Immediate free flap reconstruction of traumatic defects.

In a series of 180 transfers of free flaps were in 11 cases on upper extremities applied emergency flaps, both for immediate reconstruction of function and for the replacement and covering of tissue defects. In one case a scapular flap was used simultaneously with the replantation of the forearm. In three cases was applied a forearm flap for the reconstruction of the thumb and of the scalped index finger, and in three cases a central venous flap for traumatic defects of tissues on fingers. These flaps yielded surprisingly good functional results after an injury associated with a loss of dorsal aponeurosis of fingers and with an exposure of IP joints. A toe-to-hand transfer was used in one case for the reconstruction of the thumb. A groin flap and a lateral arm flap were applied for the covering of tissue defects.

[What does the radicality of thyroidectomy depend on?].

Reflections on the radicality of thyroidectomy are based on the persisting lack of uniformity as regards terminology, indications and techniques of thyroid surgery. The authors emphasize four basic factors involved in the solution of thyroid diseases: A. respecting the preoperative examination and recommendations of the endocrinologist, B. philosophy of the surgeon and his workplace, C. experience and skill of the surgeon, D. postoperative follow-up and treatment of patients. The necessity of multidisciplinary collaboration emphasized under A is followed under B by a detailed analysis of principles of the workplace as regards indications and technical and tactical aspects. In part C necessary theoretical and personal problems of the thyroid surgeon are summarized. Without careful postoperative check-up examinations and evaluation of the results of operations mentioned under D we cannot speak of radicality of thyroid surgery.

Inhibition of cyclin-dependent kinases by purine analogues.

While testing purines related to the non-specific protein kinase inhibitors N6-dimethylaminopurine and N6-(delta 2-isopentenyl)adenine as potential inhibitors of the p34cdc2/cyclin B kinase, we discovered a compound with high specificity, 2-(2-hydroxyethylamino)-6- benzylamino-9-methylpurine (olomoucine). Kinetic analysis of kinase inhibition reveals that olomoucine behaves as a competitive inhibitor for ATP and as a non-competitive inhibitor for histone H1 (linear inhibition for both substrates). The kinase specificity of this inhibition was investigated for 35 highly purified kinases (including p34cdk4/cyclin D1, p40cdk6/cyclin D3, cAMP-dependent and cGMP-dependent kinases, eight protein kinase C isoforms, calmodulin-dependent kinase II, myosin light-chain kinase, mitogen-activated S6 kinase, casein kinase 2, double-stranded RNA-activated protein kinase, AMP-stimulated kinase, eight tyrosine kinases). Most kinases are not significantly inhibited. Only the cell-cycle regulating p34cdc2/cyclin B, p33cdk2/cyclin A and p33cdk2/cyclin E kinases, the brain p33cdk5/p35 kinase and the ERK1/MAP-kinase (and its starfish homologue p44mpk) are substantially inhibited by olomoucine (IC50 values are 7, 7, 7, 3 and 25 microM, respectively). The cdk4/cyclin D1 and cdk6/cyclin D3 kinases are not significantly sensitive to olomoucine (IC50 values greater than 1 mM and 150 microM, respectively). N6-(delta 2-Isopentenyl)adenine is confirmed as a general kinase inhibitor with IC50 values of 50-100 microM for many kinases. The purine specificity of cyclin-dependent kinase inhibition was investigated: among 81 purine derivatives tested, only C2, N6 and N9-substituted purines exert a strong inhibitory effect on the p34cdc2/cyclin B kinase. An essentially similar sensitivity to this olomoucine family of compounds was observed for the brain-specific cdk5/p35 kinase. Structure/activity relationship studies allow speculation on the interactions of olomoucine and its analogues with the kinase catalytic subunit. Olomoucine inhibits in vitro M-phase-promoting factor activity in metaphase-arrested Xenopus egg extracts, inhibits in vitro DNA synthesis in Xenopus interphase egg extracts and inhibits the licensing factor, an essential replication factor ensuring that DNA is replicated only once in each cell cycle. Olomoucine inhibits the starfish oocyte G2/M transition in vivo. Through its unique selectivity olomoucine provides an anti-mitotic reagent that may preferentially inhibit certain steps of the cell cycle.

Microsurgical reconstruction of traumatic defects on the lower extremities.

Out of 167 free transfers 90 were applied to the lower extremities, i.e. 75 for injuries, 11 for an osteomyelitis and 4 for cancer. For these transfers were used the following 15 types of flaps; 37 from the forearm, 15 m. latissimus, 12 m. serratus ant. sup., 6 scapular, 4 fibula, 4 from the groin, 2 gracilis, 2 LAF, 1 TFL, 1 TRAM, 1 dorsalis pedis, 1 sartorius, 1 crista iliaca, 1 rectus abdominis, 1 combined latissimus+serratus. The transfer of flaps was satisfactory in 92%. The transfers to the lower extremity included in no case an emergency flap, in the acute phase within 7 days after injury were transferred 5 flaps. There were in addition 32 delayed and 38 secondary flaps. The report deals with important principles of the application of the above described transfers.

Use of two microsurgical flaps in one stage reconstructive surgery.

In the studied series of 167 free transfer two microsurgical flaps in one stage surgery were used in six cases. Two times latissimus + fibula in extensive defects of the tibia and soft tissues of the shin, two times forearm+forearm in isolated defects of the feet, in one case TRAMF + TRAMF for the reconstruction of both breasts and in one case of reconstruction of the penis and the urethra in a transsexual female to male. In this case three flaps were used--pedunculated rectus abdominis muscle and LAF for the reconstruction of the urethra and a sensitive flap from the forearm for the cover of both preceding flaps. The healing of all flaps was satisfactory and individual case records are described in detail and discussed.

[Restrictive infiltrating cardiomyopathy in primary amyloidosis].

The authors describe a typical echocardiographic finding in restrictive infiltrative cardiomyopathy which after two years led to the diagnosis of primary amyloidosis in a 51-year-old woman. The predominating manifestations of the disease were renal damage with final failure, heart failure, monoclonal gammapathy and damage of other organs. Aimed treatment of amyloidosis and repeated haemodialysis did not prevent the patient's death from cardiac and renal failure. Primary amyloidosis was confirmed by histological examination and necropsy.

Vitamin E and A and periodontium in pregnant diabetics.

The authors followed physiological levels of vitamin E and A in four-week intervals in the course of physiological pregnancy in 11 insulin dependent diabetics. The highest mean levels of both vitamins were found in the IIIrd trimester. Simultaneously, the periodontal condition and the standard of oral hygiene were evaluated by means of epidemiological indexes in two-week intervals. The most marked inflammatory changes of periodontium occurred in the 8th month and were followed by an improvement before delivery, though the standard of oral hygiene was unchanged. The possibility of the eventual contribution of both vitamins in the improvement of periodontal condition in the final stage of pregnancy seems evident.

Reconstruction of the penis in transsexual patients.

In the article, the authors offer a review of the modalities available for phalloplasty in transsexual patients. The classic methods for multi-stage transplantation of tubulized flaps in the lumbar and inguinal regions, in mid-thigh as well as current methods for one-stage reconstruction are discussed. The paper contains the case report of a transsexual patient undergoing one-stage reconstruction of the penis using m. rectus abdominis with the inferior pedicle, urethral reconstruction by a skin graft from simultaneous mastectomy with a free forearm flap covering the muscle. The paper examines the pros and cons of each method for reconstruction.

[Surgical treatment of endocrine orbital disease. Indications, methods,results].

Surgical removal of the thyroid gland in Graves-Basedow's disease has, beyond doubt, also a major effect on endocrine orbitopathy. After total or near-total removal of the thyroid gland after three months a slighter deterioration or even ameloriation of ocular protrusion is found, contrary to the effect of subtotal thyroidectomy. There exists a significant correlation between the volume of the residual gland evaluated according to 24 h-uptake of 131I, and between the regression of the protrusion (r = 0.385, p < 0.01). Total or near-total thyroidectomy alone even in the absence of other therapeutic methods leads after 1 to 4 years to an average regression of the protrusion by 1 mm which matches the results of prednisone treatment, if the latter is used as the sole treatment. Orbital decompression by the modified method of Ogura and Walsh led in patients treated concurrently with prednisone and thyroidectomy to improvement of the visual acuity on average by 50% (from the range of 0.008 to 0.33 to values from 0.4 to 1.0) and to regression of the protrusion on average by 3.3 mm. When determining the time of operation of the oculomotor muscles, in addition to the unchanged ocular finding also the echographic picture in sectorial visualization proves useful.

[Development and results of microsurgical sterility operations at the Gynecology and Obstetrics Clinics of the Brno Masaryk University].

Four periods of microsurgery of tubal sterility and present approach to individual operation techniques in respect to assisted reproduction are evaluated. The evaluation concerns 240 operated women, their average was 31.8 years, 68.8 per cent women were operated for the first time. The pregnancy rate arrived at 90 pregnancies (40.8 per cent), 80 women delivered (33.3 per cent), the abortion occurred in 7 cases (2.9 per cent) and ectopic pregnancies occurred in 11 cases (4.6 per cent). By adhesiolysis we achieved 56 per cent pregnancies, by fimbrioplastics 34 per cent pregnancies and by neostomies only 27 per cent pregnancies. Tubal anastomosis (11 operations) and tubal implantations (9 operations) had no significant influence upon evaluation. Deflection from tubal implantations, anastomoses (except refertilizing ones) or neostomies (especially iterative ones) in favour of in-vitro-fertilization is evident.

[The presence of human papillomavirus deoxyribonucleic acid in tumors in the otorhinolaryngologic area].

14 tumours in the ENT region were tested for the presence of human papillomaviruses (HPV) DNA. The dot-blot hybridization technique was used for HPV sequence detection. HPV DNA mixture of types 6b, 11, 16 and 18 served as a probe. The following five samples showed positive reaction: 3 carcinomas of the larynx, 1 carcinoma of the nasal cavity and 1 papilloma of the nasal cavity.

Acyclic nucleotide analogues: synthesis, antiviral activity and inhibitory effects on some cellular and virus-encoded enzymes in vitro.

Several N-(S)-(3-hydroxy-2-phosphonylmethoxypropyl) (HPMP) and N-(2-phosphonylmethoxyethyl) (PME) derivatives of purine bases (adenine, guanine, 2-aminoadenine, 3-deazaadenine) and cytosine inhibit the growth of various DNA viruses. PME-derivatives (PMEA, PMEG and PMEDAP) are also active against retroviruses. Both types of nucleotide analogues undergo phosphorylation by cellular nucleotide kinases to their mono- and diphosphates. The phosphorylation with crude extracts of L-1210 cells is potentiated by an ATP-regenerating system. HPMPA is phosphorylated faster than PMEA with or without the ATP-regenerating system. The HPMP and PME analogues inhibit several virus-encoded target enzymes and their cellular counterparts: (1) HSV-1 DNA polymerase is inhibited by the diphosphates of the PME series; the virus-encoded enzyme is more sensitive than HeLa DNA pol alpha and beta. PMEApp terminates the growing DNA chain; it specifically replaces dATP. HPMPApp also acts as an alternative substrate of dATP, but, in contrast with PMEApp, it permits limited chain growth. (2) Diphosphates of both series inhibit HSV-1 ribonucleotide reductase; the greatest inhibition of CDP reduction to dCDP is exhibited by HPMPApp and PMEApp. The enzyme isolated from a PMEA-resistant HSV-1 mutant proved less sensitive to PMEApp, hydroxyurea and HPMPApp. (3) Diphosphates of PME derivatives efficiently inhibit AMV(MAV) reverse transcriptase. (4) The purine HPMP and PME analogues and, even more so, their monophosphate derivatives inhibit purine nucleoside phosphorylase from L-1210 cells.

[Rational antibiotic therapy in ORL oncologic surgery].

The decisive point is the suitable time of antibiotic administration, their combination and possibility to extend the surgical repertoire by prevention in particular of general complications. Wound healing is more rapid, and intended adjuvant treatment can, due to the reduce period of wound healing, be applied much sooner. It is more effective and the attainment of target structures by the administered drugs is not blocked by prolonged processes which are sometimes associated with fibrosis, scars and regressive tissue changes.

Periodontium and vitamin E and A in pregnancy.

The evaluation of the clinical condition of periodontium by means of the epidemiological indexes and the level of oral hygiene in two weeks intervals in the course of physiological pregnancy in 39 women in good general health revealed the maximum of inflammatory changes of periodontium in the 8th month of pregnancy with the amelioration shortly before delivery. Simultaneous follow-up of the physiological levels of vitamin E and A in four weeks intervals showed the decline of the mean levels of both vitamins in the course of the 8th month and their marked elevation shortly before delivery; therefore remains questionable, if this elevation reaching over their physiological range, contributes to the amelioration of the condition of periodontium observed at the same time.

Phosphorylation of acyclic nucleotide analogs HPMPA and PMEA in L1210 mouse leukemia cell extracts.

Acyclic nucleotide analogs (S)-9-(3-hydroxy-2-phosphonylmethoxypropyl)adenine (HPMPA) and 9-(2-phosphonylmethoxyethyl)adenine (PMEA) were phosphorylated in the presence of ribonucleoside 5'-triphosphates by the crude extract from mouse leukemia cells L1210 affording the respective mono- and diphosphoryl derivatives. The donor efficiency was decreasing in the order CTP greater than UTP greater than ATP greater than GTP. The presence of an ATP regenerating system stimulated considerably the conversion of both compounds. The rate of PMEA phosphorylation was 5-times slower than that of HPMPA both with and without an ATP regenerating system.

[Personal experience with adjuvant chemotherapy of malignant tumors in otorhinolaryngology].

The authors give an account on the therapeutic results of adjuvant polychemotherapy in advanced squamous-cell carcinomas in the ENT region. Based on their conclusions they recommend in these malignities (stage III, IV) comprehensive treatment comprising neo-adjuvant polychemotherapy, surgery supplemented by simultaneous radiopolychemotherapy along with cyclic adjuvant chemotherapy and hyperthermia.

[Replantation of traumatically amputated parts of extremities-- indications and nomenclature].

The authors submit a brief report on replantation activities during the past five years and discuss the problem of indications and terminology of traumatic amputations. The paper contains instruction regarding communication by telephone between the attending surgeon and a specialized microsurgical department. There are also instruction how to handle the amputated portion of the limb and how to transport the patient and the amputated portion of the limb. The authors discusses traditional procedures in replantation surgery on the basis of his own experience with 292 replantations or revascularizations.

Arm Injuries

[Aspiration of a gestational sac controlled by vaginal ultrasound in multiple pregnancy for the purpose of selective abortion].

A case of an infertile married couple by whom the cause of infertility was oligoasthenospermia of man is described. The couple was treated by intrauterine insemination by capacitated spermatozoa in a cycle stimulated by clomiphene. The ultrasonography revealed a pregnancy of triplets. The patient refused a multiple pregnancy and asked for its interruption. As an alternative we proposed the selective abortion by means of an ultrasound-guided aspiration of two gestational sacs. Description of this method and the result are the theme of our paper.
