NHMe is described. The NG-protonated Z-Arg was coupled with methylamine by the mixed anhydride method. Z-Arg-NHMe was purified as a NG-p-toluene sulfonate salt by crystallization from water. After removal of the Z group by catalytic hydrogenation and acetylation Ac-Arg(p-TosH)-NHMe was obtained. Ac-Arg(HCl)-NHMe was prepared by chromatography of the NG-TosH derivative on Dowex 44 (in Cl- form).

A new vaginal antifungal agent--butoconazole nitrate.

The continuously increasing incidence of vulvovaginal candidiasis necessitated a search for novel therapeutic modalities. Butoconazole nitrate (BN) a new imidazole, has been singled out for clinical studies since, in experimental vaginal candidiasis, it proved more effective than either miconazole nitrate (MN) or clotrimazole. One hundred and thirty volunteers with vaginal candidiasis, verified by wet mount and positive fungal culture, were randomly assigned to receive daily, for 6 days, either 1% BN (44 patients), or 2% BN (45 patients) vaginal cream or 2% MN (41 patients) vaginal cream. The patients were comparable regarding age (85% were 18-39 years of age), gravidity and parity. Twenty-five per cent had a recent history of unsuccessfully treated fungal vaginitis. Eight days after completion of treatment, negative fungal cultures were found in 98% of the patients using 2% BN, in 91% of patients using 1% BN and in 83% using 2% MN. The recurrence rate of the disease was low; about 80% of patients using 1% and 2% BN and 68% of those using 2% MN were culture-negative 30 days after conclusion of treatment. Rapid relief of clinical symptoms was experienced by patients in all three treatment groups. No significant side effects of the treatment were observed in any of the treatment groups.

Contraception with long acting subdermal implants: I. An effective and acceptable modality in international clinical trials.

This paper presents results of a double blind, multi-centered and multi-national study of two progestin only subdermal implants used for contraception. A regimen of six capsules of levonorgestrel (Ng) used by 492 women had a net cumulative 12-month pregnancy rate of 0.6 percent and a continuation rate of 74.6 percent. 498 women used six capsules of norgestrienone (R2010) and experienced a net cumulative 12-month pregnancy rate of 3.5 percent and a continuation rate of 79.4 percent. The difference in the pregnancy rate was significant at P less than 0.01, while there was no significant difference in the continuation rates. Menstrual problems were the principal reason for termination of the levonorgestrel regimen, accounting for approximately half of all terminations. There were significantly fewer menstrual problems among users of the norgestrienone (R2010) capsules; the net cumulative 12-month termination rate for this reason was 4.3 percent. Results are compared with continuation and termination rates for acceptors of the Copper T 200 at the same clinics. The low pregnancy rate and reasonably high continuation rate of the norgestrel implants coupled with the fact that the expected effective lifetime of a set of capsules is of the order of 3-5 years appears to warrant further development of this contraceptive regimen.
