A survey of rubella immunity was conducted among prepubertal girls in Prince Edward Island. Of the 431 girls enrolled in grade 4 in a random sample of schools 83% had a school record of having been vaccinated after their first birthday. A screening test of finger-prick samples revealed that 95% of the girls with a written history of vaccination after their first birthday were immune, 79% of those vaccinated before their first birthday or for whom the date of vaccination was uncertain were immune, and 40% of unvaccinated girls were immune. Of the 49 susceptible girls 43 were vaccinated; all but 2 were found to be immune on retesting. The findings support the recommendation of the National Advisory Committee on Immunization to vaccinate all prepubertal girls without a written history of rubella vaccination after their first birthday.

Three hundred and ninety-two subjects from 89 German families were typed for restriction fragment length polymorphisms (RFLPs) detected by the probes pmetH, pmetD, pJ3.11, KM19, and XV2c known to be tightly linked to the cystic fibrosis (CF) gene. The analysis of the predictive value of this typing in individual CF families indicates that the combined use of these probes provides a powerful diagnostic system for both carrier detection and prenatal diagnosis. In 45 families the complete haplotype including all RFLPs was available. Of them 41 (91.1%) were fully informative and 4 were partly informative.
The secondary structures of bovine blood coagulation factors IX and X, as well as that of bovine prothrombin, were predicted on the basis of a computerized combination of the Chou-Fasman and Burgess algorithms. Refinements in the predictions were made after consideration of the content of various secondary structures, as determined by circular dichroism studies of these same proteins. The final turn assignments were in good agreement with those assigned with use of an algorithm involving pattern matching of beta-turns in proteins of known structure.
A new formalism has been developed in order to evaluate intermolecular interaction energies for inorganic and organometallic complexes in the framework of the extended Hückel method. In order to provide the shortest possible response time on an interactive computer graphics facility, this model should require the minimum amount of computer time, which explains why approximate procedures are used to evaluate electrostatic, charge transfer and exchange repulsion components. When applying this model to typical examples of electrophilic addition reactions to organometallic complexes, it is found that it is essential to take account of charge transfer interactions, the electrostatic component alone being not sufficient, even qualitatively, for a proper description of the reaction mechanism. The results, presented as color-coded dot molecular surfaces, show a very good agreement with experiment as to the site of attack, namely (i) on metal for the electrophilic attack on Fe(cp)2, Fe(CO)5 and X(cp)(CO)2, X = Co, Rh; (ii) on the cp ligand for the nucleophilic attack on Co(cp)2+ and Rh(cp)2+; (iii) on bz for the nucleophilic attack on Fe(cp)(bz)+. Finally, modellizations of the nucleophilic attack on a coordinated olefin and of the relation between structure and acidic properties of zeolites are presented and discussed.
The aim of this study was to assess (a) the incidence of perineal descent and (b) the relationship between radiological abnormalities of the pelvic floor and rectoanal manometric disturbances in patients consulting for constipation. Lateral radiographs in both the left lateral and supine positions studied pelvic floor descent. Results obtained in the 25 patients (mean age 47 years) studied were compared with those of 12 controls (mean age 58 years). Pelvic floor descent, never seen in controls, was demonstrated in 14 patients who were older (53 +/- 3 years, mean +/- SD) than the 11 with a normal radiological examination (38 +/- 3 years, p less than 0.05). Anal low pressures (3 cases) and a low amplitude of rectoanal inhibitory reflex (RAIR) (5 cases) were shown only in patients with perineal descent and anal high pressures only in those with normal radiology. Our results suggest that (a) perineal descent is a common finding in over 50% of constipated patients and (b) anorectal motility is related to pelvic floor function.
Three male patients without accompanying diseases, 27, 47, and 52 years of age, were admitted with fever of unknown origin. Focal liver disease was seen on ultrasound and the diagnosis of an abscess was confirmed by sonographically-guided fine-needle puncture. Microaerophilic streptococci were found in two cases, streptococcus viridans once. Two patients had cryptogenic abscesses; one had previously undergone surgery for a rectal abscess. All three patients were cured of their condition by appropriate antibiotic coverage and evacuation of the abscess by needle aspiration.
After occupational and medical radiation exposures structural chromosome aberrations may be induced in the lymphocytes of the irradiated persons. Many authors have estimated the radiation dose from the yields of dicentric aberrations. We analysed the influence on the dicentric yield of increasing time intervals between irradiation and blood sampling from a person involved in an X-radiation accident (radiography). During a 4-year follow-up we observed an approximately 7-fold decline of the dicentric yield up to the 25th month and thereafter an almost constant value. Since the decline did not commence until around the 10th month after the exposure, an exponential decrease of dicentric yield with time should be considered with reservation in this study, although it cannot be entirely ruled out. We conclude that in 'biological dosimetry' blood should be sampled as early as possible after the exposure. Furthermore, computation of 'half-times' of lymphocytes to allow for a delay in blood sampling seems uncertain after partial body exposures to high doses. Therefore in such cases dose estimates obtained 1 or more years after irradiation should be considered as minimum values.
All 17 patients with tropical spastic paraparesis (TSP) in a series seen in the United Kingdom have antibodies to the human T cell leukemia virus type 1 (HTLV-1). Cultured peripheral blood lymphocytes from these patients formed multinucleated giant cells and reacted with sera and monoclonal antibodies to HTLV-1 in a manner identical to adult T cell leukemia-lymphoma (ATLL) patient lymphocytes. Western blot analysis failed to reveal any marked difference in the antigens recognized by sera from TSP and ATLL patients. The sera from TSP patients, their asymptomatic relatives and ATLL patients were titrated using the following assays: enzyme-linked immunosorbent assays (ELISA), particle agglutination, antibody-dependent cell-mediated cytotoxicity, and pseudotype neutralization. There were significantly stronger serologic responses in the TSP patients than in their relatives or ATLL patients. High antibody titers in the presence of replicating virus often reflect the antigen load; however, these data are also consistent with the suggestion that neurologic damage in TSP may be immunologically mediated.
Mucosal uptake, mucosal transfer, and retention of a physiological dose of ferrous iron were studied in women with active rheumatoid arthritis (RA): 19 with normal and 17 with depleted bone marrow iron stores. Control subjects were 26 normal women and 20 women with uncomplicated iron deficiency. Iron absorption was measured with 59Fe as a tracer and by whole body counting. Compared with controls, final iron retention was considerably decreased in both groups of patients with RA. Analysis of the two sequential steps of iron absorption showed that mucosal uptake was normal in iron replete patients with RA but was significantly lower in patients with RA with depleted iron stores compared with iron deficient controls. Mucosal transfer of iron was considerably decreased in patients with RA with normal iron stores. The impaired absorption of iron in patients with active RA may delay the correction of the haemoglobin concentration when anaemia of chronic disease is complicated by iron deficiency.
The purpose of the present investigation was to compare the effects of some phenylalkylamine calcium entry blockers on postischemic energy metabolism in the isolated perfused rat brain. To differentiate between calcium entry blockade and nonspecific effects the optically active stereoisomers of emopamil were investigated. At a concentration of 10 mumol/l both gallopamil and emopamil enhanced the postischemic restoration of high-energy phosphate levels. The racemic mixture and the (-)-enantiomer of emopamil and noremopamil caused similar metabolic changes at 1 mumol/l. The (+)-enantiomer of emopamil proved to be ineffective at either 1 or 10 mumol/l. The effects on energy metabolism were accompanied by an increase in the postischemic perfusion rate. The results suggest that the drug efficacy necessary to accelerate the restitution of postischemic energy metabolism is in the order of noremopamil greater than emopamil greater than gallopamil. The effects of the phenylalkylamine calcium entry blockers on energy metabolism are discussed in relation to vasodilation due to calcium entry blockade.
The authors report 5 cases of in situ intraductal breast carcinoma (IDC) revealed or complicated by a clinical inflammatory syndrome. These cases showed some common features with other IDCs, such as mammographic signs and histologic forms of epitheliamatous proliferation, but were also characterized by clinical inflammatory signs different from those of rapidly developing "inflammatory" cancer, frequent nipple discharge, frequent positive results in bacteriologic examinations of nipple discharge and the role of cytologic studies of discharge in determining a definite diagnosis of carcinoma. Some hypotheses as to etiology are also discussed.

The interleukin 2 (IL 2) receptor was studied in three cases of large granular lymphocyte (LGL) lymphocytosis. All cases were nonreactive with anti-Tac monoclonal antibody (MoAb; recognizing the p55 alpha subunit of the IL 2 receptor). Sodium dodecyl sulfate (SDS)/polyacrylamide gel electrophoretic analysis (PAGE) of cells to which radio-labeled rIL 2 had been chemically crosslinked revealed uniform expression of the p70/75 beta subunit of the IL 2 receptor in the absence of the alpha subunit. Stimulation of this receptor with 2 nmol/L rIL 2 for five days led to acquisition of anti-CD3 redirected cytotoxicity. This was accompanied by a fivefold to tenfold elevation in the activity of intracellular N-alpha-benzyloxycarbonyl-L-lysine thiobenzyl esterase, an LGL granule marker enzyme. These effects of IL 2 did not require induction of the Tac peptide.
