T-lymphocytes and serum inhibitors of cell-mediated immunity in renal insufficiency.

In acute as well as chronic renal insufficiency significant immunological abnormalities were found: diminution of delayed hypersensitivity skin reactions, lymphopenia, reduction of the absolute but not relative number of peripheral blood T-lymphocytes. Responses of lymphocytes from uremic patients to PHA, allogeneic cells, or antigens were normal when lymphocyte cultures were preformed in allologous serum from a healthy individual. Sera from uremic patients had an inhibitory or toxic effect on stimulation of normal lymphocytes. Such an in vitro inhibitory activity was found not only in the whole serum but also when certain substances retained in renal failure (methylguanidine, larger molecules, etc.) were added in the lymphocyte cultures.

[Study of the criteria of evaluation of the exercise capacity of the chronic hemodialyzed subject].

Direct measurement methods (or the forecast related to indirect measurements) of the maximal oxygen uptake are reviewed. 15 patients suffering from renal failure, performed a total of 28 exercises. One can draw the conclusion that the maximum VO2 is inferior in these subjects to the values usually determined in normal healthy subjects of the same age. This decrease can be significantly correlated with the haematocrit. Comparison between direct measurement methods of the maximal oxygen uptake and indirect methods allowing its forecast from the heart rate, shows that only direct methods are to be used in these patients.

[Studies of the variations in functional renal values induced by intravenous dopamine].

We studied, on six patients, the variations in functional renal value and urinary excretion of electrolytes during the intravenous administration of dopamine and in the hours immediately following. Two groups of patients are distinguished. In groupe I, all of whom had normal functional renal values, there was no modification of these, while we observed increases in output, in excretion of electrolytes and in the clearance of the uric acid. In group II, composed of patients with renal insufficiencies, the modifications are less definite but the diuretic and saluretic effect is present. The salidiuretic effects of the dopamine would seem to be dissociated from the cardiovascular effects.
