Bone densitometry of excised vertebrae; anatomical relationships.

Bone mineral density (BMD) was determined in 32 excised vertebrae using three methods: (1) dual-energy quantitative computed tomography (QCT), (2) dual-photon absorptiometry (DPA) with 153-Gd in an anteriorposterior projection and (3) scanning slit X-ray absorptiometry (SSXA) in both AP and lateral projections. The QCT region-of-interest in the anterior vertebral body had a lower density than that of the total trabecular portion of the body, but was highly correlated to this larger region (r = 0.96; SEE = 8 mg/cm3). The anterior QCT region also correlated moderately with BMD from DPA (r = 0.77; SEE = 18 mg/cm3). Measurements of the vertebral body in lateral projection were less well correlated (r = 0.5-0.7) to QCT densities. Both the anterior QCT region (r = 0.81; SEE = 18 mg/cm3) and the BMD from DPA (r = 0.86; SEE = 16 mg/cm3) and the BMD from DPA (r = 0.86; SEE = 16 mg/cm3) were similarly predictive of density of the integral vertebral body. Differences among densitometric methods on the spine depend on the projection used and the region examined.

[Quartz crystals in the skin as a cause of progressive systemic scleroderma].

There is a close correlation between the systemic sclerosis and the exposure to crystalline silica in the GDR. The silica content was investigated in the skin of the patients. Silica particles were found using phase-contrast and polarizing microscopy. The size of the crystals varied from 1 to 90 micrometer. Below a silica content of 10% in the dust it is unlikely that systemic sclerosis is developing.

F response characteristics in type I diabetes mellitus.

Ulnar and tibial F response parameters were characterized in 17 healthy controls and 26 subjects with type I diabetes mellitus meeting or exceeding criteria for mild diabetic peripheral neuropathy. The presence of mild diabetic peripheral neuropathy was determined by utilizing conventional nerve conduction studies, the neuropathy symptoms score and a neurologic examination. Ulnar and tibial nerve F response latency, amplitude, duration, chronodispersion and persistence were then compared between populations. The relationship between tibial F response persistence and minimal F response latency was assessed in both populations. In addition, the relationship between tibial F response persistence and tibial nerve conduction velocity and tibial nerve compound action potential characteristics (e.g., latency, amplitude and duration) was assessed in the diabetic population. The results indicate that ulnar F response latency and chronodispersion failed to differentiate the subject and control populations; however, significantly decreased ulnar F response amplitude and duration were noted in the diabetic population. In the tibial nerve, the F response persistence was significantly decreased in the diabetic population but persistence did not correlate with compound muscle action potential latency, amplitude, duration or nerve conduction velocity. Finally, the tibial F response latency, amplitude, duration and chronodispersion failed to differentiate the control and diabetic populations.

Advances in noninvasive bone measurement.

Several noninvasive measurement methods are used for evaluation of metabolic disease. Single-photon (125I) scans of the peripheral skeleton are useful in some diseases but are ineffective in osteoporosis (even on the distal radius or os calcis) because they cannot predict spinal or femoral density. Also, peripheral measurements show high percentages of false negatives, that is many patients with fractures have normal peripheral density. Dual-photon (153Gd) scans of the spine, femur, and total skeleton are precise and accurate (2% error) and provide direct measurements of bone strength at fracture sites. This gives the best discrimination of abnormality and the most sensitive monitoring. Quantitative computed computed tomography (QCT) allows measurement of the spine but not the critical proximal femur area. QCT has a large accuracy error because (a) the limited area measured (under 5 cm3) fails to represent the total vertebral body, (b) technical errors, and (c) variable fat and osteoid influence the results.

Expression of synaptophysin in the rat pheochromocytoma cell line PC12.

Expression of the integral membrane protein of small synaptic vesicles, synaptophysin, was investigated in the pheochromocytoma cell line PC12 using a quantiative dot immunoassay. Specific synaptophysin contents of the cultures varied with cell density, high levels being observed in densely seeded dishes and/or after some days of subculturing. Northern blot analysis revealed these cell density-related changes in synaptophysin protein contents to result partly from corresponding alterations in mRNA levels. Treatment with nerve growth factor (NGF), but not with various other effectors of intracellular messenger systems, inhibited both synaptophysin and DNA accumulation in the cultures. These data indicate that synaptophysin expression is high in densely proliferating PC12 cells and uncoupled from process formation and neuronotypic differentiation induced by NGF.

[Basic studies of ultrasound surgery. VIII. Experimental animal studies of power ultrasound surgery of the swine pancreas].

The technique of the power ultrasound was tested in vitro and in vivo in 20 porcine pancreas. Simultaneous temperature measurements and laboratory tests do not produce any restriction. Enzymatic disorders were provable adequate to the clinical course. The ultrasonic effect guarantees a nearly riskless dissection in the layer. Joining processes, by adding Ligament-FIMOMED, produced a waterproof occlusion of parenchymal wounds. The cross-section of the pancreas with the pancreatic duct was sealed sufficiently. Also the jointed pancreato-jejunostomy remained sufficient. The reaction of the pancreatic parenchyma to the power ultrasound was small. A scarred metaplasia evolved immediately at the adhesive as a chronic atrophic interstitial pancreatitis. Only in case of occlusion of the pancreatic duct this alteration comprehended the whole pancreas.

[Occupationally-induced progressive scleroderma and the heart].

A 60-year-old man with progressive systemic sclerosis (PSS) had worked as underground miner in ore mining for 26 years with intensive exposure to crystalline silica. The PSS was acknowledged as an occupational disease. Parallel to the progress of the PSS there developed disturbances of the conduction system and cardiac rhythm with left heart failure. Despite treatment sudden death set in. A myocardial fibrosis was found histopathologically. This is a consequence of the PSS and therefore the occupational disease is the cause of death. Cardiac involvement means a poor prognosis and necessitates both evaluation of the damage in percent caused by the occupational disease and interdisciplinary teamwork in the dispensary care of patients.

Whole-body MR imaging and spectroscopy with a 4-T system.

The clinical potential and limitations of magnetic resonance imaging and spectroscopy at 4 T were investigated with the use of a newly constructed system, which has been in use since January 1987. The magnet has a warm bore that measures 1.25 m in diameter, and its homogeneity in a sphere with a diameter of 50 cm is better than +/- 2.5 ppm. It was hypothesized that the improvement in the signal-to-noise ratio (S/N) afforded by the higher field strength would be useful in reducing imaging time and in improving spatial resolution. In experiments in human volunteers, believed to be the first in which an entire human body was exposed to a magnetic flux density of that magnitude, the subjects were exposed to 4 T for 10-30 minutes. They showed no changes in well-being or heart activity. The expected gain in spectral resolution due to chemical-shift scaling was achieved with the 4-T system, and an improvement in S/N was verified for phosphorus at 34 and 68 MHz. In sodium imaging, the high flux density appears to be useful in reducing imaging time, which should increase the usefulness of sodium imaging in evaluating brain tumors and strokes. In spectroscopy, the increase in flux density improves the quality of the spectra.

[Basic research in ultrasound surgery. VII. Comparative animal experiments after intestinal anastomosis by suturing, ligament gluing with Fimomed and ultrasound sealing with ligament Fimomed].

Animal experimental anastomoses in the small intestine of Mini-Lewe-pigs showed that even sufficient anastomoses free of stenosis can be obtained by using the sticking respectively the ultrasound sealing method. The foreign-body reaction in the surroundings of the suture was approximately equally strong as on the gluten. All animals did not show any specialty in the postoperative course. The use of butylcyanidacrylic is possible in the human intestine as also shown in the literature. The use is indicated if advantages are to be expected in face of a suture anastomosis in clinical sight.

[Ultrasonic sealing of the lung parenchyma after atypical resection].

Surfaces of lung parenchyma after resection or lesion can be sealed reliably by means of monomeric tissue adhesive with immediate polymerization by ultrasound sealing. No bronchopleural fistula was observed during the animal experimental testing. Necrotic areas of the lung parenchyma are avoidable extensively. This technique is simple. It offers reliability for patient and operator in emergency situations.

[Basic research on ultrasound surgery. VI: Comparative histological liver findings following treatment of the wound with ultrasound welding and suturing].

Liver cuts were performed on 21 Mini- Lewe pigs and then closed with suture in one case, and by ultrasonic welding of Fimomed - Gelaspon conglomerate in the other. Macroscopic assessment in situ and the histological examination of cut out liver areas regularly showed perifocal inflammatory peritoneal reactions and abscess formation for the sutured regions. From a histological point of view the liver tissue in the regions subjected to ultrasonic treatment reacted more strongly than after suture, particularly in the form of lobule structure disturbances, focal hepatitis, hepatic duct proliferation and destruction, and cholangitis. All of these were limited to the immediate vicinity of the conglomerate with its fibrous demarcation, which remained identifiable until the end of the experiment after a year and a half. The experiments resulted in the sucessful closure of the liver wounds in all cases where ultrasonic welding of Fimomed - Gelaspon compound was used. Application to human medicine is, however, not yet justified.

[Histochemical-metric combination technique for the diagnostic of the acute heart failure (author's transl)].

With the combined use of the hematoxylin-basic fuchsin-picric acid staining (HBFP-staining) and sarcomere measurement of the length a new staining-metric technique for the myocardiac investigation is represented. It completes both the conventional macroscopical and histological methods and the diagnosis of a general or regional myocardiac dilatation, for instance in early infarct, acute failure of the right heart or in special intoxications can be decisively improved.
