[Analysis of the eye's anterior segment with optical coherence tomography. Static and dynamic study].

PURPOSE: To study the biometric modifications of the anterior segment depending on accommodation and age. To try and define their possible applications in certain fields of anterior segment surgery, in particular in refractive implants. MATERIAL AND METHOD: Anterior chamber biometry can be very easily studied with 1310-nm wavelength optical coherence tomography. The equipment has a fixation target that can be focused and defocused with negative lenses in order to stimulate natural accommodation. The human anterior chamber was therefore studied during accommodation. We studied 104 eyes of 56 patients aged between 7 and 82 years. Refraction was between +5D and - 5D. A single operator carried out all the measurements. The anterior chamber's horizontal diameter, the anterior chamber's depth, the horizontal pupil diameter and the horizontal radius of curvature of the crystalline lens' anterior pole were measured unaccommodated or after stimulating accommodation. RESULTS: The different static or dynamic measurements were compared to ametropia, age and accommodation. At rest, the average AC diameter was 12.33 mm, the average AC depth was 3.11 mm and the average pupil diameter was 4.26 mm. On average, for 1 D of accommodation, the crystalline lens anterior pole moved forward by 30 microm. There was a 0.3-mm reduction in its radius of curvature and a 0.15-mm reduction in pupil diameter. Several other measurements are illustrated on graphs. CONCLUSIONS: The AC OCT is a user-friendly instrument to evaluate the anterior segment and explore the anterior chamber (cornea, iris, crystalline lens, irido-corneal angle). The 1310-nm light wavelength is blocked by pigments preventing exploration behind the iris. However, the AC OCT is capable of providing good-quality images and a better visualization of the anatomical relationships of the anterior segment, even behind an opaque cornea.

[An in vivo OCT study of human natural accommodation in a 19-year-old albino].

Modifications of the anterior segment during accommodation of the eye under examination cannot be studied in a simple and direct way with anterior segment imaging techniques such as Scheimpflug photography, A Scan, B Scan, and UBM. With this equipment, it is necessary to stimulate the fellow eye in order to observe the variations of the analyzed eye. The techniques using ultrasound equipment can only be used with contact systems or with water baths that will modify the anatomical dimensions or the pressure of the anterior segment. With the Scheimpflug photographic technique, geometrical reconstructions are necessary and cannot be used in certain axes. Optical coherence tomography provides the advantage of producing non-contact images of the anterior segment in static and dynamic conditions that are then easy to use. The target of the optical system can be focused and unfocused with negative or positive lenses, thus reproducing the conditions of natural accommodation. With a normal subject, the morphological modifications of the crystalline lens behind the iris screen cannot be studied because the infrared light source used is blocked by the pigment epithelium. The absence of this pigment in an albino subject allowed us to study the modifications of the crystalline lens and the ciliary body during accommodation. In this study, we were able to definitely confirm all the modifications of the anterior segment as described by von Helmholtz in 1855, who, at the time, used very rudimentary optical procedures based on the reflections of a flame on the surface of the crystalline lens.

Survivin protein expression positively correlated with proliferative activity of cancer cells in bladder cancer.

CONTEXT: Survivin is an inhibitor of apoptosis that is selectively over-expressed in common human cancers, but not in normal tissues, and that correlates with aggressive disease and unfavorable outcomes. AIMS: To identify the role of survivin in bladder carcinogenesis and the correlation between survivin protein expression and the occurrence of spontaneous apoptosis, proliferative activity of cancer cells. SETTINGS AND DESIGN: Retrospective analysis. METHODS AND MATERIAL: Bladder transitional cell cancer (BTCC) tissue samples for 128 patients were investigated, with normal bladder tissues serving as controls. From these tumor samples, 42 (32.8%) were grade I, 59 (46.1%) were grade II, and 27 (21.1%) were grade III; 72 (56.2%) were superficial, 56 (43.8%) were invasive. The survivin protein level was quantified by Western blot analysis. The apoptotic index (AI) using in situ labeling apoptotic DNA fragment kit and the Ki-67 labeling index (Ki-67LI) with an anti-Ki-67 monoclonal antibody were analyzed in these tumors, respectively. STATISTICAL ANALYSIS USED: Differences in the S/beta ratio between tumor grade and stage were evaluated by using unpaired t-test and F-test. The relationships between the S/beta ratios and AIs, Ki-67LIs of tumors were evaluated by Pearson correlation coefficient. RESULTS: High survivin levels were detected by Western blot analysis in tumor tissue extracts. None of the expression of survivin protein was found in normal bladder tissues. Survivin levels were significantly different from different clinical stages and pathological grades of the tumors (P > 0.05, respectively). Spearman rank correlation test revealed a positively correlation between survivin protein level and the proliferative activity (P < 0.001) and failed to find significant correlation between AI and survivin protein level (P > 0.05). CONCLUSIONS: Survivin protein expression played an important role in the malignant progression of BTCC.

Mechanism of electron multipacting with a long-bunch proton beam.

The energy gain and motion of electrons can quantitatively describe the mechanism of electron multipacting in a long-bunched proton machine. Strong multipacting usually happens around the bunches' tails due to the high energy of electrons when they hit the chamber surface. We investigated several important parameters of electron multipacting, proving that it is sensitive to the beam's intensity, the shape of its longitudinal profile, its transverse size, the secondary emission yield, and the energy at peak secondary emission yield. Our analyses, simulations, and experiments are all in agreement.

Rotational state-dependent mixings between resonance states of vibrationally highly excited DCO (X2A').

Rotational state-dependent mixings between highly excited resonance states of DCO (X (2)A(')) were investigated by stimulated emission pumping spectroscopy via a series of intermediate rotational levels in the B (2)A(') electronic state of the radical. Two examples for such interactions, between pairs of accidentally nearly degenerate vibrational states at energies of E(v) approximately 6450 and E(v) approximately 10 060 cm(-1), respectively, were analyzed in detail. Deperturbations of the measured spectra provided the zeroth-order vibration-rotation term energies, widths, and rotational constants of the states and the absolute values of the vibrational coupling matrix elements. The coupled states turned out to have very different A rotational constants so that their mixings switch on or off as they are tuned relative to each other as function of the K(a) rotational quantum number. The respective zeroth-order states could be assigned to different interlaced vibrational polyads. Thus, when two states belonging to different polyads are accidentally nearly isoenergetic, even very weak interpolyad interactions may start to play important roles. The derived interpolyad coupling elements are small compared to the typical intrapolyad coupling terms so that their influences on the vibrational term energies are small. However, large effects on the widths (i.e., decay rates) of the states can be observed even from weak coupling terms when a narrow, long-lived state is perturbed by a broad, highly dissociative state. This influence contributes to the previously observed strong state-to-state fluctuations of the unimolecular decay rates of the DCO radical as function of vibrational excitation. Similar mechanisms are likely to promote the transition to "statistical" rates in many larger molecules.

Mental retardation and hypotonia seen in the knock out mouse for Canavan disease is not due to succinate semialdehyde dehydrogenase deficiency.

Canavan disease (CD) is an autosomal recessive disorder caused by aspartoacylase deficiency leading to accumulation of N-acetylaspartic acid and spongy degeneration of the brain. The mouse model for CD showed low levels of glutamate and gamma-aminobutyric acid (GABA) in the brain. Whether the low levels of glutamate and GABA observed in the CD mouse brain lead to abnormal production of glutamate-GABA associated enzymes and resulting succinate production is not obvious. While glutamate dehydrogenase and alpha-ketoglutarate dehydrogenase complex activities are lower in the cerebellum and brain stem of the CD mouse, alanine aminotransferase and succinate semialdehyde dehydrogenase (SSADH) activities and succinate level are similar to the levels observed in the wild type. Deficiency of SSADH has been suggested to be associated with mental retardation and hypotonia, similar to the clinical features of CD. The normal SSADH activity in the CD mouse brain suggests that mental retardation and hypotonia seen in the CD mouse is not due to SSADH activity and if documented also in patients with CD.

TNXB locus may be a candidate gene predisposing to schizophrenia.

We report here on the detection of nine single nucleotide polymorphisms (SNPs) near to the NOTCH4 locus in the search for schizophrenia susceptibility genes in the class III region of the human major histocompatibility complex (MHC). We totally analyzed 122 family trios recruited in the UK. The TDT analysis demonstrated that of the nine SNPs, three were associated with schizophrenia, including rs1009382 (P = 0.00047), rs204887 (P = 0.007), and rs8283 (P = 0.015). Both rs1009382 and rs204887 are present in the TNXB locus. The rs1009382 is a non-synonymous SNP located in exon 23 of the gene and its A to G base change causes a Glu2578Gly substitution. The goodness-of-fit test showed that genotypic distribution of rs1009382 was deviated from Hardy-Weinberg equilibrium due to homozygote excess in the patient group (P = 0.01), suggesting that a double dose of a genetic risk may be involved. Possibly, rs1009382 is a candidate SNP predisposing to a schizophrenic illness. Moreover, the test for linkage disequilibrium (LD) between paired SNPs showed that the nine SNPs studied may be in the same LD block with an unexpected pattern as the strength of LD was not correlated with the distance between paired SNPs. The haplotype analysis suggested that there might be more than one disease-related allele located in the class III region of the MHC, and that these alleles possibly confer either susceptibility or resistance to schizophrenia.

The CLDN5 locus may be involved in the vulnerability to schizophrenia.

The present study was designed to detect three single nucleotide polymorphisms (SNPs) located on 22q11 that was thought as being of particularly importance for genetic research into schizophrenia. We recruited a total of 176 Chinese family trios of Han descent, consisting of mothers, fathers and affected offspring with schizophrenia for the genetic analysis. The transmission disequilibrium test (TDT) showed that of three SNPs, rs10314 in the 3'-untranslated region of the CLDN5 locus was associated with schizophrenia (chi(2) = 4.75, P = 0.029). The other two SNPs, rs1548359 present in the CDC45L locus centromeric of rs10314 and rs739371 in the 5'-flanking region of the CLDN5 locus, did not show such an association. The global chi-square (chi(2)) test showed that the 3-SNP haplotype system was not associated with schizophrenia although the 1-df test for individual haplotypes showed that the rs1548359(C)-rs10314(G)-rs739371(C) haplotype was excessively non-transmitted (chi(2) = 5.32, P = 0.02). Because the claudin proteins are a major component for barrier-forming tight junctions that could play a crucial role in response to changing natural, physiological and pathological conditions, the CLDN5 association with schizophrenia may be an important clue leading to look into a meeting point of genetic and environmental factors.

Ca(2+)-calmodulin signalling pathway up-regulates GABA synaptic transmission through cytoskeleton-mediated mechanisms.

We investigated the role of calcium (Ca(2+))/calmodulin (CaM) signaling pathways in modulating GABA synaptic transmission at CA1 pyramidal neurons in hippocampal slices. Whole-cell pipettes were used to record type A GABA receptor (GABA(A)R)-gated inhibitory postsynaptic currents (IPSCs) and to perfuse intracellularly modulators in the presence of glutamate receptor antagonists. GABA(A)R-gated IPSCs were enhanced by the postsynaptic infusions of adenophostin (1 microM), a potent agonist of inositol-1,4,5-triphosphate receptor (IP(3)R) that induces Ca(2+) release. The enhancement was blocked by co-infusing either 1,2-bis(2-aminophenoxy)-ethane-N,N,N',N'-tetraacetic acid (10 mM) or CaM-binding peptide (100 microM). Moreover, the postsynaptic infusion of Ca(2+)-CaM (40/10 microM) enhanced both evoked and spontaneous GABA(A)R-gated IPSCs. The enhancement was attenuated by co-infusing 100 microM CaM-KII(281-301), an autoinhibitory peptide of CaM-dependent protein kinases. These results indicate that postsynaptic Ca(2+)-CaM signaling pathways essentially enhance GABAergic synaptic transmission. In the investigation of synaptic targets for the enhancement, we found that IP(3)R agonist-enhanced GABA(A)R-gated IPSCs were attenuated by co-infusing colchicine (30 microM), vincristine (3 microM) or cytochalasin D (1 microM) that inhibits tubulin or actin polymerization, implying that actin filament and microtubules are involved. We conclude that postsynaptic Ca(2+)-CaM signaling pathways strengthen the function of GABAergic synapses via a cytoskeleton-mediated mechanism, probably the recruitment of receptors in the postsynaptic membrane.

A study of a genetic association between the PTGS2/PLA2G4A locus and schizophrenia.

Six single nucleotide polymorphisms (SNPs) present in the PTGS2/PLA2G4A locus were detected among 118 British family trios of schizophrenia patients. The transmission disequilibrium test showed that SNP4 located in the 5'-flanking region of the PLA2G4A gene was associated with schizophrenia and that the haplotype analysis also showed a genetic association between the PTGS2 gene and schizophrenia. Because these two genes are arranged in a head-to-head configuration and separated by just about 149kb of DNA, they may have a combined effect on susceptibility to schizophrenia in some cases.

No association between the PTGS2/PLA2G4A locus and schizophrenia in a Chinese population.

The present study was undertaken to replicate an association between the PTGS2/PLA2G4A locus and schizophrenia among a Chinese population. We recruited 168 Chinese parent-offspring trios of Han descent, consisting of fathers, mothers and affected offspring with schizophrenia. Of 3 informative SNPs genotyped, no one showed allelic association with schizophrenia; the haplotype analysis also failed to capture a haplotypic association with the illness. Because the frequencies of alleles and genotypes of SNPs analyzed differ in the Chinese population as compared with a British population that initially showed the genetic association between the PTGS2/PLA2G4A locus and schizophrenia, the ethnic background may be a major reason for poor replication of the initial finding.

Heart transplant coronary artery disease in Chinese recipients.

BACKGROUND: Transplant coronary artery disease is the principle limiting factor for long-term survival of heart transplantation (HTx) recipients. We reviewed our data to assess the incidence of this disorder among Chinese HTx recipients and to compare it with the results of Western studies. MATERIAL AND METHODS: From July 1988 to May 2002, 182 patients received 184 orthotopic HTx. One hundred sixty-three recipients survived for at least 1 year with available SPECT scans or coronary angiogram studies. The data set included donor characteristics, recipient characteristics, active cytomegalovirus (CMV) infection rate, rejection episodes, immunosuppressants, and human leukocyte antigen (HLA) mismatches. RESULTS: Surgical mortality in our program was 4.3% and the actuarial freedom from coronary artery disease at 1, 3, and 5 years was 99%, 95%, and 92%, respectively. Angiogram results were stratified into coronary artery disease (n = 15) or absence of the disorder (n = 148) groups. Only older donor age showed statistical significance between the groups. Compared with the Western series, the present data show higher actuarial survival rates and freedom from coronary artery disease. There were statistically significant differences in regard to graft ischemia time, proportion of male recipients, ischemic heart disease, rejection episodes during the first year, and incidence of CMV infection. CONCLUSIONS: SPECT scan can detect coronary artery disease before there is significant stenosis of the coronary artery with acceptable survival rates. Chinese HTx recipients show a lower incidence of the disorder, lower rates of ischemia heart disease, lower proportion of male gender, lower incidence of CMV infection, fewer rejection episodes during the first year, and less ischemic time than Western recipients, which maybe the contributing factors to their better survival.

Heart transplantation at Cheng Hsin General Hospital in Taiwan: 15-year experience.

Heart transplantation (HTx) in Taiwan, which started in 1987, now includes more than 500 cases. From July 1988 to September 2003, we performed 215 cases of orthotopic HTx in 164 male and 51 female recipients of mean age of 47.3 +/- 14.3 years, (range 2.7 to 74.9 years). The leading etiologies were dilated cardiomyopathy (CMP), 68.5%; ischemic CMP, 20.2%; and valvular CMP, 4.2%. The actuarial survival rates at 1, 5, and 10 years are 88.3%, 77.1%, and 57.2%, respectively. We performed the first case of HTx in Asia after bridging for 14 days with an indigenous total artificial heart (TAH; the Phoenix-7 model); we performed the first case of infant HTx without blood transfusion and also the first case of autotransplantation of heart for repair of a left ventricular rupture after a mitral valve replacement. These cases were all successful with the longest surviving HTx recipient in Asia. We have used the biatrial anastomosis technique in all cases. We discovered familial CMP due to mitochondrial defects in two pediatric cases. Because of the scarcity of donor hearts, we have used size-mismatched hearts as well as suboptimal and hepatitis-positive donor hearts, all with satisfactory outcomes. Our experience has shown comparable results to Western programs, with efficacy and cost-effectiveness. We find the technique of biatrial anastomosis for orthotopic HTx to result in a low incidence of tricuspid regurgitation and conduction anomalies. The use of suboptimal and size-mismatched donor hearts is also promising.

Photodissociation dynamics of pyrrole: evidence for mode specific dynamics from conical intersections.

The H and D atom elimination mechanisms in the photodissociation of jet cooled pyrrole and pyrrole-d1 have been studied by photofragment velocity map imaging. The molecules were excited to the 1 1A2 (pi sigma*) state at lambda = 243 nm and to the 1 1B2 (pi pi*) state at lambda = 217 nm. H/D atoms were detected by (2 + 1) resonance enhanced multiphoton ionization (REMPI) at lambda = 243 nm. The analysis of the images and the resulting translational energy distributions from the 1 1A2 state demonstrates the existence of two decay pathways, fast mode-specific cleavage of the NH bond in the excited state (channel A) and internal conversion (IC) to the electronic ground state (S0) followed by unimolecular decomposition of the vibrationally hot S0 molecules (channel B). The angular distributions of the H/D atoms from the direct dissociation in the excited state are strongly anisotropic, whereas the decay of the S0 molecules leads to spatially isotropic distributions. The results at lambda = 217 nm indicate that the 1 1B2 state undergoes an ultrafast radiationless transition to 1 1A2 followed by the abovementioned direct mode-specific NH bond fission on the 1 1A2 potential energy surface (channel A') or conversion to S0 and subsequent unimolecular decomposition (channel B'). The latter pathway may also be initiated by a direct relaxation from 1 1B2 to S0. The anisotropy parameter of beta approximately -1 for the direct NH bond fission at lambda = 217 nm is in accordance with the expectations for a perpendicular electronic excitation and a dissociation lifetime that is short compared to the rotational period of the molecules. The fast decay dynamics of both excited electronic states can be rationalized with reference to the theoretically predicted conical intersections between the pi pi*, pi sigma*, and S0 potential energy surfaces and the antibonding nature of the pi sigma* potential energy surface with respect to the NH bond [A. Acquired inhibitor of factor V: first report in China and literature review.

We reported a 51-year-old man who developed a severe haematuria with a prolonged prothrombin time (PT) and activated partial thromboplastin time (aPTT). Laboratory evaluation revealed the presence of specific factor V inhibitors. No underlying diseases and associated conditions have been identified. The patient improved after immunosuppressive therapy. However, the FV level remained low, indicating incomplete inhibitor eradication. The clinical course of our patient was consistent with the observations by others that FV inhibitor can be persistent for months or years.

Deflections in magnet fringe fields.

A transverse multipole expansion is derived, including the longitudinal components necessarily present in regions of varying magnetic field profile. It can be used for exact numerical orbit following through the fringe-field regions of magnets whose end designs introduce no extraneous components, i.e., fields not required to be present by Maxwell's equations. Analytic evaluations of the deflections are obtained in various approximations. Mainly emphasized is a "straight-line approximation," in which particle orbits are treated as straight lines through the fringe-field regions. This approximation leads to a readily-evaluated figure of merit, the ratio of rms end deflection to nominal body deflection, that can be used to determine whether or not a fringe field can be neglected. Deflections in "critical" cases (e.g., near intersection regions) are analyzed in the same approximation.

Removal of fluoride from water by carbon nanotube supported alumina.

This paper describes a new adsorption material carbon nanotube supported alumina, used for fluoride removal from water. The effects of calcination temperature, alumina loading and pH on fluoride removal capacity were investigated. The optimum calcination temperature for preparing the adsorbents is 450 degrees C and the largest adsorption capacity takes place at alumina loading of 30 wt%. The broad pH range of 6.0-9.0 with high adsorption capability suggests that carbon nanotube supported alumina have great potential applications in practice. The adsorption data at different pH were well fitted to the Freundlich isotherm. The kinetic studies show that the adsorption reaction of fluoride removal by carbon nanotube supported alumina can be described by a second orderrate equation.
