[An abdomino-scrotal hydrocele].

Abdominoscrotal hydrocele is encountered rarely in child age. The authors present an account on two patients with abdominoscrotal hydrocele in a three- and fifteen-year-old boy. They discuss its development, draw attention to the not very marked clinical manifestations, diagnostic pitfalls and the contribution of sonography to diagnosis.

[Reactivity of antibodies to collagen types I to IV and antibodies to chondroitin sulfate in the spleen].

Antibodies to collagen type I and III reacted negatively, antibodies to collagen type IV positively with reticulin, trabeculae and circumferent reticulum of lymphatic sheaths, poorly positively with capsula, strongly positively with subcapsular zone. Antibodies to collagen type II reacted positively with capsula, poorly with subcapsular zone, strongly with sinus wall and poorly with trabeculae. They did not react with circumferent reticulum of periarterial lymphoid sheaths. Antibodies to collagen type II and IV reacted positively with central arteries. Antibodies to chondroitinsulphate C reacted poorly and antibodies to chondroitinsulphate B strongly positively with sinus walls and oval cells spread in the white and red pulpa. Antibodies to chondroitin sulphate A reacted similarly as antibodies to chondroitinsulphate B.

[Traumatic rupture of the spleen. Histopathologic findings].

The most common causes of splenic rupture are blunt injury and surgical intervention in abdominal cavity. Morphological investigation of the spleen often shows but a small capsular laceration as well as haemorrhage and granulocytic infiltration in microscopy of its margins. The laceration may be sometimes difficult to find. There are two types of microscopical haemorrhage. One in sinus-lacking subcapsular tissue zone, the other in deeper layers. In this case the tissue haemorrhage is diffuse, sometimes within or around the marginal zone. The sinuses are neither collapsed nor obturated by any material even close to margins of the wound.

[Dermoid cyst of the spleen].

The case report of a 14-year old girl is given. In her spleen of the weight of 1580 g an irregular cyst of glossy inner surface and with striking trabeculation developed. It was filled with clear greenish liquid. The wall was formed by hyalinised collagen tissue which was covered by epidermoid and cuboidal epithelium on the inner surface. Vessel conglomerates were frequent, some bleeding or mononuclear infiltrates, foci of giant cell granulation tissue, lymphoid or fatty tissue were present.

[Technical problems in the preparation of biopsy samples of the spleen].

Spleen tissue sampling for bioptical examination should be performed after mutual consultation of clinician and pathologist. Clinician should know what is expected from the pathologist. This can influence preparation and sampling method considerably. In a great amount of cases the whole organ is removed. A lab technician is sampling on her own and her contribution to a good result can be either positive or negative. Imprint preparations from the tissue sections can be useful. Specialized examination should be performed in the presence of a pathologist or his co-workers. The active approach of a lab technician can significantly contribute to the value of examined sample of the spleen, of the organ until recently considered to be not important or full of mysteries.

[Cystic lymphangioma of the retroperitoneum].

The authors present the case-history of a patient with a cystic retroperitoneal lymphangioma. Its diagnosis was established only by histological examination of the resected tissue. The authors draw attention to the symptomatology, diagnosis, therapy and discuss contemporary views regarding the cause of this disease.

[Can the system for ordering blood for elective surgery be improved?].

The authors investigated the effectiveness of ordering of blood for elective surgery. Analysis of a group of 333 operated patients revealed that blood was administered only in 9% of the operations, i.e when compared with the number ordered the ratio ordered: issued was 13:1. Based on their own experience the authors present the possibility how to improve the blood ordering system from the surgical as well as blood bank activity aspect.

[Classification of acute pancreatitis in correlation with serum protease levels].

Based on previous work of their own and data in the literature the authors outlined diagnostic and classification criteria of acute pancreatitis. The latter were applied in a prospective study. Thus a group of 71 patients with acute pancreatitis was obtained. In 10 patients of this group at the same time intervals and while the same therapeutic procedures were used, the serum protease levels were followed up. The authors assumed that it will be possible to express thus objectively the severity of the disease and to diagnose in time necrotizing forms of acute pancreatitis. The results re demonstrated the feasibility and suitability of this procedure only for assessment of the severity of the disease.

Systematized progradient multiple combined melanocytic and blue nevus.

A 44-year-old woman was diagnosed as having unilateral multiple progradient pigmented macules and papules of the upper extremity and adjacent part of the back. Microscopically increased amounts of melanin and melanocytes in the basal layer of the epidermis in the early developmental stage of macules were seen. Combinations of blue nevus with junctional or compound nevus or with a simple proliferation of melanocytes in the epidermis were present in the papules. The question of prognosis is discussed.

[Dermatology in Prague and the 1st Congress of the German Dermatology Society 1889].

Around 1900, dermatology as practised in Prague formed a link between Austria and Germany. The Congress of the German Dermatological Society held in 1889 in Prague was the first highlight of this connection. The topics and contents of the papers are characterized. The development of the dermatological clinics in Prague is described and their directors and research emphases are listed.

[Dissimulated dermatozoon delusion with severe skin artifacts].

A 37-year-old civil servant, who had been treated for 9 years for sycosis simplex, covered up the presence of his dermatozoon delusion, which had probably only developed in the course of time since the original diagnosis had been made. In a single examination performed when he was referred to our clinic, he admitted for the first time that he had been using sharp objects to dig out ingrown beard hair and mites. This practice had caused numerous sclerotic scars and ulcers on the facial skin. In our dermatological practice, this was an unusual finding in a patient with a dermatozoon delusion.
