CD23 expression on B-lymphocytes and its modulation by cytokines in allergic patients.

The aim of this study was to assess the expression of CD23 on peripheral blood B-cells, and its in vitro modulation by recombinant human interferon-gamma (IFN-gamma) in phytohaemagglutinin-(PHA) or recombinant human interleukin-4 (IL-4)-stimulated cultures in atopic patients with Dermatophagoides pteronyssinus hypersensitivity and in healthy non-atopic subjects. Atopic patients with asthma not receiving allergen-specific immunotherapy (n = 21) were studied and further compared with a group of atopic subjects with asthma under allergen-specific immunotherapy (n = 21). They were age-(+/- 5 yr) and sex-matched. The results were also compared with those obtained in the non-atopic group (n = 11). CD23 expression on B-lymphocytes and its modulation were analyzed by flow cytometry using conjugated monoclonal antibodies with a double immunofluorescence method. Atopic patients had an increase in the percentage of B-cells expressing CD23 in peripheral blood. Phytohaemagglutinin and IL-4 induced a rise in the percentage of CD23-positive B-cells in both atopic groups and non-atopic subjects. Phytohaemagglutinin provoked an increase in the intensity of CD23 expression on B-cells from stimulated cultures in all groups, while IL-4 only produced a significant increase in atopic patients. The presence of IFN-gamma decreased the CD23 expression on B-cells in PHA-stimulated culture of atopic patients, whereas it caused an increase in CD23 expression in the non-atopic group. Furthermore, the presence of IFN-gamma in IL-4-stimulated cultures induced a decrease in CD23 expression on B-cells in all cases.(ABSTRACT TRUNCATED AT 250 WORDS)

IgE antibodies against bovine serum albumin in a case of eosinophilic gastroenteritis.

Eosinophilic gastroenteritis is a disease characterized histologically by an eosinophilic infiltration of the gut. The cause of this disease remains unclear, although both food allergy and food intolerance have been implicated in its pathogenesis. We report the case of a 22-year-old man in whom gastrointestinal symptoms first appeared in childhood, with involvement of mucosa and muscularis layers of stomach and bowel. He presented high IgE blood levels, and his prick test was positive to bovine, pig, and lamb sera. Immunoblots from calf, pig, and lamb sera, incubated with the patient's serum and revealed by autoradiography, demonstrated the presence of a 65-kDa protein band that was recognized by IgE antibodies but not by IgG. This band corresponded to bovine serum albumin, while IgE did not show reactivity with human albumin. These data suggest a possible role for IgE-mediated hypersensitivity mechanisms in the pathogenesis of eosinophilic gastroenteritis.

Lymphocyte immunophenotyping by flow cytometry in normal adults. Comparison of fresh whole blood lysis technique, Ficoll-Paque separation and cryopreservation.

In the present report we have assessed the extent to which Ficoll-Paque separation and cryopreservation of mononuclear cells alter the measurement of lymphocyte subsets by flow cytometry. Standard Ficoll-Paque separation increased the percentage of CD4+, CD19+ and CD4+CD45RA+ cells, as well as decreasing that of CD8+, and CD4+CD29+ cells, compared to the fresh whole blood lysis technique. Moreover, cryopreservation caused a depletion of CD4+ p80+ cells, but normal whole blood values were restored following a short incubation.

[New concepts on the pathogenesis of uveitis].

The pathogenesis of uveitis is not entirely clear. Diverse clinical and experimental findings support the participation of immunologic mechanisms. In fact, uveitis may appear along with many immunologic systemic diseases. In this article, possible etiologic agents of uveitis are described and experimental models discussed. The immunologic mechanisms are analyzed and a unifying hypothesis is presented.

[Diabetic papillopathy. Report of a case].

A 17-year-old patient with long-standing poorly controlled type I diabetes mellitus developed bilateral transient edema of the optic disc with minimal visual impairment. Evaluation with fluoresceins angiography revealed the typical pattern of diabetic papillopathy with disc swelling and peripapillary capillary dilatation. Complete resolution occurred within 6 months, with no specific therapy other than improvement of metabolic control with insulin therapy. Early diagnosis of this complication is required to avoid ocular damage.

[Patients with primary anterior uveitis. Determination of circulating immune complexes].

The level of circulating immune complexes (CIC) in 31 patients with primary anterior uveitis was determined by an ELISA method (C1q in solid phase). In addition, we measured antinuclear antibodies (ANA), HLAB 27, C3 and C4. Increased levels of CIC were found in 38% of patients (Group I), the remainder had normal levels (Group II). Positive ANA were found in 33% of patients in Group I and 26% of patients in Group II. The clinical findings and course of uveitis was similar in both groups. Chronic uveitis was associated to greater levels of CIC as compared to recurrent uveitis among patients in Group I (p less than 0.0001). Also, moderate sequelae from uveitis were associated to higher CIC levels.

Nonproliferative diabetic retinopathy with significant capillary nonperfusion.

A clinical study was carried out in 80 eyes of 50 diabetic patients with significant capillary nonperfusion at least in one eye. Panoramic photography of the fundus and wide-angle composite fluorescein angiography was performed in all cases. Forty-nine patients had type II diabetes. The mean age of the patients was 60 years and the mean duration of diabetes was 12.1 years; 46% of the patients had hypertensive vascular disease. Small neovascular tufts were observed in the iris sphincter in 20% of eyes. In addition, abnormal leakage of dye was observed in these eyes. Abnormal leakage of dye from the iris vessels was also observed in 9 of 20 eyes without clinically visible neovascularization. Numerous soft exudates distributed in a semicircular arc pattern were observed in 49% of eyes. Soft exudates were isolated and scarce in 37% of cases and were completely absent in 14% of eyes. Thus, in a substantial proportion of cases, the severity of the retinopathy could not be assessed by ophthalmoscopic findings alone.

Juvenile retinal detachment. Pan American Association of Ophthalmology and American Journal of Ophthalmology Lecture.

In 20 patients with retinal detachments caused by the impact of a soccer ball, the interval between the injury and the diagnosis of retinal detachment was less than one month in ten cases and less than one week in four cases, suggesting that the retinal breaks occur at the time of injury. The most common breaks were superotemporal dialyses and round or oval holes. Macular damage was common. Nineteen of the 20 retinas were reattached surgically, Final visual acuities were 6/12 (20/40) or better in 12 cases, but worse than 6/60 (20/200) in five patients, all of whom had total long-standing retinal detachments or severe traumatic maculopathy. In 48 male patients with X-chromosome-linked juvenile retinoschisis, there was a high proportion with refractive errors. Vitreous veils occurred in 22 patients (47%) and macular lesions were the most common finding. In younger patients the small macular cysts were arranged in a radial pattern and separated by yellow septa. There were also fine radial folds in the internal limiting membrane in most cases. Peripheral retinoschisis was found in 19 patients (39.6%). In eight patients who underwent photocoagulation in one eye, the retinoschisis had not progressed in either eye after a mean follow-up period of 49 months. Retinal detachments occurred in five eyes (5.2%) of the youngest boys with extensive retinoschisis. In a series of 1,000 consecutive cases of rhegmatogenous retinal detachment, 9.8% were the result of nontraumatic retinal dialyses. Most o(97%) of the nontraumatic retinal dialyses occurred in the inferotemporal quadrant. Multiple dialyses occurred in 32% of the patients. In a series of 100 cases, the dialyses were bilateral in 37. A familial pattern was demonstrated in eight pedigrees. Surgery, including cryotherapy or diathermy, scleral infoldings, silicone implants, and drainage of subretinal fluid, reattached 97 of the retinas but the visual acuity results were often unsatisfactory.

Treatment of symptomatic retinal breaks.

Forty-three patients with symptomatic retinal breaks were treated with photocoagulation and a comparable group of 43 patients were treated with cryopexy. All patients had either a vitreous hemorrhage, or photopsia and floaters. Most of the breaks were of the horseshoe type. Of the patient eyes, 14% (six patients) in both groups developed new retinal tears without detachment during the long term postoperative observation. The new breaks were treated with the method used for the original breaks, and no scleral surgery was necessary. The incidence of retinal detachment was 7% in patients treated with photocoagulation and 11.6% in patients with cryocoagulation, but two cases that developed retinal detachment arising from new tears years after the initial procedure were included.

Systemic vasculitis associated with Fasciola hepatica infection.

We report the case of a 50-year-old man who presented with systemic vasculitis associated with Fasciola hepatica infection. The patient presented with severe skin, kidney, spleen, ophthalmic, and neurological compromise. An immunological examination for primary vasculitis was negative and other infections were discounted by microbiological and serological analyses. The patient was treated with steroids without clinical response. The Fasciola hepatica infection was confirmed by the presence of specific immunoglobulin G (IgG) serum antibodies detected by a quantitative enzyme-linked immunosorbent assay (ELISA) with an optical density (OD) of 0.483 OD units (normal value<0.170 OD units) and a high-titre complement fixation (1/80 dilution). The patient received treatment with triclabendazole and all symptoms and systemic manifestations resolved within weeks. Hence, this previously unreported vasculitis-associated infection, if identified opportunely, can be treated and cured.
