Chondrosarcoma of the temporal bone and otosclerosis.

Chondrosarcoma constitutes 6% of all primary bone tumors and 11% of malignant primary bone tumors. Nevertheless, in a review of the tumor registry of the University of Michigan covering a period of 50 years, there were only 3 cases involving the temporal bone. A case of a woman with a chondrosarcoma of the temporal bone that was partially resected by means of an infratemporal approach at the skull base is presented. This patient had previously undergone surgical treatment for otosclerosis of the other ear. Several considerations regarding survival factors in this type of tumor are dealt with in terms of the histological features, therapeutic options and anatomic location. The possibility that this lesion may originate from the persistence of the cartilaginous inclusions that some authors consider to be involved in the origin of otosclerosis is discussed.

Inner ear malformations: Mondini's dysplasia.

Advances in imaging techniques are enabling the detection of increasing numbers of inner ear malformations. Mondini's dysplasia, whether alone or in association with other malformations, is one of those most frequently encountered. We report 4 cases of Mondini's dysplasia treated by us, discussing recent embryological and genetic findings.

Metastatic renal cell carcinoma to the head and neck area.

AIMS AND BACKGROUND: Metastases of renal cell carcinoma to the head and neck are rare. We report on three cases with tumor spread to this area (nasal cavity, tongue and larynx) and present a review of the literature. PATIENTS: The first patient presented with lung and nasal cavity metastases five years after renal tumor resection. In patient 2 the diagnosis of primary renal carcinoma and lung and tongue metastases was concomitant. In case 3 a primary kidney tumor was not suspected until radical resection of a tongue lesion was performed. RESULTS: The first two patients received radiation therapy. They had been previously treated with interleukin + interferon and vinblastine + interleukin 2 and achieved a survival of 14 and 16 months, respectively. The third patient has not been given any treatment to date (apart from surgery) and remains asymptomatic four years after diagnosis. CONCLUSIONS: In patients with cell carcinoma the occurrence of lesions in the head and neck area may suggest metastases. In some cases they may precede the diagnosis of a renal tumor and mimic a primary head and neck tumor; otolaryngologists should be aware of this possibility. An individualized treatment approach is recommended. In the case of solitary metastases a surgical excision should be performed as palliation, if not cure.

Lennert's lymphoma with giant multivesicular lysosomal bodies optically visible.

A case of a 43-year-old male patient with lymphoepithelioid lymphoma is discussed. During the 10-year follow-up of this patient, different biopsy specimens were obtained. Immunohistochemical studies with a complete paraffin panel determined a T-cell nature of the specimens. Electron microscopy showed a peculiar finding in the cytoplasms of the tumoral cells that correlated with optical inclusions observed in optical imprints; these imprints corresponded to lysosomic multivesicular bodies, whose significance is discussed.

[Tympanosclerosis. I: proposed anatomical-surgical classification].

Tyrnpanosclerosis continues to be a clinically and pathogenically controversial topic. The options for dealing with it range from a conservative approach using hearing aids to surgery. With a view to justifying our therapeutic criteria, 29 cases of tympanosclerosis, out of a total of 200 surgical operations for otorrhea, were reviewed. An anatormical-surgical classification was established for use in approaching specific therapy for each particular case.

[Retrospective study of complications of surgery for laryngeal cancer].

Surgery, alone or in combination with other therapeutic measures, is one of the main approaches to curing laryngeal cancer. The risk of complications is implicit in any surgical procedure. We describe our experience with general and local complications in surgery for laryngeal cancer and examine their relation to tumor extension and surgical technique. A review was made of a series of 431 patients who underwent surgery for laryngeal cancer over a 10-year period (1982-1991). Twenty-two patients (5.1%) had systemic complications, including upper gastrointestinal hemorrhage (n = 5), massive cervical hemorrhage (n = 5), and four renal failure. Minor complications were recorded in 77 cases (17.8%), predominantly pharyngocutaneous salivary fistula, which developed in 55 patients (13.8%). The incidence of local complications was significantly greater in patients with extensive local spread (T4). There were no differences among patients with regional spread. The surgical technique and type of pharyngoesophageal reconstruction played no role in the development of complications. Preoperative radiotherapy did not influence on the development of salivary fistulas.

[Evaluation of tumor staging using echography in bladder tumors (2)].

The paper shows the value of echography in tumoral grading based on 87 patients who besides cystoscopy, bimanual touch and histology study by transurethral resection, had an echography performed. Sensitivity achieved for tumour detection through echography was 85% with a positive predictor value of 98.8%. On the 72 patients who were diagnosed a tumour through echography, TUR histology showed an echography under-grading of 2.7% (2 cases) and an over-grading of 9.7% (7 cases) with grading correlation in 86.3% cases.

[Complications of rigid ureteroscopy].

UNLABELLED: Peri- and post-operative complications higher than 37c (15%) encountered in 250 urethroscopies performed between May 1985 and January 1989 are presented in this paper. These experiences are compared to those from other 20 series (2,599 urethroscopies) representing 14.5% complications. In order to study their pathogeny, the sample was divided into four groups: CONTROL GROUP: 27 patients where normality of urinary tract was demonstrated in 92.6% of all evaluations made at intervals of about 20 months. Technical scope: Two groups chronologically different: 16% complications in urethroscopies performed in 1985 and 7% complications in those performed in 1986. Differences in the techniques applied: classic extraction (14%) vs. urethroscopy (13%) and electrohydraulic lithotrity (42%) vs ultrasonic lithotrity (12%). Finally, a description is made of those cause believed to be responsible for major complications: perforation, avulsion and ureteral stenosis.

[Urologic pathology in patients positive for anti-HIV antibodies].

Between May 1987 and November 1988 we performed the human immunodeficiency virus (HIV) antibody serological test on 586 patients of the Urology and Nephrology Services, and it was positive in 14 cases. Of these, 6 came on account of urological pathology: bilateral cryptorchidism, giant condylomata acuminata, acute pyelonephritis and three acute orchi-epididymitis. All the patients were intravenous drug addicts. Although it is a case of common urological pathology and not secondary to the acquired immunodeficiency syndrome, it takes on a different significance as regards the risk population in which it occurs.

[Meningeal carcinomatosis as the primary manifestation of breast cancer in a man].

Meningeal Carcinomatosis (MC) is a severe and usually late complication in the progressive development of some--5%--solid extraneural tumors. Breast and Lung adenocarcinomas are the two primary sites more often associated with this condition today. We present a case of male patient in whom MC was the initial and diagnostic manifestation of breast cancer. Autopsy study revealed a concomitant presence of a lung bronchoalveolar carcinoma. However, the cytopathology of Cerebrospinal Fluid and pathologic findings in leptomeninges showed a breast adenocarcinoma as primary site.
