[Anatomo-clinical study of a case of regenerative nodular hyperplasia of the liver with Felty's syndrome and portal hypertension].

Regencrative nodular hyperplasia (RHN) is a rare condition, the diagnosis of which is based upon histological findings. It is seen in Felty's syndrome with portal hypertension (PHT), as was the case in the patient reported here. This was a 72-year-old man, with long standing rheumatoid arthritis, hepatosplenomegaly, a neutrophil leucopaenia and oesophageal varices responsible for recurrent haematemeses. Despite a portocaval anastomosis, the patient died from postoperative acute hepatic failure. Histological study revealed changes in the hepatocytes and the reticulin system typical of RNH without cirrhosis. The relationship between Felty's syndrome and RHN, as well as the mechanism of the hypertension, are discussed in the light of cases from the literature.

[Sarcoma of the cytogenic chorion in extra-gonadal endometriosis. Review of the literature apropos of a case].

Malignant sarcoma reaching a focal endometriosis is a rare condition. It cannot be distinguished from an endometrial sarcoma except in cases which are developped in extra-uterine endometriosis without uterus involvement. The case reported is a forty-seven old white woman with a sarcoma of the endometrial stroma reaching a Douglas focal endometriosis. Cases from literature are reviewed.

[Woringer-Kolopp syndrome. Histological and ultrastructural study of a case].

The authors report a case of Woringer-Kolopp syndrome. This was a rare form with multiple cutaneous features with a typical clinical and histological appearance. Ultrastructural study revealed the presence of granules with an opaque centre in the cytoplasm of the abnormal cells, and the close relationship of the latter with neurites. On the basis of these characteristics, they may be likened to the Merkel cells of the epidermis. Thus Woringer-Kolopp disease could be related to the proliferation of Merkel cells.

["Pagetoid reticulosis" (Woringer and Kolopp disease). A disease of the Merkel cell (author's transl)].

A case of Woringer and Kolopp disease "Pagetoid reticulosis" is described. Clinical features include a ring of a target pattern of erythemato-squamous infiltrated skin lesion. The characteristic histological aspect of dense epidermal infiltrate by abnormal cells with a spared dermis has been observed. Ultrastructural findings include cytoplasmic granules with a found electron dense core and close relationship to neurites, two features of the Merkel cell of human normal epidermis. By comparing our patients with the already reported cases of Woringer and Kolopp disease it seems likely that this is a unique disease with no relationship with cutaneous lymphoma but which is defined by a Merkel cell proliferation.

[Melanoma of the penis: 6 cases].

BACKGROUND: Melanomas of the penis are rare tumors of poor prognosis. METHODS: Six cases of melanoma of the penis, followed between 1975 and 2002, were retrospectively reviewed. We collected the data on epidemiological, clinical and pathologic factors, treatment and follow-up. RESULTS: The mean age was 44 years. The time to diagnosis was 2 years. Two patients had general predisposing factors for melanoma, and 3 patients local predisposing factors. Two patients had partial penectomy and 4 patients had conservative excision. One patient had local recurrence, and another had metastatic course resulting in death. Five patients out of 6 were alive and disease free at time of the study, with a 24 month follow-up. DISCUSSION: The identified risk factors for the development of penile melanoma are melanosis and pre-existing nevus. Delayed diagnosis explains the usually bad prognosis. Classical surgical treatment used to be radical, but recently, conservative surgery has been proposed. For an early diagnosis, genital melanosis requires surgical excision, when technically feasible, and any atypical lesion of the penis should be submitted to a biopsy.

[Granuloma with lymphocytic hyperplasia following vaccination: 10 cases. Presence of aluminium in the biopsies].

BACKGROUND: Few cases of cutaneous lymphocytic hyperplasia secondary to vaccination have been published, although such lesions are not rare. PATIENTS AND METHODS: We report a series of 10 cases registered between 1993 and 2003. RESULTS: Mean age was 25. The clinical aspect was solitary or multiple subcutaneous nodules, located on the arm, developing after a delay of 1 to 18 months after vaccination. Histologic examination showed a lymphocytic infiltration of the subcutaneous fat, with diffuse and/or follicular pattern, without nuclear atypia, the morphological and immunohistochemical analysis of which revealed the benign nature. In all cases, there was fibrosis and granuloma composed of lymphocytes, plasma cells, eosinophils and macrophages with basophilic cytoplasm. Morin stain showed intralesional aluminium in the 6 investigated cases. Evolution was always benign, with no relapse following exeresis. DISCUSSION: Cutaneous lymphocytic hyperplasia secondary to vaccination has to be suspected in a young patient with subcutaneous nodules appearing at a vaccination site. Evidence of aluminium in the lesions supports the diagnosis and the hypothesis that aluminium in the vaccine excipient might have a role in the onset of such lesions.
