Endoscopic urethrotomy versus urethrotomy plus Nd-YAG laser in the treatment of urethral stricture.

Between February 1987 and May 1988, 30 patients who presented with single, iatrogenic, annular strictures of the bulbar urethra were included in this prospective study. They were randomly divided into two groups; group 1: 15 patients who underwent direct-vision endoscopic urethrotomy (cold-knife incision at 12 o'clock) and group 2: 15 patients who underwent internal urethrotomy plus Nd-YAG laser. The results obtained were analyzed and compared at 1 and 2 years by clinical evaluation, uroflowmetry and retrograde-voiding urethrography. Group 1 obtained 80% good results at 1 year, falling to 60% at 2 years follow-up. Group 2 presented good results in 73.3% both at 1 and 2 years of follow-up.

Intravesical formalin for the treatment of massive hemorrhagic cystitis: retrospective review of 25 cases.

Between 1980 and 1988 25 patients with massive bladder hemorrhage were treated with intravesical instillation of 4% formalin in 19 cases and 10% in the remaining 6 cases; the contact time was 15 min in 20 cases. The etiology of the hemorrhage was cyclophosphamide therapy (1 case), pelvic radiotherapy (15 cases) and infiltrating bladder cancer in the remaining 9 cases. In 10 cases, the instillation of formalin was performed in bladders with a prior supravesical diversion. Complications included 1 case of vesicorectal fistula, 1 case of uretero-hydronephrosis and 1 case of vesical extravasation of formalin when a concentration of 10% was used at a volume superior to 50 ml. The only complication seen with 4% formalin was 1 case of upper urinary tract dilatation. Good results were obtained in 88% of cases, who achieved correct hemostasis during a mean of 4 months.

[Laser therapy of superficial tumors of the bladder].

Based on the treatment with Nd-YAG laser of 109 superficial bladder tumors (52 initial cases and 57 recurrences), herein we report on its indications, technical aspects, and clinical advantages. In order to evaluate the difference between laser therapy and TUR, three groups were studied to compare both techniques. 1) We evaluated the incidence of tumor recurrence in 27 cases submitted to laser therapy and 26 cases submitted to TUR. At 2 years no significant differences were observed relative to tumor recurrence or progression for both groups. 2) We evaluated bladder parietal involvement for each technique. All but one of 109 cases that had been submitted to laser therapy had preserved initial bladder capacity. Biopsies performed in areas post-laser therapy (31) and post-TUR (24), and with the resector loop in 22 cases, revealed no histologic difference between both groups (replacement fibrosis and elastic fibers). 3) We evaluated the incidence of urethral stenosis after laser therapy (22) and TUR (55). Urethral stricture was significantly lower (4.5%) for those treated with laser than for those treated by TUR (16.3%). On the basis of the overall (109 cases) and the partial (study groups) results, we consider laser treatment of superficial bladder tumors to be limited to single, less than 2-3 cms, or small, multiple initial or recurrent neoformations.

Spiral urethral prosthesis as an alternative to surgery in high risk patients with benign prostatic hyperplasia: prospective study.

A spiral urethral prosthesis was inserted with use of local anesthesia in 22 patients with benign prostatic hyperplasia who had long-term indwelling bladder catheters (mean 13 months) and anesthetic contraindications to an operation (American Society of Anesthesiologists rating of 4 in 86% of the patients). Of the 22 patients 7 (31.8%) did not reach 12 months of followup for different reasons. The remaining 15 patients were followed with clinical evaluations, urine cultures, transrectal ultrasound, voiding urethrography and urine flow rates at 3, 6 and 12 months. Final evaluation at 12 months showed normal voiding in 74% of the patients with an absence of significant post-void residual in all. In 10 patients (67%) urinary flow rates varied between 6 and 14 cc per second and in the remaining it was less than 6 cc per second. In 60% of the patients urine culture was negative. Our results suggest that a spiral urethral prosthesis, placed with the patient under local anesthesia, is an excellent substitute for long-term, indwelling, bladder catheterization, and is a useful alternative to an operation in patients with an increased surgical risk.

The role of repeat transurethral resection in stage A1 carcinoma of the prostate.

Between 1983 and 1987 repeat transurethral resection (TUR) was performed in 20 patients with a diagnosis of stage A1 prostatic carcinoma. The average weight of the resected tissue in the repeat TUR was 8.81 g (range 3-20 g). In 5 of 20 cases (25%) residual prostatic cancer was identified. By adding the number of foci with cancer obtained at the initial operation with the repeat TUR, 16 cases (80%) remained as A1 and 4 cases (20%) were reclassified (upstaged) to A2. The usefulness of this technique in the restaging of patients with incidental carcinoma of the prostate is analyzed.

[Necrosis of the bone marrow and cancer].

Bone marrow necrosis (BMN) is a rare complication and is characterized by the presence of an eosinophilic amorphous material in the bone marrow. The clinical, analytical and histological characteristics of 4 patients with BMN associated to a neoplastic process are described. One of them was a gastric cancer but the neoplastic origin could not de determined in the other three cases. Three patients presented bone pain, but in all four patients thrombopenia, anemia, leuko-erythroblastic reaction and elevated LDH was found. A literature review is carried out and the possible physiopathological mechanisms are discussed.

[Second neoplasms in Hodgkin's disease].

Today, excellent therapeutic results are obtained in Hodgkin's disease however, they may be obscured by the development of long term complications due to the treatment given including second neoplasias. Out of the total of 250 cases of Hodgkin's disease reviewed during 19 years we found 13 secondary neoplasias. All the patients received alkylating drugs and in 10, both chemo and radiotherapy were associated. The average interval of time was 48 months. Bronchopulmonary carcinomas and acute leukemias were the malignancies most often encountered.

[Usefulness of sialic acid as a tumor marker].

In order to analyze the utility of sialic acid as a tumor mass marker, its levels have been determined in 253 neoplastic patients, 57 patients with non-tumoral pathology and 60 healthy controls. Significant differences in the sialic acid values were observed between the control group and the rest (p less than 0.001). When the neoplastic patients were studied according to the extension of their disease, we found that the SA levels in patients under complete remission were significantly lower than in the other stages (p less than 0.01) and that patients in stage IV presented the highest levels (p less than 0.05). Differences between stages I, II and III were not detected. Furthermore, no differences were observed regarding either the type of neoplasia or if the patients had or did not have tumor pathology. We conclude that sialic acid is a tumor marker with poor specificity that grossly relates with the tumor size.

Tumor recurrence in prostatic urethra following simultaneous resection of bladder tumor and prostate.

A comparative retrospective study was performed on 100 patients who had undergone transurethral resection of a superficial bladder tumor (Ta-Tl) with no associated carcinoma in situ (group 1) and 100 patients who had undergone simultaneous transurethral resection of a superficial bladder tumor (Ta-Tl), with no associated carcinoma in situ, and benign prostatic hyperplasia (group II). Evaluation of the recurrences of prostatic urethral tumors in both groups showed that they appeared in 10 patients (12 recurrences) in group I and 10 patients (14 recurrences) in group II (p = not significant). These data enabled us to rule out the influence of simultaneous transurethral resection in cases of recurrences of prostatic urethral tumors.

Value of in vivo mucosa-staining test with methylene blue in the diagnosis of pretumoral and tumoral lesions of the bladder.

An in vivo staining test, methylene blue was applied to 50 patients with bladder tumor. 378 biopsies were performed in 85 areas stained with methylene blue and in 293 random biopsy areas. Of these samples, 56 had carcinoma in situ: 38 of these were not stained (68% false-negative) and the remaining 18 took up the stain. In the latter corresponding to 11 patients, carcinoma in situ was also shown in unstained areas examined by random biopsies. In 37 areas with urothelial dysplasia, 31 were not stained (84% false-negatives) while 6 cases (16%) took up the stain. Bladder tumors were stained in an irregular way but the intensity of the stain was related to the tumor grade. According to our experience the in vivo staining test with methylene blue cannot be used as a marker for pretumoral and tumoral lesions of the bladder.
