Experience with ITI osseointegrated implants at five centres in the UK.

Experience with the two-part ITI titanium implant system at five centres in the UK is reported. A total of 461 implants were inserted in 176 patients, who received 189 prostheses. Criteria for the assessment of success following implantation are listed. Results over a 3-year period showed a success rate of 94% when implants were used for mandibular overdentures and crown and bridge prosthetics. A lower success rate in the edentulous maxilla was recorded.

Concentration of cefuroxime in mandibular alveolar bone following a single preoperative dose.

A pilot study was undertaken on 12 patients undergoing the bilateral removal of impacted mandibular wisdom teeth to determine the level of cefuroxime sodium within the alveolar bone adjacent to the third molar teeth following a single preoperative intravenous administration. The results indicated that a single 1.5 g intravenous bolus injection provided satisfactory alveolar bone concentrations for routine antibiotic prophylaxis at the time of wisdom tooth removal.

Dental practitioner attendances at postgraduate courses in a dental school.

The attendance pattern of dental practitioners who resided in the West Midlands and attended postgraduate courses at Birmingham University Dental School was studied. Data were collected from the participants as to their name, the location of their practice and the type of course that they attended. Other information such as their qualifications or their attendance at other postgraduate courses was sought from appropriate records. The percentage of West Midlands' dentists who attended postgraduate courses at Birmingham University Dental School was found to be 41%, although this figure was higher amongst Birmingham (53%) and Wolverhampton (51%) practitioners. It is known that practitioners participated in other types of continuing educational activities elsewhere. The majority of dentists attending courses had been qualified for less than 15 years. There were multiple attendances from participants who were already involved in other postgraduate activities such as the MGDS. This study also highlighted the important role of university employed staff in providing postgraduate education for general dental practitioners.

Tooth pulp changes following Le Fort I maxillary osteotomy in a primate model.

Le Fort I maxillary 'down-fracture' osteotomy with 8 mm advancement was performed in 15 adult rhesus monkeys. Forty-five tooth pulps were examined histologically at intervals from 0 to 24 weeks after surgery. Cell degeneration occurred in 31% of pulps examined, necrosis in 16% of pulps and osteo-dentine was found in 7% of pulps. Almost half of the teeth examined (47%) showed marked cellular changes, more frequently found in posterior teeth. Other features noted were inflammation (13%) and reactive dentine in pulps (24%). Axons degenerated initially but recovered by 24 weeks. It is concluded that Le Fort I maxillary osteotomy caused pulpal disturbances in an animal model and the extent to which this occurs in patients needs to be carefully monitored.

A histological study of stainless steel and titanium screws in bone.

This study compared histologically the tissue response of stainless steel and titanium screws when inserted into the calvaria of eight beagle dogs for periods of 1, 4, 12, and 24 weeks. There was minimal fibrous reaction around both screw types with excellent long-term bone healing. After 24 weeks there was no discernable difference in the tissue reaction between the two types of screw.

Effect of autogenous bone grafts on healing following Le Fort I maxillary osteotomies.

The healing of Le Fort I maxillary osteotomies was studied histologically in 12 adult rhesus monkeys to determine the effect of split autogenous rib grafts overlaying the osteotomy cuts on the lateral maxillary walls. The maxilla was advanced 8 mm in each animal, and examined at varying intervals up to 24 weeks after surgery. The results indicated that there were no obvious differences between the monkeys with and without the rib grafts. Both groups healed with good osseous union. It is concluded that autogenous rib grafts did not enhance bony healing.

Practice visits.

This article reviews the practice visiting procedures employed in vocational training. The particular usage examined is that employed in the West Midlands region after a 10-year development period, although most regions employ a similar format. It is hoped that the article will be of particular assistance to practitioners contemplating an application to become a trainer.

[Surgical laser in dentistry in 1989].

After many years of microscopie, cellular and molecular studies, the use of Laser rays progressively developed in dentistry. This therapeutical "tool" is now well established in numerous dental fields: cariology, pathology of oral mucosa, periodontics... The Europeans have great advance in this research area and current progress is the clear proof of their work efficiency.

A comparison of ibuprofen and dihydrocodeine in relieving pain following wisdom teeth removal.

Although dihydrocodeine (DF118) is widely prescribed by general dental practitioners, there is little evidence that it is successful in controlling post-operative dental pain. Ibuprofen is known to be effective in this situation. A single dose, double-blind study was carried out in 148 patients to compare 400 mg ibuprofen with 30 mg dihydrocodeine and placebo for treating moderate to severe pain following the removal of unilateral, impacted mandibular third molar teeth under local anaesthesia. An additional dose of either ibuprofen or dihydrocodeine was available after 2 hours. The post-operative ibuprofen reduced pain and produced more pain relief than dihydrocodeine or placebo. Furthermore, fewer patients receiving ibuprofen took additional analgesic at 2 hours. Patients who received ibuprofen as supplementary medication also experienced less pain and had greater pain relief than those receiving dihydrocodeine as supplementary medication, even when their post-operative treatment had been placebo. More patients reported the medication as having been effective if they took ibuprofen either post-operatively or as supplementary analgesia. Ibuprofen is an appropriate analgesic for treating post-operative dental pain.

Human tissue response to porous hydroxyapatite implants. A case report.

Histological examination of 2 porous hydroxyapatite implants removed from a patient showed a pattern of ossification similar to that reported in a previous animal study. New bone was laid down on the surface of the hydroxyapatite but did not progress to occupy all the pores. Although porous hydroxyapatite blocks are biocompatible and well tolerated by the tissues, they are not recommended for augmentation of the resorbed alveolar ridge where the overlying mucosa is thin and atrophic, because of the risk of mucosal dehiscence and implant exposure. They are suitable in situations where they are deeply buried in the tissues and not subjected to direct trauma.

Tongue resection with the CO2 laser: the effects of past radiotherapy on postoperative complications.

A clinical study was undertaken in 21 patients to assess the effects of past radiotherapy on complications after tongue resection with the CO2 laser. Ten of the 11 irradiated patients experienced postoperative problems such as pain, haemorrhage, oedema, infection, delayed healing and poor function, while only one of the 10 non-irradiated patients had a problem due to minor bleeding after laser surgery. It is concluded that the CO2 laser is useful for primary excision of accessible tumours in the anterior tongue, but may be associated with more postoperative complications if the tongue has previously been irradiated. Where the tongue resection is part of a more radical excision, the CO2 laser is less advantageous.
