[Disagreement between findings of 99mTc-MIBI and 99mTc-pertechnetate scintigraphy scans in patients with primary hyperparathyroidism].

Sixty-two-year-old man was admitted to hospital for increased serum level of calcium (3.85 mmol/l) and incipient renal failure (urea 7.2 mmol/l, kreatinin 117 mumol/l). The reason of hypercalcaemia was intensive explored. Serum level of intact PTH was 383.6 pg/ml. We performed two-dimensional early and delayed (2 hours) scintigram after intravenous administration of 750 MBq 99mTc-sestamibi, i.e. double-phase technique. There was detected focus (diameter 20 mm) of increased sestamibi uptake with slow wash-out in distal part right thyroid lobe. Planar scintigraphy obtained after intravenous administration of 185 MBq 99mTc-pertechnetate detected focus of increased 99mTc-pertechnetate uptake in the same point too. Patient underwent right thyroid lobectomy. There was colloid nodular goiter. Under right thyroid lobe was detected spherical particle. This particle was ablated (diameter 20 mm, weight 4 gram). It was parathyroid adenoma. One month after ablation of parathyroid adenoma serum level of PTH was 23.1 pg/ml and serum level of calcium was 2.52 mmol/l. There was overlap of scintigraphy image of parathyroid adenoma under distal part of right thyroid lobe on 99mTc-sestamibi two-dimensional scintigram and scintigraphy image of hyperplastic node with increased activity in distal part of right thyroid lobe on 99mTc-pertechnetate two-dimensional scintigram.

[Myelotoxicity after systemic radionuclide therapy of painful bone metastases with 153Samarium-EDTMP ].

AIM: The breast cancer and the prostate cancer are considered as the most frequently occurred metastatic tumors in bones. The bone pain appears in majority of patients with metastases. One possibility how to reduce or to remove this pain is the administration of the radiotherapy to the place of metastases. The present medicine disposes of two ways how to administer radiotherapy in the place of metastases: externally irradiation (loco-regional or half-body) or intravenous administration of bone-seeking therapeutic radiopharmaceuticals. MATERIAL AND METHOD: We introduced intravenous administration of 153Samarium-EDTMP in 57 patients (30 prostate cancer, 23 breast cancer, 4 renal cell carcinoma). Mean applied activity was 40 MBq per kg of patient's body weight. RESULTS: After 153Sm-EDTMP administration we observed pain relief of various degree in 75% of patients for three months. 1 and 3 months after 153Samarium-EDTMP administration the following haematologic parameters changes were seen: a) 7% reduction in red cell count in one month after administration, 15% reduction after three months. b) 43% reduction in leukocyte count in one month after administration, 24% reduction after three months. c) 50% reduction in platelet count in one month after administration, 23% reduction after three months. In all three haematological profiles (red cells, leukocytes, platelets) we detected reduction of values after treatment with 153Samarium-EDTMP. This reduction culminates one month after administration and in three months leukocytes and platelets count was getting better. CONCLUSION: Myelosuppression was mild and temporary. No patients had grade 4 hematological toxicity and only three patients (5%) had grade 3 hematological toxicity (National Cancer Institute Common Toxicity Criteria).

Radionuclide oesophageal transit scintigraphy--a useful method for verification of oesophageal dysmotility by cervical vertebropathy.

We followed up a group of 43 patients suffering from cervical vertebropathy. Dynamic scintigraphy revealed a significant prolongation of oesophageal transit time of 10 ml of labelled liquid (mean transit time, MTT) in comparison with a control group. Patients treated with acupuncture showed a significant reduction in MTT in comparison with patients receiving a placebo needle application. The group of patients treated with manipulative techniques showed in addition to symptom alleviation a significant reduction in MTT. Dynamic scintigraphy facilitates verification of oesophageal dysmotility in patients suffering from vertebropathy and enables the success of therapy to be readily monitored.

Inulin as the soluble fiber in liquid enteral nutrition.

Dietary fibers are a necessary part of human nutrition. Inulin is a fermentable fiber that improves bacterial ecology in the large bowel and is also a relevant substrate for short-chain fatty acid (SCFA) production. The aim of the present study was to evaluate the clinical effect of inulin, added as dietary fiber into the fiber-free enteral nutrition (Nutrition Standard). Inulin (30-35 g/d) was administered for 1 wk to a group of patients requiring the nutritional support of a liquid enteral diet. A significant increase in flatulence, an indication of rapid bacterial fermentation, was apparent during its infusion in comparison with a fiber-free enteral diet. Moreover, an increment in ability to dehydrate the cecal stream (stool consistency) was apparent in nearly half of patients. No changes in SCFA concentration in stool and indican output in urine were apparent after 1 wk of inulin administration. Intestinal permeability (51Cr-EDTA absorption test) was not influenced by either enteral nutrition or inulin and enteral nutrition administration. Because of good tolerance inulin seems to be a potential source of dietary fiber in clinical enteral nutrition.

[Splenosis].

Four patients where during childhood splenectomy was performed on account of a traumatic indication were after 9-12 years subjected to sonographic and scintigraphic examination focused on possible evidence of splenosis. In one case asymptomatic splenosis was detected by scintigraphy.

[A 5-year study of cardiac involvement in polymyositis/dermatomyositis].

Improvement of the diagnosis and new therapeutic opportunities revealed a number of organ affections also in patients with polymyositis and dermatomyositis. One of the prognostically important organ manifestations is affection of the heart. The prospective investigation comprises 15 patients with a confirmed diagnosis of polymyositis (10) or dermatomyositis (5). The investigation, using a number of non-invasive method (ECG, echocardiography, systolic time intervals, 99mTc pyrophosphate scan of the heart muscle), proved in the course of the 60-month investigation an increase of pathological findings from 40% at the onset (6/15) to 6 from 11 surviving patients (three died), i.e. a total of 64.3% of the group. Investigation of the fate of patients with signs of myocardial damage during the active stage of the disease (positive myocardial scan) indicates that in these patients also other signs of cardiac damage develop more frequently and that the general prognosis is more unfavourable. The authors consider therefore cardiac damage in the active stage of the disease a reason for intensive treatment.

[Scintigraphic imaging of the heart in dogs damaged by irradiation].

The paper verified literature data assuming that the heart damage caused by irradiation of thorax may be scintigraphically detected by means of 99mTc-pyrophosphate. Three dogs underwent a single irradiation of thorax with a gamma cobalt source by the Dose 40 Gy. The myocardium scan was performed 24 hours, 20 days, 50 days, 80 days and 93 days after the irradiation. The scintigraphic examination proved to be negative at all the intervals observed. A comparison of the scan with morphological findings and with radioactivity of tissue samples after the dogs were sacrificed 93 days after the irradiation proved that the negativity of the pyrophosphate scintigraphy was caused by a minimum damage of the myocardium and not by an inability of 99mTc-pyrophosphate to be incorporated into radiation lesions. The data in literature on myocardium damage by irradiation appear to be considerably exaggerated.

[The pathophysiologic basis for the incorporation of 99mTc-pyrophosphate into myocardial infarct].

The investigation studied the relation of 99mTc-pyrophosphate incorporation (99mTc-PYP) into experimental, 48-hours old myocardial infarction in dogs to tissue vascular supply and to the extent of necrotic tissue. The experimental myocardial infarction was induced in five animals during an operation by the ligation RIVA. The myocardial blood supply was measured in tissue samples, taken from transverse sections of the infarction by means of 86Rb captation. The extent of myocytolysis was measured by depletion of tissue creatine kinase (CK). In the subendocardial layer of the infarction it became obvious that 99mTc-PYP incorporation was proportional to the decrease of tissue blood supply. No relation between the extent of necrosis and the incorporation of the radioactive chemical was demonstrated. The accumulation of 99mTc-PYP requires the necrosis to be present, but its extent apparently does not influence the intensity of incorporation. In the subepicardial infarction layer there was neither a relation of the radioactive chemical cumulation to the blood flow, nor to the extent of the necrosis proved.

Scintigraphic detection of experimental myocardial infarction with 99mTc-2,3-dimercaptosuccinic acid.

Acute myocardial infarctions were produced by ligature of the left frontal descending coronary artery in 9 dogs. The possibility of scintigraphic imaging with 99mTc-DMSA 4 hrs after intravenous administration was studied. The infarctions were 4, 24 and 48 hrs old. The in vivo scan was positive in only one dog with a 4-hr old infarction. The in vivo scans were confirmed by the analysis of the radioactivity in tissue samples. The accumulation of the radiopharmaceutical increased slightly in 48-hr old lesions; however, this increase was not sufficient for a positive scintigraphic finding. Thus, we do not recommend 99mTc-DMSA for clinical use in acute lesions.
