Some principles of postoperative epidural pressure monitoring.

A retrospective analysis is presented of 115 cerebral tumour cases where continuous epidural pressure (EDP) monitoring was carried out postoperatively using an implantable transducer constructed on a coplanar principle. Correct implantation of the transducer is crucial to reliable measurement. The influence of transducer location on EDP is highly significant and can be accounted for at least partly by hydrostatic forces. The effect of postural changes similarly has a hydrostatic component. Clinically complicated cases were usually easily distinguishable from uncomplicated cases with a similar transducer location. EDP is usually low in the absence of complications during the first 6 to 12 hours postoperatively. For postoperative monitoring, an EDP transducer should be placed as near to the site of surgery as possible for maximum sensitivity to local pressure changes and the earliest detection of complications.

Regional cerebral blood flow in patients with subarachnoid haemorrhage.

With a multidetector scintillation camera regional cerebral blood flow, rCBF, was evaluated in 13 patients with subarachnoid haemorrhage. Mean CBF was subnormal, and there seems to be a relationship between CBF and severity of the neurological deficit. The regional pattern showed ischaemic as well as hyperaemic areas. Vasospasms mostly produced ischaemic areas, roughly corresponding to the region supplied by the involved artery. The hyperaemic areas were striking. This hyperaemia can perhaps be regarded as a reaction to initial ischaemia.

Techniques in transfemoral lumbar epidural phlebography.

For lumbar epidural phlebography an even distribution of the injected contrast medium over the epidural and intervertebral venous system is of the greatest importance in order to prevent the false positive diagnosis of a lumbar disk lesion due to an artificial filling defect. Various catheter placement techniques and other procedures designed to enhance epidural opacification are compared in this respect. In the author's opinion, selective injection of the contrast medium into an intervertebral vein, when possible, leads to the most reliable epidural filling.

Results of the treatment of hydrocephalus with ventriculo-jugular shunts.

260 children with hydrocephalus were treated with a ventriculo-jugular shunt. We compared the complication - rate between Holter-shunts and Pudenz-shunts. As far as we could verify 58 children died from several causes. 190 revisions were done in 114 patients, 110 revisions in 71 children with a Holter - valve and 80 revisions in 43 children with a Pudenz - valve. The complication-rate was 112 in 176 Holter - valves and 84 in 84 Pudenz - systems. In our hands the use of the Holtersystem gives better results. As one can conclude from these figures and those from others (Tsingoglou and Forrest 1971), the treatment of children with hydrocephalus is still far from ideal.

Convexity leptomeningeal cysts diagnosed by scinticisternography.

The scintigraphic diagnosis of eight convexity leptomeningeal cysts is described; the cysts appear as a local collection of abnormal radioactivity, best seen at 48 hours. The correlation of the scintigraphic findings with clinical, radiological, and operative findings is discussed.

Herniated lumbar discs in teenagers.

Forty-three patients under the age of 19 in whom a lumbar disc protrusion was diagnosed and surgically demonstrated are reviewed. The symptomatology resembles that of adults. In 19 cases there was clear relation of trauma to the onset of symptoms. The results of surgical treatment are discussed.

Leptomeningeal cysts diagnosed by isotope cisternography.

The diagnosis of leptomeningeal cysts by isotope cisternography is described in four cases. The cysts are visualized as abnormal local collections of the radiopharmaceutical, best demonstrated 48 hours after lumbar injection. The investigation makes it possible to diagnose the cyst at an early stage, before severe clinical symptoms and changes in the bones of the skull develop. Cases of leptomeningeal cysts in various areas of the brain are described.
