Effect of mazindol on feeding behaviour and on noradrenergic function of various parts of the rat brain.

The effect of mazindol on feeding behaviour was investigated in rats fasted for 24 hours. They were then given mazindol i.p. and placed in a new environment. The effect of the drug on the disappearance of 3H-noradrenaline (3H-NA) from various parts of the brain after injection into the cerebral ventricles was also investigated. Mazindol was more potent in suppressing feeding behaviour than in reducing food intake, indicating that a number of aspects of feeding behaviour may be more sensitive indices of the anorectic effect of a drug than food intake per se. Mazindol, in a dose of 30 mg/kg, accelerated the disappearance of 3H-NA from the medial hypothalamus but not from other areas of the brain, suggesting that the drug has a locus-specific action on cerebral noradrenaline metabolism.

Tiaprofenic acid (surgam) in the treatment of osteoarthritis of the knee and hip.

Tiaprofenic acid 200 mg three times a day was compared with indomethacin 25 mg three times a day in 36 patients suffering from osteoarthritis of the knee and hip. In this double-blind cross-over study little significant difference in efficacy between the drugs was seen. Although no significant difference in adverse effects was found, significantly fewer central nervous system side-effects were associated with tiaprofenic acid treatment.

[Type and frequency of cardiac arrhythmias dependent on age, therapeutic aspects (author's transl)].

Resting-, exercise and 24-h-ECG tracings of outpatients were examined for cardiac arrhythmias and were analyzed for age-dependent phenomena in the range between 20 and 100 years. Typical of higher age was especially the combination of different arrhythmias, in particular the simultaneous occurrence of slow and fast disturbances, which, of course, makes medical therapy difficult. Since ventricular arrhythmias, when classified according to Lown, have a better prognosis in the age group above 70 than between 40 and 70 years, antiarrhythmic drugs should be prescribed with extreme caution to older people and should be limited to exceptions which are detailed in this study.

Clotrimazole in rheumatoid arthritis.

Forty-seven patients with active rheumatoid arthritis took part in an 8-week controlled study in which clotrimazole was compared with a standard nonsteroidal anti-inflammatory agent, ketoprofen. Although clotrimazole was shown to be effective in the treatment of the disease and superior to ketoprofen in certain measurements, if was also responsible for a high incidence of adverse effects. Improvement with clotrimazole took place more slowly but was more sustained than with ketoprofen. A significant rise in plasma cortisol and a fall in white cell count was observed in the clotrimazole treated patients.

Epidemiological information on leprosy in the Singu area of Upper Burma.

In the course of a WHO trial designed to evaluate the possible protective action of BCG vaccine against leprosy, a longitudinal epidemiological study of the whole population was carried out in an area of very high endemicity in Burma from 1964 to 1976. Two mass surveys of the whole population with an interval of 4 years and annual re-examination of the 28 000 children (0-14 years) in the BCG trial were carried out. The data collected yielded important information about general prevalence and yearly incidence of the disease as well as on sex, age, and classification of cases. The general prevalence rate declined from 32.6 per 1000 in the first survey to 25.2 per 1000 in the second. The number of cases among males was significantly higher than among females. Incidence rate among contacts of already known cases was 9.8 per 1000 person-years. The estimated yearly incidence among non-contacts was 5.9 per 1000. Prevalence rates reached a peak in the 20-39-year age group. The prevalence rate of multibacillary patients also reached a peak in the same age bracket. It is stressed that a further period of epidemiological surveillance will be essential in order to have a correct estimate of the expected number of new infections, especially multibacillary cases, in the 20-39-year group. The value of this information is considered unique for planning and programming of future control activities.

An information system for leprosy control (OMSLEP Recording and Reporting System).

This study reports the various steps involved in the design of a simplified information system for leprosy (OMSLEP), developed in cooperation between the Unit of Epidemiology, University of Louvain, Belgium, and WHO. The objective of the system is to permit the evaluation of a) the efficiency of programs within the context of established strategies and norms; b) the effectiveness of leprosy control methods from an epidemiological point of view; c) the efficacy and productivity of certain program components. Prior to designing the system, the relevant epidemiological and operational indices have been reviewed. A survey was also made of the forms used by some 78 leprosy control schemes throughout the world in order to analyze the current information now being collected. The proposed system is described. It includes individual record form to be filled at registration and once yearly in subsequent years of follow-up, a detection form, and an annual statistics form for the tabulation of total patients. The system is presently being tested in some 15 countries.

Effect of antidiuretic hormone and indomethacin on intrarenal microsphere distribution.

The purpose of the present study was to reexamine the effect of antidiuretic hormone (ADH) and indomethacin (IM) on intrarenal blood flow distribution by applying the microsphere technique to trained consciuous dogs. ADH and IM were given separately and jointly during maximal water diuresis. ADH in itself caused moderate antidiuresis and marked natriuresis. Although IM by itself had no significant effect on urinary osmotic concentration or solute excretion it greatly enhanced the antidiuretic effect of ADH and abolished its natriuretic effect. Intrarenal blood flow distribution remained unchanged when ADH and IM were given separately but it was shifted toward the inner cortex when the drugs were administered simultaneously. Furthermore, transition from spontaneous antidiuresis to maximal water diuresis did not alter intrarenal blood flow distribution. The results indicate that ADH induced antidiuresis is dissociated from changes in intracortical blood flow distribution. However, the possibility that IM enhancement of ADH mediated antidiuresis is at least in part mediated by intrarenal hemodynamic changes cannot be excluded.

[Functional effects of micro-oil embolization following lymphography].

Lymphography, if performed correctly, has few clinically relevant complications. Nevertheless, the passage of oily contrast medium into the pulmonary vascular bed along physiological pathways can usually be verified radiographically and functionally. Since that part of the contrast medium filtered in the lungs is of no diagnostic value we think that a total amount of 14--16 ml Lipiodol Ultrafluid should normally suffice for outlining the retroperitoneal lymph passageways from the legs. With smaller amounts one may misinterpret an incomplete filling of the cranial paraaortic lymph nodes as filling defects. We agree with most authors that in patients with abnormal lung function, especially those with restrictive disorders, and in patients with pre-existing right heart strain lymphography should only be considered when truly indicated and only after testing of pulmonary function.

[Results of screening for risk factors in a rural community (author's transl)].

515 women and 372 men from a rural community of 2862 inhabitants over 14 years volunteered to participate in a screening test for risk factors. We found an exceptionally high percentage of hypertension and hypercholesterolaemia as well as overweight. Women were more frequently affected than men except for smoking, which occurred 3,6 times more often in men than in women. Since the life expectancy of men in this community was clearly lower than that of women, smoking seems to have the most weight as a risk factor for cardiovascular diseases.

Diflunisal compared with naproxen in the treatment of osteoarthrosis of hip or knee: a double-blind trial.

A randomized double-blind trial was carried out in 20 patients with osteoarthrosis of the hip or knee to compare the efficacy and tolerance of treatment with diflunisal or naproxen. During the first 4 weeks, patients received either 250 mg diflunisal or 250 mg naproxen twice daily and this was increased by 250 mg daily in 5 patients on diflunisal and in 3 on naproxen for the second 4 weeks of the trial. The results of subjective assessments made before and at the end of Week 8 showed a trend in favour of diflunisal for improvement of symptoms, except for weight-bearing pain which was improved in only 1 patient in each group. More of the patients receiving diflunisal than naproxen considered treatment to have been satisfactory, and rated their response as equally as good as or better than previous medication. Diflunisal produced significantly high incidence of gastro-intestinal upsets, leading to the withdrawal of 2 patients at Week 4.
