Outcome-oriented quality assurance in an ambulatory setting.

Quality assurance has become an increasing concern to the government, health care plans, and organized medicine as managed care systems reduce absolute dollars spent on care. Most small group practices and individual physicians have little control over the various methods used to evaluate the quality of care. A computerized quality management approach improves quality assessment and directs local changes to improve patient care.

Computerized clinical information.

The use of computers for medical practice has not evolved as rapidly as might be expected. Even though MEDLINE searches are universally used for in-depth search of medical information, the effective use of computers in direct patient care has had very limited acceptance. The Waianae Coast Comprehensive Health Center (WCCHC) has supported the development of an in-house computer billing and information system, Pacific Area Medical Manager (PAMM), that includes a Patient Health Status (PHS) module. Through local development of the system, physicians are involved in every level of planning the PHS. Acceptance of the computer system by physicians has increased.

Gestational diabetes in a rural setting.

Women who are already diabetic and become pregnant, as well as women who develop gestational diabetes, have increased risks of complications to both fetus and mother. These risks in gestational diabetes mellitus (GDM) can be reduced to near that of a non-diabetic mother by normalizing the blood sugar. The current recommended standards are reviewed. Utilizing a team approach, care was provided to patients with GDM in a rural primary care setting in order to attempt to normalize the blood sugar to the recommended level. Review of the outcomes of these pregnancies supports the conclusion that acceptable care for patients with GDM can be provided away from the tertiary care centers and in the primary care setting.
