The role of thyroid therapy in patients with thyroid cysts.

Twenty patients with benign thyroid cysts were studied in a prospective double-blind fashion to determine the effect of thyroid therapy on the recurrence of these cysts after aspiration. When the 10 patients receiving placebo medication were compared with the 10 patients ingesting thyroid hormone, no significant difference was found in the time either group was free of cyst recurrence. We conclude that thyroid therapy is not effective in preventing the recurrence of benign thyroid cysts after initial aspiration.

Hyperthyroidism and thyroid cancer. A report of three cases.

In patients with hyperthyroidism and a palpable solitary nodule, the physician should be alert to the need for further evaluation before a method of therapy is chosen. In our opinion, surgery should be the treatment of choice for a hyperthyroid patient with a "cold" solitary thyroid nodule. We no longer believe that hyperthyroidism minimizes the possibility of coexistent thyroid carcinoma in hyperthyroid patients with hypofunctioning nodules. We would also suggest a reexamination of the concept of thyroid carcinoma being dependent on thyroid-stimulating hormone.

Rehabilitation in the extended care facility.

Rehabilitation can be readily carried out in an extended care facility. A technique is presented for accomplishing this objective by organizing existing personnel within the facility into a rehabilitation team which is goal-oriented. The Barthel Index serves as an objective measurement of the patient's progress. By use of this approach, significant numbers of patients with stroke, orthopedic disorders and other neuromusculoskeletal impairments can be returned to their previous living situations in a relatively short period, and at minimal cost.

Tissue testosterone and dihydrotestosterone from bilateral testis biopsies in males with varicocele.

Nine males with varying degrees of subfertility with left varicocele underwent bilateral testicular biopsy for determination of testicular testosterone (T) and dihydrotestosterone (DHT) content. Testicular tissue T and DHT levels varied greatly, and there was no significant difference when the group T and DHT content of the left testis was compared with that of the right testis. When the respective mean preoperative sperm count was compared with the bilateral tissue T and DHT content, no consistent relationship was found.

Analysis of corneal crystalline deposits in multiple myeloma.

A 46-year old woman and a 59-year-old man had corneal crystals, multiple myeloma, and IgG kappa hypergammaglobulinemia. In one case, crystalline deposits were also present in the lens. In both patients there was a marked decrease in the amount of crystals during chemotherapy. The crystals within the cornea of one case were identified by light and electron microscopy in material obtained during a lamellar keratoplasty. The crystalline deposits were located only in the corneal epithelium, and their regular repeating internal arrangement was confirmed by monochromatic optical diffraction of electron micrographs of sections through them. By immunofluorescent and immunoperoxidase techniques, the crystals reacted positively for immunoglobulin and particularly IgG kappa chains.

Ventilatory responses to exercise and to carbon dioxide in mitral stenosis before and after valvulotomy: causes of tachypnoea.

1. The ventilation and cardiac frequency during progressive exercise and the respiratory responses to breathing carbon dioxide have been measured in 33 female patients with mitral stenosis and in 31 control subjects. Compared with the control subjects, the patients' exercise ventilation and cardiac frequency were increased; the exercise tidal volume at standard minute volume, the vital capacity and the ventilatory response to carbon dioxide were reduced. The extent to which the standardized tidal volume was lower during exercise than during breathing carbon dioxide was correlated with the severity of the stenosis, as gauged by the increase in exercise cardiac frequency above the level predicted from anthropometric measurements. 2. Twenty patients were studied postoperatively. In the 12 who showed clinical improvement the exercise ventilation and cardiac frequency were reduced and the exercise tidal volume at a given minute ventilation was increased. The latter change occurred despite a reduction in vital capacity, which was probably a residual effect of thoractomy. There was no significant change in the response to breathing carbon dioxide. No material change in function was observed in the patients whose condition was not improved by the operation. 3. It is suggested that in mitral stenosis the tachypnoea which occurs during exercise, whilst mainly a mechanical consequence of the reduced vital capacity, is also partly due to pulmonary congestion stimulating intrapulmonary receptors.

Ventilation, cardiac frequency and pattern of breathing during exercise in men exposed to O-chlorobenzylidene malononitrile (CS) and ammonia gas in low concentrations.

Ventilation minute volume, tidal volume and cardiac frequency during submaximal exercise have been measured in healthy young soldier volunteers exposed to O-chlorobenzylidene malononitrile (CS) and ammonia gas in concentrations respectively of 0.16 to 4.4 mg.m(-3) and 50 to 344 mg.m(-3). The response of ventilation minute volume to the two gases is apparently similar; both gases cause a reduction of, on average 6%. With low doses this reflects a diminution in respiratory frequency whereas with higher doses it is due to a reduction in tidal volume which is accompanied by tachypnoea. The findings may result from stimulation successively of receptors in the larynx and of irritant receptors in the large airways of the lung. The pain which is a feature of exposure to CS but not to ammonia is due to stimulation of other so far unidentified receptors. Neither gas has a direct effect upon the exercise cardiac frequency.

Calcification of soft contact lenses in patient with dry eye and elevated calcium concentration in tears.

Following radiotherapy for an extensive intraepithelial carcinoma of the conjunctiva and eyelid, a patient developed opacification of soft contact lenses used on the same side. Calcilm was demonstrated within the contact lenses by cytochemical methods and by energy-dispersive x-ray microanalysis. The phenomenon was associated with unilateral tear insufficiency and an elevated tear calcium level as determined by atomic absorption spectrophotometry.

Induced corneal vascularization remission with argon laser therapy.

Vascularization was induced in rabbit corneas by the application of silver nitrate. Argon laser photocoagulation was applied to the entire length of these corneal vessels with relatively long pulses of energy. Untreated areas of vascularization were used as controls. There was a decrease in the corneal vascularization in the treated areas that exceeded any natural tendency toward remission. The results suggest the efficacy of inducing a remission of corneal vascularization with argon laser photocoagulation.

Ultrathin coating by plasma polymerization applied to corneal contact lens.

Plasma coating (deposition of polymer under the influence of plasma) is utilized to modify the surface properties of corneal contact lens. An ultrathin layer (thickness of roughly 200 A) of plasma polymer of acetylene/H20/N2 is applied to poly(methyl methacrylate) (PMMA) corneal contact lenses. The surface becomes highly wettable with water compared to uncoated lens. When coated and uncoated contact lenses are placed on rabbit eyes, a remarkable difference in accumulation of mucous matter is observed. With the control lenses, the accumulation of mucous matter in a week is sufficient to affect the optical clarity of the lenses, whereas the coated lenses show no change after three months continuous wearing. The comparative degree of adhesion of the corneal epithelium cells onto glass, modified glass, PMMA, and coated PMMA surfaces is studied using tissue cultures and phase contrast microscopy. The coated PMMA surface exhibits a degree of tissue adhesion lower than that of control PMMA and higher than that of glass surface, and no sign of toxicity of the coated surface is observed by the tissue cultures.

Growth hormone responses following double pulse oral glucose administration in various clinical states.

The 8-h double pulse oral glucose test is proposed as an alternative screening procedure for determining the adequacy of growth hormone (GH) release. The second pulse of glucose is timely in suppressing GH release and delaying it for a more predictable controlled elevation. Peak GH (mean plus or minus SD) values following the double pulse glucose test were 17 plus or minus 10.2, 16.5 plus or minus 2.2, and 1.3 plus or minus 0.5 ng/ml in normal controls, short stature patients and GH deficient patients, respectively. Peak GH values following insulin hypoglycemia were 36.9 plus or minus 13.8, 21.9 plus or minus 23.0 and 1.8 plus or minus 1.1 ng/ml in normal controls, short stature patients and GH-deficient patients. Peak GH values during a 5-h oral glucose tolerance test were 16.4 plus or minus 6.0 and 10.2 plus or minus 3.3 ng/ml in normal controls and short patients, respectively. These differences in peak GH values between the various clinical and control groups were not significantly different except for the greater GH peak values reached in short patients in the double pulse glucose test compared to the 5-h oral glucose tolerance test. The rise in GH following double pulse oral glucose is more timely predictab-e than after the 5-h oral glucose test and involves less professional time than the insulin tolerance test as it does not require close medical supervision.

Aniridia and congenital ptosis.

Congential ptosis associated with aniridia was found in 3 patients from 2 pedigrees. Nonsurgical aphakia, a condition not previously reported in association with aniridia, was also found in one case. The association of aniridia and congenital ptosis is suggested as evidence for the common mesodermal etiology of both anomalies.
