Mode of chemotherapy does not affect complications with an implantable venous access device.

BACKGROUND: Few reports have been made regarding the long term safety of implantable venous access devices used for the delivery of chemotherapeutic agents. The authors' goals were to determine the frequency of complications in patients receiving chemotherapy with these devices; to determine whether complications were associated with the mode of chemotherapy delivery (push/bolus or infusional regimens); and to evaluate the influence of other risk factors, including home-based versus hospital-based administration. METHODS: A total of 152 oncology patients at the John L. McClellan Memorial Veterans Administration Medical Center in Little Rock, Arkansas (ages 26-81 years; mean age, 62 years), who underwent surgical placement of an Infus-a-Port (Strato, Inc., Beverly, MA) between May 1, 1992 and May 31, 1994, were evaluated retrospectively for postplacement device complications, such as infection, thrombosis, and mechanical failure. RESULTS: Twenty-seven patients experienced 1 complication each: 17 episodes of device-related sepsis, cellulitis, or fever of unknown origin; 8 episodes of thrombosis or catheter occlusion; 1 episode of drug extravasation; and 1 mechanical failure. Patient age, frequency of port accession, mode of chemotherapy delivery, tumor type, and neutropenia were evaluated as risk factors, but none was statistically significant. Complications were more frequent during the first 90 days after implantation, but they continued to occur throughout the observation period. CONCLUSIONS: Complications attributable to an implantable venous access device were infrequent in this patient population. No differences in complications for patients receiving home-based versus hospital-based chemotherapy administration were noted, opening the possibility of significant time and cost savings with home treatment.

Probing the interactions of poly(U) and tRNA(Phe) with nucleotides 1530-1542 and 1390-1417 of 16 S rRNA of Escherichia coli.

The decoding region of 16 S rRNA (nucleotides 1396-1403) is not available to complementary DNA oligomer binding in 70 S ribosomes, despite high levels of binding of these oligomers to active 30 S ribosomal subunits. While addition of poly(U) or tRNA(Phe) alone did not affect the binding of either cDNA to 30 or 70 S ribosomes, poly(U)-directed tRNA(Phe) decreased the binding of cDNA (1396-1403) oligomers by 80% to the active 30 S ribosomal subunits. But the poly(U)-directed tRNA(Phe) cannot displace a prebound cDNA in this region, nor can the cDNA displace a prebound poly(U)-directed tRNA(Phe). A cDNA to the anti-Shine-Dalgarno region (nucleotides 1534-1541) binds equally well to 30 and 70 S ribosomal particles, but the simultaneous addition of poly(U)/tRNA(Phe) and cDNA results in a 50% decrease in binding of the cDNA. In this case the poly(U)-directed tRNA(Phe) does displace the prebound cDNA, but the cDNA cannot displace a prebound poly(U)-directed tRNA(Phe). Neither cDNA oligomer inhibits the reassociation of the ribosomal subunits into 70 S particles.

Comparison of healing of lesions in the calvarium of foetal lambs and young sheep.

This study compares the healing of lesions made in the occipital region of the calvarium of 5 foetal and 4 post-natal lambs. The foetuses, operated on between the 83rd and 91st day of gestation, had a bone flap elevated and the post-natal lambs, aged about 6 weeks, had a craniectomy and replacement of the bone pieces. The foetuses were delivered spontaneously at full-term. All the animals had computed tomographic (CT) scans and, after sacrifice, naked eye examination of the bony lesions. These showed that there was poor ossification of the lesions made in the foetuses compared to those made in the post-natal lambs. These findings need to be taken into account when intra-uterine correction of cranio-facial deformities are being contemplated.

[Intracavernous injection of vasoactive substances in the treatment of erectile dysfunction].

Herein we report our experience with a clinical trial of intracavernous injection of vasoactive substances (Papaverin, Dibenzyran, Regitin) in 41 patients suffering from erectile dysfunction for longer than 1 year. In 14 out of 33 patients the application of papaverin alone was already sufficient to produce an excellent erection (T/E-grade 4) under clinical conditions. The simultaneous injection of two agents (Papaverin + Dibenzyran or Papaverin + Regitin) in patients with no or insufficient response to papaverin improved the results additionally. Besides a positively effect of delay from the injection to the erection could be observed in comparison with an injection of only papaverin. In conclusion, this method combined with an autoinjection therapy is an alternative approach to treat certain patients with erectile dysfunction.

Multiple forms of arylalkylamine N-acetyltransferases in the rat pineal gland: purification of one molecular form.

Rat pineal serotonin N-acetyltransferase (EC 2.3.1.87) activity is isolated in two molecular forms (Mr approximately equal to 10,000 and 95,000) by high performance size exclusion liquid chromatography in the presence of ammonium acetate (0.1 M, pH 6.5). In the presence of sodium citrate (0.1 M, pH 6.5), however, it is eluted as a single peak of intermediate size (Mr approximately equal to 30,000). A highly enriched preparation of one of the molecular forms has been obtained by a two-step purification procedure involving disulfide-exchange and anion-exchange chromatography. The N-acetyltransferase in 250 pineal glands obtained from isoproterenol-treated rats can be purified about 80-fold in 1 day; recovery is about 3%. Polyacrylamide gel electrophoresis of the final preparation indicates that a single major band (Mr approximately equal to 11,000) is present; this appears to be serotonin N-acetyltransferase.

Atypical synergistic alpha 1- and beta-adrenergic regulation of adenosine 3',5'-monophosphate and guanosine 3',5'-monophosphate in rat pinealocytes.

The adrenergic control of cAMP and 3',5'-cyclic GMP (cGMP) in dispersed adult rat pinealocytes was investigated. Norepinephrine treatment increased cAMP and cGMP content 60- and 400-fold, respectively; both alpha- and beta-adrenoceptors had to be activated for these responses to occur. Beta-Adrenergic stimulation alone produced only about 6- and 2-fold increase in cAMP and cGMP content, respectively. Alpha-Adrenergic stimulation, which alone had no effect on either cyclic nucleotide concentration, markedly amplified the beta-adrenergic stimulation of both cAMP and cGMP. The relative potency of alpha-adrenergic agonists and antagonists indicates the alpha 1-subclass of adrenoceptors is involved. A role of alpha 1-adrenoceptors in the control of pineal cAMP is consistent with published evidence of the presence of alpha 1-adrenoceptors on pinealocytes and their role in the regulation of N-acetyltransferase activity and melatonin production.

Psychological stress and the passage of a standard meal through the stomach and small intestine in man.

Gastric emptying half-time and mouth to caecum transit time of a solid meal were measured in eight normal volunteers, once during a period of psychological stress and again during a period of relative calm. No consistent or significant effect on gastric emptying was observed, but mouth to caecum transit times were faster in all subjects and this difference was highly significant (p<0.01).

[Relations between sperm findings and fertility prognosis in childless couples].

In eight andrological centres more than 2,000 childless married men were examined catamnestically. Fertility was compared with the findings of the routine spermatograms. Conception rate is reduced significantly if sperm density is below 10 million, in motility of spermatozoa under 50% and in ejaculates with less than 60% normal-shaped spermatozoa. However, lower values of spermatozoa must not be compared with absolute infertility.

Extracellular protein release and its response to pH level in Saccharomyces cerevisiae.

Saccharomyces cerevisiae grown in batch culture at pH 5.5 releases 0.1 to 0.2 pg protein per cell to the external medium over a period of four to five days, final concentration 20-40 micrograms/ml. Cells grown at pH 3.0 release 10-fold this quantity (1-2 pg/cell, final concentration 100-200 micrograms/ml). A kinetic model based on published behavior of periplasmic protein gave a good fit to the observed kinetics of exoprotein yield. The electrophoretic pattern of exoprotein differed from that of cell lysate protein, and exoprotein synthesis was apparently limited to early stages of the life cycle. These results are consistent with the identification of exoprotein as periplasmic protein released to the external medium through the cell wall. Analysis of the observed kinetics of exoprotein yield, utilizing the kinetic model suggests that the greater exoprotein production of cells grown at pH 3.0 was due entirely to greater synthesis of periplasmic proteins while the fraction of periplasmic protein released per unit time was greater for cells grown at pH 5.5. The latter conclusion is supported by thicker cell walls of cells grown at pH 3.0 as observed by electron microscopy. At an applied level the apparent limitation of exoprotein synthesis to the first few hours of cell life, the slow leakage of exoprotein through the cell wall, and the dilute nature of a yeast suspension do not favor the utilization of yeast cells for direct conversion of substrate into protein released to the external medium.

Pre- and post-operative CT scans of an infant with an aneurysm of the vein of Galen--a case report.

A five-month-old male infant presented with hydrocephalus and an intracranial bruit due to an aneurysm of the vein of Galen. Preoperative CT scans demonstrated the aneurysm, associated blood vessles, and the degree of hydrocephalus present. At operation the arteries feeding the aneurysm were obliterated. Post-operative CT scans showed that the greater part of the aneurysm was filled with thrombus. Information provided by the post-operative CT scans made it unnecessary to repeat the angiographic examinations which are more difficult and hazardous to perform in infants than in older subjects.

[Catamnestic study about the fertility of the man (author's transl)].

Collective investigations of fertility in childless married couples were carried out in 9 andrological health centres of the GDR. Analysis of 1937 spermatograms in comparison to fertility after 1 to 2 years shows: (1) Fertility decreases significantly at sperm density nearly 10 million per ml. (2) Motility below 60 per cent caused reduced fertility. Normal motility is more important than normal density of spermatozoa. (3) Fructose level doesn't correlate to motility or fertility of man.

[The psychological situation in a childless marriage (author's transl)].

In addition to the investigation of organic causes by gynaecological and andrological diagnostic tests the psychological problems of the infertile couples require close attention. Results of the investigation of 200 infertile couples elucidate the psychosomatic problems of infertility. There were organic cause in 72% of the cases, psychosomatic causes in 5% of the cases, probable psychologic causes in 11% of the cases and an unknown cause in 12% of the cases. The psychological problems of artificial husband and donor insemination are discussed. The importance of early psychological diagnosis in addition to organic diagnosis is emphasized.

[Sex behavior and partnership problems of the woman in climacterium with reference to her occupation].

On consideration of familiar and professional changes sexual psychologic aspects of the woman during the climacteric are being discussed. -- Changes and disturbances in sexuality, lack of libido and failure of orgasm and changed positions to sexuality expecially under the influence of the professional activity of 92 climacteric woman are stated. -- Examinations of the climacteric women were performed by the interview-method.

[First results and experiences with artificial donor insemination (ADI) in the treatment of sterile and infertile couples].

Statement of the importance, indications, contraindications and conditions of the artifical donor insemination (ADI) as a possibility of therapy in sterile and infertile married couples. Report about 34 wifes, in which hitherto in 12 cases the ADI led to a desired pregnancy. One of the pregnancies resulted in abortion, in 7 cases after a normal pregnancy and a normal delivery 7 normal developing infants were born and in 4 wifes present exists regular pregnancy in the 15 th to 30 th week. From these are 3 pregnancies, which obtained with helps of frozen semen (cryo-sperm). The successes are hitherto exclusive in the first cycles of the treatment. The biggest number of conceptions (8) have been reached, if the ADI have been led one day before rise in morning temperature. After strict consideration of all conditions and conscientious selection of the donors, ADI belongs a stable place in the therapy of sterility and infertility

[Results and experiences in the artificial maritogenous insemination as a therapeutic possibility in sterile marriages].

Statement of the importance, indications, contraindications and conditions of the artifical maritogenous insemination (AMI) as a form of therapy in the cases of a primary and secondary sterility. Report about 60 married couples during the time 1971 to 1974, in which in 10 cases (16,6%) the AMI led to a desired pregnancy. 4 of the pregnancies resulted in abortion, one of the pregnancies was anextrauterine gravidity. In 4 cases after a normal pregnancy and a normal delivery 4 normal developing boys were born. One women being in the course of treatment in the 20 th. week of her normal pregnancy. The successes are nearly exclusive in the first cycles of the treatment. The biggest number of the conceptions (5) have been reached, if AMI have been led one day before rise in morning temperature. After extensive diagnostic and proof of the generative potency of the wife and probable potency generativ of the husband, belongs AMI a stable place in the therapy of sterility.
