[99mTc-anti-granulocyte antibodies (BW 250/183) in the detection of appendicitis].

Scintigraphy with 99mTc-labeled anti-granulocyte antibodies (AGAb) was performed in 50 patients with suspected appendicitis. Sequential and static imaging as well as SPECT of the pelvis and abdomen was performed 2 h p.i. In all patients the diagnosis was confirmed either histologically or by long-term follow-up. 13 patients had histologically proven acute appendicitis. In 11 patients the appendix scan had been positive and in 2 patients the scan had shown no significant tracer uptake in the right lower abdomen. The remaining 37 patients turned out not to have acute appendicitis. 29 out of these patients had negative and 3 had positive scan findings. In 5 patients the scan was equivocal. Out of these patients 2 had pathologic findings on the left side of the abdomen which turned out to be acute diverticulitis in one patient and acute peritonitis in the other. The remaining 3 patients with unclear scintigraphic findings had no acute appendicitis. Scintigraphy with AGAb is fast and easy to perform and thus superior to cell labeling methods for diagnosing acute appendicitis. Sensitivity for the diagnosis of acute appendicitis was 85% with a specificity of 91%. Chronic or scarred non-granulocytic appendicitis--in which there is often no definite indication for surgery--was negative in our study except for two cases.

The HIV tat gene transforms human keratinocytes.

Skin disorders are frequently seen in patients with the acquired immune deficiency syndrome (AIDS). Since many of these cutaneous manifestations are accompanied by an early onset of epidermal hyperplasia, the keratinocyte is a candidate for infection by the human immunodeficiency virus (HIV). We now report that the HIV tat gene, under the control of the viral long terminal repeat (LTR), can efficiently transform human keratinocytes in culture. Our finding suggests that this activity of the tat gene may be responsible for the epidermal hyperplasia that accompanies psoriasis and precedes the development of squamous cell and basal cell carcinomas in AIDS patients.

Liver cancer in transgenic mice carrying the human immunodeficiency virus tat gene.

Patients with the acquired immunodeficiency syndrome are at risk to develop a variety of different cancers. Based on epidemiological data, Kaposi's sarcoma and non-Hodgkin's lymphoma have been clearly associated with infection by the human immunodeficiency virus (HIV). Additional cancers such as basal cell and squamous cell carcinomas, melanoma, and hepatocellular carcinoma have also been reported to be associated with a diagnosis of acquired immunodeficiency syndrome. A direct causal role of HIV has yet to be established for any of these cancers. We now report that transgenic mice carrying the HIV tat gene develop a high incidence of hepatocellular carcinoma after a long latency and that these changes in the liver are likely to be initiated by extrahepatic growth signals from the tat expressing cells in these mice. We predict that as acquired immunodeficiency syndrome patients begin to respond to therapy and show prolonged survival, such "secondary" malignancies induced by HIV will become increasingly prevalent.

[Congenital and acquired anomalies of the inferior vena cava. Differential diagnosis using digital subtraction angiography and computed tomography].

Segmental aplasias of the inferior vena cava, thrombosis of the vena cava, tumour infiltration (e.g. renal neoplasms via the renal vein) and external compression (e.g. large hepatic metastases) may produce similar radiomorphologic changes in chest x-ray DSA and CT. These radiomorphological changes are described by case reports. DSA clearly demonstrates the course of the vessels and the collaterals involved. CT may elucidate the paracaval vascular changes. It is only a synopsis of cavography (DSA) and CT with reference to embryogenesis that will lead to a reliable diagnosis and enable a definite decision on therapy and prognosis.

[Port visualization before intraperitoneal chemotherapy: scintigraphy or angiography?].

For assessing the functioning of intraperitoneal port-catheter systems prior to intraperitoneal chemotherapy, scintigraphy/SPECT and subtraction-angiography were compared. The patient under scrutiny had three port-catheter systems. Two of the three ports were functioning well. However, one port did not function. Via scintigraphy with 99m-Tc-nanocolloid this defective port was detected, but the cause was identified only by angiography. By the angiographic technique, a leakage near the port chamber caused by dislocation of the catheter could be verified. SPECT is a more useful method than angiography, since it shows very clearly intraperitoneal distribution by the possibility of reconstructing various slices. In conclusion, both techniques, the scintigraphic and the angiographic one, complement each other well.

[Problems in recognizing recurrences of craniocervical malignancies in CT].

A retrospective study was carried out involving 39 patients with malignant tumours of the head and neck in order to differentiate by CT changes due to local tumour or lymph node recurrences and various post-operative or post-irradiation changes. In these patients, two or more CT examinations of the neck were available, derived from a fourth generation scanner. The suspected lesions were classified according to their type and the appearances correlated with the treatment and clinical findings. There was frequent overlap between the appearances of benign and malignant changes, so that in individual cases a definite decision often proved impossible.

[Imaging of cerebral flow dynamics using MR angiography and selective pre-saturation: initial experiences].

MR angiography with selective pre-saturation was applied in 5 normal volunteers, 22 patients with cerebrovascular disease, 3 patients with arteriovenous malformations (AVM) and one patient with thrombosis of the superior sagittal sinus. The results were compared with Doppler ultrasound and conventional angiography. MR angiography reliably demonstrated the direction of blood flow and the presence or absence of collateral flow in the circle of Willis. Collateral flow via leptomeningeal vessels and the ophthalmic artery could not be demonstrated. In the patient with superior sagittal sinus thrombosis, the absence of flow was well seen. MR angiography was able to demonstrate the major supplying arteries in the patients with AVMs. MR angiography is a valid noninvasive means to demonstrate direction of flow and blood supply of the major intracranial arteries.

[Chievitz's organ (the juxtaoral organ): morphological and immunohistochemical studies].

The Chievitz's organ (juxtaoral organ) consisting of a long continuous mass of epithelial cells is located with its medial border by the lateral face of the buccinator muscle. It was successfully identified in 10 out of 40 corpses and histologically examined. Through immunohistological methods using markers like S-100, TPA, PAN, CK-13, NSE and vimentin the epithelial character in the centre of the parenchyma could be demonstrated as well as the marked innervation and vascular supply of the direct surroundings.

[Histological study of the remains of periodontal membrane at the alveolar wall after tooth extraction].

Histological study of 40 alveolar wounds of altogether 5 dogs have given evidence to the fact that remains of the periodontal membrane could remain in the apical area of the alveoli in their whole width. In the middle part of the alveoli, remains of it are found in a thin layer. In the marginal area, bone can be found lying open with torn remnants, which are lying free in the coagulum. In these adhering rests of the periodontal membrane plenty of dynamic cells as well as dilated vessels can be observed. All these facts lead to the idea of considering the part that the remains of the periodontal membrane may possibly play in the beginning process of alveolar wound healing.

The protective effect of cyclosporine against cirrhotic alteration of the liver.

We have successfully applied the immunosuppressive drug cyclosporine in patients with primary biliary cirrhosis and in some patients with chronic active hepatitis. After several years of treatment, we have found a histologic remission tendency in cirrhotic alteration of some patients. This observation indicates that cyclosporine could have protective effects against fibrosis or cirrhotic alteration. To clarify this, we treated Sprague-Dawley rats in which the cirrhotic alteration of the liver was induced by injection of 0.5 ml/kg body weight carbon tetrachloride intramuscularly twice a week with cyclosporine (orally); the control animals were given saline solution instead of cyclosporine. After 6 weeks, we examined the liver histologically to determine the grade of fibrosis and cirrhosis and the grade of fatty degeneration; in group 2 we gave 1 mg/kg body weight cyclosporine daily, and in group 3 it was given every second day. We found excellent protective effects of cyclosporine against cirrhotic alteration in both groups compared with the control group. In group 3 only 25% of the animals showed grade 3 fibrosis and cirrhosis; however, the rate in the control animals (group 4) was 64.3%. In the daily application of cyclosporine (group 2) we found reduced effects of the drug compared with group 3. In group 1 we ordered 10 mg/kg body weight cyclosporine, which causes severe hepatotoxicity. In group 5, animals with hepatic damage from carbon tetrachloride were treated from the third week with cyclosporine. The effect of cyclosporine was not as beneficial compared with the groups in which we ordered cyclosporine from the first week. These results suggest excellent anticirrhotic effects of cyclosporine. This drug should be ordered as early as possible in the treatment of chronic hepatic damage and in an adequate minimal dosage.

Soldier performance and mood states following a strenuous road march.

Eighty-nine soldiers performed a maximal effort 20-km road march, carrying a total load of 46 kg. Compared to pre-march values, post-march marksmanship accuracy decreased 26% for number of target hits and 33% for distance from the centroid of the target. Maximal grenade throw distance decreased 9%, but there was no change in maximal vertical jump height. The Profile of Mood States questionnaire revealed an 82% increase in self-reported fatigue and 38% decrease in vigor. Significant decrements in some aspects of military performance occur when soldiers march rapidly with heavy loads over long distances.

[Inspiratory phase angle behavior in oscilloresistometry].

Oscillatory measurement of resistance can get more practical relevance including the phase angle (PSI) and the volume dependence. Different findings during deep inspiration in patients with pulmonary emphysema (PSI unchanged) and with pulmonary fibrosis (PSI-Negativation) are known. To explain these findings transpulmonary pressure (Ptp) was measured parallel to PSI in 85 subjects. PSI-negativation was found in 31 of 43 patients with pulmonary fibrosis and in 10 of 19 healthy subjects. Patients with pulmonary emphysema have nearly constant or slight positive phase angles. Subjects with negativation of phase angle have significantly higher Ptp-levels at 100% TLC. This high pressure is reached by an in comparison to volume overproportional increase of pressure in the end part of the lung expansion curve. It is proposed to estimate the PSI-V-dependence in ranges from middle level of breathing to 75% of vital capacity VC and from 75% to 100% VC separately. By this modification the qualitative noninvasive assessment of lung compliance could be more efficient. Several findings indicate, that the observed secondary periodic of PSI are dependent from heart rhythm and therefore from intrathoracic blood volume.

[Modification of inspiratory phase angle behavior in oscilloresistometry].

The diagnosis of pulmonary emphysema and fibrosis is possible by measuring the inspiratory behaviour of phase angle PSI of oscillation method. In addition to the different lung compliance as the main reason an influence of the shifting intrathoracical blood volume as the result of the pressure change is discussed especially for the appearing secondary PSI-periodic. Because a direct experimental evidence is not present in this study it was attempted to describe qualitatively the blood volume shift. Thoracic electrical impedance plethysmography and methods of nuclear medicine were used as methodological base. Although impedance plethysmography appears theoretical suitable and there are references, that alterations being independent from the heart rhythm may be detected, the originally aim of the study was not reached by the used method. The registration of radioactive labeled blood over the thoracical region by means of methods of nuclear medicine was applied as an alternative method. In contrast to the theoretical concept of a suction effect of the decreasing pressure during inspiration a decrease of the impulse rate and an increase of the latter during expiration was found. The reason could be the isolated registration of changes in the alveolar vessels. Possible secondary oscillations, which could be in relation with findings of lung mechanics, were not observed.

Effect of thiazide on rates of bone mineral loss: a longitudinal study.

OBJECTIVE: To determine the effect of thiazide diuretic drugs on rates of bone mineral loss. DESIGN: Longitudinal, observational study with a mean follow up of five years. SETTING: Hawaii Osteoporosis Center, Honolulu. SUBJECTS: 1017 Japanese-American men born between 1900 and 1920, of whom 378 were treated for hypertension (study group) and 639 did not have hypertension (control group). INTERVENTION: Thiazide diuretics were taken by 325 men for a mean of 11.9 years; 53 men took antihypertensive drugs other than thiazides. MAIN OUTCOME MEASURE: Rate of bone loss estimated from serial photon absorptiometric scanning at three skeletal sites (calcaneus, distal radius, and proximal radius). RESULTS: Rates of bone loss at all three sites were significantly reduced among thiazide users when compared with controls. The reductions in loss rate ranged from 28.8% (p = 0.02) (distal radius) to 49.2% (p = 0.0005) (calcaneus) relative to the controls. At all three sites the men taking other antihypertensive drugs had faster loss rates (22.6-43.1%) than those of the controls but the difference was significant only for the distal radius. CONCLUSION: Thiazide diuretics slow the rate of bone loss in elderly men.
