Salivary steroid changes and physical performance in highly trained cyclists.

The purpose of this work was to study the seasonal salivary cortisol and testosterone changes, and their relationships with lean body mass variations, in highly trained cyclists. Physical fitness, body composition (6 skinfolds) and basal salivary testosterone were evaluated in 7 male cyclists, on two separate occasions. The first assessment was made at the onset of the competitive season and the second 6 months later. Two kinds of exercise tests were carried out. The first test was an incremental exercise test to determine the maximum O2 consumption (VO2max) and the maximum workload (Wmax). We also measured the VO2 and workload (W) attained at the first and second ventilatory thresholds (VO2VT1, WVT1, VO2VT2, WVT2). During the tests the VO2 was recorded every 30 seconds (Oxycon-5, Mijhardt BV, Odijk). As a second test two days later, we assessed the anaerobic capacity expressed as the maximal accumulated O2 deficit (MAOD). Briefly, each subject underwent five submaximal exercises each lasting 6 min at an intensity of 200, 220, 240, 260 and 280 W. We estimated individually the O2 demand by extrapolating the linear relationship between the power and the O2 demand previously established. Afterwards the subjects performed a supramaximal bout at an intensity producing exhaustion between 2 and 4 minutes. The accumulated O2 demand was calculated by multiplying the O2 demand by the supramaximal test duration. The MAOD was computed as the difference between the accumulated O2 demand and the O2 consumed during the supramaximal rides. We found a significant increase in some physical fitness parameters related to aerobic capacity.(ABSTRACT TRUNCATED AT 250 WORDS)

[Perception of patients from rural areas on referral resolution].

Eighty two patients from two public rural outpatient clinics were interviewed about referrals requested during June 1991, the unresolved proportion after six months, the alternative ways used to solve the health problems and their perception about this issue. A total of 95 referrals were requested in June in these clinics (4 every 100 consultations) and information about 85 was obtained. The frequency varies from 10% in the women's program, most of the referrals are directed to surgery, respiratory diseases and dermatology, in the adult's program, to radiology, gastroenterology and a wide variety of specialties. Six months later almost all the referrals had an appointment at the reference hospital. In spite of this, 20 patients did not receive the information and five sought a solution with private physicians. Of the 65 remaining patients, 12.3% lost their appointment due to diverse reasons such as preferring private physicians, oblivion or lack of money. Fifty seven patients went to the reference hospital. Thirty eight percent of these felt that their health problem had not been solved at the moment of the study, because they were still in treatment, were waiting a bed in surgery, the results of some laboratory tests were delayed or they perceived bad treatment. It is concluded that only half of the patients referred to specialists, felt that their health problem had been solved six months later.

[Synchronous lesions in advanced colorectal cancer].

From jan '82 to jun '91 were done 3701 colonoscopic procedures. Of these, we arrived to cecum in 283 patients (pat) (51.1%) (male: 175 pat; female: 108 pat) in the first six months after the diagnostic of the colorectal cancer (CRC). The localization of the primary CRC was: rectum: 17.7%; left colon: 37.8% and right colon: 44.5%. 105 synchronical polyps (p) were seen in 87 pat (30.7%): 61 pat with 1p; 16 pat with 2 p and 10 pat with 3 p. Adenomas: 50/105 p (56.19%). Early cancer was histologically confirmed in 11/105 p (11/59 p adenomas). The distribution of early were 5/11 in the same colon segment ant 6/11 in the different colon segment. 1 pat has 3 early cancer. Synchronic advanced colorectal cancer (ACC) was seen in 9 pat (M/F = 2/1). All of these in different segments. In conclusion, the high frequency of the premalignant (adenomas) an malignant (early and advanced cancer) lesions, and the high risk of these in different segments of the large bowel, shows the colonoscopy is the most efficient method in the detection of the synchronical lesions of the colorectal cancer.

Immunocytochemical localization of laccase L1 in wood decayed by Rigidoporus lignosus.

The cellular distribution of laccase L1 during degradation of wood chips by Rigidoporus lignosus, a tropical white rot fungus, was investigated by using anti-laccase L1 polyclonal antisera in conjunction with immunolabeling techniques. The enzyme was localized in the fungal cytoplasm and was associated with the plasmalemma and the fungal cell wall. An extracellular sheath, often observed around fungal cells, often contained laccase molecules. Diffusion of laccase within apparently unaltered wood was seldom observed. The enzyme penetrated all degraded cell walls, from the secondary wall toward the primary wall, including the middle lamella. Xylem cells showing advanced stages of decay were sometimes devoid of significant labeling. These data suggest that the initial attack on wood was not performed by laccase L1 of R. lignosus. Previous alteration of the lignocellulose complex may facilitate the movement of laccase within the wood cell walls. This immunogold study revealed that laccase localization during wood degradation seems limited not in space but in time.

[Adult morbidity consultations in rural medical centers].

A sample of 823 adult morbidity consultations in four rural medical centers in the VI Region in Chile were studied according to age and group of diagnosis. There was a reduction in the number of consultations with age. The following were the most frequent diagnoses: respiratory disorders 17.3%, infections and parasitic infestation 10.2%, circulatory diseases 10.6%, gastrointestinal diseases 9.8%, genitourinary diseases 8.9%, musculoskeletal disorders 8.6% and mental illnesses 8.0%. No differences were found between this pattern of morbidity and that found in similar studies in urban areas. Important differences were found between the rural centres included in the study and these should be analysed in more detail, as they may reveal different risk factors for these populations.

[Clinical and methodological considerations in the quantification of estrogen and progesterone receptors using monoclonal antibodies in breast cancer].

BACKGROUND: The aim of the present study was to provide information on several clinical and methodological aspects of the measurement of the contents of estrogen (ER) and progesterone (PgR) receptors in breast tumoral tissue using monoclonal antibodies. METHODS: The contents of ER and PgR were measured in 94 tumor specimens of breast carcinoma from pre- and postmenopausal women by the classical method of dextran-charcoal (DC) and an enzyme immunoassay (EIA) with monoclonal antibodies against receptors. RESULTS: The most remarkable results were: a significant difference for ER depending on the menopausal status, an increasing concentration of ER and PgR with age, a greater frequency of the phenotypes ER positive (+)-PgR (+) and ER negative (-) -PgR (-), a lower frequency of positive than negative receptors in the tumors measuring more than 10 cm, a greater presence of tumors with negative than with positive receptors in those located in the lower half of the breast, and the high diagnostic sensitivity of ER and PgR measurement with this method. CONCLUSIONS: There was a remarkable difference in contents of total ER in breast tumors from pre- and post menopausal women. This finding confirms that these tumors may have a different hormone modulation and a different capacity for protein induction depending on the menopausal status.

Tear pH, air pollution, and contact lenses.

We analyzed the tear pH of a random sample of 100 subjects, divided into 3 groups according to the stability of their precorneal tear film (normal eyes, borderline; and dry eyes). The average pH value obtained was 7.52. The pH for borderline and dry eyes was higher than for normal eyes. The purpose of this study was to determine the influence of air pollution, specifically sulfur dioxide (SO2), on the tear pH. We found that air pollution affected the lacrimal pH, which decreased when the atmospheric SO2 increased. Finally, we studied the effect of soft contact lens wear on tear pH after 7 days of contact lens adaptation by assessing the tear pH decrease. We took into account the influence of the sex and age of subjects on the results obtained.

Exclusive measurements of mean pion multiplicities in 4He-nucleus reactions from 200 to 800 MeV/nucleon.

Mean multiplicities of pi+ and pi- in 4He collisions with C, Cu, and Pb at 200, 600, and 800 MeV/u, and with C and Pb at 400 MeV/u have been measured using the large solid angle detector Diogene. The independence of pion multiplicity on projectile incident energy, target mass and proton multiplicity is studied in comparison with intra-nuclear cascade predictions. The discrepancy between experimental results and theory is pointed out and discussed.

[Retrospective study of prodromal symptoms in schizophrenia].

INTRODUCTION: There are some non-psychotic symptoms that can forecast the onset of psychosis. Discovering the differences between the symptoms that lead to disease and those that do not makes it possible to identify them and permits early treatment of the disease. METHODS: A sample of 689 schizophrenic patients was analyzed retrospectively. This sample was obtained from the clinical records database of the University Psychiatric Hospital Institut Pere Mata. Data were analyzed with the SPSS version 9.0 statistical package. RESULTS: The most frequent prodromal symptoms of the sample were the delusional ones, the disorganized ones and the neurotic ones. The prodromal symptoms were equally distributed in both genders. In the subtypes, paranoids showed more delusional symptoms, whereas the nonparanoids presented more disorganized symptoms. Acute onsets had more delusional prodromal symptoms whereas the insidious onsets showed more disorganized ones. CONCLUSIONS: In the prodromal stages of shizophrenia, we can also find the community neurotic prevalences regarding gender. The higher rate of neurotic symptoms in the nonparanoid subtype would be explained by the inclusion of the schizoaffective category, whereas the higher rate of disorganized symptoms categories would be due to the hebephrenic and simple categories. The latter would also explain the prodromal differences in the onset type.

[Temperament and Character Inventory Revised (TCI-R). Standardization and normative data in a general population sample].

INTRODUCTION: The revised version of the Temperament and Character Inventory (TCI-R), a tool designed by C. R. Cloninger for the evaluation of the seven dimensions defined in his psychobiological model of personality, was translated and adapted to Spanish. The aim of the study was to obtain normative data and scales with T-scores in a incidental sample of the general Spanish population. METHODS: After adaptation to Spanish, the tool was administered to 400 subjects from several areas of Spain. The sample is stratified according to age and gender according to the year 2001 Spanish population census. We have studied the differences between men and women and the association between age and dimensions. We have checked the normal distribution of the traits, and proceeded with the standardization and normalization of the scores. RESULTS: We present the mean and standard deviation according to sex for each of the main dimensions and subscales. The scores of the main dimensions obtained for general population according to gender show a normal distribution that has allowed us to standardize them into T-scores. The reliability of the dimensions is high. There are differences in the means depending on gender: women scored higher in Harm Avoidance, Reward Dependence and Cooperativeness. Men scored higher in Persistence. There were no high correlations between age and the dimensions. CONCLUSIONS: The Spanish version of the new TCI-R is an adequate tool for the study of personality dimensions of normal population.

Psychometric properties of the abbreviated Spanish version of TCI-R (TCI-140) and its relationship with the Psychopathological Personality Scales (MMPI-2 PSY-5) in patients.

INTRODUCTION: The short version of the Temperament and Character Inventory-Revised (TCI-R), the TCI-140, is presented. This study aimed: a) to obtain the psychometric properties of TCI-140; b) to analyze the relationship with the normal version of the TCI-R, and c) to study its convergent validity with the MMPI-2 PSY-5. METHOD: The TCI-R and MMPI-2 PSY-5 scales were administered to a sample of consecutive psychiatric inpatients with differential Axis I and II diagnoses. RESULTS: It was found that the TCI-140 dimensions showed reliability coefficients ranging from 0.67 (Reward dependence [RD]) to 0.86 (Self-Transcendence [ST]) and the reliability coefficients of PSY-5 ranging from 0.68 (CON) to 0.86 (NE/NEU). Correlations for the dimensions with the TCI-R original 240-item version and TCI-R 140 item version ranged from 0.91 (Self-Directedness [SD]) to 0.97 (ST). The dimensions had a normal distribution. Correlations of TCI-140 scales with PSY-5 scales provided preliminary evidence supporting the convergent validity of the constructs. Then, Novelty Seeking (NS) was associated with low Constraint, Harm Avoidance (HA) was associated with low Aggressiveness and Positive Emotionality/ Extraversion, and also with high Negative Emotionality/Neuroticism, Reward Dependence (RD) was associated with high Positive Emotionality/Extraversion. Persistence (PS) was related to high aggressiveness, and Positive Emotionality/ Extraversion. On the other hand, SD was correlated with low Psychoticism, and Negative Emotionality/Neuroticism, and also with high Positive Emotionality/Extraversion. Cooperativeness (C) had a relationship to high constraint and low psychoticism. Finally ST was associated with high psychoticism and Positive Emotionality/Extraversion. CONCLUSIONS: The short Spanish version of TCI-R is a useful inventory for the evaluation of the principals dimensions of temperament and character.

[Cavernous hemangioma of the maxillary sinus].

We describe a case of maxillary sinus hemangioma which originated in sinusal mucosa and which extended into the nasal cavity. Clinical and radiologic findings are presented and the literature reviewed. Surgery appears curative in most cases.

[Sinus aspergillosis with impact on the general health status].

We describe a patient with aspergillosis of the maxillary antrum, and systemic affectation (anemia, anorexia, fever). Through a Caldwell-Luck approach, the maxillary antrum was opened and an irregular mass removed, postoperatively the patient has done well, disappearing in two months the ORL and systemic symptoms.

[Congenital cholesteatoma of the petrous process and mastoid: a report of two cases].

Congenital cholesteatoma are unusual. They may arise in different sites of the temporal bone: petrous process, middle ear and mastoid. Primary cholesteatomas originate from embryonary ectodermal inclusions but the pathogenesis is not clear and many different hypotheses regarding its origin have been suggested. The clinical diagnosis is not easy if the tympanic membrane is normal, and computerized tomography and magnetic resonance imaging are useful. We report two cases of congenital cholesteatoma: One located in the petrous process and the other in the mastoid without affection of the tympanic cavity.

[Acute epiglottitis in the adult].

Epiglottitis is a well-defined syndrome caused by Haemophilus influenzae type B in children. It also may affect adults and has an unpredictable clinical course, sometimes complicated with airway obstruction. Two cases of adult epiglottitis are reported. The first responded favorably to medical treatment and the second required emergency airway surgery. Indicators of poor prognosis are described. Epiglottitis should be considered in the differential diagnosis as a potentially lethal cause of sore throat.
