Pulmonary embolism treated with a single dose of anisoylated lys-plasminogen streptokinase activator complex and systemic heparinization; a report of two cases.

Pulmonary embolism (PE) is an important cause of morbidity and mortality in the Western World. Systemic heparinization is usually applied as the treatment of first choice. However, a subgroup of patients presenting with massive PE and haemodynamic deterioration die in the acute phase or remain severely disabled. Several studies have demonstrated the beneficial effects of thrombolytic agents in the treatment of massive PE. This communication presents two patients with clinically massive PE of recent onset (confirmed by lung perfusion scans) who were successfully treated with a single i.v. dose of 30 mg of anisoylated lys-plasminogen streptokinase activator complex (APSAC, comparable to 1,500,000 U of streptokinase) followed by systemic heparinization for 7 days. Both patients showed a considerable improvement in their clinical condition shortly after APSAC administration. Control perfusion scans made after two days revealed marked reduction of perfusion defects.

Structure elucidation of acid reaction products of indole-3-carbinol: detection in vivo and enzyme induction in vitro.

The potency of indole-3-carbinol (I3C) to form condensation products under acidic aqueous conditions was studied. After identifying a known dimer, 3,3'-diindolylmethane (DIM), we elucidated the structures of two trimers also found in acid reaction mixtures: 5,6,11,12,17,18-hexahydrocyclonona[1,2-b:4,5-b':7,8-b"]tri-indole (CTI), and 2,3-bis[3-indolylmethyl] indole (BII). The formation of these indole oligomers was shown to be pH dependent. The highest amounts of DIM and BII were formed in aqueous solutions having a pH value ranging from 4 to 5. No CTI could be detected at pH values above 4.5. In rats that received an oral dose of I3C we could detect DIM and BII in gastric contents, stomach tissue, small intestine and liver. No CTI could be detected in vivo after oral exposure to I3C. In in vitro experiments, using rat hepatocytes, the cytochrome P-450IA1 apoprotein level, 7-ethoxyresorufin O-deethylation activity (EROD) and DT-diaphorase activity (DTD) were markedly enhanced by DIM and CTI as well as BII.

Clinical versus subclinical varicocele: venographic findings and improvement of fertility after embolization.

Venography and coil embolization of the left internal spermatic vein and/or right internal spermatic vein was performed in 75 patients with clinical or subclinical varicocele. Venograms of clinical varicocele were compared with venograms of subclinical varicocele. A major difference between the two groups was the degree of reflux. However, in terms of spermatologic improvement and restoration of fertility, embolization treatment appeared to be about equally effective in clinical and subclinical varicocele.

Evaluation of a new distal delivery guidewire for steel coils.

A new distal delivery guidewire (DDG) for steel coils is described. By means of a DDG "mini" coils as well as "new" coils can be delivered intravascularly through catheters with an inside diameter large enough to let pass a 0.97 mm. guidewire. Delivery using a DDG is compared with delivery using a pusher guidewire (conventional method); the former seems to be safer. Additionally, coil placement beyond the location of the catheter tip is only possible when using a DDG. Forty-nine venous and four arterial coil placements resulted in only one misplacement which could be corrected angiographically.
