Immunoblot (Western blot) examination of the sera of 45 patients with the Guillain-Barré syndrome demonstrated that between three and five immunoreactive bands that were characteristic of a recent Campylobacter jejuni/Campylobacter coli infection were present in 22 of these patients. The immunoblots paralleled the serological response that is found in campylobacter enteritis and confirmed the specificity of our previous enzyme-linked immunosorbent assays (ELISAs), which indicates that antecedent C. jejuni/C. coli infection is common in patients with the Guillain-Barré syndrome. We postulate that, under certain circumstances, demyelination is initiated by the leakage of campylobacter-specific antibody across the blood-nerve barrier.
The pattern of c-myc gene organization and expression has been examined in resected colonic tumors and in the adjacent normal colon from 15 patients undergoing radical surgery. DNA hybridization showed no evidence of gene amplification or rearrangement. Transcripts of the c-myc messenger ribonucleic acid (mRNA) were elevated up to 32-fold in 12 of 15 tumors. The gene product, p62c-myc, was detected by both immunoblotting and immunohistology using a monoclonal antibody raised against a synthetic peptide immunogen. There was close correlation between c-myc mRNA copy number and p62c-myc abundance. Three well differentiated tumors contained high levels of transcript and protein, whereas four poorly differentiated tumors had the lowest levels. The assay of oncogene products may provide new biologically relevant tumor markers for determining prognosis and guiding treatment.
This article attempts to convey some enduring truths about nursing on the caring edge of evolutionary human consciousness. A format of selected literary and poetic metaphoretical vignettes is used to depict the tacit, expressive elements of caring in nursing that give it an evolutionary edge in the health care system and in society.
The central problem in cancer therapy is the poor selectivity of current systemic agents against the common solid tumours. The demonstration that unique segments of DNA, constant in location and conserved in evolution are involved in growth control opens new avenues for basic and clinical research. The function of the products of these genes needs to be elucidated. Examples of growth control functions include homology to growth factors, surface receptors, protein kinases and cell cycle control proteins. From DNA sequence data peptides predicted to be exposed within intact molecules can be constructed and used to produce monoclonal antibodies to oncogene products. Such antibodies have now been successfully used to demonstrate the intracellular localization of gene products as well as the cell cycle regulatory role of the c-myc protein. By having a battery of antibodies against the different gene products their direct clinical application for diagnosis and prognosis has become a reality. Immunohistology and flow cytometry permit the geographical and quantitative analysis of function in normal and neoplastic tissues. Furthermore, by purification and biochemical analysis the molecular basis for their action can be elucidated. It is likely that by the end of the decade new drugs that inhibit oncoprotein function will be available for clinical trial.
To investigate the mechanisms of CO2 transport during constant flow ventilation, we measured arterial blood gases using air, 80% He-20% O2 (He) or 80% SF6-20% O2 (SF6) as the insufflating gas. At any given flow rate (0.2 to 1.0 L/s), PaCO2 was greatest with He and lowest with SF6. Data for all gases could be described by the equation PaCO2/Pb = 0.044 V-0.64 v0.23, where Pb = barometric pressure, PaCO2 is in mm Hg, V = insufflated flow in L/s, and v = kinematic viscosity (cm2/s). At any given flow rate, the AaPO2 was greater using SF6 than using He. These results are consistent with a 2-zone model of gas transport in which the enhancement of gas transport as V increases may be due to an increase in the turbulent diffusivity in zone I (the region affected by the jet). The decreased gas transport with He compared to air and SF6 at any V may be due to either the decreased penetration depth of zone I caused by the greater kinematic viscosity of He, or the decreased rate of gas transport in the region affected by cardiogenic oscillations (zone II) secondary to the higher molecular diffusivity of He.

Endogenous prostaglandins (PGs) have been isolated in large amounts from the gastrointestinal tract of man. These substances are intimately related to muscle contraction and cytoprotection. The gastric juice and serum levels of PGE2 and F2 alpha have been assayed in nine infants with infantile hypertrophic pyloric stenosis (IHPS) and seven age-matched controls. The gastric juice levels were significantly raised in the IHPS group for both PGE2 (P = 0.001) and PGF2 alpha (P = 0.02) as compared to controls. The possible role of PGs in the etiology of IHPS is discussed.
