Open aortic bioprosthesis at zero pressure. A new concept in glutaraldehyde fixation of tissue valve.

An experimental trial has been made to fix a porcine aortic valve in a semi-open position using 1 percent glutaraldehyde (open model). Four valves of open model xenograft were examined with use of our pulse-simulator. Four valves fixed in the closed position (closed model) and one untreated valve were studied for comparison. Comparison of the opening characteristics at the pump pressure of 120 gr/sq.cm revealed the significant difference in that the open model valves gave the simultaneous cusp opening with a circular orifice, while the closed model valves opened in sequence with irregular orifice. This relationship was substantiated by forward flow measurement. Analysis of the flow velocity curve confirmed the competence of the open model xenograft. These results suggest that the open aortic bioprosthesis may provide an improved hemodynamic function under low flow conditions.

Prosthetic valve surgery in tricuspid area.

Up to September 20, 1975, 1,020 cases of valvular heart diseases were surgically treated at the Sapporo Medical University Hospital. In these series, there were 33 cases done for the tricuspid valve which included 27 valve replacements (81.8%) and 6 annuloplasties (18.2%). We have demonstrated that TVR may improve hemodynamic condition for any type of TI, however, postoperative A-V block and sinus bradycardias seemed to be serious complications with relatively high incidence. On the other hand, long term hemodynamic results of TAP was demonstrated to be comparable to that with TVR. Therefore, much more reliable technique of tricuspid annuloplasty may bring much better long term results. We have shown our annuloplasty "Circumferential Annular Constriction" in which a paired purse-string sutures along the tricuspid annulus may produce almost the same constricting effect as the use of a ring material.

Bcl-2 expression and apoptosis in nephrotoxic nephritis.

The product of the Bcl-2 proto-oncogene has been shown to prolong cell survival by preventing apoptosis in several cell lineages. To investigate the regulatory mechanisms of apoptosis in glomerulonephritis, we examined the expression pattern of the Bcl-2 protein together with cellular events in rat nephrotoxic nephritis. Bcl-2 protein and proliferating cell nuclear antigen were detected in glomeruli by immunohistochemistry. Morphologic changes of apoptosis were identified by electron and light microscopy and an in situ DNA nick end labeling method. The first (heterologous) phase began with significant neutrophil infiltration shortly after the injection of nephrotoxic serum. Both Bcl-2 expression and the number of proliferating cells in the glomeruli were at maximum at 24 h in the heterologous phase. Glomerular hypercellularity with an influx of macrophages and the number of apoptotic glomerular cells peaked on day 14 in the second (autologous) phase. Glomerulonephritis resolved after that. These results suggest that overexpression of the Bcl-2 protein may play a role in glomerular cell survival and exacerbation of glomerulonephritis. Apoptosis may occur as an active mechanism in the resolution of the autologous phase in nephrotoxic nephritis.
