[Meningoencephalomyelitis caused by Borrelia burgdorferi: a case without epidemiologic history or chronic migratory erythema].

A patient is reported with meningoencephalomyelitis with polyradiculitis caused by Borrelia burgdorferi infection. Neurological features developed without previously known tick bite nor the characteristic skin lesion, chronic migratory erythema (CME). The vector of the disease (the tick Ixodes ricinus) exists in Spain, but only one case of meningopolyradiculitis with CME has been reported in Asturias. Our case stresses that B. burgdorferi infection should be suspected in cases of meningoencephalomyelitis or meningopolyradiculitis even without previous skin or joint lesion.

[Treatment of atypical pneumonia with josamycin].

In the present study the clinical efficacy and tolerance of josamycin (1 g every 12 hours) was prospectively evaluated for the empirical therapy of patients below 70 years with community acquired pneumonia of atypical presentation (AP), without respiratory failure, radiological cavitation or risk factors of pharyngeal colonization by gram-negative bacilli. During a 28-month period 168 patients adequate for the study were included. The etiological diagnosis was established in 56 cases (33.3%); in 55 by seroconversion (28 Mycoplasma pneumoniae, 22 Legionella pneumophila, 1 Chlamydia psittaci and 4 Coxiella burnetii) and in only one case by positive blood culture (Streptococcus pneumoniae). The mean duration of therapy was 9 days and that of fever 1.4 days. Five patients (3%) had mild transient gastrointestinal complaints. No patient required a change of therapy. Relapses were not observed in the 45 days of follow up. In conclusion, josamycin (1 g/12 h p.o.) is an effective and well tolerated antibiotic for the therapy of pneumonia by M. pneumoniae or L. pneumophila, and it represents a good empirical treatment of AP in patients below 70 years without risk factors of GNB infection, respiratory failure or radiological cavitation.

Motor evoked responses after lumbar spinal stimulation in patients with L5 or S1 radicular involvement.

Motor evoked responses were recorded bilaterally in the tibialis anterior and soleus muscles after lumbar spinal stimulation in 45 patients suffering from L5 or S1 radiculopathies due to disc protrusion, and in 25 healthy controls. A significant prolongation (P less than 0.01) of motor evoked response latency was observed in 72% of the patients with L5 radiculopathy and in 66% of the patients with S1 radiculopathy. These anomalies were observed in patients with objective neurological signs as well as in patients free of objective signs. These findings indicate that subclinical compressive radiculopathy can be detected by this method which represents a useful adjunct in the assessment of radicular pain of discopathic origin.

[Prevalence of antitetanus antibodies in pregnant women from Catalonia].

A seroepidemiological survey was carried out to evaluate the immune status against tetanus in a representative sample of pregnant women in Catalonia. Only 21.2% of females from the evaluated sample had immunity against tetanus. The observed rate of protection in women below 25 years of age (40.6%) was significantly higher than in those aged 25-45 years (17.8%). The results of the study show that the risk of neonatal tetanus is still present in Catalonia, and that it is necessary to enhance tetanus vaccination, particularly among the adult population.

[Serologic markers of viral hepatitis B in drug addicted++ patients at the Hermilio Valdizán Hospital].

A study was done in 36 drug addict patients at the H. Valdizan Hospital We studied serological markers for Hepatitis B: HBsAg Anti-HBc, and anti-HIV was also made. They were not IV abusers. Five patients (13.8%) were positive for HBsAg and 6 (16.6%) for anti-HBc. Anti-HIV was negative in all the cases, that obtained from our results compared with the general adult population from peruvian coast are high. In relation to the information obtained, the transmission route would be diverse, considering an important one the sexual route. 44 drug addicts were seen in 1988 at this Hospital, 36 cases could be a representative number, although we think we need to extend this study to a bigger population to establish the actual prevalence of this infection among these particular patients. This information could help to advice preventive and control health measures.

[Prevalence of antibodies against Toxoplasma gondii, rubella virus, cytomegalovirus and herpes simplex virus in pregnant women of Catalonia].

A seroepidemiological survey was carried out to evaluate the prevalence of antibodies against toxoplasma, rubella virus, cytomegalovirus (CMV) and herpes simplex virus in pregnant women from Catalonia during 1985. The study was carried out in a representative sample of the pregnant women cared for in public and private hospitals from Catalonia, which was selected in maternal clinics by random sampling. Antibody measurements were carried out with the following techniques: toxoplasma, IFI (positive values greater than or equal to 1:10); rubella virus, Iha (greater than or equal to 1:10); CMV and herpes simplex virus, CF (greater than or equal to 1:10). 50.21% of pregnant women had antibodies against toxoplasma, 97.48% against rubella virus, 73.31% against CMV, and 80.93% against herpes simplex virus. The presence of antibodies against CMV and herpes simplex virus were significantly associated ( less than 0.00001). The demographic and socioeconomic variables associated with the prevalence of antibodies were analyzed, and they were compared with other results from the literature.

[Aztreonam versus tobramycin in acute pyelonephritis. A comparative study].

We performed a comparative study between the monobactam antibiotic aztreonam and the aminoglycoside tobramycin in patients diagnosed as having acute pyelonephritis. The respective doses were 1 gr. IM daily for 7 days, and 100 mgr. IM q 12 h for the same period of time. Clinically, 100% of uncomplicated acute pyelonephritis and 87.5% of complicated infections cured with aztreonam. Tobramycin achieved an 80% cure rate for both types of infections. Microbiologically aztreonam was effective in all uncomplicated acute pyelonephritis, and in 69.56% of the complicated cases (overall microbiological cure rate = 78.7%). The therapeutic failures were ascribable to infections from S. faecalis, an organism naturally resistant to aztreonam. Thus, the microbiological cure rate was 84.2% in complicated pyelonephritis from organisms sensitive to this antimicrobial. The microbiological cure rate for tobramycin was 70% in acute uncomplicated, and 80% in complicated infections. We observed a good clinical and biological tolerance to both antimicrobials. No side effects were observed. Serum and blood biochemical analyses, and coagulation tests revealed no changes.

[Modification of the echographic pattern of carcinoma of the prostate treated with Lh-rh agonists].

We present the results of a study undertaken to assess the echo patterns and prostate size in 13 patients with prostatic carcinoma treated with Lh-rh agonists. The study revealed a gradual reduction of the anteroposterior and transverse diameters. After 12 months, these were 28% and 22% of their respective initial values. Furthermore, the prostatic echo patterns changed significantly in 66% of the patients, and infiltration of the seminal vesicle improved or became stable in 50%. These findings suggest that transrectal ultrasonography may be useful in the follow up of prostatic carcinoma patients receiving hormone therapy.

[Intradural spinal metastasis from visceral neoplasms. Apropos of 4 cases. Review of the literature].

Four patients were referred during the last year for treatment of an intradural metastasis. Primary sites were a bronchogenic carcinoma (one case), a prostatic carcinoma (one case) and in the two other cases the primary tumor was not found. In all cases Noot pain had lasted 1 to 5 months, despite CT examination during this painful period. Except in case n.4, diagnosis was not established until the onset of myelopathy. Surgical removal was complete in one case and subtotal in the three other cases, with complete postoperative relief of the pain in all cases and significative improvement of neurological status. The authors emphasize the importance of root pain as an early sign in these patients as well as the difficulty in establishing diagnosis. They discuss the different pathogenic mechanisms and the methods of treatment.

[Acrylic denture base materials in dentistry. The influence of immersion of bases on the palatal gap].

The method of experimental design, we used, is versatile enough to allow the study of immersion in water at 37 degrees C during 15 days after curing, as a new parameter able to influence the deformation of denture bases, without any additional experiment. According to the experimental conditions used to make up denture bases, this parameter can, in some cases, modify appreciably the deviation in the palate region.

[Acrylic materials for denture bases in dentistry. Study of measurement variations and the influence of three new parameters on construction of bases and their deformation].

A further study of the variance of deviation measurements of denture bases in the palate region shows that the variability of measurements is not essentially due to measuring, but also to the variability of denture processing. No more than the other previously studied parameters, the heating and cooling rates as well as the mode of insertion of resin in the cast have no effect on the dimensional accuracy of denture bases after curing.

Purification and comparative properties of the cytosolic isocitrate dehydrogenases (NADP) from pea (Pisum sativum) roots and green leaves.

The cytosolic isocitrate dehydrogenases (NADP-IDH) were purified to homogeneity from pea roots and green leaves with a high yield by ammonium sulfate precipitation, DEAE-cellulose chromatography, Sephacryl S-200 gel filtration, Matrex red-A affinity chromatography and phenyl-Superose HR 5/5 HPLC. Both isoenzymes were dimeric proteins, consisting of two apparently identical 41-kDa subunits, having similar secondary structures with an alpha-helical content of 20% and a beta-pleated sheet content of 43%. Similarly immunoassays suggested that the two isoenzymes were closely related in terms of antigenic determinants. However, the two proteins were distinguishable by their electrophoretic mobilities and amino acid compositions. The profiles of the two isoenzymes as a function of pH were similar and exhibited a broad pH optimum from 8.5 to 9.0 with Mg2+ as cofactor and 8.0 to 8.5 when Mn2+ was used. Compared to the root isoenzyme, the leaf NADP-IDH appeared to be more heat-labile. However, these isoenzymes exhibited similar behavior for thermal denaturation in the presence of bovine serum albumin and were stabilized upon addition of substrate, metal and coenzyme. Their values of activation energy were estimated as 47 kJ/mol. When using Mn2+ as cofactor, the two isoenzymes displayed identical Km(Mn2+), Km(DL-isocitrate) and Km(NADP) values, which were calculated to be 2.1 microM, 5.7 microM and 2.7 microM respectively. With Mg2+ as cofactor, their Km(Mg2+) K(DL-isocitrate)m and Km(NADP) values were also not statistically different, being 34.0 microM, 15.2 microM and 2.6 microM for the root NADP-IDH, and 29.0 microM, 20.3 microM and 3.1 microM for the leaf isoenzyme. From the above data it can be concluded that although the cytosolic NADP-IDH in pea roots and leaves are organ-specific isozymes, their similar physicochemical and kinetic properties suggest that the two isozymes might be involved in identical metabolic functions.

Identification of a cDNA clone for sorghum leaf malate dehydrogenase (NADP). Light-dependent mRNA accumulation.

The mechanisms underlying the photoregulation of the synthesis of sorghum leaf malate dehydrogenase (NADP) (EC 1.1.1.82) (NADP-MDH), a key enzyme in C4 photosynthesis, have been investigated. During the greening process a light-dependent increase in enzyme activity took place, accompanied by de novo synthesis of the protein. In vitro translation experiments showed that this chloroplastic protein is synthesized as a precursor (46 kDa) with a 'transit peptide' of about 2.5 kDa. A large increase in NADP-MDH-translatable RNAs was also observed during greening. We describe also the construction and characterization of a cDNA clone for NADP-MDH (pCM18A) in the expression vector lambda gt11. The use of this homologous probe demonstrated a light-dependent mRNA accumulation.
