Private psychiatric hospitals, mental health care management, and wellness: an interface with industry.

Hospitals doing business is good business. Hospitals that use their professional staff to enhance their interface with industry are usually pleased with the outcome. Health care professionals must reach beyond their doors and be willing to understand the needs of a large corporate bureaucracy and the aspiring entrepreneur. Using hospital professionals in a consultative model with gatekeepers of industry is an ideal way to market and enhance the hospital's image in the community. Professionals employed by private hospitals are usually quite willing to expand their roles into the community as trainers, consultants, educators, diagnosticians, and treatment resource consultants to the business world. Business people understand business problems, and health care is a business issue as well as a humanitarian issue. In the current climate of cost containment, the hospital's ability to help the business work with paying for health care, if properly presented, will be welcomed.

Increased leukocyte sensitivity to phosphodiesterase inhibitors in atopic dermatitis: tachyphylaxis after theophylline therapy.

Previous studies have demonstrated elevated cyclic 3',5'-adenosine monophosphate specific phosphodiesterase (PDE) activity in mononuclear leukocytes (MNL) from patients with atopic dermatitis (AD) and with allergic respiratory disease. We have studied the effect of PDE inhibitors on MNL from normal subjects and from patients with AD. We measured PDE activity in MNL after in vitro exposure to Ro20-1724, theophylline, and isobutylmethylxanthine. The effect of PDE inhibitors was also measured in MNL of patients during and after oral theophylline therapy. There was a dose-response effect of Ro20-1724 on PDE activity of AD cells. This activity was significantly more inhibited than that in MNL of normal subjects or in MNL of theophylline-treated patients with AD. The inhibition pattern of PDE in MNL of treated patients with AD did not vary significantly from the pattern observed in MNL from normal patients. Two weeks after oral theophylline was stopped, the PDE activity in MNL was once again susceptible to in vitro inhibition. The relative resistance of MNL to PDE activity in normal subjects to in vitro PDE inhibitors may be due to the existence of different PDE enzyme forms or the presence of endogenous inhibitors. The clinical importance of the tachyphylaxis observed is uncertain, but our results suggest that intermittent dosing with PDE inhibitors may provide a therapeutic advantage in treatment of AD and other atopic diseases.

Measurement of spatial stability of spark position in burst-mode breakdown with the Nd:YAG laser.

Measurements to determine the precise position of the spark produced by optical breakdown in ocular media with a repetitive sequence of pulses from a Nd:YAG laser have been made. The results show that the location of the spark along the beam axis remains fixed within +/- 10 micrometers (the resolution of the experiment) for pulses with a time separation of 20 milliseconds, indicating that the surgical effects of a sequence of pulses can be cumulative.

Value of maternal anti-D concentration in predicting the outcome of Rh(D) haemolytic disease.

The outcomes of 43 pregnancies complicated by the presence of Rhesus antibodies were studied in relation to the peak concentrations of anti-D reached during pregnancy. Antibody concentration was measured by an automated method calibrated against the British Anti-D Working Standard. Where the anti-D concentration remained below 5 IU/ml, the infants at worst suffered only moderate jaundice controllable with phototherapy. Above this level the incidence of requirement for exchange or top-up transfusion was high with concentrations greater than 50 IU/ml being predictive of a very severely affected fetus. It is recommended that amniocentesis in these patients be deferred until maternal anti-D levels exceed 5 IU/ml.

Combined treatment of advanced cancer of the laryngopharynx and cervical esophagus.

Advanced cancer of the laryngopharynx and cervical esophagus is difficult to treat because of the malnutrition produced by pain and obstruction and the problems related to reconstruction. This paper presents the initial results of a regimen used in 14 patients in which there is rapid reversal of nutritional deficiencies, radical resection, and reconstruction using the gastric pull-up technique and administration of postoperative radiotherapy. Excellent overall palliation and decreased hospitalization have been achieved using this regimen without diminishing chances for cure.

Anti-C haemolytic disease requiring intrauterine and exchange transfusion.

A case is presented of anti-C haemolytic disease of the newborn in which the infant required three intrauterine transfusions and an exchange transfusion after birth. Anti-C is not a common cause of haemolytic disease of the newborn and this is the first recorded case requiring intrauterine transfusion.

Combination chemotherapy of metastatic breast cancer with vincristine adriamycin and prednisolone.

Fifty patients with metastatic breast cancer were treated with a combination of vincristine, Adriamycin and prednisolone (VAP) at four weekly intervals. Response rates of 78% for soft tissue disease, 66% for parenchymal lung disease, 67% for liver disease, and 64% for pleural effusions were seen. Only 26% of patients with bone metastases showed an objective response. These figures are as good as any previously reported for chemotherapy of breast cancer.
