The short term effects of bezafibrate on the hypertriglyceridaemia of moderate to severe uraemia.

Hypertriglyceridaemia, an atherogenic risk factor, is a well recognised complication of uraemia, and is present in the earliest stages of the disease. Bezafibrate is an effective hypolipidaemic agent, and its effect in moderate to severe uraemia is documented in this study. Significant reductions in serum triglyceride and cholesterol have been achieved after 1 month's therapy with a reduced dosage of bezafibrate. A reduction in the hyperinsulinaemia was also seen, but no change in the fractional removal rate of injected lipid emulsion (K2) was observed. An accelerated decline in some patients' renal function was observed, which was partially reversed on cessation of treatment. Reversible elevations in the serum creatinine phosphokinase were also seen, but no patient exhibited the myositis-like syndrome associated with clofibrate.

Fatal self-poisoning with benoxaprofen.

A report of the first fatal self-poisoning due to primary toxicity of benoxaprofen. Benoxaprofen is toxic in acute overdosage to the central nervous system, myocardium and kidneys, but appears to spare the liver.

Acute ventilatory failure and myoglobinuria.

The prognosis of myoglobinuria is good even when acute renal failure occurs. We describe two patients who died from acute ventilatory failure due to myopathic involvement of the respiratory muscles. This rare complication of myoglobinuria has a high mortality despite ventilatory support.

The effect of renal transplantation with a minimal steroid regime on uraemic hypertriglyceridaemia.

We have studied the lipid and associated biochemical indices in 18 renal transplant patients maintained on an alternate day prednisolone regime. At an average 27 months following transplantation serum lipids were not significantly different from normals despite some renal impairment in the transplant recipients. In addition the blood sugar and plasma immunoreactive insulin levels were elevated in transplanted patients. The intravenous fat tolerance test (IVFTT) was not found to be different from that of normals. In some patients IVFTT was performed before and after transplantation and it was shown that K2 (fractional removal rate of Intralipid) improved from abnormal to normal values. Thus, it has been demonstrated that previously abnormal lipid levels can return to normal after transplantation and this may be related to alternate day steroid regimes. Impaired removal of triglyceride seen in chronic renal failure is improved by transplantation.

Predictors of renal function in diabetic and non-diabetic renal disease.

Forty-two patients with renal disease due to diabetic nephropathy (14 patients), tubulointerstitial disease (14 patients) and glomerular disease (14 patients) underwent measurement of glomerular filtration rate (GFR) by the 51Cr-EDTA 'one-shot' method and simultaneous serum beta 2-microglobulin, serum creatinine and creatinine clearance estimation. Serum creatinine was a significantly better predictor of GFR than serum beta 2-microglobulin in patients with diabetic nephropathy, whereas both methods were equally useful predictors of GFR in non-diabetic renal disease. Serum creatinine measurement remains the best method for detecting early reduction of GFR on a serum sample.

Accelerated hypertriglyceridemia in patients on continuous ambulatory peritoneal dialysis - a preventable abnormality.

Increasing hypertriglyceridemia is a recognised complication of CAPD. To investigate the etiology lipid clearance studies using the intravenous fat tolerance test were performed in control subjects and in uremic patients before and after six months CAPD treatment. Oral carbohydrate intake was restricted and the use of hypertonic dialysate kept to a minimum. Although serum triglycerides were elevated in the uremic patients before CAPD (3.1 +/- 0.3 mMol/L) no further increase occurred during therapy. Oral carbohydrate intake was restricted to 240-250 Gms/24 hrs and hypertonic dialysate to 5.7 +/- 4.1% of the total. There were no significant changes in fractional removal rate of lipid. Therefore further increases in serum triglycerides in CAPD may be prevented by simple dietary manoeuvres.

Recurrent proteinuria in chronic lymphocytic leukemia.

A case of chronic lymphocytic leukemia complicated by the nephrotic syndrome (due to membranoproliferative glomerulonephritis) is reported. There was complete remission of the proteinuria on two occasions in response to treatment of the leukemia. Possible etiological mechanisms are discussed and review of the literature is presented.

Evaluation of the intravenous fat tolerance test in chronic renal disease.

The intravenous fat tolerance test (IVFTT) was performed on 17 controls, 25 patients with non dialyzed, non nephrotic chronic renal failure and 32 patients on hemodialysis. The fractional removal rate of triglyceride (K2) from plasma is decreased to the same degree in renal disease whether treated by hemodialysis or not. There was an inverse relationship between K2 and both serum triglyceride and fasting blood sugar, which were significantly increased compared to controls. The IVFTT also separated patients with renal disease and hypertriglyceridemia into two groups. One, where there was a low K2, suggesting that defective clearance of triglyceride was the major etiological factor in the hypertriglyceridemia and two, those patients with a normal K2 value, where other factors appear to be more important.

Fulminant psittacosis.

Two patients died of psittacosis after presenting with generalised toxaemia, acute renal failure, and evidence of pancreatitis. Death was attributed to the virulence of the chlamydial strain and the delay in antemortem diagnosis. In one case Chlamydia psittaci was isolated from necropsy lung tissue. A third case of psittacosis suggested person-to-person or fomite spread, which is rarely reported. Infection was acquired from an apparently healthy, imported and quarantined cockatiel. Import restrictions on psittacines should thus be tightened and psittacosis should be made a notifiable disease.
