Supersensitivity and gastric emptying after vagotomy.

Investigations were carried out in man and dog to study the validity of Cannon's law of denervation supersensitivity in relation to parasympathetic denervation of the stomach. Subthreshold doses of carbachol did not accelerate gastric emptying before vagotomy but caused a significant increase in the rate of gastric emptying after vagotomy. These findings may be relevant to aberrations of gastrointestinal motility after vagotomy and may provide the basis of a test for completeness of gastric vagotomy.

Effect of metoclopramide on transport in the small intestine of the dog.

A method is described for the simultaneous measurement of flow rate, mean transit time, and volume of the intestine during the transport of fluid through a segment of the small intestine of the dog. Using this method, the effect of metoclopramide on motility of the small bowel was assessed. The drug resulted in a decrease in transit time and volume in the test segment of bowel while the flow rate remained constant. This finding suggests that metoclopramide-stimulated contraction of intestinal smooth muscle.

Future training of hospital doctors.

Many changes are occurring in the organization and management of postgraduate medical education and training. Hospital Doctors: Training for the Future (Department of Health, 1993) highlights the need for shorter training programmes and structured curricula. A number of issues still require analysis, particularly the resource implications of expanding the consultant grade.
