[Thrombolysis in acute myocardial infarction in the aged. A greater risk of cardiac rupture?].

OBJECTIVE: To evaluate age group and i.v. thrombolytic therapy (TT) influences on cardiac rupture complicating acute myocardial infarction (AMI). CONCEPTION: Retrospective analysis of patients (pts) admitted during 1988-89 to an Intensive Care Unit of a Terciary Hospital (UCIM) with AMI. POPULATION: 430 pts were admitted with AMI. During this period 89 pts were submitted to TT (25% age greater than or equal to 65 years). Eighty four pts died during hospital stay and 42 had autopsy study (50%). Only 7 of nonautopsied pts died of undetermined cause. Dead pts had age m +/- SD 72 +/- 11 years old (yo) (37% female, 63% male and 74% age greater than or equal to 65 yo). RESULTS: Nineteen pts died of cardiac rupture (CR) (23%). CR prevalence was 1.4% (3/218) in pts less than 65 yo and 7.5% (16/212) in pts greater than or equal to 65 yo (p less than 0.01). CR prevalence in pts submitted to TT (4.5%) 4/89, was similar to pts not submitted to TT (4.4%) 15/341 (n.s.). Pts less than 65 yo had nonsignificant differences in CR wether submitted or not to TT (0% vs 2%). Elderly pts (greater than or equal to 65 yo) CR prevalence was 18% (4/22) in those submitted to TT vs 6% (12/190) in those not submitted to TT (p less than 0.05). CONCLUSIONS: Thrombolytic therapy may carry an additional risk for cardiac rupture in elderly patients (greater than or equal to 65 yo).

Production and characterization of human gamma interferon from Escherichia coli.

The production of human gamma interferon as intracellular inclusion bodies in Escherichia coli, which simplified the purification process, is described. An expression plasmid carrying lipoprotein and the tryptophane promoters in tandem was used. Preparation of highly pure interferon was achieved using high resolution chromatography after denaturation and renaturation steps. Structural characteristics of this protein were verified by mass spectrometric analysis. Additional control tests have shown the suitability of the final product for clinical purposes.

[Prolonged hospitalization in the intensive care unit: a worthless effort?].

Hospitalization of a patient in critical care unit for over a month is exceptional and often raises the question of an irreversible disease process. From 2715 consecutive patients admitted to an intensive care unit in the last 4 years we identified 20 patients remaining in the unit for over a month. Acute pancreatitis and septic abortion were the most common diagnosis at admission. Overall mortality in this series was 30%. None of the complications developing or the therapeutic procedures used in the first 3 weeks predicted the outcome. In contrast, the presence of respiratory failure, septic shock, requirement of ventilation or parenteral nutrition at day 30 was highly associated with mortality.

Biologicals production by recombinant DNA technology in Cuba.

During 1981 we started an intensive programme for the development of biotechnology in Cuba. The first project was related to the production of leucocyte interferon and the cloning an expression of these genes in prokaryotic and yeast cells. These products are currently obtained in large amounts and their physical and chemical characterization are presented. The use of mammalian cells in culture for the production of recombinant proteins has also been carried out. The results obtained with the expression and amplification of the hepatitis B surface antigen (HBsAg) in CHO cells are discussed and compared with that obtained in yeast. The construction of a recombinant vaccinia virus carrying the HBsAg was also achieved. The specific transcripts were characterized and the Ab levels tested in animals. The results presented here indicate that the host system is a very important aspect to be taken into consideration and in some cases mammalian cells appear to be the best choice.

[Rhabdomyolysis caused by severe sepsis: discussion on its role in the development of acute renal failure].

Rhabdomyolysis was evaluated by measurement of total creatine kinase (CK) and lactic dehydrogenase (LDH) in 19 patients with severe sepsis; 12 developed acute renal failure (Group B) and 7 did not (Group A). Results were compared to 7 patients with trauma (Group C) and 6 patients with chronic renal failure and minor infections (Group D). CK was higher (p less than 0.005) in Group B than in A. Results in Group C were similar to those in A. Elevation of CK correlated to increases in creatinine (r = 0.655, p less than 0.005). CK levels of Group D patients were lower than those of Group B. Blood pressure, lactate and pO2 were similar in both groups but thrombopenia was noted in Group B patients. Our results suggest that rhabdomyolysis and thrombopenia play a role in the development of renal failure in patients with severe sepsis.

Drosophila topoisomerase I: isolation, purification and characterization.

We have purified and characterized topoisomerase I from Drosophila melanogaster. The molecular weight of the enzyme is 135,000; 100,000, 90,000, and 65,000 molecular weight products result from degradation of the enzyme. The enzyme relaxes both positive and negative supercoiled DNA. Mg++ is not absolutely required, but stimulates the enzymatic activity considerably.

Antibody-independent complement activation by a subpopulation of mitogen-induced human T lymphoblasts.

A study was made of the complement activation (CA) capacity of human T lymphoblasts obtained in vitro by PHA stimulation of highly purified T cells. The CA was studied by immunofluorescent detection of the C3 fragments deposited onto the cell membrane following incubation in autologous normal human serum (NHS). Whereas resting T cells did not stain, a mean of 12% of the blast cells were positive for monospecific anti-human C3 fluorescent serum. The possible mechanism through which C3 was deposited on the cell membrane may be a consequence of CA, because complement membrane fluorescence (CMF) was abolished by chelation of divalent cations with EDTA, complement inactivation by heating or incubation at low temperature. This reaction appears to occur in the absence of antibodies, because similar results were found when human agammaglobulinaemic serum was used as the complement source. The addition of ethyleneglycolbis-(aminoethylether)-tetraacetate (EGTA), supplemented with MgCl2 to the NHS failed to abolish the fluorescence, indicating that the alternative pathway was involved in the phenomenon. It is likely that the capacity to activate complement may distinguish a functional subpopulation of human T lymphoblasts.
