Effects of antrectomy or porta-caval shunting on the histamine-storing endocrine-like cells in oxyntic mucosa of rat stomach. A fluorescence histochemical, electron microscopic and chemical study.

1. The argyrophil (enterochromaffin-like) cells in the oxyntic gland area of the rat stomach contain histamine, which can be demonstrated fluorescence microscopically after exposure to gaseous OPT. After administration of L-dopa (or L-5-hydroxytryptophan), these cells produce and temporarily store dopamine (or 5-hydroxytryptamine), demonstrable by its characteristic formaldehyde-induced fluorescence. Ultrastructurally, the enterochromaffin-like cells, which have the appearance of polypeptide hormone-secreting cells, comprise two main cell types, the most predominant one having vesicular type granules (EGL cells), the second most predominant one having smaller, uniformly electron dense granules (A-like cells). 2. Rats were subjected to the following surgical treatments: antrectomy; porta-caval shunting; antrectomy+porta-caval shunting; or sham-operation. Three to eight weeks after surgery the histamine-storing cells (enterochromaffin-like cells) of the oxyntic mucosa were analysed by fluorescence histochemistry, light and (quantitative) electron microscopy, and fluorometric determination of amines. 3. After antrectomy, fluorescence histochemistry and silver staining revealed a reduced number of enterochromaffin-like cells. The histamine content in the oxyntic mucosa was reduced by about 50%. As in unoperated injection of pentagastrin seemed to mobilize histamine. Feeding or injection of insulin failed to do so in antrectomized as opposed to control rats. Ultrastructurally, the cytoplasmic granules of both endocrine-like cell types were less numerous than in the unoperated rats. The reduction in cell number and granularity was particularly conspicuous with regard to the EGL cells. 4. After porta-caval shunting the number of enterochromaffin-like cells increased markedly. Chemical determination revealed a twofold increase in the histamine concentration of the oxyntic mucosa. Feeding or injection of insulin or pentagastrin lowered the histamine concentration. As judged by electron microscopy, the proliferation of endocrine-like cells induced by porta-caval shunting was restricted to the ECL cell type. Besides occurring in greater number, these cells were larger than those in unoperated controls, and their cytoplasm was densely packed with granules that were increased in size. 5. Following antrectomy of the porta-caval shunted rats the number of enterochromaffin-like cells and the oxyntic histamine concentration was reduced. 6. The results support the idea that gastrin exerts trophic as well as excitatory effects on oxyntic endocrine-like cells.

Hemodynamic changes after distal splenorenal shunt studied by sequential angiography.

Distal splenorenal shunts were made in 25 patients with cirrhosis of the liver and portal hypertension. Angiograms were obtained in 16 patients at one week, three and six months, and at one year postoperatively. Portal vein flow had diminished in all patients compared with flow seen in preoperative angiograms. Hepatofugal flow developed in 6 patients during the follow-up period, and in 2 patients only one week postoperatively. The shunt remained patent in all of the patients but one.

Enzyme histochemistry studies of transverse and longitudinal incisions in the common bile duct in rabbits.

The healing process of longitudinal and transverse incisions of the common bile duct in rabbits was studied by identification of 10 different enzymes in the duct wall at various times after the operation. Bile was deviated during the healing. An almost complete disappearance of the subepithelial alkaline phosphatase activity was noted early after longitudinal incisions in contrast to unchanged alkaline phosphatase activity after transverse incisions. Transversely incised ducts almost uniformly developed stricutre formation. The size of the lumen after longitudinal incisions was generally normal. The difference in enzyme pattern following the two types of lesion might reflect a different response of the tissues to the two different types of trauma.

Absorption of blocking activity from human tumor-bearer sera by Staphylococcus aureus, Cowan I.

Staphylococcus aureus, strain Cowan I, contains a cell-wall substance, protein A, which combines with the Fc part of IgG in most mammalian species. It can therefore be used as a solid-phase immunoabsorbant for elimination of the reacting immunoglobulins. Since it has been shown that Cowan I could absorb out the blocking activity of sera from rats bearing isografts of polyoma-virus-induced sarcomas or chemically induced colon carcinomas, we investigated what effects Cowan I absorption of human tumor-bearer sera might have. In all tumor-bearer sera tested, from patients with melanomas or colon carcinomas, treatment with protein-A-containing staphylococci decreased the sera's ability to inhibit lymphocyte-mediated cytotoxicity in vitro. Cowan-I-treated sera from healthy controls had no effect on lymphocyte cytotoxicity. Nor did Cowan-I-treated tumor-bearer sera potentiate or "arm" normal lymphocytes against tumor target cells. There was no evidence of complement-dependent cytotoxicity with added human complement in sera from melanoma and colon carcinoma bearing patients either before or after absorption with Staphylococcus aureus, Cowan I, The concentrations of IgA, IgG and IgM were determined in sera used for in vitro tests of blocking activity and complement-dependent cytotoxicity before and after absorption. No reduction of IgA, reduction to undetectable levels of IgG and 20-30 percent reduction of IgM immunoglobulins as compared to unabsorbed sera were demonstrated.

Spontaneous rupture of the liver.

Four cases of spontaneous rupture of the liver are reported. In all these cases, pathological processes in the liver caused the rupture, i.e. primary hepatoma in 3 cases and a benign angiomatous tumour in one case. In one case, the diagnosis was not established until autopsy and in 3 cases at operation. In one of these latter cases, however, the bleeding source was not found until at a secondary surgical exploration. In none of the patients was the diagnosis suspected preoperatively. Four-quadrant puncture is highly recommended even in patients with only a slight suspicion of intra-abdominal hemorrhage. In cases with a ruptured solitary primary liver tumour, an acute liver resection might be performed. If there are multiple tumours--primary or secondary--simple hemostasis seems to be the treatment of choice. In patients with intra-abdominal hemorrhage, thorough palpation of the whole liver is an obligatory manovre during surgery.
