EOG findings in patients with lesions in cerebellar peduncles.

EOG tests were performed in patients with lesions in either the superior or the middle cerebellar peduncle, because these patients showed cerebellar signs and symptoms and were difficult to distinguish from patients with cerebellar lesions symptomatically. EOGs of these patients showed a predominant decrease of pursuit gains and OKN velocity gains toward the lesioned side, whereas the percent reduction of fixation-suppression of caloric nystagmus decreased predominantly toward the side contralateral to the lesioned side, regardless of whether lesions were located either in the superior or the middle cerebellar peduncle. The present EOGs are best interpreted as a corresponding to a deficit of the neuronal events of the flocculus in the monkey and are solely ascribed to lesions in the cerebellar peduncles, because even well-defined cerebellar lesions on one side showed bilateral impairments of these visually induced eye movements.

[Extracorporeal shock wave lithotripsy monotherapy in the treatment of staghorn calculi].

From October, 1987 to September, 1989, 53 staghorn calculi of 51 patients underwent extracorporeal shock wave lithotripsy (ESWL) monotherapy by using Dornier HM3 lithotriptor. All patients were treated with double J stenting preoperatively. Mean number of shock waves was 6092 and mean number of sessions was 2.1. In 52 out of 53 kidneys (98%), the stones were disintegrated completely. Complete removal of the stone were observed in 29 kidneys (55%) 3 months after the last ESWL treatment. Complications consisted of fever attack (more than 38 degrees C) (26 patients), ileus (2), subcapsular hematoma (2) and gastrointestinal hemorrhage (1). They could be conservatively treated except one case with percutaneous nephrostomy. Supplementary procedures for the stone street were necessary in 23 patients. They consisted of ESWL (16 patients) and transuretheral lithotripsy (7). The indication of this procedure for the treatment of staghorn calculi was also discussed.

[Extracorporeal shock wave lithotripsy using Dornier modified HM3 lithotripter comparison with the results by Dornier HM3 lithotripter].

Ninety-four kidneys with renal stones less than or equal to 20 mm in diameter were treated by extracorporeal shock wave litotripsy (ESWL) using a Dornier modified HM3 lithotripter and the results were compared with those of 98 kidneys with similar size stones treated with a Dornier HM3 lithotripter. The Dornier modified HM3 lithotripter is equipped with a new type of shock wave generator with a reduced capacity for 30% less pressure peakes at the same voltage. It has an enlarged ellipsoid leading to a smaller focus and a reduced pressure per area at the shock wave entry into the skin. All treatments of modified HM3 litotripter series were performed under only intravenous analgosedation, without epidural anesthesia. The number of shock waves in the modified HM3 series ranged 900 to 6000, with the mean values of 2863 +/- 1234, which was 1.55 times as that in the Dornier HM3 series. Complete disintegration was achieved in 94 of 94 modified HM3 series kidneys and 98 of 98 kidneys of HM3 series. Complete removal of the stone was done at 72.6% in the modified HM3 series and at 70.4% in the HM3 series 3 months after ESWL. There were no severe complications in both modified HM3 series and HM3 series. Renal damage caused by ESWL was monitored by the level of urinary enzyme, N-acetyl-beta-glucosaminidase (NAG) and beta 2 microglobulin (beta 2MG) and the level of urinary protein. The levels of NAG, beta 2MG and urinary protein in the HM3 series were higher than those of the modified HM3 series.(ABSTRACT TRUNCATED AT 250 WORDS)

Cyclothiazomycin, a novel polythiazole-containing peptide with renin inhibitory activity. Taxonomy, fermentation, isolation and physico-chemical characterization.

Cyclothiazomycin is a novel renin inhibitor produced by Streptomyces sp. NR0516. It was isolated from fermentation broth by extraction with butyl alcohol, QAE-Toyopearl column chromatography and preparative HPLC. Cyclothiazomycin, which was determined to be a unique polythiazole-containing bicyclic peptide, exhibited inhibitory activity against human plasma renin with IC50 being 1.7 microM.

Structural studies on avidinorubicin, a novel anthracycline with platelet aggregation inhibitory activity.

Avidinorubicin (MW 1,214, C60H86N4O22) was isolated from the cultured broth of strain NR0576 (which was identified as Streptomyces avidinii Stapley et al.) by butyl alcohol extraction, Sephadex LH-20 column chromatography and preparative HPLC. Avidinorubicin inhibited thrombin-induced platelet aggregation with an IC50 being 7.9 microM and was determined to be a novel anthracycline possessing two units of a new aminosugar, avidinosamine, in place of two decilonitrose groups in decilorubicin.

Identity of brain-associated small cell lung cancer antigen and the CD56 (NKH-1/Leu-19) leukocyte differentiation antigen and the neural cell adhesion molecule.

Previous studies have demonstrated the monoclonal antibody, TFS-4, to recognize a cell surface antigen, 124,000 Daltons, of molecular weight expressed selectively on small cell lung cancer but not on non-small cell lung cancer, and to cross-react with the human brain, cardiac muscle and some smooth muscle cells. The cross-reactivity of TFS-4 with peripheral blood lymphocytes was examined by two color immunofluorescence analysis, using monoclonal antibodies to leukocyte differentiation antigens. Flow cytometric analysis revealed an identical subset of cells to express both BASCA and CD56(NKH-1 antigen). The immunofluorescence profiles for both TFS-4 and NKH-1 were, furthermore, identical to those of the background controls, identicating identical quantities of the antigens to be present on each cell within the population. Since CD56 has been show to be identical to the neural cell adhesion molecule (N-CAM), we determined the amino-terminal amino acid sequence of BASCA purified from the human brain. The amino-terminal amino acid sequence of BASCA was identical to that of N-CAM.

[The physiological function of human endometrium].

The cyclic change of human endometrial cells are controlled by the interaction between hypothalamus, pituitary gland and ovaries, thus making the endometrium proliferate, differentiate, exfoliate and then reproduce. The menstrual cycle is divided into three phases which are called follicle, ovulatory and luteal phase by the morphological change of the ovarium. The endometrial cycle is also classified to proliferative, secretory and menstrual phase. Estradiol (E2) stimulates the proliferation of endometrial cells by the indirect positive mechanism activated by the binding of E2 to E2 receptor. Growth factors (IGF- I, EGF, TGF- alpha etc.) induced by the transcription of the gene promote the proliferation of endometrial cells. Progesterone (P) has antagonistic effects on E2 actions and transform proliferative phase to secretory phase in endometrium. It is suggested that the possible mechanism of carcinogenesis of normal endometrium is the progression of endometrial hyperplasia due to the prolonged and unphysiological exposure to E2. The additional role of oncogenes (fos, fms, myc, myb, erb-B, neu) and growth factors on the mechanism of carcinogenesis of hyperplasia to cancer is very interested.

[Beneficial effect of diltiazem on pulmonary hypertension in a patient with Hughes-Stovin's syndrome].

A 54 year-old female who had a history of hemoptysis was admitted to our hospital because of dyspnea on effort. Pulmonary arterial pressure was elevated and pulmonary arteriography showed multiple pulmonary arterial aneurysms and occlusion of the left upper lobe pulmonary artery. Systolic pulmonary arterial pressure was 110 mmHg when measured by continuous wave Doppler echocardiography. From the clinical and angiographical findings, we diagnosed this patient as having Hughes-Stovin's syndrome (forme fruste). 30 mg of diltiazem per day was initially used, and 80 mg was used thereafter. After 2 months follow up of the medication with diltiazem, systolic pulmonary arterial pressure decreased from 110 mmHg to 61 mmHg and clinical symptoms improved dramatically.

[Various characteristics of Helicobacter pylori infection in Perú].

The rate of infection by Helicobacter pylori in peruvian dyspeptic patients (84%) is higher than that reported in symptomatic patients from developed countries. Helicobacter pylori infection did not increase with age. The ecological distribution is similar in the three different geographical regions of Peru. Only women of high economic status had significantly lower rates of Helicobacter pylori infection compared to all other groups. The recurrence rate after treatment is higher than that reported from developed countries.

Identification of genetically high risk individuals to lung cancer by DNA polymorphisms of the cytochrome P450IA1 gene.

A good correlation was observed between enhanced lung cancer risk and restriction fragment length polymorphisms (RFLPs) of the P450IA1 gene with the restriction enzyme MspI. Genotype frequencies of 0.49 for the predominant homozygote, 0.40 for the heterozygote, and 0.11 for the homozygous rare allele were observed in a healthy population. Among lung cancer patients, the frequency of homozygous rare allele of P450IA1 gene was found to be about 3-fold higher than that among healthy population, and this difference was statistically significant. This is the first report to identify the genetically high risk individuals to lung cancer at the gene level.

Neuronal regulation of substrate cycle between glucose 6-phosphate and glucose in brown adipose tissues of cold-exposed mice.

The increase in both glucose 6-phosphatase and hexokinase activities in brown adipose tissues of cold-exposed mice probably relates to thermogenesis by the substrate cycle between glucose 6-phosphate and glucose (Watanabe et al.: Anatomical Record 219:39-44, 1987). To clarify the factors causing the simultaneous increase, we examined biochemically the effects of uni- or bilateral surgical denervation of brown adipose tissues, of adrenalectomy, or of streptozotocin injection on the increase in the two activities in the tissues of cold-exposed mice. Further, the effects of denervation on the increase were also examined histochemically. The simultaneous increase in the two activities was inhibited in the denervated tissues of cold-exposed animals in biochemical and histochemical experiments. However, the increase in the activities was not inhibited in the tissues of animals exposed to cold after adrenalectomy or streptozotocin injection. The results suggest strongly that the activation of the substrate cycle in brown adipose tissues of cold-exposed mice is caused by a transmitter released from sympathetic nerve endings, probably norepinephrine.

Fine structure and function of hepatocytes during development.

This paper reviews the fine structure and function of hepatocytes during fetal and postnatal development. Bile canaliculi develop to a mature appearance during perinatal and early postnatal periods, while bile secretory function is immature at birth and develops during the postnatal period. The rough endoplasmic reticulum is prominent and remains unchanged in amount during development, and the Golgi complex is large from early stages of fetal life. The smooth endoplasmic reticulum (SER) appears shortly before birth and increases in quantity to the adult level after birth. In mouse hepatocytes, Sv (area per unit cytoplasmic volume) of SER increases in perivenular cells between 1 and 10 days of age, although it remains low in periportal cells. Similarly, Sv of total ER increases in both periportal and perivenular cells between 1 and 5 days of age and then becomes greater in perivenular than periportal cells. This suggests that the postnatal increase in the drug-metabolizing capacity occurs predominantly in perivenular hepatocytes. SER proliferates after phenobarbital (PB) administration in both perivenular and periportal cells in 3-, 5-, and 10-day-old mice, and predominantly in perivenular cells in 20-day-old and adult mice. Thus the conspicuous proliferation of SER in perivenular hepatocytes after PB administration, characteristic of adult liver, becomes manifest during postnatal development. In mouse hepatocytes, Vv (volume per unit cytoplasmic volume) of mitochondrial matrix and peroxisomes and Sv of mitochondrial inner membrane and cristae increase in both periportal and perivenular cells between birth and 10 days of age. Then, Vv of mitochondrial matrix remains unchanged in periportal cells but decreases in perivenular cells. In general, the process of postnatal hepatocyte differentiation appears to include several phases of development; cell organelles develop during the early postnatal period, subsequently the cells undergo both functional and structural heterogeneity, and the late postnatal period after weaning is the time for a marked increase in cell size.
