[Results of treating ophthalmic traumas in circular saw operators].

Twenty two cases of ophthalmic traumas acquired while operating a circular saw are presented. Useful vision of the injured eye was maintained only in 7 patients. In 15 subjects vision acuity ranged from photopsia and localisation of light to lack of photopsia. Thirteen patients were qualified for ophthalmectomy. The majority of patients reported that while operating a circular saw they were removing the blade guard which in their opinion made their work more difficult. These observations stress an urgent need for training of workers and redesigning of circular saws so that a blade guard should not disturb the work but provide and adequate protection against injuries.

[Effect of prostaglandin inhibitors on leukocyte infiltration and on leukocytes in tears after experimental injury of the corneal epithelium].

The effect of drug inhibiting prostaglandin biosynthesis on permeation of leucocytes to tears and cornea after experimental removal of the epithelium was observed. It was shown that 1% indomethacin solution and 0.5% ibuprofen solution considerably decreased the number of leucocytes in tears and their infiltration to the proper substance and negatively affected regeneration of corneal epithelium. The investigations have shown that prostaglandin inhibitors inhibit leucocyte infiltration and may, in effect, decrease the number of proteolytic enzymes that are secreted by these cells.

[Treatment of acute optic neuritis with large doses of corticosteroids].

UNLABELLED: The aim of this study is to compare two modes of treatment of acute optic neuritis (AON). MATERIALS AND METHODS: 98 patients, treated between January 1992 and February 1995, after diagnosis of AON were included in this study. The diagnosis and results of treatment were determined on the basis of ophthalmological examination including visual acuity, perimetry, colour vision testing and neurological assessment. A comparison was made between the two treatment regimens of AON: oral and peribulbar injections of steroids versus intravenous administration followed by oral medication with steroids. The results are in agreement with other studies and show that intravenous steroids followed by oral steroids are more effective treatment of AON on its own or in association with MS.

Acute Disease

[Experimental studies of changes in corneal refraction after use of Nd: YAG laser].

Using Nd: YAG laser 600-800 photodisruptions were made in the cornea of freshly enucleated pigs' eyes. The coagulations were placed in 1/3 superficial part of corneal stroma along circles of 4 mm and 5 mm in diameter. Histological examination revealed spherical empty spaces in the area of coagulations. Corneal epithelium and endothelium were not impaired. The mean decrease of corneal refraction was 1.6 D with the 5 mm circle and 1.3 D with the 4 mm circle. After 24 hours the refraction was further decreased, about 1.3 D on the average in both groups.

[Studies on possible ocular refraction changes after use of Nd: YAG laser].

In 3 human eyes with intraocular tumors, before enucleation, 300-350 photodisruptions were made in the corneal stroma using Nd: YAG laser. Coagulations were placed in 1/3 superficial part of corneal stroma along circles of 5 mm and 6 mm in diameter. Histological examinations revealed spherical empty spaces and cracks of the surrounding stroma in the area coagulations. Corneal refraction examined with Javal's ophthalmometer revealed decrease 1.2-1/4 D, maintained after 24 and 48 hours.

Cornea

[Treatment of idiopathic blepharospasm with botulinum toxin A].

The preparations of botulinum toxin A--Botox and Porton were injected into the orbicular muscle of eye in 18 patients with idiopathic blepharospasm or Meige's syndrome. Improvement was achieved in 17 patients; in 10 injections should be repeated after about 8 weeks. Minor side effects declined after several days. Injection of botulinum toxin could be effective in this disease, otherwise hard to cure.

Aged

[Treatment of orbital hemangioma].

Hemangiomas occur in 1%-2.6% of newborn infants and, after 7 years, they disappear in 70% of them. Indications for the treatment are the deterioration of visual function and great cosmetic defects. Both surgical and radiological therapy are likely to cause numerous complications. The reports concerning the beneficial effect of corticosteroids, injected directly into hemangiomas, have recently been presented. In two of our patients, aged 32 and 43, the injection of steroids resulted in the disappearance of hemangioma during 2-4 weeks.

Adult

[Local use of cyclosporin in nonbacterial corneal ulceration].

The authors present the results of local use of cyclosporin solution in 6 patients with corneal ulcerations occurring in Sjögren disease, rheumatic disease and after corneal transplantation. The ulceration was healed in all patients. The cyclosporin, locally applied, has no side effects and may be an efficient way in the treatment of some immunological diseases of cornea.

Thirty years after the creation of cryoophthalmology.

Low temperatures in ophthalmology were first applied in the end of the previous century. However, those experiments were not sufficiently documented and were not widely used. So the report concerning the Krwawicz (6) method of the cataract cryoextraction published in 1960 is commonly treated as the beginning of the modern cryoophthalmology. This extremely effective and simple method very quickly met with approbation and became commonly applied. After the surgical exposure of the opacificated lens the copper cryoextractor frozen to about -72 degrees C was applied to it. The deep lens freezing to the apparatus occurred enclosing the subcapsular masses. Then the fibres of Zinn ligament were broken and the cataract was gently removed. Due to its safety and the lack of complications the above technique led the real revolution in the cataract surgery. Its was compared to the epochal contribution of Daviel who was the first to remove the lens in 1774. Its importance however, was not just limited to the improvement of the surgical technique. Thanks to the cryoextraction the possibilities of the low temperature application in medicine were noticed. In a short period of time low temperatures began to be used in some other specialties: laryngology, neurosurgery, oncology, etc. It was found out that great number of the eye diseases could be treated in that way, including viral keratitis, haemorrhages to the vitreous, glaucoma, etc. The gradually gained experience gave rise to the new branch of ophthalmology called cryoophthalmology.

Cataract Extraction

[Effect of drugs inhibiting the formation of hydroxide radicals on healing of experimental corneal ulcer].

The 60 per cent water solution of dimethyl sulfoxide (DMSO) was used in the investigations. It was instilled into the conjunctival sac of an eye with experimental corneal ulceration. The dimethyl sulfoxide is inhibiting the formation of hydroxide radicals which cause a damage of the tissue in the course of inflammation. Clinical observations as well as histochemical and histopathological examinations showed that ulcerations treated by dimethyl sulfoxide healed faster than those from the control group and reactions to acid mucopolysaccharides++ returned more rapidly to normal values.

[Effect of drugs inhibiting prostaglandin activity on the appearance of leukocytes in the tears after experimental injury to the corneal epithelium].

The investigations were carried out on 3 groups of rabbits in which the corneal epithelium was removed and into the conjunctival sac were instilled drugs inhibiting the activity of prostaglandins such as indomethacin solution or a 0.5 p.c. solution of ibuprofen. It was demonstrated that these drugs lead in the period of the first 8 hours to reduction of the number of leucocytes in tears from 2 to 20 times in comparison with the control group. The presented observation show that prostaglandin inhibitors may be a successful mean leading to decrease of number of leucocytes in tears after corneal injury.

[Experimental studies of retinal ultrastructure after administration of highly fluorinated alkane into the vitreous body].

In experimental investigations the hydrogen-fluoroalkane was introduced into the rabbit vitreous and subsequently the retina was examined in the electron microscope. The observations were carried out in 1, 4 and 8 weeks after introduction of the compound. It was demonstrated that the fluoroalkane penetrated into the internal retinal layers. These layers showed the presence of macrophages, the cytoplasm of which indicated the absorption of the compound. The structure of the retina was preserved till the end of the observation. The compound did not exert any toxic action.

Alkanes

[Comparative studies of the effects of fly ash, soil dust and clay on selected structures of the visual system of female albino rats].

Observations were carried out on 65 white she-rats. The animals received by+intraconjunctival+route a suspension of volatile ashes from ventilators of electricity works, a suspension of the soil from the area of the power-houses and a suspension of clay from clay mines. The animals exhibited marginal blepharitis and conjunctivitis appearing in the 3-5 days of investigation. The observations point to pollution of the soil of the power-house area and to the noxiousness of the investigated dusts which should be classified to the category of burdensome dusts.

Air Pollutants

[Our observations concerning the use of low temperature in the treatment of glaucoma].

Cyclocryoapplication in glaucomatous patients is performed since 1983 by the method settled on the basis of results of experimental investigations. Observations concern 174 persons with various forms of glaucoma. Best results were obtained in patients with glaucoma in the course of uveitis, with secondary glaucoma after perforating injuries and after intracapsular cataract extraction. Fewest favourable results were observed in patients with closed angle glaucoma.
