Comparison of sonography and computed tomography in the study of orbital space-occupying lesions.

Seventy cases of space-occupying lesions in the eyeball and 140 extraocular masses in the orbit were explored by both sonography and compute tomography (CT). The merits of each method were demonstrated relative to defining the location, extension, and nature of the lesion. In adults with ocular masses, sonography is usually sufficient. For retinoblastoma, CT is usually necessary to rule out an extraocular extension. When an orbital process is involved, greater diagnostic accuracy can be achieved if the two methods are combined.

Computed tomography in 75 clinical cases of syringomyelia.

Seventy-five patients with a clinical diagnosis of syringomyelia were examined by computed tomography after intrathecal injection of metrizamide. A central cavity was demonstrated in 67 patients. Tilting the patient head down did not increase the rate of cavity opacification. This evidence favors transneural migration of metrizamide into the cavity. The spinal cord was measurably enlarged in only a minority of the patients. In some, the cavity appeared to have clefts or wall defects. These results are discussed according to the etiopathogenic theories advanced by Gardner, Aboulker, and Williams.

[Mondini's syndrome. Recurrent meningitis. Surgical cure (author's transl)].

The authors report 3 cases of meningitis in children. In every case of meningitis, and furthermore in recurrent in cases of meningitis, it is essential to examine not only the eardrum but also hearing and, when there is a deafness, tomographic shots should be taken of the labyrinth to look for a malformation of the internal auditory canal or the cochlea so as to detect the portal of entry for the infection and to be able to close it surgically.

[MRI of the internal auditory meatus. Is injection of gadolinium indispensable?].

OBJECT: To compare prospectively high resolution fast spin echo T2 weighted (FSE-T2W) images and gadolinium-enhanced spin echo T1 weighted images for diagnosis of pathology of the internal auditory meatus (IAM). PATIENTS AND METHODS: 114 patients underwent MR imaging at 1.5 T with the following protocol: axial images; whole brain FSE-T2W sequence (256 matrix); 2 sequences focused on IAM with high resolution FSE-T2W acquisitions (512 matrix, 3 mm sections with 1.5 mm overlapping); pre and postcontrast T1 weighted sequences focused on IAM. Examinations were viewed by senior neuroradiologists. RESULTS: 31 lesions were detected in the IAM by high resolution FSE-T2W images; 25 lesions were found in the IAM by postcontrast T1 weighted images, i.e. 6 false-positive results and none false-negative. On the basis of 228 studied IAMs the sensitivity, specificity and negative predictive value of this high resolution FSE-T2W sequence were 100%, 97% and 100% respectively. CONCLUSION: Gadolinium enhanced sequence is not indispensable if high resolution FSE-T2W images are normal. Gadolinium-enhanced sequence remains essential in others equivocal cases (difficult diagnosis or abnormal IAM).

[Techniques for opacification of the lumbar spinal venous plexuses (author's transl)].

No single technique exists for lumbar phlebography. Whilst catheterisation of the ilio-lumbar veins is always the first step, it should be routinely followed by injection of the lateral sacral veins whenever the leel L5-S1 is clinically suspicious. Finally, when only poor opacification of the longitudinal epidural plexuses is obtained by filateral injection of the ilio-lumbar or ilio-lumbar and sacral systems, catheterisation of the vein in L2 on the right or L3 on the left makes it possible in virtually all cases to obtain satisfactory filling of the lumbar plexuses. It may be considered at the present time that, from a technical standpoint, failures of lumbar phlebography by bilateral femoral catheterisation do not exceed 1% of cases.
